Abstract:
Introduction: HIV/AIDS is a worldwide health problem that has claimed a total of 36.7 million lives due to the HIV virus. In Indonesia, there are 30,935 cases of HIV and 7,185 cases of AIDS. HIV/AIDS can affect a person’s mental and psychological health. For example, the most common psychological problem faced by HIV/AIDS patients is depression. Depression experienced by HIV/AIDS patients is caused by the burden of life and social stigma. To overcome the psychological problems experienced by PLHIV, it is necessary to provide psychotherapy such as Acceptance and Commitment Therapy (ACT). ACT is a form of psychotherapy that focuses on changing a person’s mindset through their experiences so that they can move on with their lives. Objective: The purpose of this study was to identify the effect of ACT on depression in PLHIV. Methods: This study used a quasi-experimental design with a one group pretest-posttest design. This study’s population consisted of all HIV/AIDS patients who were also depressed. The number of samples in this study is 33 PLWHA who had depression problems using a purposive sampling technique. The intervention given to treat depression in HIV/AIDS patients is ACT with 4 sessions, and the analysis of this study used a t-test. Results: The average depression score of respondents decreased from 38.73 before being given Acceptance and Commitment Therapy to 19.27 after being given ACT. Conclusion: There was a significant difference in the average depression score between before and after being given ACT (t= 63,162 and p=0.000).

Abstract:
Abstract Kaposi’s sarcoma (KS) is the most frequent malignancy associated with HIV infection (AIDS-KS), a complication that leads to high mortality and morbidity. AIDS-KS cells are resistant to killing by chemotherapeutic drugs/NK cells and Fas-induced apoptosis, suggesting that the acquisition of antiapoptotic characteristics by AIDS-KS cells may contribute to their prolonged survival. Apo-2 ligand (Apo-2L)/TNF-related apoptosis-inducing ligand, a new member of the TNF family, has been identified as an apoptosis-inducing molecule. In this study we examined the sensitivity of 10 different AIDS-KS isolates to Apo-2L-mediated cytotoxicity. AIDS-KS cells were relatively resistant to Apo-2L; however, Apo-2L and actinomycin D (Act D) used in combination synergistically potentiated the induction of cell death in nine of the 10 isolates. Apo-2L induced apoptosis in >80% of AIDS-KS cells pretreated with Act D. The caspase inhibitors, zIETD-fmk and zDEVD-fmk, inhibited apoptosis in AIDS-KS by sApo-2L, suggesting that caspase 3-like and caspase 8 or 10 activities are essential for Apo-2L-mediated apoptosis. Act D treatment of AIDS-KS cells markedly and selectively down-regulated Bcl-xL expression, while the expressions of decoy receptors 1 and 2, Bax, cellular FLICE (Fas-associated death domain protein-like IL-1-converting enzyme) inhibitory protein, FADD (Fas-associated death domain protein), procaspase 8, and p53 were not affected. These findings suggest the possible involvement of Bcl-xL in Act D-induced sensitization of AIDS-KS cells to Apo-2L-mediated apoptosis. Furthermore, Act D did not sensitize PBMC or fibroblast cells to Apo-2L. Thus, Apo-2L and Act D used in combination may be of therapeutic value in the treatment of AIDS-KS.

This is a study of 89 community-based nonprofit organizations in northern California that provide prevention and care services to persons at risk of or infected by HIV. Executive directors responded to a telephone and mail survey concerning community HIV consortia mandated by the Title II provisions of the Ryan White CARE Act Mandated coalitions are hypothesized to increase the power of the mandating agency over the member organizations; increase interorganizational awareness, interdependence, domain consensus, and cooperation; provide benefits for consortium members; address gaps in community services; influence the division of labor; encourage interorganizational standardization; and stabilize resource exchanges. Although the hypotheses regarding power and stability are not supported, HIV consortia appear to increase organizational interdependence and service-system efficiency. Implications of these findings for theory, research, and policy development are discussed.

Abstract:
Introduction. Our goal was to assess the long-term impact of AIDS activism of ACT UP/New York on the current adjustment of those who were members during its peak years (1987–1992), including assessment of trauma sequelae as well as posttraumatic growth.Methods. A 90-minute semistructured interview and 6 validated self-report scales were administered. We relied on purposive and snowball sampling to recruit potential participants. Areas covered include demographics, ACT UP participation, and psychiatric problems. Self-report scales provided approximate diagnoses of PTSD and depression, as well as coping, optimism, and related concepts.Results. Participants included 102 men (40% HIV-positive) and 23 women. Seventeen percent reported current symptoms suggesting PTSD, slightly above the range in general population studies. Symptoms consistent with depression were reported by 8% overall, with higher rates for HIV+ men. Enhanced sense of self, belief in change, and empowerment were reported by 93% of respondents, independent of concurrent PTSD or depression.Conclusions. Twenty-eight years later, ACT UP study participants recall their activist days during the AIDS epidemic as the peak experience of their lives. While some continue to have symptoms of stress and depression, most found that their activism has enriched their subsequent lives.

Abstract:
The study reported here explored the HIV/AIDS-related lexicon in two mainstream Arabic dailies in Jordan over a period of 21 years. Throughout the time period, the most frequently used term to refer to HIV/AIDS was al-eidz, the Arabicized acronym for 'AIDS'. Both Jordanian and Western media tended to emphasize the role of religion and social values in the fight against HIV/AIDS. The data also showed a significant attitudinal shift, over time, towards HIV/AIDS. This was evident in the gradual disappearance of the negative attributes, e.g. ‘plague of our time’, which were used intensively in the earlier stages to describe HIV/AIDS and people living with it. It was also clear in the slogans used to mobilize the public and other stakeholders to act against the disease. Finally, the study suggested that future research examine the portrayal of HIV/AIDS in the lexicon of tabloids and broadcast and electronic media..

Abstract:
IntroductionThere is a worldwide need to enhance the capacity of audiometry testing. The objective of this study is to compare the User-operated Audiometry (UAud) system with traditional audiometry in a clinical setting, by investigating if hearing aid effectiveness based on UAud is non-inferior to hearing aid effectiveness based on traditional audiometry, and whether thresholds obtained with the user-operated version of the Audible Contrast Threshold (ACT) test correlates to traditional measures of speech intelligibility. Methods and analysisThe design will be a blinded non-inferiority randomised controlled trial. 250 adults referred for hearing aid treatment will be enrolled in the study. Study participants will be tested using both traditional audiometry as well as the UAud system and they will answer the questionnaire Speech, Spatial and Qualities of Hearing Scale (SSQ12) at baseline. Participants will be randomly divided to receive hearing aids fitted based on either UAud or traditional audiometry. Three months after participants have started using their hearing aids, they will undergo a hearing in noise test with hearing aids to measure their speech-in-noise performance and answer the following questionnaires: SSQ12, the Abbreviated Profile of Hearing Aid Benefit and the International Outcome Inventory for Hearing Aids. The primary outcome is a comparison of the change in SSQ12 scores from baseline to follow-up between the two groups. Participants will undergo the user-operated ACT test of spectro-temporal modulation sensitivity as part of the UAud system. The ACT results will be compared with measures of speech intelligibility from the traditional audiometry session and follow-up measurements.Ethics and disseminationThe project was evaluated by the Research Ethics Committee of Southern Denmark and judged not to need approval. The findings will be submitted to an international peer-reviewed journal and presented at national and international conferences.Trial registration numberNCT05043207.

Abstract:
The study aims to apprehend the subjectivity production possibilities concerning sexuality in a group of women living with HIV/aids, based on the sociopoetic method. The subjects were nine women with HIV/aids attended at the public referral hospital for infectious diseases in Fortaleza-CE. The results appoint that sexuality appears in several dimensions: in the sexual act, in knowing their own body, in professional accomplishment, in feelings of desire and love, besides the feeling of freedom. We concluded that sexuality is present in the individual's totality; it is not limited to the sexual act, but goes much further and is characterized as a dynamic reality

Abstract:
This article summarizes key findings from evaluation and research studies that have received financial support from the HIV/AIDS Bureau of the Health Resources and Services Administration or from Ryan White Comprehensive AIDS Resources Emergency (CARE) Act grantees. These studies suggest that the CARE Act has improved but not equalized service accessibility, quality, and outcomes for different populations living with HIV disease. Evaluations of access to highly active antiretroviral therapy (HAART) found that uninsured patients, women, people of color, and injection drug users waited much longer than others to receive the new therapies. These disparities were not uniform across study sites, suggesting that clinic characteristics and geographic location have a major influence on prescribing patterns. Once patients gained access to HAART, health insurance status made little difference in clinical outcomes.

Abstract:
This study examines the dynamics of grass-roots decision making processes involved in the implementation of the Ryan White CARE Act. Providing social services to persons with HIV/AIDS, the CARE act requires participation of all relevant groups, including representatives of the HIV/AIDS and gay communities. Decision making behavior is explored by applying a political (zero-sum) model and a bureaucratic (the Herbert Thesis) model. Using qualitative research techniques, the Kern County (California) HIV Consortium is used as a case study. Findings shed light on the decision making behavior of social service organizations characterized by intense advocacy and structured on the basis of voluntarism and non-hierarchical relationships. Findings affirm bureaucratic behavior predicted by the Herbert Thesis, and they also discern factors which seem to trigger more conflictual zero-sum behavior.

Abstract:
The thesis proposes a theory for the development of substantive representation among social movement organisations (SMOs). Substantive representation (SR) is the extent to which political institutions advance the policy interests of their constituents, in particular the most disenfranchised. Despite their noble proclamations, institutions of representative democracy often fail to advance the interests of groups who have been ignored and absent at the proverbial table. The thesis establishes a causal process to explain the divergence in SR outcomes among informal SMOs, or all-volunteer groups that disavow formal hierarchy in favour of egalitarian modes of decision-making. It utilises a case study of the AIDS Coalition to Unleash Power (ACT UP), an umbrella organisation dedicated to ending the HIV/AIDS crisis in the United States and worldwide. It explains an anomalous story of SR attainment through the ACT UP Philadelphia chapter, compared to sister groups in New York City and Boston. The analysis draws from 92 semi-structured interviews, 13 months of participant observation, periodical review, and archival databases. ACT UP Philadelphia translated common SMO intentions of inclusivity into the uncommon rituals of practice. It forged a deliberate pipeline to invest not only in the presence but also the power of disenfranchised people with HIV, people too dark and poor to interest counterpart groups in other cities. Through an analytic retelling of ACT UP's history, the thesis argues that the fulfilment of SR depends on the ability of SMOs to appeal to member self-interest. Critically, SMOs can offer material incentives and nurture feelings of debt and obligation: causal steps to recruitment and sustainability of a heterogeneous membership. In building a crucial if contentious core of dissimilar people and partnerships, SMOs can unleash an oft-unrealised power for collective action and SR, by and for disenfranchised peoples who had thought change to be impossible.

Abstract:
This is a two-stage study of HIV/AIDS prevention. In Stage One, the study examines the HIV/AIDS approach of Western aid organisations (INGOs) and compares it to the lived realities of people who are most 'at risk', sex workers, unemployed street youth, and married low income families, in the poor migrant community of Ajegunle in Nigerias Lagos state. The study found that INGOs and their client NGOs emphasised Western medical models of HIV/AIDS for both intervention (e.g. testing and ARV drugs for management) and prevention (e.g. through education and behaviour change). In contrast, among 'at-risk' groups, the study revealed a high degree of knowledge about the transmission of HIV/AIDS (contrary to Western medical assumptions), but also detected strong feelings of powerlessness in being able to address it. INGOs and their client organisations were operating at the levels of tertiary or curative and secondary or behaviour change prevention, whereas the views of the local 'at-risk people' indicated relevance of the primary prevention level, the social structural conditions of the people. In analysing the results of the first stage of the study, the findings identified a process of 'talking past each other' by official aid agencies and those most at risk, thereby inhibiting effective prevention. The INGOs and NGOs used their financial power, based on the gross inequality in the world distribution of resources, to dictate their own agendas, omitting primary intervention and instead concentrating on secondary and tertiary prevention. The study suggests that new thinking about multi-sectoral responses with full community participation is necessary in order to engage in more effective preventive action. The study then sought out alternative sources of power that might permit that to happen, notably the strengths of the local Ajegunle community. As a poor community, they lacked financial resources and human capital, such as skilled workers, but they had significant knowledge capital about their own circumstances and the realities people faced. The community also had considerable cultural capital and local organisations with considerable relational capital around community links, broad based support and commitment to such action. This analysis suggests the need to identify and work through the power differentials using community development processes, especially seeking to empower local communities to take part in decision-making over prevention, if effective action is to take place. The process required is one of a negotiated, inclusive partnerships for sharing information, experience, and decision-making, involving all the relevant stakeholders - the International Organisations (INGOs), National NGOs, Community Groups and the community itself.

Nadim, Hussain. "The Politics of the Security–Development Nexus: A Case Study of the Kerry-Lugar-Berman Act in Pakistan." Thesis, The University of Sydney, 2019. http://hdl.handle.net/2123/21029.

Full text
Abstract:
The adage that there is ‘no security without development and no development without security’ (commonly referred to as the security–development nexus) has become the dominant discourse of Western donor engagement with the developing world since the 2001 terrorist attacks in the United States (US). This thesis argues that the existing literature on the nexus, as seen from a critical neocolonial and securitisation lens, is Western-centric in its parochial attention to the conceptual, policy and programming concerns of the Western donors. As a result, the literature excludes the voices of the local recipient countries on this subject, robbing them of their agency and subjecthood. By doing this, the literature is inadequate in explaining the deeper level politics of the nexus that are present on the ground in the recipient countries. Therefore, this thesis takes a decolonial approach, using a case study of the Kerry-Lugar-Berman Act in Pakistan to explore the politics of the security–development nexus with regard to the issues of ‘sovereignty’ and ‘agency’. The thesis argues that the security–development nexus activates a dialectical power struggle between donor and recipient countries and between different actors within the recipient countries that use the indivisibility of security and development to advance their strategic interests over each other. For instance, the nexus has enabled the US to influence the national security policies and civil–military relationships of Pakistan, challenging its sovereignty. However, the local actors in Pakistan, notably the political and military stakeholders have not been passive bystanders; they, too, have exerted their agency to co-produce the nexus and use it to their own advantage, such as by modernising their armed forces or promoting civilian supremacy in the country. The thesis demonstrates that the security–development nexus is not only a Western donor construct but also actively constructed by recipient countries like Pakistan, which mould it to serve their own interests. Recognising this co-constitutivity allows the prevailing understanding of the security–development nexus to be simultaneously enriched and challenged.

Abstract:
Community, Attachment, Structures and the Epidemic maps some of the changes in gay mens experience and conceptualisation of community that have occurred during the AIDS epidemic. Social identity theory has been employed to investigate the social-psychological aspects of gay identity at personal, social and community levels. The study compares three generations of gay men in Melbourne; pre- peri- and post-AIDS. As a starting point, the study employed focus groups to explore basic conceptions of gay community. In the first of two major data collection phases, 32 gay men between the ages of 18 and 40 participated in semistructured interviews of between 45 minutes and one and a half hours. The interviews explored the mens social networks, past and present relationship to the commercial gay scene, feelings about gay organisations businesses, neighbourhoods, entertainment, aesthetics, the way they see other gay people, their thoughts about the impact of AIDS on gay communities and their aspirations for gay communities. The second phase of data collection utilised a questionnaire developed from the analysis of the interviews. The 55 item questionnaire covered demographic information, coming out history, initial experiences of the gay world, friendship networks, feelings about the institutions, people and conceptual elements of gay community, items concerning practices of gay community, the community attachment subscale from the SAPA study and items on HIV/AIDS. The questionnaire was completed by 432 gay men, 207 recruited at the Midsumma carnival, an annual gay and lesbian event in Melbourne and 225 through the mailing list of the Victorian AIDS Council. Analysis of the questionnaire data uncovered a complex constellation of difference in the conceptualisation and experience of gay community between the groups, particularly with regard to the content and boundaries of the category gay community. The research challenges practice based models of gay community attachment and proposes a more dynamic, fluid and multi-dimensional conceptualisation of gay social identity.

Abstract:
Elite coaches consider aspects of sports science when preparing athletes for competition. Sports scientists conduct applied research and a fundamental purpose of sports science research is to produce knowledge that helps improve the performance of elite athletes. In view of the considerable resources being directed toward research and coaching at the elite level, there is a need to conduct research to identify the relationship between research and coaching practice at the elite level. Australia has an institute of sport or academy of sport in each state and territory dedicated to the development of team and individual sports, both Olympic and non- Olympic. In the area of elite athlete performance, the Australian Institute of Sport (AIS) has gained an international reputation for excellence, and the AIS Research Centre has achieved international recognition for the quality of research projects directed toward the performance of elite athletes. Sports scientists at Australian universities also undertake research related to elite coaching, some of which has occurred in partnership with researchers at Australian institutes of sport. The purpose of the study was to identify the relationship between sports science research at Australian institutes of sport and post-graduate sports science research in Australian universities, and how elite coaches in Australia perceive sports science research practice meeting the needs of elite coaching. A case study method was selected for this thesis, which involved the following data gathering instruments: a survey of 225 elite coaches and 125 sports science researchers, follow-up interviews of elite coaches and sports science researchers, and document analysis of 725 research projects conducted at Australian institutes of sport and postgraduate theses at Master and Doctoral level at Australian universities. An analysis was also conducted to assess the sports science content contained in the National Coaching Accreditation Scheme's Level Three course material. A schedule was developed for the document analysis called the "Williams Sports Science Research Schedule". Interviews were conducted with elite key informants to validate a model that was developed fiom the study. Results of the study revealed a degree of congruence between the perceptions of elite coaches and sports science researchers regarding the research needs of elite coaches and the research activity of sports science researchers. A model, called the "Elite Sports Research Model" was developed to describe that relationship. The Elite Sports Research Model contains four components, namely: coach knowledge, information seeking/dissemination strategies, qualities valued in an elite coach and a sports science researcher, and application of research. Within the model, particular perspectives of elite coaches and particular perspectives of sports science researchers were identified. Some differences were found between elite coaches of team sports and elite coaches of individual sports, as well as some differences between researchers at institutes of sports and researchers at universities. At the elite level in Australia a relationship was found between sports science research activity and the research needs of elite coaches. With the increase in support for elite coaching and sports science research in Australia and internationally, the results of this study should help to inform improvement in sports science research programs that support elite coaching practice.

Abstract:
The purpose of this study was to focus on the Omnibus Budget Reconciliation Act of 1987 and the amendments of 1989 and 1990. Nursing assistants were placed in cluster groups of 300-hours, 120-hours, and 0-hours. Each subject's job performance of nursing care was observed using the Francis's Task Performance Rating Scale. The purpose of the analysis was to determine (1) if there was a significant difference in job performance of patient care between program completers and the challengers, (2) if there was a significant difference in the job performance of patient care between 300-hour and 120-hour, and (3) if there was a significant difference between the content required in OBRA mandated nursing assistant programs and actual job performance skills needed in patient care. It was determined that program completers' job performance of patient care was significantly different from the challengers.

Full text
Abstract:
Policies advocating the use of digital technologies in government schools are promoted by all public school education systems in Australia. This is reflected in the release of political media statements, policies, plans, budgets, digital networking rollouts, curriculum developments, and professional development activities. Resources are being directed towards such initiatives from within school education budgets and from departmental and 'whole of government' initiatives, at state, territory and federal levels. While there is considerable activity being supported by governments, outside of these activities academic publications specifically about these school level initiatives are limited. This research sets out to answer the question: 'what does public schooling mean in Australia in the 21st century given its past tradition of free, compulsory and secular schooling, and given the present policies that are urging the ubiquitous use of digital technologies?' The purpose of this research is to interpret, understand and explain the policies of the public schooling systems in Australia advocating the use of digital technologies. In doing so, this thesis aims to contribute to the development of a stock of Australian research specifically in the schooling sector, about the use of digital technologies in schools. Further, this thesis aims to stimulate and add to the conversations concerning these policies. It is argued that the use of digital technologies in schooling has the capacity to redefine what has previously been understood by 'public schooling'. This thesis is the outcome of an interpretative social inquiry where narrative theory and hegemony have provided its theoretical bases. This thesis has not set out to merge these theories nor has it attempted to reconcile the internal differences within them, but rather, to draw from them, and to use approaches that are pertinent to this study. While such an approach may be contentious and bring some inherent difficulties, the intention of the research has been to draw upon the abstract understandings afforded by these theories and apply them to concrete, particular, yet newly emerging educational activities. This is to provide interpretative and explanatory perspectives to the advocated use of digital technologies in Australian schools and systems, and, in Chapter Six, to forward a proposition for future action. There are several different ways in which this thesis could have been approached and finally could have been structured. Likewise, there are many avenues that require research but have been left without investigation due to limitations of size, space and time. This is not to negate their importance, but rather it is to recognise the limits of this project and to highlight the necessity for more research to be undertaken. Throughout the thesis distance education has been considered in conjunction with the policies directly impinging upon 'face to face' schooling. It is argued that with the advocated use of digital technologies as an inherent part of public schooling, there is emerging, a convergence in these two styles of schooling. Further it is argued that experiences from school level distance education practitioners have the potential to offer some insights that may be useful for those in 'face to face' schools using digital technologies. It is intended then, that the implications from this research will have the capacity to influence how we view centrally developed school education policies, curriculum leadership and management as well as what is intended to happen in the classroom. The thesis has been arranged into three parts. The first three chapters comprise Part One. Chapter One identifies the research space for the thesis. This is achieved by describing the fields of research from which this thesis draws, and introduces the theoretical bases used in the research space identified for this thesis. Chapter Two provides the theoretical bases for the thesis in more detail. In doing so, positivist approaches to the research are rejected. Chapter Three describes the research methods used to interpret, understand and explain the public schooling sectors' digital technologies policies. Together, these three chapters provide an outline of the nature of the research undertaking, and the theories and methods used. Part Two also has three chapters. These are structured around the temporal concept important to narrative theory; that of the past, the present and the future. Chapter Four looks to the past and provides an account of the history and three traditions, it is argued, impinge upon this research project. In particular, this chapter discusses what was intended by the phrase 'public education' in Australia during the 19th and 20th centuries. This chapter establishes the context for the interpretations of the policies that follow. Chapter Five seeks to understand and explain the policy narratives of the present, defined as the research period between 1997 and 2001. This period of time is thought of as sitting temporally between the past history and traditions outlined in Chapter Four and the possible scenarios for the future, proposed in Chapter Six. Part Three brings the thesis to its conclusion by reflecting on the central question identified for this thesis: 'what does public schooling mean in Australia in the 21st century, given its past tradition of free, compulsory and secular schooling, and given the present policies that are urging the ubiquitous use of digital technologies?'

Abstract:
In the second half of the 16th century, in the garden of the Medicean villa of Pratolino the sculptor Valerio Cioli created, among other groups, the Villano con la falce, which represented a peasant in the act of sawing the reeds in a marshy lake where there was a salamander that spurted water from its mouth. The salamander is now lost while the Villano has been identified so far by critics with the statue of the so-called Mietitore attributed to Cioli and now in the Boboli garden deposits. In the 1990s, during restorations at Pratolino, a stone fragment of a male statue was found: through a careful analysis of the documentary and figurative sources as well as a close comparison with other works certainly by Cioli, this paper aims to recognize the fragment as the Villano con la falce.

Abstract:
AbstractUnruptured brain AVMs (bAVMs) remain a controversial subject for practicing neurosurgeons, especially in the light of ARUBA and other observational studies. This retrospective study aims to analyze our experience with unruptured bAVMs to see whether it is beneficial in the long-term and how it corresponds to large literature trials. The study comprised 160 adult patients with unruptured bAVMs surgically treated in Burdenko NMC (Moscow) in 2009–2017. Mean age: 33.4 ± 10лет. Clinical presentations were: seizures in 99 (61.9%), chronical headaches—49 (30.6%), ischemic symptoms—4 (2.5%), asymptomatic in 8 (5%) patients. Spetzler-Martin scale: I—18 pt. (11.3%), II—71 pt. (44.4%), III—60 pt. (37.5%), IV—11 pt. (6.8%). Good outcomes (mRS = 0–2) at discharge were achieved in 149 (93.1%), satisfactory (mRS—3)—9 (5.6%). Follow-up was complete for 97 (60.6%) patients, mean—59.3 (13–108 month). Excellent outcomes (mRS = 0–1) reached in 94.8%. For epilepsy patients, Engel I outcome was found in 50 (84.8%); for chronic headaches, 43 (66.1%) patients reported improvement. Postoperative visual field defects were followed in 22 of 55 (40%), complete recovery was reported in 6 (27%) and partial recovery in 8 (36%) patients. Overall, our results support the conclusion that surgery for low-grade bAVMs (S-M I–II) is a beneficial, low-risk option.

AbstractThe actual role of preoperative embolization of brain arteriovenous malformations (AVMs) is undervalued. The present study aims to describe the effectiveness, safety, technics, and results of the endovascular-based functional downgrading of brain AVMs.Data regarding 31 Spetzler-Martin (SM) grade III AVMs that consecutively underwent a combined endovascular-surgical treatment were reviewed. Clinical and radiological outcomes were evaluated according to modified Ranking Scale score (mRS) and postoperative angiography, respectively.Low-density Onyx was used in all cases, and the timeframe between the embolizations was 10–15 days. Procedures were 1.6 (±0.6) on average. Superselective nidal catheterization was essential to reach the deepest parts of the AVMs, which were the targets. Onyx made the nidus compact and easier to dissect. The Average obliteration rate was 29.6%, with negligible morbidity. Surgery was performed after 3.7 days on average and the AVM exclusion was complete in 83.4% of cases. An mRS score between 0 and 2 was reported in 77.5% of patients, while the best outcome was achieved in small and medium-deep malformations. An effective and safe functional downgrading of brain AVMs must be based upon specific technical key aspects, which make surgery easier and, ultimately, allow for the achievement of the best outcome.

Abstract:
AbstractThis study aims to broaden the scope of attention paid to multicultural societies in existing migration and mobilities literature, through expanding on the topic of spatial design, planning and management of contemporary cemeteries in light of increasing multiculturalism. The chapter explores how national judicial acts and practices facilitate or hinder inclusion of minorities’ needs and the existing local level measures in three Scandinavian towns to safeguard the needs of minorities in cemeteries and crematoria. The empirical data consists of analysis of legislative acts and qualitative interviews with 28 stakeholders in three medium/large-sized towns. As will be shown, Swedish and Norwegian burial acts share many similarities – for example, both acknowledge the need to meet particular burial and cremation needs for people not belonging to the Christian church. Differences mainly relate to how religion and belief communities are involved in the development of cemeteries. Through the interviews, we extract a nuanced picture of how individual towns work with ‘multiculturalism’ by elaborating on the following components that emerged from the analysis: presence of diversity, accommodation of various needs, cooperation with minorities, and reflections over special grave areas or cemetery sections for minorities. This study’s comparative outlook informs current debates on migration and mobilities and further sheds light on the specific inputs for forming policies and practices aiming at facilitating a multicultural approach when designing, planning and managing cemeteries and crematoria.

AbstractThis chapter aims to identify the consequences of seizing correspondence at sea and the subsequent revealing of state secrets in seventeenth-century Brazil. The wars of the colonial empires of the early modern era highlight the centrality of overseas territories in these disputes. The conflict against the Dutch West India Company in the South Atlantic (1630–1654) demonstrates how the Portuguese had to deal with problems arising from the invasion and the constant attack on navigation routes, which affected communication and commerce. The present analysis uses a set of sources: correspondence issued by Governors-general, orders, regiments, instructions, ciphered letters, and printed leaflets. The study of the Portuguese policies of secrecy allows us to understand the role strategies employed to secure privacy and secrecy by Portuguese authorities. This chapter also proposes that the sea enabled and challenged notions of privacy at the state level, evidentiating how colonial disputes in this in-between space demanded secrecy.

Abstract:
AbstractCoal is a rock composed of organic compounds (C, H, and O), inorganic or mineral impurities, and ash content. Burning coal produces waste in fly ash and bottom ash (FABA). FABA that accumulates for a long time can cause environmental problems, such as pollution. Petrokimia Gresik Ltd. Produces FABA in large quantities, so it must be managed by external parties who have a permit to manage it. With the regulatory changes, FABA can be utilized for various products. This study aims to utilize FABA as a filler substitute for raw materials for NPK fertilizer and find the right proportion so that utilization can be carried out optimally. This study applied the Plan-Do-Check-Act (PDAC) method to achieve continuous improvement. The results show that the most optimal FABA ratio is 3:1 with the addition of ZA raw materials with a specific ratio. After analyzing the test product on NPK 15-10-12 plus fertilizer, it is found that the NPK 15-10-12 plus fertilizer using FABA as a filler complies with SNI 2083:2012 about NPK fertilizer. The effectiveness test results on paddy plants at the Experimental Garden of Petrokimia Gresik Ltd. Show that FABA as a fertilizer filler does not affect the effectiveness of NPK fertilizer. This study shows that FABA can be used as a filler substitute for raw materials for NPK fertilizer.

Abstract:
Abstract The hydraulic fracturing technique is based on pumping a given fluid at a high pressure, greater than the rupture pressure of the reservoir rock, which causes the creation of high permeability channels inside the rock. This process also makes the fluid contained in the formation flow preferentially through the newly cracked channels. After stabilizing the pressure inside the rock formation, the fracturing fluid may settle inside the fracture or even infiltrate into the formation. The next step, known as flowback, consists in removing all fracturing fluid, so that the well can start producing. Under certain conditions, significant amounts of hydrocarbons can remain inaccessible and problems such as water blockage, fluid trapping and flowback efficiency reduction can occur, which can cause loss of fluid in the formation and a low production of hydrocarbons. The present work aims to present the effect of flowback aids, composed of combinations of surfactants, in brines and fracturing gels. Flowback aids act mainly on the surface of fracturing fluids and at their interface with the reservoir hydrocarbons. They reduce the deformation resistance of fluid droplets allowing them to cross narrow formation channels. They also reduce the resistance of the fracturing fluid to return to the surface, helping it avoid blocked channels and, consequently, these additives reduce the formation damage and improve the production of reservoir fluids. Experiments were carried out to measure rheology, stability, surface and interfacial properties, and a physical simulator of flow in porous media was used. The rheology results showed that the use of flowback aids did not change the consistency of the fracturing gel. The experiments with the physical simulator showed that flowback aid solutions in brine flow through a porous medium faster, compared to flowback aid solutions in deionized water. That is, the flowback aids have been designed to perform better in saline solutions over a wide range of concentrations. The results of the surface and interfacial properties measurements showed that a small amount of flowback aids can efficiently change the surface properties of brines and their interfacial properties with the crude oil. These changes improve the malleability of the injected fracturing fluid and reduce the crude oil's resistance to push the fluid back to the surface, allowing a more efficient production of hydrocarbons.

Abstract:
A marine Aids of Navigation (AtoN) is a man-made object in waterways used by mariners to determine their ship's location or a safe route. They act like traffic signs on roads used to reduce the risk of traffic accidents. However, traffic signs can't do their job if you can't recognize them, don't understand their meaning, or violate them. In this study, ship accidents caused by the malfunction of navigation aids were investigated. First, maritime officers’ work in the ship bridge work was analyzed using the Operator Function Model (OFM), and among all the functions derived from the analysis, only the functions related to aids to navigation were selected. Using the selected functions as a classification system for ship accidents, recent ship accidents in the Korean coast were analyzed. The ship accident analysis was conducted using written verdicts of the Korean Maritime Safety Tribunal. Cases of ship accidents were confirmed in most of the selected functions. The results of this study can be used for safety education of maritime officers and the deployment, maintenance, and design of AtoN.

Abstract:
Space travel imposes significant risks to crew health due to physiological adaptations, exposure to physical and psychological stressors, and limited capabilities to provide medical care. When medical emergencies occur, appropriate use of diagnostic and procedural guidance tools are crucial countermeasures against the risks of injury and mission failure. Point-of-care ultrasound (POCUS) is the only portable imaging modality available during exploration missions that can provide critical and dynamic medical information. Developing competency in POCUS is time-consuming and it is usually achieved through years of medical residency or clinical fellowship training programs. Due to the amount of astronaut training currently required, it is not feasible to also provide them with in-depth POCUS training. Current cognitive aids for POCUS-based procedures are either paper-based or static electronic checklists, which can be cumbersome to use, non-intuitive, and sometimes distracting; applying their written guidance to real actions can be difficult. To overcome these limitations, we developed a proof of concept of an augmented reality (AR) Coach (AR-Coach) as an Augmented Clinical Tool (ACT): a hands-free virtual coach system that guides the crew in real-time on how to perform POCUS during medical emergencies in space. To better understand the context and design requirements for the proposed ACT, we applied a human-centered design approach as part of our wider space medicine research program. We convened a multidisciplinary expert panel (n=46), including astronauts, flight surgeons, clinicians, XR and AI experts, to identify essential capability requirements. Task analysis with five experts, including clinicians, human factors researchers, and an XR developer, was used to create a process model of a POCUS-guided procedure to diagnose a potentially life-threatening condition (i.e., pneumothorax) that could occur during space travel. An iterative design and prototyping process was conducted. Informed by the expert panel and task analysis, we created a proof of concept of the AR-Coach which includes holographic panels that guide the crew in confirming the diagnosis of pneumothorax using POCUS. The results of this study can be applied in advancing space technologies that support astronauts in managing medical events during space exploration missions, optimizing performance, and improving crew safety.

Abstract:
In politics, a concession is the act of a losing candidate publicly yielding to a winning candidate after an election after the overall result of the vote has become clear. The present paper finds it is important to trace the concession speeches of the political figures and tries to make a view on the general format of such speeches. So the present paper aims at analyzing the social factor of gender in American concessions sociopragmatically. Then, finding out the way that male and female American politicians use the expressive speech acts. It also aims at investigating the most frequent expressive speech act used by male and female in American concessions. The procedures that are followed in this paper are: 1. presenting a theoretical background about sociolinguistics, gender, socio-pragmatics and political concessions. 2.selecting the data, which is the American concession to be analysed socio-pragmatically. 3.making a conclusion based on what have been found in the analysis. This paper is limited to analyze gender socio-pragmatically according to Searle's classifications of speech acts (1969). The selected data are John Kerry's and Hillary Clinton’s concession speeches after conceding defeat in the US presidential elections.

Abstract:
Nowadays, teaching languages for specific purposes, in particular in the field of entrepreneurship, has to focus on pragmatic and intercultural aspects in response to a multicultural professional reality that comprises different areas of knowledge. Our study aims at analyzing the speech act of making a request in German and Spanish by Portuguese native speakers, i.e. BA students of Languages and Business Relations at University of Aveiro. For this study, two different types of tests were performed: the Discourse Completion Task (DCT) and the Rating Assessment Test. The data provided by the answers given to these two surveys will help the teacher to understand the pragmatic difficulties students have when making a request in these two foreign languages. The information obtained will help the teacher to focus on aspects that are really problematic from the pragmatic point of view, and at the same time, to find and implement strategies and activities that help students improve their pragmatic awareness and overcome difficulties that may arise in intercultural communication. Hence, the objective is to contribute to an adequate development of the students' pragmatic and intercultural communicative competence.

Abstract:
With the Coronavirus pandemic and the necessary social isolation, purchases made by the digital channel gained a notable space, evidecing the changes in shopping habits that have been taking place over the last ten years. In Brazil, the growth of online shopping was 75% and in China, sales through e-commerce are expected to surpass sales in physical stores for the next two years to come (e-Marketer, 2021). In the age of digital interactions, consumers demand a new level of concern. Being at the center in the making of products and services, users define any and all design decisions. Euromonitor, in a report issued in late 2020, pointed out that approximately 60% of consumers between 15 and 29 years old used augmented reality or virtual reality in the last year at the same time that 68% of consumers over 60 years old prefer to talk to human representatives when making transactions or purchases. This proves the need to rethink the experience offered in physical retail, incorporating different generations and shopping habits. Although attracted by the speed and convenience of the virtual environment, customers are looking for personalized and multi-sensory shopping experiences that only physical stores are able to offer. In order to claim their importance and permanence in the current scenario that moves with dynamism, these points of sale must include in the experience they offer personalized solutions that involve, for instance, sensory stimulation and technology. When consistent with the brand image, product, and target audience, these solutions can, together, create a more compelling experience.Marketing or sensory branding consists of the precise use of environmental elements in order to act on the senses and generate affective, cognitive and behavioral reactions, which can contribute to usability and satisfaction, as well as memory and decision-making processes. Through technology, in addition to simplifying tasks, it becomes possible to make visible what would otherwise be invisible or even hide what is irrelevante in the shaping of the shopping journey. In attendance as mnemonic aids, like smartphones and tablets, or even solutions for immersive experiences, as augmented or virtural reality devices, technologies must be designed according to the audience they intend to interact with. This study is characterized as an exploratory research that, through literature review, seeks to raise ideas and thoughts on technology and sensory stimulation acting as support for experience design in physical retail environments, understanding the influence they can exert on users cognitive processes and responses. Based on different fields of knowledge, such as cognitive ergonomics and marketing, the study intends to promote an integrated view of the topic, facilitating its approach and understanding for both designers and retailers.

Abstract:
Review question / Objective: This study aims to evaluate the effectiveness and safety of Tangcao tablet (Tangcao) for treating people with HIV/AIDS. Condition being studied: Acquired immunodeficiency syndrome (AIDS) is a chronic infectious disease characterized by severe immunodeficiency caused by the human immunodeficiency virus (HIV). The infection attacks specifically the white blood cells, CD4+T (CD4) cells, weakening the immunity of individuals against infections such as tuberculosis. Without treatment, patients with AIDS may survive up to 2 years. Pneumocystis pneumonia and infections of the central nervous system are two of the most common causes of death in people with AIDS. AIDS still remains a significant global public health problem, with an estimated 37.7 million people infected with HIV at the end of 2020.

Abstract:
This Coastal and Hydraulics Engineering Technical Note (CHETN) describes a method for evaluating the received coverage from Automatic Identification System (AIS) shore sites and the availability of historic vessel position reports along the Ohio River. The network of AIS shoreside sites installed and operated by the US Army Corps of Engineers (USACE) and the US Coast Guard (USCG) receive information transmitted from vessels; however, reception of these transmissions is generally line-of-sight between the vessel and the AIS site antenna. Reception may also be affected by factors such as the quality of the transceiver installation aboard the vessel as well as the state of the equipment at the receiving site. Understanding how to define and quantify coverage gaps along the inland river system can inform research utilizing AIS data, provide information on the performance of the AIS network, and provide guidance for efforts to address identified coverage gaps

Abstract:
This paper focuses on a rather unexplored dimension to date of the HIV/AIDS epidemic: the resulting stigma and discrimination and its impact on the effectiveness of HIV/AIDS programs. It presents a conceptual framework that explains the interplay between stigma, discrimination and human rights. The study also provides guidelines for developing programmatic activities that situate HIV/AIDS-related stigma and discrimination within the broader social context where a variety of stigmas related to class, race, gender and ethnicity exist. Thus, the framework and the paper can be useful both to those fighting the spread of HIV/AIDS as well as to those fighting exclusion in other social contexts. This paper was prepared as a background document for a seminar entitled HIV/AIDS and Development: Challenges and Responses in Latin America and the Caribbean held at the Annual Meeting of the Boards of Governors of the Inter-American Development Bank and Inter-American Investment Corporation in March 2002.

Tetreault, Brian, Marin Kress, and Patricia DiJoseph. AIS data case study : evaluating reception of AIS position reports on the Missouri River by LOMA AIS sites in April and August 2020. Engineer Research and Development Center (U.S.), January 2022. http://dx.doi.org/10.21079/11681/42980.

Full text
Abstract:
This Coastal and Hydraulics Engineering Technical Note (CHETN) describes a method for evaluating the received coverage from Automatic Identification System (AIS) shoreside sites along the Missouri River managed by the US Army Corps of Engineers (USACE) Lock Operations Management Application (LOMA), and presents the results of that analysis. The purpose is to identify AIS coverage gaps in the current system. Reception of AIS transmissions between shore-based transceivers and vessels is generally line-of-sight between the vessel and the AIS site antenna. However, signal reception may be affected by factors such as the distance and terrain between the vessel and the transceiver site, quality of the transceiver installation, state of the equipment either aboard the vessel or at the shore transceiver station, and atmospheric phenomena. Quantifying coverage gaps along the inland waterways system can inform research that uses AIS data, provide information on the performance of the AIS network, and provide guidance for efforts to address coverage gaps to improve navigation safety. In autumn 2020, severe shoaling was occurring on the Missouri River. As the shoals were identified, the Kansas City District requested the LOMA system transmit AIS Aid to Navigation (AtoN) to mark the shoals in several critical areas. However, vessel pilots sometimes reported that they were not receiving the AIS AtoN being transmitted. At the request of the Kansas City District, the US Army Engineer Research and Development Center, Coastal and Hydraulics Laboratory (ERDC-CHL), conducted a coverage analysis using data collected from the LOMA AIS transceivers in the area to determine if there were coverage issues and their extent and to aid in determining the best means of addressing any coverage gaps.

Kress, Marin. Automatic Identification System (AIS) data case study : identifying unofficial mooring areas along the Upper Mississippi River. Engineer Research and Development Center (U.S.), May 2023. http://dx.doi.org/10.21079/11681/47081.

Full text
Abstract:
This Dredging Operations and Technical Support (DOTS) program technical note presents the results of a study undertaken at the request of staff from the US Army Corps of Engineers (USACE) Rock Island District (MVR) as part of a larger effort examining the potential creation of seven new permanent mooring cells along the Upper Mississippi River in proximity to lock and dam (LD) locations selected by MVR. MVR staff were interested in evaluating vessel traffic and identifying unofficial mooring areas (i.e., waiting areas) in the vicinity of LD7, LD10, LD11, LD14, LD15, LD20, and LD22; they were also interested in travel times from those unofficial mooring areas to the destination lock. The search distance for unofficial mooring areas was limited to 20 miles from the lock, or the distance to the next closest lock if less than 20 miles, in the appropriate direction (i.e., upstream or downstream), as specified by MVR staff.

Kress, Marin. Automatic Identification System (AIS) data case study : vessel traffic through the Yaquina Bay breakwater at Newport, Oregon. Engineer Research and Development Center (U.S.), March 2023. http://dx.doi.org/10.21079/11681/46549.

Full text
Abstract:
The navigation staff at US Army Corps of Engineers (USACE) Portland District (NWP) asked for information on vessel transits through the two existing openings in the breakwater on the north side of Yaquina Bay in Newport, Oregon. Currently, no authorized federal channel passes through the breakwater openings; however, the design for a possible federal channel is under consideration. NWP staff were interested in historical vessel transits, with a special focus on isolating transits for the largest (i.e., longest) vessels, identified as vessels 80 feet or longer, currently utilizing the area inside the breakwater. The Automatic Identification System Analysis Package (AISAP) software created by USACE-ERDC (2018) was used to analyze vessel traffic.

Milante, Gary, and Jannie Lilja. Chronic Crisis Financing? Fifty Years of Humanitarian Aid and Future Prospects. Stockholm International Peace Research Institute, April 2022. http://dx.doi.org/10.55163/agar2561.

Full text
Abstract:
This study maps trends of humanitarian funding in the context of total aid at the coun-try level between 1969 and 2019 and estimates how these trends will change in the future. Historical trends show that the com-position of aid has changed significantly in the last two dec-ades: the humanitarian share of total aid to countries has increased from approxi-mately 5 per cent in the 1990s to 23 per cent in 2019. Humanitarian aid was originally intended to respond to short-term emergencies, how-ever, most of today’s humani-tarian financing goes to pro-tracted situations. Today many countries receive high levels of humanitarian assistance for a decade or longer, referred to here as ‘chronic’ cases. More than half of total global humanitarian assistance (59 per cent) in the decade 2010–19 went to chronic crises, typically synonymous with conflict-affected, fragile and refugee-hosting settings. These chronic crises have become more prevalent since 1995. Conservative forecasts suggest that 71 per cent of humanitarian assistance over the next decade will continue to go to countries receiving high levels of assistance today. The study concludes with open questions to policymakers regarding the use of the humanitarian financing instrument. In chronic crisis settings, the strategic balance between humanitarian, development and peacebuilding financing and the interplay between these streams will remain critical.

Anton, Pablo, Nassim Alemany, and Luis Schloeter. Assessing the Regional Economic Benefits of Public Investments: A Replicable Methodology and Case Study Application to the Dominican Republic. Inter-American Development Bank, 2024. http://dx.doi.org/10.18235/0005689.

Full text
Abstract:
This paper presents a comprehensive methodological framework designed to assess the regional economic benefits of public investments. Recognizing the challenges of limited resources and data, especially at the regional level, the study aims to enhance operational efficiency in the early phases of investment projects. The framework supports more informed decision-making by facilitating a rapid assessment of potential benefits, using a data-driven approach that leverages input-output analysis to refine understanding of local economic linkages. Adaptable to various regions with readily available economic statistics, the tool requires minimal input. In collaboration with the Ministry of Economics, Planning, and Development and the Ministry of Tourism the framework was applied to the Ozama region in the Dominican Republic, yielding a pilot assessment tool. The model elucidates supply chain connections across fifteen economic sectors. A case study of the IDB's investment in Santo Domingo's Colonial City demonstrates the tool's utility, revealing significant localized GVA and employment impacts. This study highlights the framework's potential to aid early-stage decision-making and cost-benefit analyses. While the current version focuses on short-term impacts, future enhancements aim to address long-term effects and additionality, further broadening the tool's applicability.
