Abstract:
This study explores how student participation in the development of a school computer-network (SCN) motivated students to learn and promoted service and collegial relationships in the school. Students participated in a Technology Leadership (TL) community and engaged in activities that were central to the development of the SCN. The research examines the co-evolution of the SCN and student activities and the relationships between TL students and the school. In the study, data on students' experiences in the TL program came from non-participant observation, conversations, semi-structured interviews and document analyses. Using a sociocultural perspective of identity construction and informed by Lave and Wenger's notion of participation in a community-of-practice, with actor-network approaches, the analysis of the data showed that student level of engagement increased when the activities were relevant to their in-school and out-of-school technology experiences, or to their future career goals. Program participants provided technical support to the SCN and taught what teachers and students wanted to learn at a time when they needed to know it. In so doing, these leadership students moved towards greater technical expertise, improved interpersonal skills and increased leadership responsibilities as demonstrated by the availability of improved technical support services in the SCN. As newcomers to the TL community gradually advanced to full participation and old-timers became computer consultants to the school before they eventually graduated, the TL community was subjected to a continual process of renewal in terms of participants. With progressive student participation and with translations of diverse technology actors, the services the SCN provided to the school improved. Over time, the SCN's technical character changed and the relationships of service and collegiality between TL students and the school were enhanced. Thus, both participants and the school realized educational value. The implication for curriculum and pedagogy of discipline-based courses is that if students are to be attracted to school initiatives and retained, the curriculum and its delivery need to increase opportunities for students' changed relationships with the school community to take place, and for student participation in a relevant community-of-practice that is responsive to students' future aspirations.

Abstract:
People with heart failure frequently experience poor quality of life, often exacerbated by co-morbidities. This demands complex self-management activities, often supported by family members in care roles, and includes input from community Heart Failure Specialist Nurses (HFSNs) that is known to reduce hospital readmissions. There is, however, a paucity of knowledge about the perceptions of people living with heart failure concerning self-management strategies. Aim This thesis is a longitudinal study exploring emergent patterns and styles of self- management of persons with heart failure, the impact of family members and HFSNs. Method Thirty-one theoretically sampled participants with heart failure were recruited from a district general hospital between October 2004 and September 2006. The sample participated in home-based, audio-taped, semi-structured interviews prior to nurse intervention, describing how heart failure affected their lives, focusing on self-management strategies. The HFSNs conducted home visits and telephone contacts according to individual needs independent of the research. At five months, 23 participants completed a diary for 14 days. Twenty- seven participants (three died, and one withdrew) were interviewed at six 3 months to explore how self-management strategies had changed. The HFSNs participated in a focus group. Constructivist grounded theory and reflexive accounting informed data collection and analysis, leading to the inductive development of an explanatory framework for emergent themes, one of which, Informed Support, serves as an original and significant contribution to the literature. Results Successful management is a complex biopsychosocial activity, involving reconstructions of identity, symptom management, and relationship management, while living with a chronic and uncertain condition that severely compromises life activities. Central to successful management is Informed Support, where family members collaboratively develop a range of activities and attributes to augment self-management. This was perceived as most effective when the HFSNs were able to adopt a 'whole family' approach. Conclusion Heart failure self-management is a dynamic, complex and adaptive activity that can be positively and significantly shaped by Informed Support from family members. Community based HFSNs can influence, but need to recognise differing styles of, self-management.

Abstract:
Background Type 2 diabetes demands self-management over time, to maintain health and reduce the risk for diabetes complications. However, despite efforts, many persons with type 2 diabetes are not reaching the treatment targets. In diabetes, person-centred care and group education are recommended. Diabetes specialist nurses (DSNs) working in primary healthcare have an important role in supporting patients with type 2 diabetes in their self-management to adapt to the demands of the disease in everyday life. Therefore, it is important to explore the DSNs’ professional role and their experiences of practising person-centred care. The overall aim of the thesis was to explore the professional role of DSNs in primary healthcare, and to describe their experiences of person-centred diabetes care. Methods The thesis includes three studies with qualitative, and one with a quantitative, approach. Data collection consisted of focus group interviews, individual interviews, observations, and questionnaires. Qualitative content analysis and statistics were used in the analysis. In studies I and II, 29 and 31 DSNs participated, respectively. In study III, 10 DSNs and 44 persons with type 2 diabetes participated. Lastly, in study IV, 10 DSNs participated. Results The results in the thesis showed that DSNs have a complex and multifaceted professional role that entails striving to be an expert, a fosterer, a leader, an executive, and a role model, which they found challenging. The DSNs perceived high job demands, such as decision-making and learning. The thesis also showed that the interaction between DSNs and persons with type 2 diabetes shifted from empowerment to authority struggles during group support sessions based on person-centred care. The experience of person-centred care was described as enriching, but DSNs also expressed ambivalence, related to an altered professional role. Conclusion There is a desire by DSNs to be close to persons with type 2 diabetes, although they have several challenges to fulfil, which makes it difficult to uphold a relation with proximity; thus, distance is also present. Even though person-centred care is recommended in healthcare, and despite DSNs’ efforts to practise PCC, the result of this thesis shows that it also implies an altered professional role for DSNs that has to be addressed.

Abstract:
Nurse education is continually adapting to meet the requirements of employers to develop increasingly autonomous practitioners who can provide evidence-based, high quality care. The work-based project examines the support available to mentors, known as practice teachers, in their role as assessors of nursing students in higher level practice. A qualitative study: the project employs a grounded theory approach to the analysis of data elicited from practice teachers and academics. Semi-structured interviews and focus groups alongside regular reviews of the literature are utilised to elicit data, and via an inductive process, categories emerging from the analysis are constructed to present new insights and understanding of the subject under scrutiny. Whilst it is clear that a degree of support is available to practice teachers in response to a rudimentary understanding of their role in higher level practice, it is also clear that this support is limited by a number of factors. The product of practice assessment for the employer is a newly-qualified practitioner who is able to carry out a role based on a specified set of competencies. For the educator, whether within the higher education institution or in practice, the process of education is ongoing; producing a practitioner with the capability to utilise higher level practice in ever-changing contexts and situations. Support for the practice teacher can only be enhanced if recognition of the role is promoted. This requires a shared understanding of the importance of developing both competence and capability for higher level practice. Only then will the vital contribution made by the practice teacher in the student’s development be understood by those supporting them. Converging rather than competing philosophies of training for competence and educating for capability are necessary to maintain the status and commitment of the practice teacher and consequently the rigour required of assessment in practice.

Breast cancer has had, and will continue to have, a devastating impact on the lives of many Australian women, their families, friends and the wider community. The concomitant treatment of this disease places a considerable burden on the health care system and the supporters of the person diagnosed with this disease. While there are many government and non-government organisations that provide treatment and support services for the person with breast cancer, these services are usually provided in person either in the home or at the organisation’s offices. This study extended the information advice and support aspects of these services to the online or Internet based realm via the design and development of a breast cancer focused online support community www.breastcancerclick.com.au and explored the role of the expert nurse through the employment of a specialist breast care nurse as a member, moderator and health professional within this online community. This study used an ethnonetnographic approach, including online (on the Internet) and offline (face-to-face) methods, to explore the role of the specialist breast care nurse within the online, breast cancer support, community. The study was comprised of three phases, Phase One, the offline and online identification of the information, advice and support needs of Western Australian women with breast cancer and their Internet use; development of a website designed to meet those needs and to foster the development of an online support community; Phase Two, the employment and introduction of a specialist breast care nurse as a member and provider of evidence-based information, advice and support for online community members; Phase Three, the online and offline collection of data relevant to the role of the specialist breast care nurse within the online support community. The identification of the expert nurse as a linchpin in the patient’s care and communication has implications for future nursing practice and curricula as well as consumers of health care. Recommendations arose from the findings in relation to further research, nursing practice, education these recommendations indicate an innovative extension to expert nursing practice and together the elementary guidelines for health professional when developing an illness specific online support community foreshadow a future direction for nursing, in line with the digital age.

Self-management embedded early in the lung cancer pathway improves the quality of life of people living with the disease. However, specialist-level lung cancer nurses have limited guidance on how to best enable people to self-manage. Importantly, social, cultural and personal influences mediate the ways in which these nurses facilitate meaningful interactions with patients. A Vygotskian-inspired model of engagement was developed that asserts that learning occurs during interactions and is constructed around the contextual behaviours and identities of nurses and patients. The model may augment thinking and influence behaviour around self-management education more generally.

Abstract:
In the UK since the early 1990s there has been a growing trend for certain hospital initiated specialist drugs to be prescribed by GPs within primary care. GPs have been encouraged to undertake this activity supported by the use of so-called “shared care protocols.” Despite these arrangements this has proven difficult with concerns over the quality and availability of shared care protocols and reported safety related incidents including fatalities linked to the use of certain specialist drugs. GPs have benefited from the complete computerisation of general practice unlike their hospital based colleagues who on the whole continue to work and prescribe within paper-based systems. These benefits have included the use of electronic prescribing and the introduction of computerised decision support systems (CDSS) in the form of reminders and drug related warnings and alerts. Scoping reviews are an established way of assessing the evidence base of a subject area which can then be summarised to reflect the broad nature of the field. For this thesis a scoping review of CDSS was used to evaluate how this intervention has been used within primary care and to identify areas where it can be further developed. Although the use of information technology has been suggested as a possible solution to some of the problems at the primary and secondary care interface, no published studies have evaluated the potential for a CDSS to support the prescribing of specialist drugs by GPs. In order to explore this concept further and gain additional knowledge of current CDSS usage, key informants were identified and interviewed. The key informants described the current availability and characteristics of CDSS within UK general practice. The concept of developing a CDSS to support GPs in the use of specialist drugs to include prescribing was acknowledged as beneficial and was widely supported. Enablers and barriers to development and implementation were identified including a number of potential operating models. Key enablers included data quality and functionality features, joint development and implementation and to make use of existing systems and frameworks. Key barriers included addressing the needs of end users, security, regulation and funding. Human ergonomics was used to further investigate GPs and the actual level and use of computers and software programs including CDSS at the point of prescribing both during and outside of patient consultations. The application of an analytical approach to these processes through a task analysis framework identified failings in existing arrangements for GPs to safely prescribe specialist drugs.

INTRODUCTION: Access to health care, like childhood survival, often depends on where one lives. The infant mortality rate in rural South Africa (SA) is 52.6 per 1000 births, compared to 32.6 per 1000 births in urban areas. Furthermore, three of the four districts in SA with the highest HIV prevalence are rural. These being two commonly used health indicators, it is clear that rural populations have significantly poorer health outcomes than their urban counterparts. About half the world’s population live outside major urban centres, where health services and specialist medical services are concentrated. Rural SA are home to 43.6% of the population, but are served by only 12% of doctors and 19% of nurses. Of the 1200 medical students graduating in the country annually, only about 35 work in rural areas in the long term. There are 30 generalists and 30 specialists/100 000 people in urban areas, compared to an average of 13 generalists and two specialists/100 000 people in rural areas. The question arises whether the poorer access to particularly specialist services is a contributing factor towards poorer outcomes. Specialist outreach to rural communities is one way of improving access to care. In the Eden and Central-Karoo districts of the Western Cape of SA there are one level 2 (regional) hospital and ten level 1 (district) hospitals. All clinical disciplines reach out, with varying frequencies. On average, the four main district hospitals receive 17 specialist outreach visits per month; while the smaller district hospitals receive three specialist visits per month (Appendix 1). A typical outreach visit includes a problem ward round, outpatient session, theatre list for some surgical disciplines and formal/informal educational sessions. In principle, stakeholders agree that specialist outreach and support (O&S) to rural populations is necessary, as it improves access to specialized health care services. In practise however, there are factors that influence whether or not O&S reaches its goals. This in turn affects the sustainability of O&S projects. Understanding these factors would aid recommendations for a suitable model for O&S.

I Sverige är rollen som specialist-sjuksköterska inom kirurgisk vård ganska okänd och antalet specialistsjuksköterskor inom denna specialitet är få.Syfte: Syftet var att beskriva vilka förväntningar specialistsjuksköterskor inom kirurgisk vård har på sin yrkesroll samt att ta del av och beskriva deras erfarenheter av att arbeta som specialistsjuksköterskor.Metod: Kvalitativ studie med deskriptiv design. Specialistsjuksköterskor inom kirurgisk vård (n=29) från fem olika sjukhus deltog i studien. Urvalet gjordes med bekvämlighetsurval. För datainsamling användes ett studiespecifikt formulär med öppna frågor. Data analyserades med en kvalitativ innehållsanalys.Huvudresultat: Specialistsjuksköterskornas förväntningar på yrkesrollen var att med sin kompetens kunna vara en tillgång för kollegor och patienter. De beskrev att deras kompetens behövs på vårdavdelningarna, både i det kliniska arbetet och i förbättringsarbeten. Specialistsjuksköterskorna upplevde varierande grad av stöd från chefer och kollegor. De beskrev även att det är viktigt att tydliggöra yrkesrollen.Slutsats: Specialistsjuksköterskors kompetens gör skillnad, både direkt i patientvården och indirekt via förbättringsarbeten. Det är viktigt att fortsätta arbetet med att tydliggöra yrkesrollen till exempel genom en kompetensbeskrivning, och möjlighet att söka tjänster eller förordnanden som specialistsjuksköterska. För att utvecklas professionellt behöver specialistsjuksköterskor en ledning som tror på yrkesrollen och stöd från arbetsgruppen.
Background: Several studies show that nurses’ educational level has impact on patient safety in surgical wards. In Sweden the role of the clinical nurse specialist in surgical care is quite unknown and the numbers of clinical nurse specialists are few.Aim: The aim was to describe the expectations clinical nurse specialists in surgical care have on their professional role and to explore their experiences of their work.Method: A qualitative study with a descriptive design. A convenient sample of clinical nurse specialists in surgical care (n = 29) from five different hospitals participated in the study. A study specific questionnaire with open ended questions was used for data collection. The narrative data was analyzed using a qualitative content analysis.Result: The clinical nurse specialists’ expectations of their professional role were to be able to support and contribute with their competence in clinical practice. They described that their competence is needed in clinical work as well as in the quality improvement work in the surgical ward. Furthermore, the nurses described that they received varying levels of support in their new position from managers and colleagues. The clinical nurse specialists pointed out the importance of clarifying the role.Conclusion: The competence of the clinical nurse specialist makes a difference in patient care and in quality improvement work in the surgical ward. It is important to continue the process to clarify the role and to develop national guidelines. In the professional development the clinical nurse specialist needs to be supported by the managers as well as the colleagues.

Abstract:
Bröstmjölk är en komplett näringskälla för det nyfödda barnet. Förutom att tillföra barnet alla näringsämnen hen behöver så ger bröstmjölken energi, bygger upp kroppen samt skyddar det nyfödda barnet mot sjukdomar. Amning är ett samspel mellan mamma och barn som påverkas av många faktorer. För en tvillingmamma är initiering av amning en större utmaning än för en mamma som fött ett barn, då omvårdnaden kring barnen blir dubbel. Syftet med studien var att beskriva tvillingmammors erfarenhet av amningsstöd, dels från vårdpersonal, dels från sociala medier, för att initiera och upprätthålla en fungerande amning. Studien är en intervjustudie med kvalitativ metod. Datamaterial samlades in genom intervjuer av sex utvalda tvillingmammor. Mammorna söktes och valdes ut via en Facebookgrupp. I resultatet framkom två kategorier: Mammans initiativ och Vårdpersonalens engagemang. Resultatet tyder på att de intervjuade mammornas erfarenhet är att vårdpersonal idag saknar den specifika kunskapen om/och förståelse för hur mycket arbete det innebär för en tvillingmamma att få amningen att fungera. Många mammor söker idag den information de inte erhållit via vårdpersonal på annat sätt, oftast via sociala medier. Utifrån den här studien kan vårdpersonal få en ökad förståelse för tvillingmammors behov av extra amningsstöd och information.
Breast milk is a complete food source for the newborn. In addition to supplying the baby all the nutrients it needs breast milk also provides energy, builds up the body and protects the newborn against disease. Breastfeeding is an interaction between mother and child that is affected by many factors. For a twin mother is initiation of breastfeeding more of a challenge than for a mother who gave birth to a child, then care about the children becomes twice.The purpose of this study was to describe the mother of twin´s experience of breastfeeding support, both from health professionals as well as from social media, to initiate and maintain a functional breastfeeding. The study is an interview study of qualitative method. Data material was collected through interviews with six selected twin mothers. The informants were sought and selected through a Facebook group. The results revealed two categories: The mother's initiative and The involvement of health professionals. The result indicates that the interviewed mothers experience is that health professionals today lack the specific knowledge and understanding of how much work it means for a twin mother to get a functional breastfeeding. Many mothers today are using other sources in seeking the information they did not receive by health professionals, mostly through social media. Based on this study, health professionals might gain a greater understanding of the need twin mothers have for more support and information regarding breast feeding twins.

Bakgrund: Regionalt Läkarstöd (RLS) har utformats för att säkra prehospital involvering från läkare. Dagens ambulanssjukvård ställer höga krav på att bedöma och triagera patienter till rätt vårdnivå. Till sin hjälp i sitt dagliga arbete med detta behöver sjuksköterskan en läkare som är väl insatt i verksamheten. Syfte: Att beskriva läkares erfarenhet av att fungera som rådgivare till sjuksköterskor inom ambulanssjukvården Metod: Kvalitativ intervjustudie med tio läkare som tjänstgör som RLS. Analysen genomfördes med en kvalitativ innehållsanalys. Resultat: Resultatet beskrivs med de fyra kategorierna Teamsammanhållning, Frustration, Bristande ansvarskänsla och Lyhördhet, som speglade läkarnas erfarenheter av att vara telefonstöd till sjuksköterskor. Ett latent huvudbudskap identifierades som Skör tillit. Att som en del i teamet finnas som stöd och rådgivare till sjuksköterskor i ambulanssjukvården upplevdes som positivt och funktionen ses som relevant och viktig. Det krävs en god teamsamverkan mellan professionerna för en patientsäker vård och för att uppnå det utkristalliserades tillit som en nyckelfaktor. Konklusion: En telefonbaserad bakjourslinje dit sjuksköterskor kan vända sig för råd och stöd är en relevant funktion för läkarna, där de känner sig som en del i det prehospitala teamet. I arbetet med bättre teamsamverkan inkluderande god kommunikation för säker vård, är gemensamma möten och övningar en viktig del för att ge professionerna en chans att mötas som personer och främja ökad förståelse för varandras kunskap och utforma gemensamma mål.
Background: Regional Medical support has been designed to ensure an involvement of doctors in the prehospital field. The current system for ambulance care raise high demands to ensure a correct patient assessment and triage to the right level of care, and for their assistance in this daily work, the nurse needs a doctor who is very knowledgeable in the organization. Purpose: To describe doctors’ experience of functioning as advisors for nurses working in the ambulance. Method: A qualitative research design was conducted including interviews with nine doctors serving as Regional Medical support. The content analysis was conducted according to a qualitative content analysis method.  Result: The analysis resulted in the four categories team spirit, frustration, lack of sense of responsibility and responsiveness, which mirrored the doctors’ experiences of acting as a phone support for specialist nurses on the ambulance. An underlying main message was identified as delicate trust. To be part of the team and act as a support and advisor for nurses working on the ambulance was perceived as positive, relevant and important. Good teamwork between the professions is required to guarantee patient safe care, and to obtain that, trust came through as a key factor. Conclusion: For the nurses to have an on call doctor that they can reach on the phone for support and advice was seen as a relevant system for the doctors, where they feel as a part of the prehospital team. In the continuous work for better teamwork, including good communication for safe care, it's important with common gatherings and training sessions to give the professions a chance to meet in person and gain a better understanding for each other’s roles, and enunciate common goals.

There is a lack of narrative research in the United Kingdom (UK) that captures the stories of young people who are, or have previously been, dealing with issues surrounding their use of drugs and/or alcohol. This study, through the utilisation of a narrative approach, aims to provide a rich and detailed description of young people’s experiences, and the difficulties they’ve faced, with their use of drugs and/or alcohol. In this study I have presented the stories of three males aged seventeen to eighteen in rough verse form and then individually analysed the narratives using Engels’ narrative analysis approach. The young males have recently been discharged from a specialist support service for their use of drugs and/or alcohol which has caused concern. The specialist support service supports adolescents (aged fourteen – eighteen) with their drug and/or alcohol use. Each young male was interviewed twice, the first interview allowed the young person to tell their story in its entirety. During the second interview, each young person’s narrative was presented back to them in rough verse form and they were asked to reflect upon reading their story as well as check the anonymity of their identity in their narrative. It is hoped that the findings will help inform Educational Psychologists of some of the challenges faced by young adults who have experienced difficulties as a result of their substance use. In turn it is hoped that this may lead to better informed hypothesis generation when encountering young people who have problematic relationships with drugs and/or alcohol.

Em virtude disso, foi elaborado um Sistema de Apoio à Decisão Médica com o intuito de otimizar e agilizar de forma confiável os diagnósticos médicos. A ideia é dar qualidade e agilidade à prática médica, adotando a tecnologia como ferramenta básica: “Quem tem mais informação, tem melhores condições para escolher e tomar decisões”. Na construção deste sistema, foram utilizados um banco de dados relacional (MySQL) e aplicadas técnicas de inteligência artificial, tais como: a construção de Árvores de Decisão, Aprendizado não supervisionado e a utilização das Redes de Bayes (onde estão envolvidos domínios de conhecimento com significativo grau de incerteza, como é o caso da área médica). Abstract: The knowledge about any pathology can be easily found on the internet, but it is difficult to find any tool that makes the analysis and reasoning between the data of a patient and obtain the most probable diagnosis. In our daily lives, due to a greater demand in the health area, there is a growing need for fast and accurate medical diagnoses. As a result, a Medical Decision Support System was developed in order to reliably optimize and streamline medical diagnostics. The idea is to give quality and agility to medical practice, adopting technology as a basic tool: “Who has more information, has better conditions to choose and make decisions”. In the construction of this system, a relational database (MySQL) was used and artificial intelligence techniques were applied, such as: the construction of Decision Trees, Unsupervised Learning and the use of Bayes Networks (where knowledge domains are involved with significant degree of uncertainty, as is the case in the medical field). Through the union of these techniques, the selection and classification of the most probable diseases are made, where they can be examined in more detail by the doctor, thus ensuring greater security in the choice of possible diagnoses. Pre-school education institution – an institution, which affects social and emotional development of the child, where educational responsibilities of parents are extended by qualified pre-school educators. Pre-school institutions are responsible for child's socialization, quality of care and education, but parents and educators do not always take into account child's abilities and needs, because of lack of time and knowledge. Educators do not learn from socialization programs, which could give them proper social skills development. Social educator - a competent person, who can work in pre-school education institutions and facilitate child's socialization, allow early prevention and social pedagogical rehabilitation.

The purpose of this study was to better understand the self-care practices of female peer support specialists (PSS) with co-occurring mood and substance use disorders. The researcher took a qualitative grounded theory approach conducting in-depth semi-structured interviews with ten women employed at peer-run agencies in Maricopa County, Arizona. Data from these interviews were transcribed, then analyzed manually, as well as with NVivo 10.0 software, to identify the key terms, nodes, categories and emergent themes of the participants' experiences. Self-care practices of peer support specialists included accessing personal and professional support networks; maintaining a daily routine to balance the demands of recovery, parenting, and working; taking medications; sleeping; practicing spirituality; participating in service work; eating nutritiously; exercising, and building a sense of coherence. Although a variety of practices were being used and identified as helpful, spirituality was identified as the most important self-care practice to achieve overall wellness. Employment improved the ability for PSSs to practice self-care because they valued the support of their supervisors and coworkers, were reminded of the consequences of not practicing self-care by working with individuals who were unstable, gained knowledge from teaching others, found healing in telling their stories, and reported higher self-esteems from working and helping others. However, participants did identify ways that employment as a PSS could interfere with practicing self-care, such as staff turnover, limited access to supervisors, or being unprepared to work in the field. Several recommendations were suggested as a result of this study, such as the importance of understanding and using effective self-care practices, building personal and professional support networks, and establishing daily routines to balance recovery with personal and professional demands.

Majority of the psychologists note that it is natural of a man to evolve gradually. In the beginning of the career, a teacher survives some stages of pedagogical development progress (Frances Fuler, 1969; Feiman – Nemsener, 1983; Leitwood, 1992; Arends, 1998; Šernas, 1998 etc.), but first and most of all (s)he experiences diffidence and self-concern. Only later (s)he starts to care about the teaching situation and the learners. After surveying a range of studies considering first-year teaching problems, a conclusion can be made that there are still very few programs helping beginner teachers’ work. As one of the ways which could help a young teacher solve raising problems, mentorship is being studied as method to establish and maintain partnership with young and little- or non-experienced colleagues. Good partnership should be grounded on common interests, mutual help and knowledge. The present article the deals with mentorship as way to help a beginner teacher’s adaptation and his formation as an educator, in which mentor means a person specially appointed to admonish a young teacher. Since Lithuania joined the international APartMent mentor preparation project in 2001, mentor has been defined as a qualified specialist able to conduct future teachers’ school practice. Syftet med studien är att undersöka och följa matematiklärares resonemang kring ämnesplanens förmågor, genom problematisering av nationella provuppgifter.   Matematikundervisningen både på grundskolan och gymnasiet har visat sig vara begränsad när det gäller målstyrning av undervisningen som bygger på medvetenhet och kunskap om förmågorna. En förutsättning för att undervisningen ska kunna vara målstyrd med avseende på förmågorna ser vi är lärares kunskap och medvetenhet om desamma. Avsikten med studien är därför att ur ett utvecklingsperspektiv hitta sätt att för lärare ge ämnesplanens förmågor innebörd. De nationella proven konkretiserar flera av förmågorna, och skulle därför kunna fungera som vägledare i lärares kompetensutveckling kring förmågornas innebörd.  Undersökningen baseras på två filmade fokusgruppsdiskussioner i vilka gymnasielärare i matematik deltar. Diskussionerna utgår från nationella provuppgifter i matematik, med stegvis introducering av skolverkets definitioner av förmågorna och senare också av skolverkets bedömning av desamma.  Studiens teoretiska ramverk utgörs av ett kollegialt lärandeperspektiv och ett multimodalt och socialsemiotiskt perspektiv, som också används i den kvalitativa videoanalysen.   Studien visar att lärarnas resonemang förändras under diskussionen utan Skolverkets definitioner, så till vida att en slags överenskommelse görs, av på vilket sätt förmågorna kan tolkas. Skolverkets definitioner visade sig ha marginell inverkan på resonemangets utveckling i riktning mot Skolverkets intentioner. Skolverkets bedömning tillsammans med definitionerna fungerade däremot som utmanare till lärarnas egen tolkning och skapade genom det ett ökat resonemang. Resonemanget visade dock inte på ökad kunskap och medvetenhet kring förmågornas innebörd, som är förenligt med Skolverkets intentioner. Med stöd av forskning kring kollegialt lärande, skulle ledning av expertis på området kunnat möjliggöra ett resonemang i riktning mot intentionerna.
The aim of this study is to investigate and to monitor teachers of mathematics when they reason on the abilities as expressed in the curriculum, through problematisation of Swedish national test questions.   Teaching mathematics in Sweden has shown itself to be limited when it comes to setting objectives when based on awareness and knowledge of the abilities. One condition for the teaching to be manageable is that the teachers are aware of the abilities. The intention of this study is to find a way for the teachers through a perspective of development, where the abilities are given meaning. The national tests define many of the abilities and therefore the tests could be a guide in the teachers’ development of the reasoning of the abilities.   The investigation is based on two videotaped focus discussions by high school teachers of mathematics. These discussions are based on national test questions, where the teachers are gradually introduced to the national educational agency’s (henceforth Skolverket) definitions of the abilities and assessments that are given by Skolverket. The theoretical framework of the study is based on collegial teaching perspective and a multimodal- and social semiotic perspective, which is also being used in the qualitative video analysis.   The study shows that the teachers’ reasoning changes during the discussions without Skolverket’s definitions, and the teachers reach a consensus on how the abilities should be interpreted. Skolverket’s definitions had only marginal effects on the teachers’ reasoning regarding the intentions of the abilities. However, the teachers’ interpretations were challenged by the assessments which resulted in an increase in the teachers’ reasoning and interpretations. But the outcome of the study has not given any convincing evidence that an actual increase in knowledge nor awareness of the abilities has occurred. According to research into collegial learning, experts in this field would have been able to support more developed reasoning in line with the intentions.

Introduction: The presence of psychological distress following a diagnosis of cancer is well evidenced. To meet this need, the role of oncology clinical nurse specialists (CNSs) has expanded in line with national guidance to include the provision of psychological support to patients and their families. Skills training and supervision has been provided by clinical psychologists. However, there has been little research focusing on the role and experience of the CNS doing this work. This present study researched the experience of CNSs working with patients with cancer and their families in order to understand more fully their experiences. Method: Eight CNSs from four NHS trusts were interviewed about their experience of their role, including the recent expectation of offering psychological support. These interviews were transcribed and individually analysed using Interpretative Phenomenological Analysis, before conducting a group analysis to identify overall themes. Results: Four key themes and fourteen sub-themes emerged through this analysis. The first theme ‘The everyday experience’ captured the experiences and demands of participants in their day-to-day work. ‘The impact of working with patients’ captured a range of experiences of the emotional and existential impact that doing this work involves. ‘Understanding and working out the role’ illustrated the way in which participants must work out their role and identity within their organisational context. Finally, participants experienced ‘Needing recognition and support’ as they carry out this vital role. Two overarching phenomenological themes were also identified as ‘ambivalence’ and ‘uncertainty’ and these run throughout the experiences of all the participants. Discussion: The findings were examined in relation to existing literature. The strengths and limitations of the study were presented and future research suggested. Finally, the clinical implications of this research were identified which included suggestions for training, the use of supervision and greater role clarity.

El presente estudio tuvo como objetivo abordar el tema del abandono de la Terapia de Exposición Prolongada (TEP) en ex-combatientes de Guerra (Veteranos), uno de los tratamientos con mayor basamento empírico sobre su efectividad para el tratamiento del Trastorno de Estrés Posttraumático (TEPT). La TEP típicamente produce resultados positivos duraderos si se llega a completar. Como tal, la Administración de Salud de Veteranos (VHA por sus siglas en inglés) ha invertido recursos extensos en investigación y en clínicas para poner este tratamiento a disposición de los Veteranos bajo el cuidado de la VHA. Esto es de especial preocupación para los Veteranos en los Estados Unidos, que son más propensos a sufrir de trastorno de estrés postraumático que el resto de la población civil.Un estudio previo de la autora de la presente tesis (Hernández-Tejada, Zoller, Ruggiero, Kazley y Acierno, 2014) examinó la deserción de dicha terapia en Veteranos que participaban en la TEP via la modalidad tradicional en persona o via telesalud. Este estudio mostró que en el caso de aquellos que recibían la TEP llevada a cabo en persona, la causa de abandon se derivaba de los problemas relacionados con la logística como tráfico, distancia del hospital, problemas para estacionar el vehículo y el costo indirecto (por ejemplo, horas de trabajo perdido); mientras que para aquellos que recibían la TEP a través de la telesalud en el hogar, la deserción se relacionaba más específicamente con los propios componentes del tratamiento. Específicamente, estos pacientes informaron que las asignaciones de exposición in vivo eran muy incómodas, difíciles y casi intolerables, y por lo tanto el factor principal conducente al abandon del tratamiento. Este trabajo describe un programa para solucionar el abandono de TEP a través del uso de "especialistas de pares" (como se les llama dentro de la VHA) de una manera novedosa, es decir, directamente durante las asignaciones de exposición in vivo. Hasta los momentos, los pares no habían sido utilizados de esta manera, y de hecho tal uso había sido prohibido. En cambio, los pares se usan principalmente para alentar a otros Veteranos a buscar tratamiento y para ayudarles a ‘navegar’ el sistema hospitalario VHA. Por lo tanto, una innovación importante del presente trabajo es elevar el papel de dicho par o igual a un miembro central y clave del equipo de tratamiento. La idea de usar pares o acompañamiento durante la terapis proviene de una amplia evidencia empírica a favor del apoyo social como herramienta para mejorar la recuperación de la salud mental y física. A través del desarrollo de esta tesis, los conceptos de TEPT, TEP, y apoyo usando a pares, se discuten en la sección I. Los métodos se discuten en la sección II. The following study aimed to address the issue of Veteran dropout from Prolonged Exposure (PE) therapy, one of the most effective evidence-based treatments for PTSD. PE typically produces long lasting positive results if treatment is actually completed. As such, the Veterans Health Administration (VHA) has invested extensive resources into research and practices to make this treatment available to the Veterans under VHA care. Unfortunately, little research and attention has been paid to reversing dropout from PE, which is not insignificant at about 30%. This is of particular concern for Veterans in the United States, who are more likely to suffer from PTSD than civilians.   A previous study by the author of the present thesis (Hernandez-Tejada, Zoller, Ruggiero, Kazley & Acierno, 2014) examined Veteran dropout from in person delivered and home-telehealth delivered PE. This study showed that, for in person delivered PE, logistics-related problems such as driving time to appointments, distance to the hospital, parking issues, and indirect cost (i.e., missed work) incurred through participation in therapy were related to dropout, whereas for those receiving PE via home telehealth, dropout related more specificallyto treatment components themselves. In particular, participants in home telehealth delivered PE were more likely to report that in vivo exposure assignments were very uncomfortable, difficult, and almost intolerable, and noted that this was a factor leading to their attrition from treatment.   The following work describes a program to address dropout from PE that uses ‘peer specialists” (as they are called within the VHA) in a novel way, that is, directly during in vivo exposure assignments. Peer specialists have not been used in this manner before, and in fact such use has been prohibited. Instead, peers are used primarily to encourage other Veterans to obtain treatment, and to help Veterans navigate the VHA hospital system. Thus, a major innovation of the present work is elevating the role of a trained peer to a central and key member of the treatment team.   The idea of using peers comes from a wide evidence base in favor of leveraging social support to enhance mental and physical health recovery. Through the development of this proposal, concepts related to PTSD, PE and peer support are discussed in the Background in section I. Methods are discussed in section II. The manuscripts published in peer-reviewed journals are presented, as is a summary of the results, in section III, and finally the discussion is presented in section IV. This manuscript also includes the Appendices within which informed consent and all study instruments are given.

Taikyti teoriniai (mokslinės literatūros ir dokumentų, antrinių dokumentų analizė), empiriniai (anketinė apklausa ir interviu) bei statistiniai metodai. Statistinėms tendencijoms nustatyti, taikyti aprašomosios statistikos metodai (absoliutiniai ir santykiniai dažniai). Anketinėje apklausoje dalyvavo198 tėvai, auginantys ikimokyklinio amžiaus vaikus, kurie gauna logopedinę pagalbą ikimokyklinėse įstaigose. Interviu raštu dalyvavo 4 respondentai (N=4), siekiant išsiaiškinti nuomonių bei nuostatų į teikiamą pagalbą ir pasitenkinimą ja turiniui atskleisti. Svarbiausios empirinio tyrimo išvados: 1. The theoretical part of the research analyses the main focuses concerning parents' satisfaction with the services provided by speech therapists and support to their children as well as to the families in pre-school in Lithuania as well as in other European countries. The aim of the research is to assess the parents’ raising up pre-school children, who need speech therapy, satisfaction with speech therapy support. There were applied the theoretical analysis of literature and documents, secondary analysis of documents as well as empirical (questionnaire and interview), and statistical methods. Methods of descriptive statistics (absolute and relative frequencies) were applied for statistical analysis. The sample of research (questionnaire survey) consists of 198 parents with pre-school children, who receive speech therapy at pre-school, as well as 4 respondents who took part in an interview in order to ascertain the views and attitudes towards the given support and its contents. The main conclusions of the empirical part of the research: 1. The scientific literature and analysis of documents show that Lithuania, focusing on the European Union's education policy, shifts from a narrow orientation to the child‘s and the support to him/her, based on the traditional institutional assistance of a speech therapist to a child in educational institution, to a systemic educational model focused not only on a child but also on a families, involved into the child's education. 2. Barriers to renewable energy development in the past have been identified as high upfront costs with challenges around equity and debt financing, as well as limited legal frameworks and limited regulatory support. Although it is noted that barriers to development are country specific, as well as the solutions to overcome such barriers. Policy measures implemented in various countries to support renewable energy have been seen to have a marked impact on its development. The renewable energy industry in Australia is showing unprecedented growth with a drastic change in the energy landscape expected in the future, should all proposed developments come to fruition. Renewable energy in Australia is currently in its infancy though. It is expected that the planned integration of all the proposed developments in the National Electricity Market will result in certain barriers to development emerging at higher levels of integration. Australia's energy policy has taken significant steps to supporting the deployment of cleaner generation technologies, although hurdles at federal and state level have in the past frustrated the development of renewable energy. The history and status quo of renewable energy development in Australia has been investigated. The current framework and support structures in place as well as potential barriers have been reviewed. A qualitative investigation utilising a single case study approach was selected as the research method. The case investigated was the renewable energy division of a leading global engineering consultancy operating in Australia. The participants of the study included the senior management staff in the renewable energy division of the company. Standard open-ended interviews were conducted. The data was analysed by employing a method of pattern matching. The observed data from the interview process was analysed and compared against the expected pattern which had emerged from the literature. Potential barriers and support strategies have been outlined as a conclusion to the study. The study found federal energy policy to be favourable, but not vital for continued growth of RE development. It was acknowledged that a focus on grid infrastructure would be vital to avoid constricting asset development. The development of Renewable Energy Zones was considered a favourable strategy to overcoming barriers identified, although it was noted that community acceptance would be a significant consideration. Gas generation may form part of the future energy mix, but will be dependent on price and emergent technology. Pumped hydro power is expected to be an important compliment to renewable energy in future, to assist in addressing the technical barriers associated with high levels of non-synchronous generation in the network. A reform of the NEM may also be required to cater for a scenario of a significant penetration of renewable energy and is a topic recommended for future research.

Maybe, one of the answers could be found in the way the inclusive practitioners in practice use disability terminologies in educational settings. In this view, the study seeks to sensitize education professionals practicing inclusion in all educational settings to pay attention to the terminologies that they use in their day to day language communication with others to describe people with disabilities. In writing and speaking about, and with people with disabilities, whether they are young or old, it is most important to use appropriate terminology. Firstly, it demonstrates to all that we value people with disabilities as members of our society. Secondly, it educates those who read and hear what we say, about appropriate terminology, and therefore gives them an opportunity too, to help develop and promote, inclusive and equitable values. Abstract:

The teachers of Years 6 and 7 at a rural State Primary School in the South -Burnett Region of South -East Queensland, in response to concerns and issues raised in the 1993 Collaborative School Campus Review, initiated an action research study during which all teachers of Years 6 and 7 students would specialise in a maxium of three subjects. This research study was conducted over a six month period in 1994 and was a natural progression from other initiatives already established within the school campus. Timetable blocking, the establishment of school management teams of teachers in the P-2. Years 3-5, Years 6-7 and Specialist/Support Staff areas had already been completed.

The action research study, officially included within the school campus operational plan, sought such outcomes as (a) the improvement of teaching practices; (b) enhanced learning opportunities/outcomes for students; (c) enhanced professional support for teachers; (d) promotion of Years 7-8 transition ease for students: and (e) working smarter, not harder within a context of devolved education departmental responsibilities and accountabilities for school -based staff. Conducted by the school principal in collaboration with the School Child Needs and Staff Professional Development Committees, the intent of the study was to (1) develop a plan of action to improve present teaching practices; (2) act on and implement the plan devised collaboratively by the teaching team, comprising the principal and Year 6 and 7 teachers; (3) observe and record the effects of the action within the classroom teaching context, using student opinion to provide an additional perspective; and (4) reflect on these effects as a basis for further planning and action.

Each teacher of Years 6 and 7 specialised in a combination of up to three of the following subjects: (a) English,(b) Mathematics, (c) Science.(d) Social Studies,(e) Environmental Education/Health/Personal Development,(f) Art,(g) Physical Education,(h) Music.(i) German; and (j) Library/Research Studies. Each subject was taught by each teacher at least four times per week for a minimum period of 3Omins. Classroom teachers retained English and Mathematics in their subject selection and specialist teachers retained their official subject area of responsibility. It was thought that by focusing a teacher's time and expertise on a maximum of three subjects, teachers would be able to reflect on their teaching strategies and through improved practice, enhance student learning.

The study found that in transition from successful general teaching practice to that of successful specialist teaching practice, many aspects of a teacher's professional craft knowledge remain constant enabling such a transition to successfully be undertaken. It was also found that over time, many aspects of effective teaching practice evolve and/or specialise.

When referring physicians decide to refer their patients to specialist care, they rarely, if ever, make a referral decision with the benefit of having access to all of the desirable information. It is therefore highly unlikely that the referring physician will make the optimal choice of specialist for that particular referral. A specialist selection optimization algorithm was developed to guarantee that the “right specialist” for each patient’s referral was chosen. The specialist selection optimization algorithm was developed based on feedback from over 120 users of the e-referral program. The developed algorithm was simulated, tested, and validated in MATLAB. Results from the MATLAB simulation demonstrate that the algorithm functioned as it was designed to. The developed algorithm provides referring physicians with an unprecedented level of support for their decision of which specialist to refer their patient to.

Electronic document and record management systems (EDRMS), as an enterprise records management tool, have become well-recognised applications for academic users in higher education institutes. The systems often show limited decision support options for academic users such as educators and students, resulting in a lack of coordination and duplication of academic efforts. For sustainable records management, it is beneficial to have a more context-specific and systematic EDRMS for an individual‟s effective decision-making. This study intends to make two distinct contributions in the following ways: Firstly, the study outlines a four-phased approach adapting the step-by-step process to conduct the empirical investigation through case study data collection and analysis in order to create a new ontology-based records management approach. Secondly, the study proposes a new architecture of the domain ontology-based ERM approach that satisfies user requirements and promotes the value of using it to meet organisational standards.

Abstract:

Marketing education services to Asian countries is a relatively new phenomena which has received limited attention in the Australian literature and has generated only a small amount of research. The literature reveals the need for expertise not only by educationalists but also by marketing specialists to ensure high quality education servicing. In this way, satisfaction experienced by Asian students with the quality of tertiary education services may translate into future recommendations to prospective overseas students to study in Australia.

An exploratory study was undertaken to measure whether participating Asian students had been satisfied with the quality of Australian tertiary education support services which was measured against the base line of Australian tertiary students' satisfaction with services. To narrow the focus of the study, teaching/learning was excluded from tertiary education services. These services were considered to consist of 22 items contained in the widely utilised survey instrument, SERVQUAL. This was used to conduct the investigation. These 22 items of service included items such as equipment, physical facilities, record keeping and staff responses to students. The 22 items were clustered into five basic service elements of 'tangibles', 'reliability', 'responsiveness', 'assurance' and 'empathy'. Using the SERVQUAL survey clusters, the degree of Asian and Australian students' satisfaction with education services, as well as the degree of dissatisfaction, was measured across the five elements.

Forty full -fee paying Asian. and forty Australian, students undertaking first year studies at a South -East Queensland university completed a questionnaire of 22 questions relating to five elements of service. Responses by Asian and Australian students were compared to ascertain which elements of service were considered more, or less, important to each group and what degree of satisfaction/dissatisfaction was experienced with the services.

The questionnaire revealed that all students experienced dissatisfaction with the five service elements. indicating a need for Australian higher education institutions to provide better services for all students. increasing since the introduction of full -fee payment where value for money becomes an imperative. Further longitudinal quantitative and qualitative research are suggested that might result in higher quality higher education services which target the needs of Asian students.

The limitations of using the SERVQUAL instrument for measuring education support services is discussed. It is suggested that this instrument may prove useful in future studies if adaptation to offset the generic nature of the survey is achieved through specific tailoring of the instrument to tertiary education requirements.

This professional practicum seeks to explore methods for teachers to develop knowledge and skills in the effective use of multimedia computer technology in promoting quality teaching and learning. The limitations and advantages of computer generated multimedia in the primary classroom are discussed in the light of current literature. Information regarding appropriate computer hardware and software to implement a multimedia approach to teaching and learning in the primary school is provided, and classroom applications discussed. Various models for the delivery of professional development of teachers are detailed. The identification of some of the problems disclosed by the study are discussed and solutions are offered.

The practicum encompasses two separate studies: an initial study conducted at five separate sites with computer literate teachers and a further study conducted with several teachers (who were not computer literate) at one large school. The research method adopted in each case was collaborative action research involving planning, implementation, observation, reflection and reporting. The researcher provided leadership and professional development for the participating teachers in exploring the potential of computer generated multimedia in the classroom. The initial study and subsequent implementation offer varied methods of using computer generated multimedia in teaching and learning. In this instance all were successful to a degree because the implementation models were tailored to the participants' skills and needs. In the main study subsequent to the initial one, problems in the levels of teacher computer literacy had a marked bearing on the extent of implementation that could be achieved.

A significant outcome of this program is the highlighting of the ever-increasing gap between the advance of multimedia computer technology and the skills required by teachers to utilise the potential of this educational tool. This research project shows that it is possible to address this problem through a professional development model and recognises the importance of training in the technology area coupled with peer coaching and collegial support.

In this study, the controversies surrounding the diagnosis and management of children with Attention Deficit Hyperactivity Disorder (ADHD) are discussed. The influence of the historical background on the current debate is considered, along with the way that the information about this disorder is accessed, communicated and interpreted by parents and service providers. The experiences of parents whose children have been diagnosed with ADHD are examined in order to understand their needs in terms of service delivery options. The experiences of service providers are also examined to determine the nature of support that is provided. Outcomes of current service provision options are discussed, along with suggestions for improvements in service delivery. The difficulties that parents have had in accessing support and the problems that service providers report highlight the need for an approach to service delivery that fosters cooperative partnerships between parents and service providers. Finally, a vision which incorporates the suggestions from parents and service providers and thc findings from current researchers is proposed.

The aim of this study was to investigate the importance, frequency in practice, and readiness of job functions from the perspectives of supported employment specialists in Taiwan. Considering the unique developing process of supported employment practice (SES) in Taiwan, the instruments used were the Job Placement Job Task Inventory and the Rehabilitation Skills Inventory, which consisted of 11 subscales and 169 items in total. Using a total sampling survey approach, the research population was the 260 supported employment specialists who were employed by the SES project funding in 2012. The data collection sessions began on the first of March, and lasted till April 12, 2012. A total of 151 valid respondents were collected, with a response rate of 58%. Data collected consisted of their self-assessments of importance, frequency in practice, and readiness of job functions. Correlations among the three variables as well as differences due to demographic characteristics were analyzed. The result revealed three findings as follows. First, the job functions that supported employment specialists valued were job placement and rehabilitation counseling oriented, and their self-assessments of readiness were acceptable. Second, use of diverse systems to facilitate the job placement was the only job function that was less valued and seldom implemented, but considered so-so in readiness. And third, all of the three variables appeared to be moderately to highly correlate. Suggestions were provided based on the study findings. For the practitioners, a formal training and professional development curriculum and a clinical supervision system were recommended. For policy-makers, the development of supported employment specialists’ licensure and the standard revision of SES program evaluation were recommended. Abstract:
The aim of the diploma thesis is to outline the possibilities of information support of project teams by an Informational Specialist. The theoretical part introduces different project management methodologies and further specifies the individual project phases as seen by PMBoK methodology. In the chapters focusing on each of the phases of a project, all necessary input sources as well as all output documents that are essential for the project to move on to its next phase are defined and elaborated on. This part is followed by a description of possibilities of information support for working with information sources and creating output documents with regard to competencies, which an Information Specialist should have. Key in this respect is the third and the fourth chapter, including specific proposals on the possibility of information support of a project team by an Information Specialist. The practical part is focused on the role of an Information Specialist in the implementation of a specific project in a company - in particular, a major financial institution on the Czech market. While the theoretical part is based on the study of literature, the practical part is mainly based on personal consultation with a Project Manager of the company.
