Abstract:
Substance abuse in pregnancy is a cause of maternal and neonatal morbidity and mortality. It can lead to a wide range of health, social, and psychological problems. Many of these mothers are young, single, socially deprived, and often present late for antenatal care. The prevalence is unclear as substance abuse is often concealed, but it is most common in young adults, and may be around 4% in the United Kingdom and 6% in the United States. It is estimated that 200,000–300,000 children living in England and Wales have one or both parents with a drug problem. Patterns and prevalence of substance abuse vary between and within countries, but polysubstance abuse is common. Obstetric anaesthetists may be involved in care of mothers who have known or covert substance abuse. Common problems include poor nutrition, dentition, difficult intravenous access, immunosuppression, and altered drug metabolism. Use of some illicit drugs can cause obstetric complications, and others can mimic serious issues such as pre-eclampsia. The incidence of emergency caesarean delivery is higher. Neonates tend to be premature, small for gestational age, at risk of withdrawal, and have ongoing health issues throughout life. Healthcare workers should enquire about tobacco, alcohol, and illicit drug use early in pregnancy as advice and support may motivate women to alter their lifestyle. The impact of tobacco, caffeine, alcohol, marijuana, solvents, opioids, cocaine, and amphetamine use on the mother and fetus, and the implications for the obstetric anaesthetist, are presented.

Abstract:
This go-to resource on substance abuse supplies the broad background knowledge and historical information needed to understand this important sociological issue and provides readers with a range of additional sources for continuing their study of the topic. From the pharmaceuticals advertised on television for various specific medical conditions; to alcohol, which is consumed regularly as a societal norm; to illicit drugs such as cocaine, heroin, and methamphetamine; to marijuana, which is becoming legal in an increasing number of U.S. states, drugs are all around us and are ingrained in our culture. The difficult reality is that any drug is a substance that can be abused. Substance Abuse: A Reference Handbook provides a detailed discussion of the history of substance abuse, covers the classification of drugs, explains how drugs work in the body, includes a general survey of both legal and illegal drugs, and describes the methods of substance abuse prevention and treatment. Readers receive a comprehensive introduction to the broad topic of substance abuse and a variety of additional resources with which to conduct extensive research. In addition to describing the nature of licit and illicit drugs, the beneficial and harmful effects drugs can have on the human body, and factors that may lead to abuse and/or substance abuse, this book covers subtopics such as drug testing in a variety of settings including the workplace and sports, drug control mechanisms, and the debates relating to the legalization of drugs such as medical and recreational marijuana. The book also offers primary source resources that enable readers to directly examine the text of documents, such as significant laws and court cases dealing with aspects of substance abuse, alcohol prohibition amendments, the Controlled Substances Act of 1970, a series of memoranda from the U.S. Department of Justice regarding the prosecution of marijuana cases, and state doctor shopping laws.

It can also have far-reaching impacts on psychological well-being, relationships, and finances. A part of Greenwood’s Q&A Health Guides series, Substance Abuse: Your Questions Answered provides clear, concise information for readers interested in or struggling with this difficult subject. It explores the substances most frequently abused in the United States, the causes and effects of their abuse, treatment options, and the role that popular culture and the media have in shaping our understanding of these behaviors. Abstract:
This text provides readers with an in-depth understanding of the essential aspects of youth substance abuse—an important contemporary personal, social, and public health issue. Humans have been using natural and synthetic chemicals for at least two millennia—primarily for the purpose of treating medical problems, but also for recreational purposes. The 2014 Monitoring the Future survey of eighth, tenth, and twelfth grade students indicates a general decline in the use of illicit drugs over the last two decades. On the other hand, perceptions among youth that certain types of drug use—like marijuana and e-cigarettes—are harmless are growing. Youth Substance Abuse: A Reference Handbook provides an overview of the history and development of youth substance abuse along with a discussion of the medical, social, psychological, legal, and economic issues associated with youth substance abuse both in the United States and other parts of the world. The book begins with a comprehensive introduction to the subject of youth substance abuse that explains how modern societies have reached the point where abuse of both legal and illegal substances is a major health issue in many nations. Readers will learn about the effects of substances such as cocaine, marijuana, and heroin as well as substances that are typically legal but have deleterious health, social, or psychological effects, such as tobacco, alcohol, prescription drugs, and electronic cigarettes. Subsequent chapters address how and why youth substance abuse has become a problem in the United States and other countries, the demographics of this widespread problem, the direct and indirect effects of youth substance abuse and addiction, and the range of services and methods that are available for treating substance abuse.
