Full textIn the last few years, ultrafine metals, with a crystalline grain below the micron, have gained particular interest due to their high mechanical strength combined with adequate ductility. Through severe plastic deformation techniques (Severe Plastic Deformation, SPD) it is possible to obtain ultra-fine grained materials. Among these, the technique of the Equal Channel Angular Pressing (ECAP) is very widespread because it allows an ultrafine structure throughout the volume of the material, which retains the same cross-section after pressing and its free from any residual porosity. Furthermore, ECAP offers the possibility to extend this process even to an industrial level. Aluminum alloys are widely used for different technological and industrial applications, due to their lightness, good mechanical properties and their low cost. On the one hand, in the literature, the mechanical properties of the materials processed by severe plastic deformation techniques have been widely discussed, on the other hand many uncertainties and controversial results remain on the corrosion behavior of these materials. In fact, these processes that lead to grain refinement also cause other metallurgical and microstructural changes: precipitation of the phases, distribution of impurities, surface finishing, etc. As a result, the corrosion behavior of the materials that have undergone these processes can be influenced by these changes. The works present in the literature in this area are also contradictory, even those related to the same alloy, because these changes occur in a complex and different way. In this research the corrosion behavior of two aluminum alloys, AA6012 and AA5083, subjected to the ECAP process was studied. In particular, the influence on the corrosion behavior of aluminum alloys by ECAP and cryogenic and thermal treatments carried out before or after severe plastic deformation was evaluated. The analysis of the corrosion resistance of aluminum alloys was carried out at room temperature by means of electrochemical characterization in aqueous solutions at the same chloride concentration (0.1 M Cl-), but at different pH (pH 2 and pH 6.5). For this purpose, cyclic potentiodynamic polarization, Tafel curves, polarization resistance and impedance spectroscopy electrochemical tests were performed. The experimental results show that the ECAP process deteriorates the corrosion behavior of the alloys in acid environment to a small extent, result that is clear before the aging of the alloy and in short exposure tests. In the neutral environment, on the other hand, neither the ECAP process nor the thermal treatments and the cryogenic treatment have influenced the corrosion resistance of the alloys. The cryogenic treatment, carried out before ECAP, allows a recovery of corrosion resistance of AA6012 alloy only before natural or artificial aging.

Full textSeveral studies from around the world have focused on integrated care models as key solutions to build more effective and efficient healthcare systems that can better meet the needs of patients and the populations served. Often, the coordination is mostly clinical, and consists of the promotion of workgroup activities centered on specific oncological pathologies and in the adoption of common guidelines. However, the process needs to be developed and assessed also with respect to the patient perspective and patient experience. The purposes of this research are to explore the relationship between the integration of cancer services and the patient experience; to describe the representation of “patient engagement” and to examine what kind of application exists in the cancer services; finally to examine if patient experience can be measured. The research’s method consisted of a literature review and analysis, and also it consisted of a case study in the Area Vasta Romagna Cancer Network, via the use of a survey distributed to 310 patients with breast or colo-rectal cancer. From the findings, it appears a positive relationship between integration and patient experience. Also,we identified four main themes of patient experience related to integration: “individual care provider”, “team/system care providers”, “mixed approach”, “continuity and quality of care”. Furthermore, it was possible to describe a consistent representation of patient engagement models and to identify four main fields of application: “screening”, “treatment”, “care”, “research”. Finally, the continuity of care framework results an appropriate measure to analyse the integrated cancer care from patient perspective and to evaluate the patient experience over the entire cancer pathway.

Full textBackground. Delirium is a neuropsychiatric syndrome more frequent in advanced cancer patients. The Memorial Delirium Assessment Scale (MDAS) is a validated tool used for diagnosis, severity and measuring phenomenology of delirium. The aim of this study is to compare the prevalence at admission, incidence during hospitalization and phenomenology of delirium in advanced cancer patients in two different settings: a hospice and an oncology ward. Methods. We conducted a prospective observational study of patients admitted at Hospice Bentivoglio of the Hospice MT Chiantore Seràgnoli Onlus Foundation (Bentivoglio, Bologna, Italy) (FHS) and attended by the Palliative Care Supportive Team at the Oncology Ward of the University Clinic of Navarra (Pamplona, Spain) (CUN). MDAS was administered at initial hospitalization and repeated every week. Frequency analysis, Chi-squared Pearson Test, Fisher test, Anova Test and Wilcoxon test were analyzed. Results. 227 were enrolled (176 in FHS, 51 in CUN). Delirium prevalence was 26% (46/176) FHS, 22% (11/51) CUN (p<0.585). Delirium incidence was diagnosed in 18% (31/176) of patients in FHS, in 8% (4/51) at CUN (p<0.208). At the time of discharge/death, irreversible delirium was present in 37% (65/176) of patients at FHS, in 6% (3/51) at CUN (p<0.001). In a subset of 32 patients, MDAS was compared at the time of diagnosis of delirium and one week later. In 22 patients with reversible delirium, all MDAS items showed a reduction in the level of intensity of delirium, but in 10 patients with irreversible delirium, the level of all items reminded at the same intensity. Conclusion. Delirium prevalence at admission and incidence during hospitalization was similar in both settings, but the evolution was different: in FSH at discharge/death, there was a higher prevalence of irreversible delirium. Signs of delirium can improve in reversible delirium and not worsen in irreversible delirium by early diagnosis and proper treatment.

Developmental Care -an approach using a range of medical and nursing interventions aimed to decrease the stress of preterm neonates in Neonatal Intensive Care Units (NICUs)- have quite important effects on neonates health both in the short and medium-long run. Though, it is still not enough clear how and how frequently these procedures are used. Aims The present survey deals with forms and frequencies of use of Developmental Care in both Italian and European NICUs. Through comparable data, the survey takes into consideration the different aspects of structures and physical environment in which the Developmental Care is used, the concrete medical and nursing practices and the main obstacles in the diffusion of Care activities. Methods. A questionnaire was sent to 364 NICUs in Belgium, Denmark, France, UK, Italy, Netherlands, Spain and Sweden. The main sections of the questionnaire were the following: NICUs environment, neonate’s management, pain management, policies towards parental visiting. Frequency distribution, average value, median and relative confidence intervals, range and percentiles were computed by total population and by country. For Italy, a log-linear model has been applied in order to evaluate the association between the Developmental Care indicators and the NICUs’ main features (geographical location, unit’s level, dimension, affiliation to university). Results. The total response rate is 78% (100% in Denmark, Netherlands and Sweden and 70% in UK and Italy). The environment is controlled through a 24 hours moderate lighting in 59% of NICUs (80% in Netherlands, 44% in Spain). As for neonate’s management, the nesting is largely used in most countries (94% of NICUs). As for pain control, 58% of units use pharmacological analgesia during endotracheal intubation (94% in Denmark, 28% in Italy) and 50% of NICUs use non-pharmacological analgesia during lumbar puncture (94% in Denmark , 27% in Spain). Both Denmark and Sweden have unlimited parental visiting, when in Italy and Spain this is an opportunity offered only in respectively 33% and 27% of the cases. In Italy the most powerful factor influencing the differences in Developmental Care use is the geographical repartition. NICUs in Southern regions show, in terms of pharmacological (OR 0,36; IC 95% 0,13-0,98) and non- pharmacological (OR 0,34; IC 95% 0,11-0,99) analgesia, a lower attention to pain management than in the Northern areas. Southern Italy opens the department to parental visits much less than the rest of the country. In the first case, mothers are free to be by their child less frequently than in the North (South vs North OR = 0.29; IC 95% 0,10-0,87) and fathers less free to practice the kangaroo cares (South vs North OR=0.19; IC 95% 0,06-0,58). Conclusions. Many of the Developmental Care elements are presently consolidated in the medical and nursing practices of the European NUCUs. The areas in which a wider differentiation is shown is the pain management and the opening to parental visits. North European countries show a higher attention to these aspects than Italy and Spain and a similar North-South differentiation is also shown in Italian Developmental Care application.

Full textDiabetes is one of the most common chronic diseases in nearly all countries. Subjects’ socioeconomic status (SES) is associated with diabetes incidence and prevalence, use of healthcare services and prevalence of diabetic complications. Considering the need to analyse if appropriate services are equally provided to different socio-economic groups at local level, and considering the role of the general practitioners (GPs) in the management of chronic disease, the project AEQUITAS was conducted. The aim of the current study was to evaluate social and economic disparity in diabetes care, analysing the association between preventable hospitalization for diabetes and frequency of controls of glicated haemoglobin (A1c) and indicators of inequality. We also evaluated the diabetes care demand estimating diabetes prevalence. This study was based on administrative archives with GPs database as the main source for case detection. A total of 6 494 subjects suffering from diabetes mellitus aged ≥ 20 years were analysed in the study period. Overall prevalence of diabetes ranged from 5.4% in 2003 to 7.8% in 2010, with a significant positive trend of 0.31%. Only 21% of subjects had two or more A1c tests in each year of observation.We found people aged ≤44 years at a significant higher risk of preventable hospitalization and of poor frequency of control of A1C than people aged >44, consistently with other Italian studies. We observed an excess of preventable admissions for people living in socioeconomic disadvantage areas and they was at significant higher risks of poor frequency of A1C controls if they were aged more than 44 years. As shown in other studies, the severity of the disease increased the frequency of controls, maybe through the care of a diabetes centre. However, educational level was significantly associated neither with the rate of preventable admissions nor with the frequency of controls. These results may indicate that patients independently by their SES appropriately use health services, but outpatient healthcare services continue to be not equally available on the regional territory. The study showed that there are considerable opportunities for improving the management of diabetes, especially in the youngest and in the disadvantage areas.

Full textOptimisation of busulfan dosage in patients undergoing bone marrow transplantation is recommended in order to reduce toxic effects associated with high drug exposure. Variation in the area under the concentration /time curve (AUC) results in risk of over or under treatment with excess risk of toxicity or relapse. A rapid, sensitive and specific assay for detection of busulfan in human plasma was developed. The assay is based on a liquid-liquid extraction with ethyl acetate and detection by high performance liquid chromatography with electrospray ionization and tandem mass spectrometry (HPLC-ESI-MS/MS). Deuterated busulfan was used as internal standard. The method was linear over the range 39-2500 ng/mL, with r2 > 0.99 and a run time of only 5 minutes (busulfan retention time of 1.62 minutes). The inter-day and intra-day precision were in the range 1,01- 3,72 % and 0,35-4,32 %, respectively. The recovery was >70%. The limits of detection and quantification were 6 and 10 ng/mL, respectively. The reported assay required only 200 µL of plasma for the analysis. The validated method was successfully applied to analyze plasma samples from children with thalassemia undergoing a conditioning regimen with busulfan and submitted to bone marrow transplantation. This method permits dose correction allowing a better dosing adjustment towards the target level of busulfan. This method is currently used to analyze the plasma concentrations of busulfan after intravenous administration and it is applied in therapeutic drug monitoring.

However, the introduction of the PCM may have a profound impact on the social organisation of work, changing lines of demarcation, challenging well established inter-/intra-professional relationships, and prompting the development of new roles and modes of working. This thesis explores the mutual effects between the new organisational model and the pre-existent social organisation of hospital work. The research design is organised in three phases: an extensive document analysis; an interview study; an in-depth ethnographic case study conducted for over one year in a PCM hospital. The findings are organised in three studies. The first shows that the PCM was interpreted differently by hospital managers and by frontline clinicians, thus giving rise to two divergent narratives of change. The second study focuses on the boundaries to collaboration and care integration in newly created hospital teams within PCM hospitals. The third study looks at the impact of the PCM on the medical-nursing boundary. The thesis contributes to management learning and practice by providing recommendations on how to accompany complex innovations, comprising of both their expected and unexpected consequences. It also enriches academic debates on professional boundaries, relations, and identities in healthcare.
