Full textBackground: Streptococcus pneumoniae or pneumococcus, is responsible for severe invasive infections (IPD) in high risk groups and in the elderly. Moreover the pneumococcus is the most common cause of community acquired pneumonia (CAP) in adults. The aim of this study is the budget impact analysis of pneumococcal vaccination in elderly population in Italy from the perspectives of the National Health System (NHS) and Societal at 10 years of follow-up. Methods: A Markov model was developed based on the cohort of the Italian population to describe pneumococcal disease in elderly (=65 years). Three vaccination strategy scenarios were considered; No vaccination (Scenario 1), vaccination with 20% VCR (Scenario 2) and vaccination with 75% VCR (Scenario 3). The model considers costs of vaccination and other direct and indirect medical costs related to pneumococcal disease (non-bacteraemic pneumococcal pneumonia and pneumococcal meningitis). The NHS and Societal perspective and a simulated follow-up of 10 years were adopted. Results: In the base case, considering the cohort of population vaccinated followed for 10 years, the model estimated a total direct and indirect cost of €92.9, €88.8 and €77.5 million for the Scenario 1, 2 and 3. The ROI was -10% in the scenario without vaccination and 37% in the scenario with vaccination considering the impact of direct and indirect costs. Conclusions: In this study has been demonstrated that a pneumococcal vaccination in elderly populations is a strategy with a positive ROI if also indirect costs have been considered in the analysis. VCR do not affect the ROI but the budget impact.

Full textThe research project was finalized to develop an innovative decision support system based on key performance indicators for the choice among process alternatives. In particular, the project was focused on the definition of indicators able to support decisions in the debottlenecking of a process plant. Two indicators that allow evaluating the need and the potentialities for debottlenecking, identifying the causes of production losses and the utilisation of the equipment, were developed. The practical value and the potentialities of such indicators were evaluated analyzing the section of a production process of a high molecular weight organic substance. As a following step, the issue of deciding among process alternatives for debottlenecking was afforded. A methodology based on sustainability indicators was applied to select the best alternative. The methodology, that allows comparing the alternatives through the analysis of environmental, social and economic impacts, was further developed within the present study. Two area hazard indicators aiming at the assessment of the impacts of fugitive emissions were developed to integrate these aspects in the sustainability analysis of process alternatives. To increase the accuracy in the quantifications of these impacts, a new predictive model, based on available data in design phase, considering the different categories of emission sources, the mechanism of losses, and the maintenance, was developed to estimate the fugitive emissions in a process plant. The comparison between the experimental measurements of fugitive emissions rate in a process plant and the model results confirmed that this new predictive model was needed for an adequate comparison of the process alternatives, because the existing predictive models overestimate the real emission rate. Finally applying the indicators on a real process plant it was confirmed that these indicators are very important to simplify the decisional process, because they provide accurate indications based on a limited number of information.

Full textThe exponential increase of medico-legal claims - originated in the USA in the sixties in such proportions as to talk of medical liability crisis, and developed in Italy since the mid-eighties - has led and continues to lead, together with other negative consequences, to the more frequent use of health care practices of defensive medicine, with very high costs to the NHS due to the overabundance of unnecessary treatments and hospitalizations, and worthless invasive diagnostic procedures, which generate emotional distress in patients. The cause of the increase in litigation is to be found largely in the doctor-patient relationship, especially with reference to the information that precedes the acquisition of the informed consent for clinical treatment. In Italy, the limits which mostly characterize the studies regarding informed consent derive mainly from the fact that they tend to focus solely on the written component of the same. The focus of informed consent, however, must be considered represented by a communication between healthcare and patient concerning a proposed treatment and its possible alternatives, to not subjecting the patient to any treatment, and the risks and benefits of each of these options. In such a context, the issue of communication between the physician and the patient is attracting interest because it is expected that it will lead to improvements in patient outcomes and to fewer malpractice claims. The increased attention to the doctor - patient relationship has highlighted the need to improve and enhance the communication skills of doctors, in a perspective in which the time dedicated to communication can be perceived by the professional as the core of the doctor-patient relationship, with a view to developing strategies to prevent the phenomena of defensive medicine.

Full textArthropod pests represent one of the major threats worldwide as they have an impact on agricultural and forestry production, and on the health of animals, humans and the environment. In particular, plant pests will play a prominent role in shaping the management practices and the technologies to be adopted in food production. With an increasing growth of human population and the consequent increase in food demand, a major challenge of the future will be to ensure safe and secure food. Decreasing crop losses due to pests and reducing both the direct and indirect costs linked to pest management is a fundamental aspect for ensuring global food security. The design and the implementation of rational and scientifically sound decisions in pest management can be supported by the use of models. In particular, physiologically-based models have proven to be useful tools able to provide quantitative information supporting decision-makers in the management of pests. In this work, I present a general overview on the development and the application of models for pest management with particular emphasis on physiologically-based models applied at different contexts and at different spatial (from local to area-wide management) and temporal (from short to long term management) scales. In Chapter One I present an overview on the impacts of arthropod pests and the requirements to be satisfied when developing a model for pest management considering the spatio-temporal scale and the context of application. I discuss the different modelling approaches applied to pest ecology and the importance to realistically represent the biological system under investigation. In Chapter Two I review the current state of the art on the development and the use of models supporting Integrated Pest Management. I discuss the modelling approaches, the fields of application, the benefits and the drivers influencing their adoption as tools for decision-making in crop protection. In Chapter Three I present a physiologically-based model predicting the multi-annual phenology of the Japanese beetle (Popillia japonica). In particular, is introduced a realistic description of the overwintering process of the species and discussed the consequences on the predicted timing of emergence of the different life-stages. In Chapter Four I present a physiologically-based model simulating the population dynamics of the fall armyworm (Spodoptera frugiperda). I discuss the implications linked to the introduction of a density-dependent control factor for the realistic simulation of intra-specific and inter-specific competition and the influence on the predicted dynamics of the species. In Chapter Five I present a physiologically-based model simulating the potential distribution, abundance and activity of the Mediterranean fruitfly (Ceratitis capitata) in Europe under current and future climatic scenarios. In particular, I discuss the importance of capturing the non-linear relation between environmental drivers and the species’ physiological responses as well as the consequences at the individual and at the population level. In Chapter Six I briefly summarise the main results of the models presented and discuss their application for the assessment and the management of the risks linked to pests in different spatio-temporal scales and contexts of application. I also discuss how models for pest management might support decision-making in relation to the future global challenges we are called to face in the near future.

Full textThe research activity described in this thesis has been conducted at Nerviano Medical Sciences (NMS), a research-based company dedicated to the discovery and development of innovative target drugs for the treatment of cancer. A target therapy is a drug able to block cancer growth by interfering with specific molecules (target), involved in cancer development. Typically, a target is a protein specifically or at least preferentially expressed in cancer but not in normal cells. Kinases are a family of about 500 enzymes involved in several key cellular functions, which have been often found deregulated in cancer. They are characterized by a conserved ATP-binding pocket, which can be exploited for the binding of small molecules, blocking the catalytic activity of the enzyme, thus representing ideal targets for drug development. Kinases in tumors are activated by gene mutations or by overexpression, as a consequence of copy number alteration or more complex genomic rearrangements, like gene fusions, which are rare events resulting in the overexpression of the driver kinase. In this thesis, in order to identify potential new targets for the development of novel drugs, the implementation of a tool, called KAOS (Kinase Automatic Outliers Search) is described. KAOS was specifically developed for the identification of kinases showing an outlier gene expression profile, when compared to other samples from the same tumor subtype. The tool requires in input gene expression data and uses the anomalous overexpression of a kinase as readout of the presence of a gene fusion event. In addition, the implementation of a comprehensive whole kinome expression screening, called KING-REX (KINase Gene RNA EXpression), is also described, enabling the analysis of the kinase only portion of the transcriptome, with reduced time and costs. The platform allows investigating kinase expression and identifying potential gene fusion events using a customized Illumina RNAseq targeted NGS approach (Illumina TruSeq Targeted RNA expression kit, TREx), together with an ad hoc analysis pipeline. KING-REX has been conceived for the profiling of the human “kinome” on small/medium scale Illumina sequencers, requiring reduced computational resources in terms of storage space and data processing, thus representing a rapid and cost effective kinome investigation tool in the field of kinase target identification, for applications in cancer biology. In parallel, with the development of new drugs targeting specific kinase rearrangements, it is important the development of screening and validation methods, allowing the selection of the patient population harboring a specific driver gene, for treatment prescription. With the advent of Next Generation Sequencing (NGS), the detection of specific gene fusions can benefit from the high sensitivity of target-RNAseq approaches and has been proposed as diagnostic platforms. One of these methods is the Anchored Multiplex PCR (AMP) (ArcherDx, Inc.), an NGS-based system allowing the detection of rearrangements for a selected number of kinases, without requiring the knowledge of the rearrangement partner. In this thesis the use of AMP technology is described for the analysis of colorectal cancer (CRC) clinical specimens. The use of this test allowed the identification of patients harboring novel rearrangements of the kinases NTRK and ALK in CRC patients, responsive to the treatment with entrectinib, a drug initially developed at NMS specifically targeting these kinases.

Full textThe European Common Agricultural Policy is one of the major drivers of change in rural landscapes. This study is focused on the Standards of Good Agricultural and Environmental Condition (GAEC) as part of the Cross Compliance requirements in the framework of Pillar I (Direct Payment) and of the objectives of Pillar II (Rural Development). In particular, it focuses on the implementation of the standard GAEC 5.2 “Buffer Strips” along watercourses, which was introduced in Italy in January 2011, and was adopted by the Marche Region in early 2012. This study has two main aims: the first is to characterize the rural areas, where the CAP regulations have an important role; the second is to develop appropriate strategies to assess and guide the impact of the implementation of new measures like GAEC 5.2, on the rural landscape, using innovative tools for data analysis and integration. Thus, the first aspect of this study was to develop a framework of analysis to improve the characterization of rural areas in a GIS environment. According to the National Strategic Plan for Rural Development, the Marche Region adopted a definition of rural areas based on population density, along the lines of the recommendations of the OECD. This subdivided the region in five different gradients of ‘rurality’ that are strictly delimited by administrative boundaries, and ignore the spatial structural aspects of the rural patterns. To overcome this limitation, we have improved the characterization of the rural settings at a regional scale by building in a new layer that is based on both landscape metrics and socio-economic descriptors. This has been applied to three test areas in the Marche Region. The methodology makes use of statistical (i.e., PCA and Varimax) and clustering (i.e., K-Means) analyses. Through several analyses of intersections dealt with in a GIS environment, and several steps of semantic interpretation, we obtained the spatial integration of the descriptors. These “Integrated Territorial Descriptors” describe a specific area through definition of the different gradients of rurality. These different Integrated Territorial Descriptors defined for each test area have been aggregated into the Integrated Homogeneous Macro-Descriptors, to allow comparisons between the three test areas. In the second part of the study, we performed several analyses based on a multi-scale approach in the GIS environment, at two different spatial scales: regional and local. At the regional level, EU CLUE Scanner 100 was used as forecasting model. This model allows for different implementation scenarios based on the Corine Land Cover across the whole of Europe. These results were used to identify three test areas of about 1500 km2 each. At the local scale, a parametric GIS-Script model was tested, which was developed at KU Leuven as an instrument to address the choice of the optimal location and typology of buffer strips at the parcel and local scales. In this step, a first model was tested with a dataset for the Marche Region. The results shown what the critical points are to improve the use of the parameters evaluated by the model. Overall, this study represents an important step towards the development of novel strategies using innovative analytical tools for the appropriate assessment of land use and planning in rural areas.

Full textThe main challenge of Smart Swap Building project, carried out in collaboration with Aster by Construction Platform of Emilia-Romagna High Technology Network, is to identify possible solutions to the crisis that is severely affecting the construction industry, through new approaches to urban regeneration based on housing refurbishment and soil consumption reduction. The recent downturn in the housing market has generated a significant amount of unsold assets. The absorption rate of 80% in 2007 fell to 35% in 2012. At the end of 2014, the total number of unsold houses was over 540 thousand units in Italy, 26% of them were new constructions. Investments in refurbishment now exceed those for new buildings. Actually the 70% of residential housing stock was built before 1970 and it needs to be adapted to new levels of energy efficiency, seismic safety, inclusiveness and comfort. The swap term, borrowed from finance field, indicates the flow exchange between two counterparts: Smart Swap Building wants to use the unsold housing stock as temporary accommodation for the inhabitants of areas in which a renewal program take place. The overall objective of this doctoral research is to evaluate the feasibility of BIM tools adoption in the Smart Swap Building workflow. These electronic tools not only allow a transparent design phase but covers the whole building process, relating technological and structural elements with construction scheduling (4D), cost estimation (5D), certification (6D) and management of the life cycle (7D + CAFM). Trying to meet European challenges of soil consumption and emission reduction, energy efficiency and living comfort, and involving the growing unsold housing stock, the Smart Swap Building project, aimed at the refurbishment of existing housing stock, has the strong need to define a set of advanced information tools to support the complex decision-making process, integrating different skills in different stages of the building life cycle. The representative tool is an integrated part of the design process and because product innovations are strictly connected to the building processes, from design to construction, to management, it becomes itself an element of project quality, allowing more sustainable and aware choices. In conclusion, the main output of the research will be guidelines aimed at digital informative tools adoption in the Smart Swap Building workflow management, and their validation through test-beds and online dissemination.

Full textReducing emissions responsible for the climate change is recognized as a strategic goal at European and global level. A higher deployment of renewable energy sources (RES) is considered as essential for a transition towards a more sustainable energy system. This low-carbon energy transition requires both the development and use of innovative technologies, particularly at end-use sectors (buildings, industry and transport), and new management approaches as well as new market design and business models. This study explores the sustainability driven opportunities offered by the energy based Industrial and Urban-Industrial Symbiosis approach. The Industrial Energy Symbiosis (IES) considers the sharing of energy-related resources, facilities and infrastructures as an effective model to promote energy conservation measures and the renewable energy sources uptake at the industrial level. In addition, an improved low-carbon strategy can be achieved creating energy synergies between industrial districts and the adjacent urban areas. Establishing Urban-Industrial Energy Symbiosis (UIES) allows optimizing the energy production and consumption and exploiting the local knowhow and human resources. These new integrated system needs a change of perspective, considering a multi-stakeholder action: energy service companies, local communities, industry sector, consumers, policy makers, researchers must get actively involved in participatory planning processes to guide the transformation of the energy system and the research and innovation process, and respond adequately to the needs of the territory. Thus, an in-depth analysis of the manifold technical, economic, organizational, regulatory, environmental and social drivers and barriers of the energy symbiosis approach are presented, with the aim of modelling the optimal energy synergies configurations among firms including RES. A methodology is developed to support energy managers, single firms, groups of firms within industrial parks, and decision-makers to evaluate energy synergies and projects involving RES, taking into account the economic, environmental and social impacts of the projects. Lastly, a sustainability-driven framework is developed, with the aim of modeling Urban–Industrial Energy Symbiosis networks integrating RES from a multi-stakeholder point of view and supporting decision-making on the economic, environmental, and social sustainability of the energy synergies. The application of the developed decision-making tools to specific case studies emphasizes how collective strategies (IES or UIES) allow better management of the energy supplied by renewable sources.
