Cytomegalovirus (CMV) infection is an important cause of morbidity and mortality in the immunocompromised; however, even within this group of patients, infection with CMV is not synonymous with disease. A number of factors affect the pathogenicity of the virus. An understanding of the mechanisms by which CMV causes disease at different sites in the body, and of how U1at might vary with different patient groups, is essential to the design of stategies for the prevention and treatment of disease. This review discusses the factors which affect the occurrence and type of CMV disease in different patient populations, and considers some of the underlying mechanisms which might contribute to pathology.

One of the lessons drawn by many scholars from the 1982 U.S. sanctions against the Soviet-European gas pipeline was that the decline of American hegemony and the global spread of American business placed the overseas networks of U.S. multinational corporations beyond the control of the U.S. government for the purposes of economic sanctions. Through systematically examining three subsequent sanctions efforts (Nicaragua, Libya, and South Africa), this study qualifies the generalizability of this “lesson.” In none of the cases was the United States willing to incur alliance costs through applying extraterritorial controls, nor was it able to persuade American firms to substitute public preferences for private ones. Nonetheless, in each case, the U.S. government influenced corporate decision making by augmenting corporate perceptions of risk so that prudent business stategies reinforced diplomatic preferences.

Based upon many quantitative case histories of basins, evaluated by several basin analysis groups, some general patterns of application of codes are developing. Here we set down some ideas for using quantitative basin analysis codes when minimal, moderate or maximal quantities of high quality data are available. While strategies for each basin have to be tailored to the specific geologic setting of each basin, nevertheless some general patterns of development seem to be of generic validity. Hypervitaminosis A can lead to hypercalcemia by increasing osteoclasts and subsequent bone resporption. Dietary supplements including vitamin A are new popular treatment stategies for autism.Case presentationWe report a five-year old boy with autism spectrum disorder presenting with severe abdominal pain and bilateral lower extremity pain, who was found to have persistent hypercalcemia due to hypervitaminosis A. The patient ingested over 700 times the recommended intake of Vitamin A per day for age. Retention of vitamin A in the liver and adipose tissue causes toxic levels of retinoids and hypercalcemia.ConclusionsAcute treatment included intravenous rehydration, furosemide, and calcitonin. Pamidronate was the definitive treatment for hypercalcemia from hypervitaminosis A due to its osteoclast inhibition and long biologic half-life. Parents should be counseled on risks of toxicity and absence of evidence showing benefits of vitamin A therapy for autism.

Within the project “Functional Surfaces via Micro- and Nanoscaled Structures” an investment casting process to produce 3-dimensional functional surfaces down to a structural size of 1µm on near-net-shape-casting parts will be developed. The common way to realise functional microscale features on metallic surfaces is to use laser ablation, electro discharge machining or micro milling. The handicap of these processes is their limited productivity. In order to raise the efficiency, microscale features will be replicated by use of the investment casting process. The main research objective deals with the investigation of the single process steps with regard to the moulding accuracy. Actual results concerning making of the wax pattern and the ceramic mould as well as the casting of an Aluminium alloy will be presented. By using the example of an intake manifold of a gasoline race car engine a technical shark skin surface was defined in order to reduce the drag of the in-coming air. Possible process stategies to realise microscale features on an inner surface of a casting part were developed.

Three analytical techniques are available to describe the observed response of stream-dwelling organisms to variations in their physicochemical environment: the incremental method, polynomial regression on a single factor, and multiple regression. The efficacy of these tools as descriptors of the responses has not been compared. We used the three methods to describe density and microhabitat–preference curves for Prosimulium mixtum/fuscum, Stegopterna mutata, and Simulium aureum in response to distance from the lake, current velocity, and water depth in a stream draining a Laurentian lake. The incremental method yielded the least precise estimates of density and biased estimates of optimal current velocity for two of the three species; multiple regression yielded the most precise estimates of density and unbiased estimates of optimal conditions, whereas polynomial regression on a single factor was intermediate. From this comparison, we suggest that the multiple-regression approach to estimate microhabitat–preference curves be used in developing optimal management stategies. Further, we suggest that rare species should be excluded from such analysis because of the low precision of density estimates for rare organisms.

Abstract. A problem encountered by many distributed hydrological modelling studies is high simulation errors at interior gauges when the model is only globally calibrated at the outlet. We simulated river runoff in the Elbe River basin in Central Europe (148 268 km2) with the semi-distributed eco-hydrological model SWIM. While global parameter optimisation led to Nash–Sutcliffe efficiencies of 0.9 at the main outlet gauge, comparisons with measured runoff series at interior points revealed large deviations. Therefore, we compared three different stategies for deriving sub-basin evapotranspiration: (1) modelled by SWIM without any spatial calibration, (2) derived from remotely sensed surface temperatures, and (3) calculated from long-term precipitation and discharge data. The results show certain consistencies between the modelled and the remote sensing based evapotranspiration rates, but there seems to be no correlation between remote sensing and water balance based estimations. Subsequent analyses for single sub-basins identify input weather data and systematic error amplification in inter-gauge discharge calculations as sources of uncertainty. Further probable causes for epistemic uncertainties could be pinpointed. The results encourage careful utilisation of different data sources for calibration and validation procedures in distributed hydrological modelling.

AbstractI clarify in what sense one might want to claim that thought or language are public. I distinguish among four forms that each of these claims might take, and two general ways of establishing them that might be contemplated. The first infers the public character of thought from the public character of language, and the second infers the latter from the former. I show that neither of these stategies seems to be able to dispense with the claim that thought and language are interdependent, and that the second strategy raises more difficulties than the first. I then examine the reasoning by which Davidson means to establish that thought depends on language. I claim that this reasoning is not conclusive, and that it can be adapted in such a way as to establish a version of the thesis that thought is public which does not presuppose that language is public, and a version of the thesis that language is public which does not imply that thought depends on language. I conclude with the suggestion that despite appearances to the contrary Davidson's doctrine is defensible only if it implies at least the conceivability of intentional systems that would lack language altogether.

The article discusses the features of the development and course of the atherosclerotic process in various vascular basins. Variability of factors such as local hemodynamics, differences in the structure of the arteries can influence the formation of atherosclerotic plaque and the progression of the atherosclerotic process. A better understanding of the nature and causes of these differences can help improve the tactics of treatment and prevention of vascular complications. Analysis of the results of the COMPASS study (cardiovascular outomes for people using anticoagulation stategies - evaluation of cardiovascular outcomes in patients receiving anticoagulant therapy) showed for the first time the effectiveness of combination therapy using rivaroxaban direct anticoagulant at a dose of 2.5 mg 2 times a day in combination with acetylsalicylic acid at a dose of 100 mg per day. It was shown that, against the background of combination therapy, patients with coronary heart disease or atherosclerotic peripheral artery disease showed a significant decrease in the incidence of events of the primary end point of efficacy: a reduced risk of stroke, cardiovascular mortality and myocardial infarction, and also a reduction in the risk of major lower amputations extremities and total mortality in patients with stable forms of atherosclerosis. The emergence of combination therapy can help to achieve both an improvement in the overall prognosis and the prognosis of limb preservation in patients with multifocal arterial lesion.

Reproductive events of Nicotiana glauca Grah. from the Pedregal de San Angel (SW of Cuenca del Valle de México, México) were studied. The species is esencially ornitophilous because it is visited by several hummingbird species (fam. Trochilidae) in the american continent, while in Africa and Asia is visited by sunbirds (fam . Nectariniidae) and honeyeaters (fam . Meliphagidae) respectively. N . glauca is highly beneficia! to the hummingbird populations in the Pedregal de San Angel because of its long flowering period and large amount of nectar produced; it is specially notorious where are not alternative sources of food. Introduced recently in Mexico, N. glauca has played an important role on the ecology, and particulary on the seasonal movements of hummingbirds. Both, trap-line and territoriality, are feeding stategies founded in the four hummingbird species identified in the Pedregal de San Angel; these feeding strategies have implications in the pollen flux between individuals of N. glauca. At this respect, the role played by the ilegitimate visitors is discussed. Experimental techniques carried out to know the breeding system showed that N. glauca is self-compatible and able to self-pollination, which is an important feature in its success as a colonizing species. Finally, three different dispersal mechanisms are described, and its importance in the present pattern of distribution of the species is discussed.

Abstract:
Seedlings of western hemlock (Tsugaheterophylla (Raf.) Sarg.) and early huckleberry (Vacciniumovalifolium Smith) were studied for 5 years following windthrow in a dense 45-year-old forest to better understand the contrasting reproductive and growth stategies of woody shrubs and trees to small natural disturbances in temperate rain forest environments. Seedlings of both species responded to the disturbance with increased growth and increased recruitment. Tsuga seedling establishment and Vaccinium seedling density increased after the disturbance. Tsuga height growth, number of branches, and biomass were linearly related to solar radiation transmission in all 3 years. Vaccinium exhibited an exponential shoot growth pattern characterized by an initial delay of 3–4 years, whereas Tsuga increased productivity within months of the disturbance and continued to increase its growth exponentially throughout the study. Vaccinium increased rate of branching, total biomass, and allocation of biomass to roots following the windthrow, but did not significantly increase twig elongation rates until the fourth growing season. No significant difference in soil or air temperatures was detected between the forest and open windthrown areas, but litter temperatures increased by as much as 1 °C in the windthrow during the summer. Infrequent opening of dense second-growth conifer canopies whether by thinning or by small-scale disturbances is likely to result in dense secondary canopies of shade-tolerant trees and in little understory vegetation or forage for wildlife.

Objective: Cardiopulmonary bypass (CPB) and deep hypothermic circulatory arrest (DHCA) are commonly used in cardiac surgery. However, the mortality and morbidity are still high in practice. Developing novel protective stategies and elucidating the underlying mechanisms for the pathophysiological consequences of DHCA have been hampered because of the absence of a satisfactory recovery animal model. The aim of this study was to establish a novel and safe DHCA model without blood priming in rats to study the pathophysiology of potential complications. Methods: Ten adult male Sprague-Dawley rats (age, 14-16 weeks; weight, 200-300g) were used. The entire CPB circuit consisted of a modified reservoir, a custom-designed small-volume membrane oxygenator, a roller pump and a home-made heat exchanger, all of which were connected via silicon tubing. The volume of the priming solution was less than 10 ml. The right jugular vein, right carotid artery and left femoral artery were cannulated. The blood was drained from the right atrium through the right jugular vein and fed back to the rat via the left femoral artery. CPB was commenced at a full flow rate. The animals were cooled to a pericranial temperature of 18°C and then subjected to 45 minutes of DHCA with global ischemia. Circulatory arrest was followed by rewarming and over 60 minutes of reperfusion. CPB was terminated carefully. Blood in the circuit was centrifuged and slowly transfused to achieve optimal hematocrit. Blood gas and hemodynamic parameters were recorded at each time point before CPB, during CPB and after CPB. Results: All CPB and DHCA processes were achieved successfully. No rat died in our research. Blood gas analyses at different times were normal. Cardiac function and blood pressure were stable after the operation. The vital signs of all the rats were stable. Conclusion: The novel augmented venous-drainage CPB and DHCA model in rats could be established successfully without blood priming.

Dalam penelitian ini peneliti juga mencobamerumuskan strategi perusahaan dalam rangka meningkatkan penggunaan jasa dry port, dengan metode Matrik IFE dan EFE, Matriks Internal-Eksternal, Matriks TOWS atau SWOT, hingga Quantitative Stategic Planning Matriks (QSPM). Berdasarkan hasil perhitungan diketahui Customer Satisfaction Index di Dry Port Cikarang sebesar 71,85%. Dari 33 variabel pertanyaan Importance Performance Grid didapatkan ada 9 variabel pada kuadran 1, 9 variabel pada kuadran 2, 10 variabel pada kuadran 3, dan 5 variabel pada kuadran 4. Hasil perhitungan Matrik Internal-Eksternal didapatkan posisi stategi beada posisi 1 atau cocok dengan tumbuh kembang. Sedangkan untuk Matriks TOWS atau SWOT berada pada strategi Threats-Weakness (TW). Berdasarkan perhitungan QSPM dalam penelitian ini juga menunjukkan bahwa alternatif terbaik dalam meningkatkan penggunaandry port oleh pengguna jasa (freigt forwarde/EMKL) adalah melakukan promosi yang besar-besaran.

The result of the research shows that in improving writing skill through STAD type cooperative stategial strategy IPA 4 SMA Negeri 1 Pasangkayu Pasangkayu regency in the stage of planning, implementation, and assessment of cycle I have not maximized because of obstacles faced by teachers and students in learning. However, in the second cycle of implementation planning, and assessment can be maximized with the limitation of all the obstacles experienced in the first cycle so that students' writing skills have a good change. or increase. Thus, STAD type cooperative strategy can improve the writing skill of grade XII students IPA4 SMA Negeri 1 Pasangkayu Pasangkayu regency.

They are confronted with a particular institutional environment that pre-determines strategic opportunities for the enterprise and limits the application of Western firms business practices and organizational concepts. Thus, the strategies observed in transition countries are different from those applied in successful and developed economies, and strategies that prove successful in one country may be a complete failure in another. Corporate strategies in transition countries and other emerging markets can only be explained by including a specific institutional context in the analysis. This creates challenges that are fundamentally different from the experience of managers of developed countries and firms with foreign business partners. a particular challenge is the acquisition and subsequent integration of state-owned enterprises.

Untuk mengatasi kecemasan yang dialami siswa SMP dan SMA di Kabupaten Magelang perlu dirancang dan diterapkan Strategi Intervensi Konseling yang tepat, tingkat kecemasan yang dialami para siswa dapat diturunkan secara maksimal dengan cara mendesain Strategi Intervensi Konseling dengan teknik Cognitive Restructuring (CR) dan Systematic Desensitization (SD) yang efektif diterapkan untuk mengatasi kecemasan para siswa SMP dan SMA di Kabupaten Magelang. Dengan melakukan kegiatan sebagai berikut: 1) menganalisis tingkat kecemasan para siswa SMP dan SMA di Kabupaten Magelang, 2) mengidentifikasi factor penyebab timbulnya kecemasan siswa, 3) menganalisis terori-teori yang mendukung pemecahan masalah kecemasan, dan 4) mendesain Strategi Intervensi Konseling yang cocok digunakan untuk mengentaskan kecemasan siswa. Abstract Recent CCG and POG treatment stategies for patients with T-ALL are significantly different. POG treats T-ALL patients on a separate protocol from that used for B precursor ALL (B-ALL) while CCG treats T-ALL patients on the same protocols utilized for B-ALL patients with protocol assignment determined by the NCI risk classification. We recently reviewed the outcome of patients with T-ALL and high-risk features (age ≥ 10 years and/or initial WBC > 50,000/ul) treated on either the CCG-1961 or POG-9404 clinical trials. POG utilized an intensive anthracycline based multidrug regimen based on the DFCI treatment program and randomized patients at registration to receive or not receive additional high dose methotrexate(HDMTX) with leucovorin rescue. All patients received cranial irradiation (CRT). CCG therapy for high risk T-ALL was based on CCG modified BFM(S-BFM) and Augmented BFM(A-BFM). Patients with <25% blasts on day 7 bone marrow(RER) who achieved remission were randomized in a 2X2 design to standard or augmented intensity and standard duration(1 interim maintenance(IM) and 1 delayed intensification(DI) phase or prolonged duration (2 IM and DI phases). Only RER patients with CNS-3 status received CRT. Patients with a slow response(SER) to induction therapy received A-BFM therapy +/− idarubicin/cyclophosphamide courses during DI phases. All SER patients received CRT. In the POG trial, HDMTX produced a significant improvement in event free survival (EFS). In the CCG trial, for RER patients, augmented intensity regimens produced a better EFS than standard intensity regimens. There was no difference in outcome for patients receiving standard or prolonged duration therapy. Thus we compared the high dose MTX arm from POG 9404 to a composite EFS result for CCG l961 based on induction failure/toxic death rate, augmented intensity therapy(1 or 2 DI phases) for RER patients and A-BFM therapy for SER patients. POG 9404, (opened 6/1996; closed 9/2001) and enrolled 363 T-ALL patients, with 77% having high risk features of which 155 were randomized to HDMTX arms. 5 yr EFS for patients on these arms was 75% and overall survival (S) of 84%. CCG 1961(opened 11/1996;closed 5/2002) entered 2077 patients of which 410 had T-cell disease with 263 RERs. The projected composite 5 year EFS and S estimated for high risk T-ALL patients treated on CCG l961 was 76.9% and 82.5% respectively. Isolated CNS relapse accounted for approximately 2/3 of the relapses in RER patients patients.CNS 3 patients treated on 9404 demonstrated 75% 5 yr EFS as compared to 82% for patients on 1961. Outcome for higher risk T-ALL patients treated by POG and CCG were quite similar. However, the BFM based strategy utilizes significantly less anthracycline and appears to produce a lower rate of bone marrow relapse. We postulate that incorporating low dose cranial RT(1200 cGy) will significantly reduce the incidence of CNS relapse and improve the overall EFS rate. Therefore, COG will utilize an augmented intensity BFM backbone for the first T-ALL protocol.
