ZusammenfassungAnlässlich der Veröffentlichung des Positionspapiers ‚Inklusion und Sportwissenschaft‘ fragte die Autorin, Vizepräsidentin der dvs und Initiatorin der o. g. Publikation, bei verschiedenen sportwissenschaftlichen Standorten nach Resonanz und Umsetzung des im Papier konturierten Aufgabenbereichs ‚Ausbildung‘. Dabei entstand der Eindruck, dass sich der Umgang mit dem Thema „Inklusion“ in den letzten Jahren offensichtlich verändert hat. Ging es vor einigen Jahren in den akademisch geprägten Diskussionen noch primär (und mehr oder weniger erregt) um die Auslegung von Inklusion und um das Pro und Contra der geforderten gesellschaftlichen und bildungspolitischen Veränderungen, so deutet sich derzeit eine pragmatische Betriebsamkeit an. Inklusion erhält insofern Einzug in den Alltag sportwissenschaftlicher Ausbildung, als einschlägige Begriffe in Modulverzeichnisse eingespeist werden, Seminarpläne das Thema Inklusion punktuell aufgreifen und gezielte Oberflächenanpassungen vorgenommen werden. Strukturelle Änderungen bleiben jedoch weitgehend aus. Dieser Eindruck korrespondiert u. a. mit ministeriellen Erlassen und universitären Auflagen, die durch einschlägige Formulierungen oder Auslassungen anzeigen: Fürchtet Euch nicht vor Inklusion. Alles bleibt in etwa so, wie es ist.

Parodontiitti on kansansairaudeksi luokiteltava tulehduksellinen kiinnityskudossairaus. Sen käynnistäjänä pidetään bakteereja, jotka aikaansaavat isännän tulehdusvasteen käynnistymisen. Pitkittynyt tulehdusreaktio aiheuttaa vaurioita parodontiumissa, mikä johtaa lopulta hampaan kiinnityskudoksen menetykseen. Parodontiittia sairastaa arvioilta noin 2/3 suomalaisista (Suominen ym. 2012). Sen hoidossa käytetään ensisijaisesti anti-infektiivista terapiaa, johon voidaan tarvittaessa yhdistää mikrobilääkitys ja/tai kirurgista hoitoa.
Systeeminen lupus erytematosus (SLE) on monimuotoinen autoimmuunireaktioista johtuva systeeminen sidekudostauti, joka koskettaa lähinnä lisääntymisiässä olevia naisia. Taudin esiintyvyys vaihtelee maittain välillä 4–250/100 000 (Gripenberg-Gahmberg ja Konttinen 2002). Sen etiologia on edelleen tuntematon, mutta sairastavuudesta puolet selittyy perimällä. SLE:hen näyttää liittyvän useiden eri geenien polymorfismi. Erilaisten ympäristötekijöiden on ajateltu vaikuttavan siihen, että sairauden oireet laukeavat geneettisesti alttiilla henkilöillä. SLE:hen liittyvät lukuiset immuunijärjestelmän poikkeavuudet, kuten B-solujen hyperaktiivisuus ja vasta-aineiden ylituotanto. Autovasta-aineet ovat keskeisiä SLE:ssä, sillä ne aiheuttavat pahimmillaan jopa potilaan kaikkien elinten sairastumisen. Tumavasta-aineet ovat yleisimpiä ja niistä DNA-vasta-aineet ovat keskeisiä, sillä erityisesti ne muodostavat verenkierrossa kiertäviä kudosvaurioita aiheuttavia immunokomplekseja. Yleisiä oireita ovat mm. ihomuutokset, nivel- ja lihaskivut sekä munuaistulehdus. SLE:n vaikeusaste on hyvin yksilöllinen taudin monitahoisen etiologian takia ja sen aktiivisuus vaihtelee. Lievimmillään sairauden hoitoon riittää aktivoivien ympäristötekijöiden välttäminen sekä tulehduskipulääkkeet, mutta vakavaan tautimuotoon vaaditaan glukokortikoideja sekä vahvaa immunosuppressiivista lääkitystä.
Syventävien opintojen tutkielmana tein kirjallisuuskatsauksen SLE:n ja parodontiitin välisestä yhteydestä. Aineistona tutkielmaan olivat aiheeseen soveltuvat PubMedin tieteelliset artikkelit hakusanoilla ”systemic lupus erythematosus” ja ”periodontitis”, Terveysportin soveltuvat artikkelit hakusanalla ”SLE” sekä Duodecimin sisätautioppikirjat. Katsauksen perusteella sairauksien välillä vaikuttaa olevan jonkinlainen assosiaatio. Niiden patogeneettisten reittien välillä on yhtäläisyyksiä. Tutkimuksissa havaittiin, että parodontiitin esiintyvyys SLE-potilailla on suuri. On myös viitteitä vaikeammasta taudinkuvasta SLE-potilailla. On löydetty tiettyjä geneettisiä tekijöitä, jotka altistavat molemmille sairauksille. Parodontiitti on riskitekijä systeemiselle inflammaatiolle ja täten SLE:lle. Se voi toimia tulehdusvasteen ylläpitäjänä SLE:ssä lukuisten mekanismien kautta (esim. autovasta-aineiden muodostuksen indusointi). Parodontiitin hoidolla havaittiin olevan hyödyllisiä vaikutuksia SLE:n aktiivisuuteen, oireisiin ja mahdollisesti komplikaatioihin. Voidaan myös spekuloida parodontiitin mahdollisuutta SLE:n laukaisevana tekijänä. Kirjallisuuskatsauksen perusteella havaittiin myöskin, että SLE on riskitekijä parodontiitille. Tämä voi johtua sairauden patogeneesista tai siihen liittyvistä oireista. Lisäksi SLE:n immunosuppressiivinen lääkitys saattaa heikentää parodontiumin immuunivastetta.

Abstract This material culture study focuses on the residents of Oulu via small clothing-related items (buttons, buckles, pins). The source material is comprised of not only excavation material, but also of the probate inventories of the townsfolk. Gender archaeology, previously unused in Finnish urban archaeology, is used as the theoretical framework. This study discusses early modern clothing from the perspective of identity with particular emphasis on gender and class; why did townsfolk dress as they did in the 18th century Oulu? Although clothing was mainly tied to class and gender in early modern Oulu, on a personal level there was room for negotiation and transgression. Classes were an integral part of society in the 18th century, and the abolishment of the class system was still ahead. However, the signs of the formation of the middle class and the deterioration of the strict class boundaries are visible in the research material. Sumptuary laws notwithstanding, identity could be expressed through clothing, in particular with small details, and clothing gave people some leeway to negotiate their position in the communityTiivistelmä Valtaosassa 1700-luvun pukeutumista käsittelevässä suomalaisessa tutkimuskirjallisuudessa pukeutuminen jaetaan säätyläisten ja rahvaan pukuun. Tässä materiaalisesta kulttuurista ammentavassa historiallisen ajan arkeologian tutkimuksessa näkökulma on toinen; keskiössä on kaupunkilaisten pukeutuminen uuden ajan kynnyksellä. Tutkimus ei lähde liikkeelle säädyistä, vaan pukeutumisesta kertovasta kaivausaineistosta ja kaupunkilaisten perukirjoista. Tutkimus kohdentuu Suomen historiallisen ajan arkeologiassa vain vähän huomiota saaneisiin esineryhmiin (napit, neulat, soljet) yhdistäen arkeologisen ja dokumentaarisen lähdemateriaalin. Teoreettisena viitekehyksenä käytetään Suomen kaupunkiarkeologiassa aiemmin hyödyntämätöntä sukupuoliarkeologiaa. Tutkimus käsittelee varhaismodernia pukeutumista identiteetin näkökulmasta keskittyen erityisesti sukupuoleen ja luokkaan; miksi 1700-luvun Oulussa pukeuduttiin niin kuin pukeuduttiin? Vaikka varhaismodernissa Oulussa ihmisten pukeutuminen oli pitkälti sääty- ja sukupuolisidonnaista, henkilökohtaisella tasolla oli tilaa neuvottelulle ja rajanylityksille. Säädyt olivat 1700-luvulla olennainen osa yhteiskuntajärjestystä ja säätyjärjestelmän lakkauttamiseen oli vielä runsaasti aikaa, mutta merkit keskiluokan muodostumisesta ja tiukkojen säätyrajojen murenemisesta ovat havaittavissa tutkimusaineistossa. Abstract Best available technology (BAT) should be used for peat extraction runoff purification. One currently used BAT technique is treatment wetland construction on pristine peatland, but under Finnish national strategies, new peat extraction areas should only be established on previously drained peatland and it can be challenging to find natural, intact peatland for treatment wetlands near these areas. This thesis evaluated the function and purification efficiency of treatment wetlands constructed on drained peatland and examined factors that could indicate their purification efficiency. The year-round removal efficiency of treatment wetlands established in drained or pristine peatland areas in a cold climate was also studied. A first study examined the water flow processes in treatment wetlands constructed on drained peatland. In more than 50% of the 20 wetlands studied, the active flow depth was under 20 cm and water distribution was unequal. A second study determined the purification efficiency of treatment wetlands constructed on drained peatland and examined factors indicating removal efficiency. The removal efficiency of suspended solids and inorganic nitrogen was good, but in some wetlands leaching of phosphorus and iron was observed, e.g. due to changes in peat properties, first after drainage and then after rewetting, altering e.g., oxygen conditions in the peat layer. The surface peat content of elements such as phosphorus, iron, aluminum, magnesium, manganese, and calcium can be used as a proxy to assess leaching of phosphorus. A third study examined the year-round purification efficiency of treatment wetlands based on peat extraction load monitoring program values. Seasonal variation in the purification efficiency of constructed wetlands was observed for total nitrogen, ammonium-nitrogen, and chemical oxygen demand (CODMn). Based on these results, it is possible to evaluate the suitability of different drained peatland areas for water purification purposes. Treatment wetlands were shown to generally purify runoff water during winter mainly as well as in other seasons, except for nitrogen and CODMn, but there may be variations between wetlandsTiivistelmä Turvetuotannon valumavesien käsittelyssä tulee käyttää parasta saatavilla olevaa käyttökelpoista tekniikkaa, esimerkiksi luonnontilaisille suoalueille rakennettuja pintavalutuskenttiä. Valtakunnallisilla alueidenkäyttöstrategioilla turvetuotantoa ohjataan yhä enemmän jo aiemmin ojitetuille suoalueille. Näiden läheisyydestä ei välttämättä löydy sopivaa ojittamatonta suoaluetta pintavalutuskentän perustamista varten. Tämän tutkimuksen tavoitteena oli selvittää miten ojitetuille suoalueille rakennetut pintavalutuskentät (kosteikot) toimivat ja puhdistavat valumavesiä sekä mitkä tekijät voivat kuvata niiden toimivuutta. Työssä selvitettiin myös ojittamattomille ja ojitetuille suoalueille rakennettujen pintavalutuskenttien ympärivuotista toimivuutta. Työn ensimmäisessä osassa tutkittiin veden virtausta ojitetulle suoalueille rakennetuilla pintavalutuskentillä. Yli puolella tutkituista pintavalutuskentistä oli havaittavissa, että veden virtauskerros turpeessa oli matala (alle 20 cm) ja vesi jakautui pintavalutuskentälle epätasaisesti. Työn toisessa osassa selvitettiin ojitetuille suoalueille rakennettujen pintavalutuskenttien puhdistustehokkuutta ja sitä indikoivia tekijöitä. Ojitetut pintavalutuskentät puhdistivat hyvin kiintoainetta ja epäorgaanista typpeä, mutta joillakin kohteilla oli havaittavissa fosforin ja raudan huuhtoutumista, mikä johtunee osaltaan kuivatuksen aiheuttamista muutoksista turpeessa ja happiolosuhteiden muutoksista uudelleen vesittämisen jälkeen. Erityisesti fosforin huuhtoumista voitiin alustavasti arvioida pintaturpeen fosfori-, rauta-, alumiini-, magnesium-, mangaani- ja kalsiumpitoisuuksien perusteella. Työn kolmannessa osassa määritettiin olemassa olevan vedenlaatuaineiston avulla pintavalutuskenttien ympärivuotista puhdistustehokkuutta. Selvää vuodenaikaisvaihtelua ei havaittu kokonaistyppeä, ammoniumtyppeä ja kemiallisen hapenkulutuksen (CODMn)-arvoa lukuun ottamatta. Työn tuloksia voidaan käyttää, kun arvioidaan etukäteen ojitettujen suoalueiden soveltuvuutta vedenpuhdistukseen. Kosteikot vaikuttivat pääasiassa typpeä ja CODMn-arvoa lukuun ottamatta olevan yhtä tehokkaita valumavesien puhdistajia myös talvella kuin muina vuodenaikoina, tosin kohteiden välillä oli vaihtelua

Tutkielmani aiheena on alaskalaisen Eowyn Iveyn esikoisromaani The Snow Child (2012) lapsettomuuden kriisin ja surutyön prosessin allegorisena kuvauksena. Teos käsittelee raskaita asioita ihmeellisen herkällä tavalla: Ivey kuvaa masennusta, surua ja lapsettomuuden kriisiä osin realistisesti, osin maagisen realismin, sadun ja fantastisen keinoin. Runsas symboliikka ja maagiset elementit realistisen masennus- ja selviytymiskertomuksen rinnalla tarjoavat mahdollisuuden koko teoksen allegoriseen lukemiseen. Tutkielmassani keskityn lapsettomuuden kriisiin ja surutyön prosessiin lähinnä naishahmo Mabelin kautta. Miehiset hahmot jäävät sivummalle johtuen feministisen ja ekofeministisen tutkimusotteen merkityksestä tutkielmassani. Tutkielmani nimessä esiintyvä lainaus teoksesta on allegorinen viitaten melankolisen Mabelin tilaan ja Mabelin ja talvisin ilmestyvän lumityttö Fainan suhteeseen: jää kantaa masentunutta Mabelia sittenkin, eikä hän huku. Lainauksen allegorisuus selittyy tulkinnallani Fainasta Mabelin pelastajana, keinona, jolla Mabel selviytyy surustaan. Faina, Mabelin lisäksi teoksen toinen merkittävä naishahmo, on maaginen mysteeri ja sellaiseksi myös jää teoksen lopussa. Fainan alkuperästä, olemuksesta ja sen merkityksestä teoksessa esitän pohdintoja ja hypoteeseja useassa luvussa. Pyrin perustelemaan päätelmiäni tutkimalla teoksessa esiintyviä eri genrejä ja niiden suhdetta allegoriaan, osoittamalla teoksen intertekstuaalisia yhteyksiä eri hypoteksteihin, erityisesti satuihin, ja ennen kaikkea analysoimalla löytämiäni allegorisia piirteitä lähemmin sekä viittaamalla psykologisiin tutkimuksiin. Teos sijoittuu Alaskaan, ja luonnon kuvaus on siinä oleellista. Käsittelenkin melko laajasti teoksen luontosymboliikkaa allegorisuuden tuottajana ja merkittävänä osana tulkintaa. Teoksen suhde luontoon perustelee myös ekofeministisen lähestymistavan feministisen ja psykologisen tutkimusotteeni ohessa. Ekofeminismi liittyy ajatukseeni käsittää Faina paitsi liminaalitilassa olevaksi tytöksi ja osin Mabelin psyyken luomaksi fantasiaksi, myös Alaskan ja Toiseuden symboliksi. Osoitan Iveyn teosta analysoimalla myös sen kantaaottavuuden äitiyden rooliin ja sen ongelmiin. Psykologista tutkimusotetta edustavat äitiyden kriisien tutkimukseen keskittyneet psykoanalyyttiset ja psykologiset tutkimukset, joita käytän Mabelin tilan selittämisessä. Mabelin masennuksen ja surutyön kaaren tarkastelun yhteydessä käytän myös lääketieteellisiä lähteitä havainnollistamaan ja perustelemaan Mabelin toimintaa. Jo mainitsemani liminaalisuus on työssäni olennaista, ja pyrin lääketieteellisin ja psykologisin lähtein täsmentämään tutkielmassa esittämiäni ajatuksia masennuksesta liminaalitilana. Haluan myös nostaa esille uutta alaskalaista naiskirjallisuutta, sillä Iveystä ei ole juuri tehty tutkimuksia. Tutkimukseni perusteella koko teoksen voi lukea surutyön allegoriana, jossa on elementtejä maagisesta realismista ja fantastisesta. Teoksessa kuvattujen lapsettomuuden surun ja masennuksen ollessa fiktion ulkopuolellakin olemassa olevia ongelmia esitän, että teosta voisi hyvin käyttää myös kirjallisuusterapiassa. Allegorisen tulkinnan kautta teoksen voi käsittää melankolisuudestaan huolimatta viestivän toivosta ja elämän hyvyydestä.

Abstract This thesis focuses on the design of coordinated downlink beamforming techniques for wireless multi-cell multi-user multi-antenna systems. In particular, cellular and cognitive radio networks are considered. In general, coordinated beamforming schemes aim to improve system performance, especially at the cell-edge area, by controlling inter-cell interference. In this work, special emphasis is put on practical coordinated beamforming designs that can be implemented in a decentralized manner by relying on local channel state information (CSI) and low-rate backhaul signaling. The network design objective is the sum power minimization (SPMin) of base stations (BSs) while providing the guaranteed minimum rate for each user. Decentralized coordinated beamforming techniques are developed for cellular multi-user multiple-input single-output (MISO) systems. The proposed iterative algorithms are based on classical primal and dual decomposition methods. The SPMin problem is decomposed into two optimization levels, i.e., BS-specific subproblems for the beamforming design and a network-wide master problem for the inter-cell interference coordination. After the acquisition of local CSI, each BS can independently compute its transmit beamformers by solving the subproblem via standard convex optimization techniques. Interference coordination is managed by solving the master problem via a traditional subgradient method that requires scalar information exchange between the BSs. The algorithms make it possible to satisfy the user-specific rate constraints for any iteration. Hence, delay and signaling overhead can be reduced by limiting the number of performed iterations. In this respect, the proposed algorithms are applicable to practical implementations unlike most of the existing decentralized approaches. The numerical results demonstrate that the algorithms provide significant performance gains over zero-forcing beamforming strategies. Coordinated beamforming is also studied in cellular multi-user multiple-input multiple-output (MIMO) systems. The corresponding non-convex SPMin problem is divided into transmit and receive beamforming optimization steps that are alternately solved via successive convex approximation method and the linear minimum mean square error criterion, respectively, until the desired level of convergence is attained. In addition to centralized design, two decentralized primal decomposition-based algorithms are proposed wherein the transmit and receive beamforming designs are facilitated by a combination of pilot and backhaul signaling. The results show that the proposed MIMO algorithms notably outperform the MISO ones. Finally, cellular coordinated beamforming strategies are extended to multi-user MISO cognitive radio systems, where primary and secondary networks share the same spectrum. Here, network optimization is performed for the secondary system with additional interference constraints imposed for the primary users. Yhteistoiminnalliset keilanmuodostustekniikat pyrkivät parantamaan verkkojen suorituskykyä kontrolloimalla monisoluhäiriötä, erityisesti tukiasemasolujen reuna-alueilla. Tässä työssä painotetaan erityisesti käytännöllisten yhteistoiminnallisten keilanmuodostustekniikoiden suunnittelua, joka voidaan toteuttaa hajautetusti perustuen paikalliseen kanavatietoon ja tukiasemien väliseen informaationvaihtoon. Verkon suunnittelutavoite on minimoida tukiasemien kokonaislähetysteho samalla, kun jokaiselle käyttäjälle taataan tietty vähimmäistiedonsiirtonopeus. Hajautettuja yhteistoiminnallisia keilanmuodostustekniikoita kehitetään moni-tulo yksi-lähtö -solukkoverkoille. Monisoluhäiriötä kontrolloidaan ratkaisemalla pääongelma käyttäen perinteistä aligradienttimenetelmää. Tämä vaatii tukiasemien välistä informaationvaihtoa. Ehdotetut algoritmit takaavat käyttäjäkohtaiset tiedonsiirtonopeustavoitteet jokaisella iterointikierroksella. Tämä mahdollistaa viiveen pienentämisen ja tukiasemien välisen informaatiovaihdon kontrolloimisen. Yhteistoiminnallista keilanmuodostusta tutkitaan myös moni-tulo moni-lähtö -solukkoverkoissa, joissa tukiasemat sekä päätelaitteet ovat varustettuja monilla antenneilla. Tällaisessa verkossa lähetystehon minimointiongelma on ei-konveksi. Optimointiongelma jaetaan lähetys- ja vastaanottokeilanmuodostukseen, jotka toistetaan vuorotellen, kunnes algoritmi konvergoituu. Lähetyskeilanmuodostusongelma ratkaistaan peräkkäisillä konvekseilla approksimaatioilla. Vastaanottimen keilanmuodostus toteutetaan summaneliövirheen minimoinnin kautta. Keskitetyn algoritmin lisäksi tässä työssä kehitetään myös kaksi hajautettua algoritmia, jotka perustuvat primaalihajotelmaan. Hajautettua toteutusta helpotetaan pilottisignaloinnilla ja tukiasemien välisellä informaationvaihdolla. Numeeriset tulokset osoittavat, että moni-tulo moni-lähtö -tekniikoilla on merkittävästi parempi suorituskyky kuin moni-tulo yksi-lähtö -tekniikoilla. Lopuksi yhteistoiminnallista keilanmuodostusta tarkastellaan kognitiiviradioverkoissa, joissa primaari- ja sekundaarijärjestelmät jakavat saman taajuuskaistan. A anorexia nervosa restritiva configura-se a uma perturbacao alimentar com falta aparente de interesse na alimentacao, aversao a alimentos e preocupacao com o possivel ganho de peso, manifestada por fracasso persistente em satisfazer as necessidades nutricionais. O objetivo do trabalho foi identificar as intervencoes dieteticas utilizadas para a recuperacao do estado nutricional e clinico de adolescentes acometidas de anorexia nervosa aguda. O estudo trata-se de uma revisao sistematica, por meio de pesquisa bibliografica em levantamento nas bases de dados, foram observados que em relacao ao perfil dos pacientes acometidos por Anorexia Nervosa a prevalencia foi do sexo feminino, a media de faixa etaria de 16 anos. Todo o tratamento da anorexia restritiva aborda um processo muitas vezes lento e complexo, com acompanhamento psicologico e psiquiatrico associado a terapia nutricional, onde observamos que na maioria dos casos citados neste estudo o paciente alcancou bom exito frente a recuperacao com a presentas das terapias familiar e nutricional associados. Ha uma procura por tratamento e melhoria por parte do paciente ja tardia, dificultando a interferencia e analise por parte da equipe, onde sua efetividade esta atrelada a aderencia do paciente ao tratamento. Contudo faz-se necessario a realizacao de mais estudos e pesquisas no periodo da adolescencia, voltados para aplicacao das intervencoes estando a patologia ainda em seu estado primitivo.,

Abstract The main objective of this work is to develop a numerical model to analyze heat transfer and condensation of a rising spherical bubble. The model included the bubble shrinkage during condensation, which can be utilized to analyze the bubble’s total energy loss, raising velocity, and condensation rate of a single bubble compared to multiple bubbles with the same total thermal energy. The equations of motion, heat, and mass transfer were developed. The model results were verified with the bubble condensation experiment data in the literature, in which they exhibited a good agreement. For the validation, the model results were compared with bubble condensation experiment data in the literature, which showed a good agreement with the experimental results. The dynamic term of the model is developed using the force balance on a gravity-driven bubble, including hydrodynamic drag force and gravity/buoyancy force, which acting with and against the bubble’s motion direction. For the thermal part of the model, a condensation correlation has been adapted to represent the Nusselt number as a function of Reynolds number (Re), Jakob number (Ja), and Prandtl number (Pr). A MATLAB code is developed in order to calculate the instantaneous velocity, the radius, and the mass loss of the vapor bubble in each time step. Moreover, the fundamental behavior for a single bubble and multiple bubbles was investigated in various initial conditions under the same total thermal energy. The effects of the initial bubble radius and the temperature difference between the liquid and vapor phases were analyzed for both scenarios in order to examine the condensation rate. It was found that the thermal behavior of the condensing bubble can be improved by forcing the bubble to collapse into sub bubbles, which will increase the total interfacial area and the rising velocity. Farther, due to generated sub bubbles, the resultant velocity increased the turbulency and the heat transfer rate accordingly. This study can lead to improve the heat transfer rate and allow for more intensive research to enhance the condensation rate.

During the last twenty years, the Russian adverb davno ‘long ago, for a long time’ was widely discussed in literature. It was recognized that the unique parameter of davno is its inability to be the theme of a sentence. Moreover, if davno functions in the context of aspectual forms relating to the past it can only be the rheme. In the context of the aspectual verbal forms relating to the past but preserving the connection with the moment of speech, davno can be either the rheme proper, or a component of the rheme. A classic example of an aspectual verb form referring to the past is the general factual meaning of the imperfective aspect. At present, the spoken data corpora can shed light on the communicative structure analysis, since the prosodic structure of the sound speech provides a straightforward access to the communicative structure. Novel parameters of davno are as follows. 1) Whereas davno is traditionally recognized as a word of rhematic polarity it can nevertheless function as a component of the theme in the context of attributive clauses and constructions (Davno soglasovannyj visit dolzhen byl sostojatjsja v aprele ‘A visit planned long ago would take place in April’). 2) The general factual meaning of the imperfective aspect, contrary to what was assumed before, is not an absolute prerequisite for davno to function as the rheme. The spoken corpus showed that in the context of negation and in the context of the verbs of speech, the general factual allows for davno to function as a component of the rheme but not the rheme proper (Ja davno tebja ne videl ‘I have not been seeing you for a long time’; My davno govorili, chto nasha zadacha — eto borjba s terrorismom ‘We have been insisting for a long time that our main goal is the struggle against terrorism’). 3) A specific type of questions with the initial davno (as well as with other adverbs with the meaning of a considerable quantity like chasto ‘often’, mnogo ‘much’, and daleko ‘far away’) is singled out. Such questions cannot be unambiguously classified either as yes-no-questions or as wh-questions (I davno vy zdesj stoite? ‘And how long are you staying here?’). A description of unique prosody of such questions is given. 4) In the context of discourse continuity, davno acquires the rising prosody which is in fact uncharacteristic of a word, which is unable be the theme (Xotel eto sdelat’ davno, no teperj sdelaju tochno ‘I wished to do it long ago, but now I will do it for sure’). The rising tone is accounted for by the meaning of continuity, which has the same prosody as the theme. 5) In constructions kogda-to davno ‘once upon a time’, ochenj davno ‘very long ago’, davno-davno ‘very long ago’, davnymdavno ‘very long ago’, dovoljno davno ‘quite long ago’, ne tak davno ‘not so long ago’ davno loses its rhematic polarity. The parameters of davno are exemplified by spoken fragments taken from the Multimodal corpus of the Russian National corpus, and the minor working collection of the Russian speech recordings specifically set up for this investigation. The software program Praat was used in the process of analyzing the sound data.

This imbalance between the need for care and the supply of care not only leads to under-treatment and over-treatment and thus to less (experienced) quality, but also entails the risk of mis-treatment, which means that patient safety is at stake (Berwick, 2005).
