Abstract:
John Lionel Stretton was born in Kidderminster in 1860. Apprenticed to his father, Samuel, at the age of 17, he trained at Bart's hospital, obtaining his MRCS in 1881, and was immediately appointed Junior Assistant Anaesthetist before he had to resign and return to Kidderminster, to help his father who was seriously ill. He worked in Kidderminster as a surgeon for 56 years, promoting healthcare for the working people and falling out with the industrialists whom he badgered persistently for funding. He is best remembered for his medical innovations, including the introduction of tincture of iodine for skin sterilisation, for which he should be recognised alongside Joseph Lister, the 'father of antispetic surgery'. This article is based on anecdotes from Lionel's own story from his manuscript that was eventually published after he died.

Abstract:
Catastrophes present leaders with complex and significant challenges that they have not previously experienced nor have had time to deeply analyse. Leaders must collaborate and demonstrate agility. To develop such leadership skills, it is useful to reflect on the experiences of people who have faced catastrophe before. This paper examines the leadership of Major General Alan Stretton AO, CBE in the aftermath of Cyclone Tracy in 1974. Alan Stretton’s personal accounts and archival interviews with other leaders were reviewed as source material. These showed that he demonstrated decisiveness and courage and ‘over-responded’ if necessary. He worked collaboratively with community leaders and acted in a confident, empathetic and reassuring manner. He led with agility and with a focus on an overarching plan. He prioritised communication with the community and negotiated political challenges. Lessons from this experience can help to guide leaders who may be called on to lead during times of future disaster events.

Abstract:
Healthcare education and practice has significantly been impacted by COVID-19. This includes the challenge on pedagogical approaches that highlight the potential of technology to facilitate innovative new approaches in response to social distancing, lockdowns, remote learning and improving the patient experience and positive outcomes. Many of these innovative approaches are not fundamentally new but are now seeing relevance beyond early adopters to mainstream implementation. This presentation draws upon collaborations with educational researchers and technologists that have explored the integration of technology into healthcare education and practice. COVID-19 Adversity to Opportunity Many healthcare programmes required reenvisaging teaching and learning approaches in response to COVID-19 restrictions. This had a particular impact on the development of interpersonal and practical knowledge and skills essential for healthcare graduates. The limited access to on-campus learning provided an opportunity for both institutional and individual evaluation of pedagogical practices. The affordances of traditional, didactic, and “hands-on” skills were compared with those that could be facilitated using online asynchronous/ synchronous strategies. A particular concern was the development of the interpersonal and practical skills required in safe and effective healthcare practice. Alongside easing of restrictions, these skills were adapted using online demonstrations within the limits of socially distanced “bubbles”, telehealth and limited clinical placements. Reconsideration of summative assessments was also required- with the introduction online synchronous and asynchronous verbal assessments, and asynchronous submissions of practical skills online (Cochrane et al., 2021; Narayan et al., 2021). In the prospect of COVID-19 restrictions continuing to lift, it is envisioned that most of the reenvisaged pedagogical approaches to healthcare education will persist, without compromising student critical thinking or practical skills. Interprofessional Collaboration This presentation will highlight the importance of interprofessional collaboration in healthcare curriculum design using a Design-Based-Research methodology (Chen et al., 2020; Kartoğlu et al., 2020) to facilitate authentic learning and develop self-determined learning capabilities for healthcare professionals. DBR- Design Principles in response to COVID Transferable design principles will be introduced for enhancing healthcare education that will improve practice in a COVID19 world, particularly drawing from eight healthcare projects including: STUDIO602 – enhancing clinical practice with mobile technologies (Cochrane & Sinfield, 2021), developing a virtual reality handover experience for healthcare students (Cochrane et al., 2018), using immersive reality to develop critical thinking in clinical health education (Stretton et al., 2018), enhancing first responder clinical simulation education using immersive reality and biometrics (Cochrane et al., 2020), designing authentic learning for graduate entry nursing students (Macdiarmid et al., 2021), designing public and environmental health education (Kersey et al., 2018), Biomedical engineering (Lam et al., 2021), and physiology education (Fabris et al., 2019). Abstract:
Body and trace fossils of Ediacaran affinity are described from strata of the late Neoproterozoic Longmyndian Supergroup exposed near Church Stretton, Shropshire, UK. The almost spherical soft-bodied Ediacaran fossil Beltanelliformis brunsae Menner occurs rarely in the Burway Formation, but much smaller, simpler, discoidal structures are common in both the Burway and Synalds formations and are referred to Beltanelliformis minutae sp. nov. Similar discoidal structures, but with a distinct central depression, are included in Intrites punctatus Fedonkin and are common at several horizons. Two blocks with numerous examples of Medusinites aff. asteroides (Sprigg) Glaessner & Wade were recovered from the Burway Formation. The purported Ediacaran body fossil ‘Arumberia’ Glaessner & Walter is common at several horizons but its biogenicity is not accepted herein. ‘Arumberia’ is thus treated along with evidence for microbially bound sediment surfaces or matgrounds that have been suggested by several authors to be necessary for some types of Ediacaran preservation. The assemblage of simple trace and body fossils along with matgrounds is typical of latest Neoproterozoic time, though some elements range into the Phanerozoic.

The development of an operability model analysis tool to support design requirement definition for advanced ASW combat systems has evolved over the last four years. Previous NUWC ASW Combat System Operability Modeling efforts using SLAM II have produced comparisons between various ASW combat systems to examine the relative benefits of equipment features and operating concepts. Development of these operability models enabled the representation of contact data handling processes, ASW combat system operators, and the man-to-man, man-to-machine, and machine-to-machine interfaces that occur during an ASW mission. Early operability model studies (Stretton, Swiontek, Morris, Conway, and Wachter, 1991) investigated data handling bottlenecks and contact data throughput for ASW systems by focusing on the sensor operators, supervisory personnel, and command decision makers. Earlier models did not incorporate variations in team member productivity as might be found under high-workload conditions. Also, ASW team member skill levels were not varied as might be found due to latency from training-to-shipboard activities or to individual differences in operators and their training. As a follow-on-effort, this study examined how variations in operator proficiency on critical tasks affect overall ASW combat system performance. Results of this effort appear to have wide-reaching operational impacts in the area of team performance and operator variability.

It is easy to describe the mess the world is in, and to preach large changes of heart. Short, saleable books which do that may be useful if they persuade people in good directions, but only if people know how to move in those directions. (Hugh Stretton, 1976, p. 2)In June 1992, the UN Conference on Environment and Development will be held in Brazil. A follow up to the World Commission on Environment and Development, it will mark the twentieth anniversary of the first UN conference on the environment, held in Stockholm in 1972. Promoting environmental education is one of 23 objectives for the conference.Conference delegates in Brazil are likely to hear that the global ecological crisis has worsened, that sustainable development is not being realised, and that there is a continuing need for education. Yet more initiatives in environmental education are likely to be advocated and some delegates will go away feeling happier.What they and others may fail to realise is that much environmental education is part of the problem rather than the solution. Current practice fails to reveal the true causes of environmental problems and to educate pupils in ways which enable them to realise sustainable development. It is based on inadequate theory and practice yet receives increasing support from powerful elites who must manage the global ecological crisis in their own interests.

AbstractBasaltic, andesitic and rhyodacitic/rhyolitic volcanism was widespread during Ordovician time in the Welsh Basin. New chemical data are presented for Llanvirn to Caradoc lavas and tuffs from the Welsh Borderland which, during Ordovician time, formed the southeastern margin of the Welsh Basin. In view of the observed chemical alteration, immobile elements are used in the interpretation of the original geochemical character. The data indicate that the Llanvirn Stapeley volcanic group of the Shelve inlier was a bimodal basalt/basaltic andesite – rhyodacite/rhyolite association. The basalts have trace element contents of tholeiitic associations with a subduction-related character. The Caradoc Whittery and Hagley volcanic groups of the Shelve inlier comprise lavas and tuffs of calc-alkaline andesite. Blocks sampled from the Breidden Hills show that these were also derived from a calc-alkaline volcano. Associated Caradoc pumice- and ash-flow deposits from the Breidden Hills are probably of altered calc-alkaline rhyodacite/rhyolite composition. The Sibdon Carwood basalt flow, the only known example of Ordovician volcanism east of the Pontesford–Linley and Church Stretton lineaments, has transitional tholeiitic to alkaline character, with trace element contents influenced by subduction-related processes. The overall tholeiitic to calc-alkaline nature of the magmatism is consistent with the view that, during Llanvirn to Caradoc time, the Welsh Basin was an ensialic marginal basin.

ABSTRACT Two broad-host-range vectors previously constructed for use in soil bacteria (A. G. Matthysse, S. Stretton, C. Dandie, N. C. McClure, and A. E. Goodman, FEMS Microbiol. Lett. 145:87–94, 1996) were assessed by epifluorescence microscopy for use in tagging three marine bacterial species. Expression of gfp could be visualized in Vibrio sp. strain S141 cells at uniform levels of intensity from either the lac or thenpt-2 promoter, whereas expression of gfp could be visualized in Psychrobacter sp. strain SW5H cells at various levels of intensity only from the npt-2 promoter. Green fluorescent protein (GFP) fluorescence was not detected in the third species, Pseudoalteromonas sp. strain S91, when thegfp gene was expressed from either promoter. A new mini-Tn10-kan-gfp transposon was constructed to investigate further the possibilities of fluorescence tagging of marine bacteria. Insertion of mini-Tn10-kan-gfp generated random stable mutants at high frequencies with all three marine species. With this transposon, strongly and weakly expressed S91 promoters were isolated. Visualization of GFP by epifluorescence microscopy was markedly reduced when S91 (mini-Tn10-kan-gfp) cells were grown in rich medium compared to that when cells were grown in minimal medium. Mini-Tn10-kan-gfp was used to create an S91 chitinase-negative, GFP-positive mutant. Expression of the chi-gfp fusion was induced in cells exposed toN′-acetylglucosamine or attached to chitin particles. By laser scanning confocal microscopy, biofilms consisting of microcolonies of chi-negative, GFP+ S91 cells were found to be localized several microns from a natural chitin substratum. Tagging bacterial strains with GFP enables visualization of, as well as monitoring of gene expression in, living single cells in situ and in real time.

Il rapporto tra le dimensioni fondamentali del vivere sociale e le caratteristiche dei ‘nuovi' turisti sono molto strette, a dimostrazione del ruolo rilevante assunto dal fenomeno turistico; la dimensione globale, lo spazio estetico, la ridefinizione delle coordinate spazio-temporali, il desiderio di recuperare relazioni interpersonali significative determinano in modo decisivo le scelte turistiche. L'autore si sofferma sulla crescente attenzione per la natura e per la ruralitŕ, che conduce alla diffusione di nuove forme di fruizione del territorio, come le greenways, e di modelli innovativi di ricettivitŕ come gli agriturismi. Lo stretto legame che si instaura tra turismo e ambito rurale si esprime in particolare nella riscoperta delle culture e delle tradizioni dei luoghi d'origine dei prodotti enogastronomici. Il binomio turismo-agricoltura č rafforzato dal fatto che la ristorazione e le ricette locali sono parte integrante di ogni tipologia di prodotto turistico. Anche alcune aree montane in difficoltŕ possono trovare occasioni di rinascita sociale ed economica sviluppando il turismo enogastronomico. Nell’ambito dei monitoraggi ambientali propedeutici alla realizzazione dell’Attraversamento stabile dello Stretto di Messina sono state indagate le faune dei Macrolepidoptera presenti sul versante siciliano e su quello calabrese. Il monitoraggio è stato effettuato in 23 siti siciliani e 15 calabresi, cercando di coprire al meglio la diversità ambientale presente sulle due sponde dello stretto. Le ricerche sono state condotte dal 2010 al 2011, escludendo i mesi invernali. Sono stati rinvenuti 10.399 esemplari appartenenti a 355 specie, un numero relativamente alto se si considera l’elevata antropizzazione delle aree indagate e la sospensione dei campionamenti nei mesi invernali, anche se di queste solo Euplagia quadripunctaria (Poda, 1761) ha un interesse conservazionistico ai sensi della Direttiva “Habitat”. Fra le 168 specie rinvenute solo su una sponda dello stretto (97 sulla sponda calabrese e 71 su quella siciliana), 26 non sono presenti su entrambe le sponde per motivi biogeografici. Di queste 21 sono presenti in quasi tutta Italia, ma ad oggi non sono mai state rinvenute in Sicilia e 5 sono specie endemiche siciliane di relativamente recente differenziamento o sono specie più diffuse nel bacino sudoccidentale del Mediterraneo. Inoltre, Zanclognatha lunalis (Scopoli, 1763), viene segnalata per la prima volta sul territorio siciliano. Questi dati sottolineano ancora una volta la già nota importanza dello Stretto di Messina come barriera biogeografica che la eventuale costruzione di un suo attraversamento stabile potrebbe quanto meno indebolire.

Nella presente ricerca si è andati ad investigare in modo contrastivo quanto il comportamento e le caratteristiche degli insetti (in generale considerati universali) differiscano nelle tre lingue (italiano, croato e ceco) nelle espressioni idiomatiche. Si è partiti dalle espressioni idiomatiche italiane contenenti una componente entomologica (17 insetti), le quali cercano i loro equivalenti nel croato e nel ceco. Significa che l’analisi viene fatta soltanto unidirezionalmente dall’italiano al croato e ceco. Da un lato, la lingua croata e quella ceca, essendo della stessa origine (slava), dovrebbero accomunare equivalenti fraseologici totali (attraverso la variante protoslava), oppure attraverso i prestiti di terminologie religiose, ad esempio bibliche. D'altra parte, per via del diretto contatto italiano-croato, il croato dovrebbe avere più similarità fraseologica con l'italiano, a differenza del ceco. In base ai 4 passi della linguistica contrastiva e selezionando gli equivalenti totali, parziali e quelli diversi ci si è concentrati a spiegare (anche attraverso un’analisi concettuale) il perché delle diversità e quali siano le scelte linguistiche nei diversi equivalenti. Delle 72 espressioni in italiano, soltanto 9 (il 12%) sono equivalenti totali in tutte e tre le lingue. Differenziando le lingue, si nota un’affiliazione più stretta tra le lingue slave (24 equivalenze totali), che non tra il croato e l’italiano (15) e tra il ceco e l’italiano (11). L’analisi concettuale conferma questa affiliazione slava. Le lingue slave (croato e ceco), anche se storicamente e geograficamente più lontane, hanno conservato le loro specificità idiomatiche. D’altra parte l’italiano, essendo nell’ultimo millennio in stretto rapporto con il croato, ha sicuramente lasciato più repertorio idiomatico nel croato che nel ceco.

Оба термина широко представлены в современных словарях и руководствах, а их трактовка близка к синонимической. Вероятно, по этой причине определения данных терминов практически всегда рассматриваются порознь и не сопоставляются. Следствием такого положения оказывается не вполне адекватное их понимание и теоретическое обоснование. Статья посвящена сравнению стретты и канона — как близких, но разных явлений и как отражающих их терминов. Общая для обоих явлений каноническая природа проявляется в них существенно по-разному. Показать, в чем суть их родства и расхождений между ними и как это должно отражаться в комплексе дефиниций, — задача, поставленная в статье. В основе рассуждений лежат два тезиса, касающиеся принципиального различия между соответствующими каноническими построениями. Один тезис касается природы пропост в стретте и в каноне — функциональных и композиционно-синтаксических различий между темой фуги в стретте и одноголосным началом (вступительным отделом) пропосты в каноне. Другой тезис касается интонационно-содержательных различий между завершениями собственно канонического развертывания стретты и строгой (регламентированной) части канона. Общий анализ всех показанных различий позволяет внести уточнения в соответствующие определения. Both terms are widely represented in modern dictionaries and manuals, and their interpretation is close to synonymous. This feature is probably the reason that the definitions of these terms are almost always considered separately and not compared, which results in not quite adequate understanding and theoretical explanation. As the title suggests, the article is devoted to comparing stretta and canon — as close though different phenomena (as well as the terms reflecting them). The authors seek to show, what is the essence of their relatedness and differences and how this should be reflected in the complex of definitions. The canonical nature, which is common to both phenomena, demonstrates itself in stretta and in canon in a different way. The reasoning is based on two theses concerning the fundamental difference between the corresponding canonical constructions. One of them refers to the different nature of the propostas in stretta and canon (we are talking about the difference between the theme in the fugue and the monophonic part of proposta in a canon). The other thesis is connected with the differences in the completion of the canonical section of the stretta and the strict part of the canon. A general analysis of the differences allows us to refine the respective definitions.

Три типа имитации — простая, стреттная, каноническая — постоянно фигурируют в современной музыкально-теоретической литературе. Трактовки сути простой и канонической имитации в разных источниках совпадают, хотя формулировки в деталях разнятся. При этом значительны расхождения в понимании стретты. По традиции, заложенной учениями XVIII века, в теоретическом музыкознании стретту рассматривают в контексте формы фуги. Одновременно ряд исследователей считает возможным использовать понятие стретты по отношению к имитационному многоголосию Ренессанса. При этом термин употребляется в разных значениях. Материал исследования — начальные имитационные секции четырех-шестиголосных мотетов Палестрины без c. f. Тематическая организация рассматривается с учетом тексто-музыкальной формы мотета, в соответствии с которой функцию темы выполняет тексто-музыкальная строка, построенная на относительно стабильном соединении текстовой строки и развернутого мелодического soggetto. Ее неоднократные повторения позволяют обнаружить сходства и различия канонов и стретт в строгостильном многоголосии. The article is devoted to the problems of studying the theory and practice of imitation technique in polyphonic music of strict writing. Three types of imitation — simple, stretto and canonical — appear as relevant in modern musical theoretical literature. An analysis of the existing concepts showed that the interpretations of the essence of simple and canonical imitation in different sources coincide, although the formulations in details, as a rule, differ. Against this background, significant differences in the understanding of stretto (narrow, tight) imitation are especially noticeable. Many authors, foreign and domestic, starting from the teachings of the 18th century, consider the stretto in the context of the fugue form. At the same time, a number of researchers of the 20th century (domestic and foreign) have formed a different position. They believe that it is possible to expand the musical and historical boundaries of the use of the concept of stretto, its use in relation to the imitative polyphony of the Renaissance. The authors talk about the stretto in at least three different cases: the effect of a compressed temporary introduction of imitation voices (S. Skrebkov, T. Dubravskaya), narrow introduction of voices with their subsequent non-imitation promotion (N. Simakova), the tight entry of the rispost before the end of the theme in the propost, which does not fit into the canon (K. Eppessen, S. Skrebkov). The analysis of the musical material showed that the broadly understood stretto (the conciseness of the timing of the introduction of voices) is very typical of the polyphony of strict writing and manifests itself in many and different methods. The musical material of the study was the one-theme initial imitation sections of the four-six-part Palestrina motets, the compositional foundation of which lacks cantus firmus. The thematic organization was considered taking into account the genre of the motet, which belongs to the class of text-musical forms. In accordance with the nature of the genre, the function of the theme in the imitation section is performed by a text-musical line built on a relatively stable connection of a text line and an expanded melodic soggetto.

BackgroundEndoscopic antireflux radiofrequency treatment (Stretta) offers a therapeutic alternative for patients suffering from refractory gastro-oesophageal reflux disease (GORD). Current evidence suggests that the treatment may improve symptoms of GORD and decrease requirement for proton pump inhibitor (PPI) therapy.MethodsProspective assessment of patients undergoing Stretta, between October 2014 and February 2016, in a UK tertiary referral centre was carried. All patients were assessed for suitability using endoscopy, contrast studies, and pH and manometry studies. The Gastro-oesophageal Reflux Disease-Health-Related Quality of Life (GERD-HRQL) was used to evaluate symptoms along with PPI dependency, pre-Stretta and post-Stretta treatment. Patients were followed up by outpatient clinic appointment and telephone consultation.ResultsFifty consecutive patients were followed up for a median of 771 days (range 499–1162) following treatment with Stretta. The average GERD-HRQL score improved from 46.2/75 (±14.2) preprocedure to 15.2/75 (±17.3) postprocedure. Dissatisfaction with GORD as measured in the GERD-HRQL decreased from 100% to 10% with three patients showing no improvement (non-responders) at follow up and two late failures at the time of this review. There were no complications and all cases were carried out as day cases.ConclusionThere are currently few effective therapeutic alternatives to antireflux surgery for refractory GORD. This series corroborates the value and safety of Stretta as a viable option for selected patients who are unwilling or unable to undergo an operation. Stretta improves quality of life and decreases PPI dependency in selected patients with GORD.

Abstract:
Gastro-oesophageal reflux disease (GORD) is common with a prevalence of 10-20% in the Western World. During the last two decades, amongst introduced several endo-luminal techniques, evidence has emerged about the efficacy and safety of two treatment modalities for GORD; 'Stretta' procedure and trans-oral incisionless fundoplication (TIF)' or EsophyX. Author in this study aim to conduct a systematic review of published 'level 1' evidence, to evaluate all evidence on the efficacy of Stretta procedure and TIF (EsophyX) for the management of GORD. A robust literature that included RCT trials only, searched on MEDLINE, EMBASE, and Cochrane library was undertaken from January 2007 until January 2017. The outcomes were normalisation of oesophageal pH values, augmentation of lower oesophageal sphincter pressure (LESP), health-related quality of life (HRQOL) score, and ability to stop or reduce PPI after procedure. For quality assurance purposes, two researchers were involved in the data collection process and its analysis. Author collected data from 9 RCT trials, 3 for Stretta and 6 for EsophyX. From the Stretta trials 101 patients were recruited, 92 patients were analyzed. Of the EsophyX trials, 3 of six papers were from single trial. A total of 296 patients were included and 203 were analysed. The pooled results show both stretta and esophyX significantly improved GORD symptoms. In a meta-analysis of trials, author found that both Stretta and EsophyX significantly improves GERD-HRQL score and enables patients to reduce or stop PPI intake compared with sham/PPI therapy alone. The overall quality of evidence is superior for EsophyX than it is for Stretta.
