Full textA proliferated number of electronic healthcare databases has increased the role of observational studies as a tool to provide scientific evidence. Challenges in conducting observational studies based on Electronic Databases (ED) include concern about errors that can compromise the validity of the results. This thesis analyses methods and approaches to manage analytical challenges with regard to the study design and the statistical analysis of observational studies based on ED. Some of these techniques are performed in a study aimed to estimate the risk of a rare event, acute kidney injury (AKI) after contrast media (CM) administration. For that purpose, a cohort and a nested case-control study were performed using the discharged hospital database of Marche region. The cohort included 29925 subjects with a CM administration during 2008-2011. AKI occurred in 324 subjects; for 129 of them the time period after the CM administration was <7 days, considered therefore associated to AKI. The overall risk of AKI after CM administration was 0.43%. Subjects over 65 years old, suffering from a renal disease were at higher risk. In the nested case-control study all 324 cases were matched to 1258 controls. CM exposure and renal disease were significantly associated with a higher risk of AKI. A reduction of the exposure effect on AKI risk was found in those aged >65 years and with more than one CM administration, while an increased risk was found in subjects suffering from diseases different from renal and circulatory diseases. The study allowed us to estimate the risk of a rare event and the risk factors. Furthermore, it represents a concrete application of the current epidemiological methodology concerning problems related to the use of a potential data source but in the meantime should be cautiously applied.

Full textRIASSUNTO Obiettivo. Studio e analisi di metodi dell’epidemiologia clinica descrittiva e valutativa applicata al tema della patologia cronica congenita ed acquisita, in soggetti in età pediatrica, in particolare all’assessment psicodiagnostico nella valutazione del danno attuale, potenziale ed evolutivo e del potenziale di sviluppo del bambino con patologia cronica congenita e acquisita. Materiali e metodi. Sono stati selezionati 3 gruppi di pazienti affetti da patologia cronica. Un gruppo di 19 bambini affetti da Sindrome da Shaken Baby, un gruppo affetto da sindrome di DiGeorge (20 bambini) ed un gruppo affetto da paralisi cerebrale infantile in forma emiplegica che hanno fatto parte di un trial multicentrico prospettico nazionale (105 soggetti). Ciascuno dei bambini è stato sottoposto ad un assessment completo comprendente l’analisi dello sviluppo cognitivo, lo studio del profilo comportamentale. Inoltre è stato somministrato un test di analisi dello stress genitoriale. È stata condotta un’analisi di tipo descrittivo univariata e multivariata. Dopo una prima analisi della casistica, si è proceduto a confrontare i tre gruppi di bambini (con PCI, con Sindrome di DiGeorge e con SBS) analizzando separatamente la sfera del profilo cognitivo, comportamentale e dello stress familiare. Infine si sono descritti e confrontati i tre gruppi studiando l’interazione delle tre sfere. Risultati. Dai risultati è emerso che i bambini con patologia cronica acquisita da trauma inferto come lo Shaken Baby Syndrome, congenita nel caso dei bambini affetti dalla Sindrome di DiGeorge, o con genesi mista come la Paralisi Cerebrale Infantile, abbiano performance intellettive e di sviluppo minori rispetto alla norma, dati confermati anche in letteratura. Lo studio si è inoltre focalizzato sui risultati ottenuti dai genitori al Parenting Stress Index (PSI); I risultati ottenuti evidenziano i livelli di stress, che se elevati possono dipendere da vari fattori, tra cui anche la difficoltà ad accettare lo stato di disabilità del figlio, che può spingere i genitori a negarlo o a nasconderlo. Le caratteristiche del bambino attraverso il test Child-Behaviour Check List CBCL sono risultate essere un importante fattore nello sviluppo dello stress parentale come elemento destabilizzante nella famiglia. Conclusioni. Dal presente lavoro di tesi è emerso che le patologie ad alta complessità assistenziale in età evolutiva rappresentano un’area clinica variegata, caratterizzata dalla presenza di problemi di salute e/o deficit funzionali multipli, che richiedono un’assistenza multi-specialistica e multi-disciplinare, medica, psicologica e sociale.

Full textWithin the current cultural and social context in which the economic and financial crisis of public administration is firmly rooted in the effects of climate change, international environmental policies and the emergence of new technologies, an innovative methodology, that analyzes energy planning (regional, district, and urban) at different scales, has been investigated. Regional (Regional Energy and Environmental Plans - PEAR), district (Pact of Islands) and urban/metropolitan (Sustainable Energy and Climate Action Plans - SECAP) energy plans have been analyzed by identifying opportunities and critical issues. The methodology identified has allowed to introduce and coordinate the proper integration of renewable sources (RES) and emission (GHG) reduction policies in the transport sector. In this context, a territorial planning and management effort, starting from the regional and national planning (PEAR), will have to relate more to the participatory push, which in the footsteps of Community policies is animating the energy discipline of the communities in particular through the instrument of the Action Plan for Sustainable Energy and Climate. The opportunity offered will enable the development of multi-level instruments (regional but declinable at the district and urban level), SECAPs to improve and adapt to each other through a comprehensive policy on energy planning . The vehicle will be to develop an integrated tool able to harmonize the SACAPs to promote distributed RES based generation at multi-scalar level.
