ABSTRACT Auditors often rely on the assistance of specialists from such fields as tax, information technology, valuation, and forensic accounting. Integration of the work of specialists with the work of audit team members is a challenge for both groups. This interview-based study of 34 practitioners from six accounting firms, including 12 auditors (partners and managers) and 22 specialists (tax, IT, valuation, forensic) examines auditors' and specialists' views about the current state of specialist use on audits. The regulatory environment creates pressure for financial statement auditors to use specialists on audits; however, financial statement auditors often seek to limit specialist involvement. Both auditors and specialists are dissatisfied with the current situation, but for different reasons. Auditors are concerned about budget overruns, delays, and harm to client relationships by (overly) meticulous specialists. Specialists are concerned about auditors limiting the scope of specialist involvement, and its effect on audit quality. JEL Classifications: M4; M40; M42.

As rural Queenslanders are isolated geographically due to dispersed populationpatterns, they are often required to travel long distances to access services, especiallyservices of a specialist nature. The distress of this relocation for treatment is particularlyintensified for patients with leukaemia and associated haematological disorders andtheir carers, as they must often relocate for long periods of time and face invasive anddemanding treatments away from the comfort of their own homes. Because suchtreatments are now highly technical and specialised, even patients from moreurbanised areas are also required to relocate for prolonged specialist treatment notavailable locally. Consequently, for many rural and urban patients with leukaemia,relocation for specialist treatment is a major concern.This discussion presents findings from recent research on a Queensland Governmentinitiative, the Patient Transit Assistance Scheme, designed to address this concern.These findings indicate a high level of hardship for these patients and their familieswho must travel long distances, often relocate for long periods, and endure additionalfinancial burdens at a time when a majority are dependent on government assistance.

The effective application of a Decision Tree (DT) process is beset with many difficult and technical decisions about the choice of algorithms, parameters, evaluation, etc. Therefore, we propose assistance by using ontologies for addressing the above-mentioned challenges that face the non-specialist DT miner (person). Ontologies have been used in various research areas such as computer science, including data mining tools. In this paper, we propose the realisation of a domain ontology for DT OntoDTA to empower the non-specialist DT miner throughout the key phases of the DT process. OntoDTA ontology contains the knowledge of DT and provides a common terminology that can be shared and processed by DT miners.

Abstract:
The article examines the complex problems of the participation of a specialist in the production of procedural actions during the preliminary investigation. Conclusions are drawn about the need for a clear distinction between the functions of a specialist and an expert in the process of involving them in investigative actions, about the independence of a specialist in criminal proceedings. The authors also consider it necessary to classify such participants in the criminal process as a teacher and a psychologist as specialists. It is noted that the criminal procedure legislation does not require a detailed differentiation of the functions of a specialist, since, depending on the type of investigative action, the assistance of a specialist can be of a diverse nature. In various investigative actions and at their various stages, it can be of a consulting nature or the nature of performing physical actions that require special knowledge, skills and abilities.

Objective: The present paper aims to provide an overview of the Medical Specialist Outreach Assistance Programme (MSOAP) and its implementation in South Australia with particular reference to the expansion of visiting psychiatric services to rural and remote areas. Included is a discussion of a number of the practical issues and challenges experienced by the author in the development of a visiting psychiatric service to the remote community of Coober Pedy in northern South Australia. Conclusions: There has been much success to date with the expansion of visiting psychiatric services to rural and remote areas within South Australia under MSOAP. MSOAP appears to have considerable merit, particularly for psychiatrists working in private practice. There are several practical issues to be considered in taking on this type of work. The professional rewards are substantial.

The analysis of the results of the introduction of the professional standard "Specialist in Working with the Family" in social organizations is given. It is emphasized that the complexity of the work of a family work specialist is related to the need for interaction with specialists from different departments, while the organization of rendering assistance and support of families with children is interdepartmental. The role of interdepartmental and interdisciplinary approaches in solving family support problems, providing it with various types of assistance, protecting the rights and legitimate interests of children, and social support in accordance with the generalized labor functions of the professional standard are pointed out. The factors influencing the introduction of the professional standard in the Russian Federation, as well as the main directions of the strategy for applying the professional standard, are described. The process of organizing the activities of a specialist in working with the family in the field of prevention of social orphanhood - the development, approbation and dissemination of technologies for working with the family, adapting the best foreign practices for working with the family in Moscow, is described.

PANDA, the Pumper Apparatus Novice Design Assistant, is a case-based design system developed to assist firefighters who wish to design their pumper engines. In contrast to other such systems, PANDA addresses the unique needs of a novice, non-specialist who performs design in a highly specialized domain, where the design is decomposable into elements which each fulfill their own identifiable function. In PANDA we study how to create an interactive case-based design assistant that can understand the needs and desires of a non-specialist designer and can translate them into formal specifications; that can provide assistance by using case-based design methodologies; that can deal with non-functional design criteria such as aesthetics and traditional practices; and that can guide the novice designer by discussing alternatives, tradeoffs, and adaptations. Our prototypical system verifies our methodological approach to supporting novice design activities.

Abstract:
The article suggests that there are two different levels of skills and opportunities for professional interaction between a helping specialist and subjects of the educational process. The first level of skills involves ensuring safe interaction through the manifestation of tolerant attitudes and behaviour through the ethical regulation of activities within a professional role. The second level involves the emotional, personal inclusion of a specialist in professional interaction through the manifestation of special personal qualities (mercy, empathy, empathy), which provides a healing effect through deep personal interaction. Weanalyse Russian socio-cultural and ideological characteristics of help, based on mercy and empathy. European and Russian ideas about the content of the training of specialists in helping professions, going back to the peculiarities of the religious beliefs of three Christian denominations, which define the meaning of assistance both in the east and in the west, are compared. It is shown that the Russian experience of assistance focuses on the internal motives of help and sacrificial love, which provides a deep personal level of professional interaction. The conclusion is made about the need for a scientific study of traditional Russian experience in order to create programmes for developing future helpers with personal readiness education for effective professional interaction at the second level.

Introduction. The Health Education Specialist Practice Analysis II 2020 (HESPA II 2020) described the contemporary practice of health education specialists across work settings at entry and advanced levels. The purpose of the HESPA II 2020 manuscript was to report the research process and resulting data associated with the HESPA II 2020 and to provide recommendations for future practice analyses in health education. Method. Two data collection instruments were developed with the assistance of a 17-member Health Education Practice Panel to survey practicing health education specialists on the knowledge and skills needed in their ongoing roles. The instruments were designed to assess the degree to which the elements of the model had importance to practice ( Importance), how often health education specialists performed the elements of the model ( Frequency), and the point in time that health education specialists were expected to perform the various elements of the model ( Performance Expectation). Composite scores for Sub-Competencies were calculated and subgroup comparisons were conducted to distinguish between levels of practice. Results. A total of 3,851 health education specialists provided usable responses. The final validated hierarchical model included eight Areas of Responsibility, 35 Competencies, and 193 Sub-Competencies. Of the Sub-Competencies, 114 were Entry level, 59 Advanced 1 level, and 20 Advanced 2 level. In addition, 145 knowledge items were verified. Discussion. HESPA II 2020 produced a validated hierarchical model descriptive of current practice of health education specialists across work settings. The results have implications for professional preparation, professional development, and certification.

A postal questionnaire survey was undertaken to explore the views of specialist registrars and academic programme coordinators identified nationally with regard to their local academic programme.ResultsSixty per cent of specialist registrars (152 out of 253) and 90% of coordinators (17 out of 19) responded. All schemes offered an academic programme with protected time. Teaching methods were diverse, and satisfaction varied within and between schemes, with trainees reporting greater satisfaction associated with active involvement of coordinators.Clinical ImplicationsThe authors provide recommendations for local planning of academic programmes. The authors request the assistance of CAPSAC in standardising the appointment and training of coordinators and facilitating their release from clinical commitments.

This paper proposes a walker system with power assistance device for standing-up motion. Our system focuses on family use for aged people who need nursing in their daily life. Our key ideas are based on two topics. The first topic is new assistance manipulator mechanism with four parallel linkages. Our proposed manipulator mechanism requires only smaller actuators and realizes rigid structure with lighter linkages compared with general manipulator. Thus, we can design our assistance system compactly with low-cost using our mechanism. The second topic is the combination of force and position control. According to the patient's posture during standing-up, our control system selects more appropriate control method from them. Furthermore, the walker coordinates its assistance position and maintains the stable posture of the patient during standing assistance. We use the reference of standing-up motion which is based on the typical standing-up motion by a nursing specialist for realizing the natural assistance. The performance of our proposed assistance system is verified by experiments using our prototype.

This relatively new role in New Zealand is modelled on the role of the intermediary in England and Wales. This research provides a qualitative analysis of professionals’ perspectives (n = 28 participants) on the challenges of communication assistance for young people facing criminal charges in the New Zealand youth justice system. The findings of this study do not question whether or not communication assistance should exist, but rather how it might best function in practice. The overall implications are that more education and guidance for youth justice professionals is needed.

The aim of this work was the development of a surgical assistance system based on augmented reality to support joint replacement procedures and implantation of prosthetic knee. Images of the scene were captured in order to detect the visual markers located on the lateral surface of the patient’s leg for overlapping the 3D models of the prosthesis and the joint, as well as the tool used by the medical specialist. With the marker identification, it was possible to compute its position and orientation for locating the virtual models, obtaining a monitoring system for giving accurate information about the procedure. Also it can be used as training platform for surgeons, without having volunteers or patients for performing real surgeries; instead they can train in a virtual environment. The results have shown an efficient system in terms of cost-benefit relation, taking into account the materials used for developing the system; nevertheless, the accuracy of the algorithm decreases according to the distance between the markers.

Abstract:
The article shows the effectiveness of informal education assistance as a method of young specialist`s anxiety overcoming study. The sample consisted of 150 people between the ages of 21 and 49, divided into three groups. The first group included 44 people, short-term students, the second group (60 people) – middle-term students, and the third group (46 people) – long-term informal education assistance courses. We used Spilberger-Khanin`s anxiety test to define the level of personal and situational anxiety. To define professional difficulties, we used a half-structured interview and self-report. Short-term educational programs students have a low level of personal anxiety and middle-level situational anxiety, which changed slightly after the educational program (the difference is insignificant). The members of this group, as usual, have about 2 years of work experience and apply for academic help on specific professional issues. Middle-term educational programs participants mostly do not have work experience are somewhat confused about their professional specialization (for example: «I want to work with children or with adults, I do not know», «I want certain instruments, but I am not sure if I need them»), do not see the point of an in-depth study that or another theme. At the beginning of the course, they have a high level of personal and situational anxiety. They try to get the basics for the future work, quite quickly overcome the confusion, in studying situational anxiety of this group decreased with a significant difference (p < 0.05). Examined members of long-term educational programs have a middle level of individual and situational anxiety at the beginning. They try to understand many professional issues and feel tension and confusion. Situational anxiety in this group decreased with a significant difference (p < 0.05) in the process of studying. Members of this group feel the necessity to set goals and to plan their professional activity (for example: «I do not know where to start», «I do not understand why we need to do this», «I need consistency in work»), that is why they choose programs with a significant substance and content. The proposed system of educational assistance is based on the problem-oriented approach. We have identified the lack of beginner’s practical skills, tools, and professional activity techniques as the main problem that leads to difficulties during a professional crisis. We suppose that overcoming this deficit affects changes in specialists’ activity quality and their psycho-emotional condition. Our study results suggest that the proposed program of informal education has a positive effect on overcoming anxiety during the professional development crisis.

Aim: To explore how the Canadian Medical Assistance in Dying legislation affects end-of-life care discussions between patients and multidisciplinary specialist palliative care providers. Design: Qualitative thematic analysis of semi-structured interviews. Participants: Forty-eight specialist palliative care providers from Vancouver (n = 26) and Toronto ( n = 22) were interviewed in person or by phone. Participants included physicians ( n = 22), nurses ( n = 15), social workers ( n = 7), and allied health professionals ( n = 4). Results: Qualitative thematic analysis identified five notable considerations associated with Medical Assistance in Dying affecting end-of-life care discussions: (1) concerns over having proactive conversations about the desire to hasten death, (2) uncertainties regarding wish-to-die statements, (3) conversation complexities around procedural matters, (4) shifting discussions about suffering and quality of life, and (5) the need and challenges of promoting open-ended discussions. Conclusion: Medical Assistance in Dying challenges end-of-life care discussions and requires education and support for all concerned to enable compassionate health professional communication. It remains essential to address psychosocial and existential suffering in medicine, but also to provide timely palliative care to ensure suffering is addressed before it is deemed irremediable. Hence, clarification is required regarding assisted dying as an intervention of last resort. Furthermore, professional and institutional guidance needs to better support palliative care providers in maintaining their holistic standard of care.

The regulation of specialist medical appointments represents one of the problematic areas of the Brazilian Public Health System. In this regulation process, 2 issues stand out: 1) which patients should have the highest attendance priority, and 2) which service suppliers can best resolve the specific health problem of a patient? Based on the consideration of the existing Brazilian context in the field of medical assistance, this study proposes a model designed to aid regulation centers deal with the decisions related to the process of allocating specialist medical appointments. The model integrates multicriteria decision analysis and linear programming for the specialist medical appointment allocation, in which the allocation of patients is defined as a function of the relative significance of a set of criteria related to the notion of effectiveness of the specialist medical care and the capability of the accredited specialist health care units. The integrated model was implemented in a computer-based system and validated using cardiology and vein surgery data from the regulation center in Porto Alegre, Brazil. The validated computational system was applied to mammography services in another regulation center. The system successfully implemented a prioritization scheme, decreasing significantly the examination waiting time of severe cases.

Under such conditions, specialists in physical therapy and ergotherapy become subjects of medical, social and psychological assistance, associates of the improvement of physical qualities, psycho-emotional stability and adaptation reserves. The aim of the study is to develop and substantiate the medical aspects of training a specialist in physical therapy and ergotherapy in the system of higher education in Ukraine. Materials and methods: To achieve the goal, a set of research methods, in particular theoretical ones, is used: historical-comparative and logical, terminological, quantitativequalitative (bibliometry), generalization, abstraction, analysis and synthesis, modeling, studying documents; empirical: observation, survey, analysis of products of activity, complex pre-nasol diagnostics. Conclusions: The theoretical synthesis of domestic and foreign experience is carried out and the author’s approach to solving an important and actual scientific problem of forming the medical competence of future specialists in physiotherapy and ergotherapy is proposed. An analysis of the international experience of training physical rehabilitation specialists has made it possible to identify the most promising American and European models that have a number of significant differences in the principles of organization and financing of pedagogical processes, professional etiquette and self-discipline of specialists.

Background. The article deals with measures for providing medical and social assistance with the aim of preservation of health, improvement of the quality of life of the elderly and senile, and of their adaptation in the society. Aim. study of the opinion of elderly and senile people of the rendered medical, social and geriatric care. Materials and Methods. A survey of 456 residents of the Moscow region older than working age was conducted. All respondents at one time were patients of the Moscow Regional Hospital of Wars Veterans. Results. The majority of respondents noted availability of various types of medical care, but only a small part of them evaluated its quality positively. Among the reasons for dissatisfaction, the most frequently cited were the lack of specialist doctors, spending much time in the waiting room in visiting therapists and narrow specialists, ineffective treatment, payment for necessary medical services, waiting lists for inpatient treatment, and in some cases violation of the patients rights. Dissatisfaction with medical and social assistance was mainly related to the problems of disability registration and implementation of individual rehabilitation programs. Conclusion. The results of the survey for studying satisfaction of older people with medical, social and geriatric care should be used for elaboration of proposals for its improvement.

The Disability Specialist intervention utilized a variety of approaches, including: a) increasing awareness of disabilities and services by providing learning opportunity sessions for families and staff, and outreach activities to youth through interactive theater; b) developing in house “disability specialists” to offer ongoing leadership and technical expertise for after-school programs and their community centers; c) developing a network of technical consultants in order to connect families and after-school programs to specialized community resources; d) providing financial assistance to enable community center staff to allocate time to outreach activities; and e) providing families with support in educational advocacy efforts by partnering with a local parent mentoring program. The success of the project depended upon building partnerships with families, community centers, human service agencies, schools, and local funding sources.

Abstract Nutritional management is a crucial component of animal production and enterprise economic sustainability. Nutritional value of feedstuffs and nutrient requirements of livestock are often misunderstood by producers and feedstuff providers. Extension specialists support the continuing education of the livestock industry by communicating nutritional research results to county agents and clientele. A survey of Extension specialists in the ASAS Southern Section was conducted to understand the current state of nutritional education strategies in the region. The goal was to identify: 1) specialist background knowledge, 2) industries specialists serve, and 3) processes, tools, and resources used to maximize nutritional efficiency. The survey was answered by 25 specialists from 13 states. Among the 13 states, 1 state has 5 or more specialists providing nutritional assistance, while 6 indicated only 1 or 2, and the average number of specialists was 2.8 per state. Of the specialists, 13% indicated their background was feedlot nutrition, 47% forages, and 40% both. Animal industries supported included 85% for beef cattle, 3.5% for sheep and goats, and the remainder for other species such as dairy and horses. Currently, 9 states offer forage/feed analysis, and in the states that do not offer the service, specialists indicated that commercial laboratories were utilized. More specialists indicated they may require a forage analysis (70%) compared to grain/byproduct feeds (55%) before providing recommendations. When formulating feed recommendations, specialists indicated that 33% were performed by hand and 67% by computer program. All states indicated that they perform at least 1 agent and (or) stakeholder training per year on ration balancing. Additionally, when asked to characterize the nutritional knowledge of agents, specialists indicated 16% were capable of handling most nutritional issues, 23% could handle some issues, 41% had minimal familiarity with animal nutrition, and 19% were considered focused on other areas. These results indicate specialist knowledge base and tools to understand feedstuffs are present in the region. Stakeholders may benefit by understanding information required for nutritional assessment and recommended changes to meet production goals. Additionally, there is potential to work across state lines to utilize expertise in specific areas.

Each teacher-psychologist faces the task of effectively organizing a system of correctional and developmental classes with pupils and students with special needs. To solve it, a team of psychologists from four educational organizations in Moscow (Secondary School named after Artyom Borovik, Secondary School No. 1357 “On Bratislavskaya”, Secondary School No. 1547, Maryinsky Secondary School No. 1566 in memory of the Heroes of the Battle of Stalingrad) and the specialists of the City Psychological and Pedagogical Center of the Department of Education and Science of Moscow have developed the so-called “Work Programs Constructor” technology (hereinafter referred to as the Constructor). This article discusses structural features of the Constructor, which allow the specialist to take into account the full range of correctional and developmental assistance for children with disabilities when developing the program, taking into account their special educational needs and age. The issue of ensuring the continuity of work programs at different age stages in the implementation of each direction of correctional and developmental work is touched upon. When analyzing the substantive aspects of the Constructor, special attention is paid to the consideration of the age development norms as guidelines for the organization of correctional development work and indicators of its effectiveness. The article reveals how this technology provides a unified approach to helping children with special educational needs.

This study is based on a corpus of 110 dialogs recorded in a medical assistance telephonist's workstation. Given the nature of the task, we have considered five main variables in dialog (topics) as well as their order of occurrence (sequences). These data were analyzed with a lexical analysis program. Results show a great difference between Operator/Specialist dialog and Operator/Nonspecialist dialog. Dialog “script” is very strong in the first case in which the operator merely plays a feedback role (routine procedures). If the caller is a private individual, the situation is often an indefinite problem, and the operator may have to adapt to the person (weak script) to obtain relevant information as quickly as possible (problem-solving procedures). This provides confirmation of the operator's twofold competence (efficient decision-making, dialog management).

Through the use of a HTML editor and the assistance from staff knowledgeable about the WWW, a web site on a main campus server was created by untrained off-campus personnel at a remote site to provide information on commercial vegetable production. Planning of the web site included targeting potential audiences, determining what information would be shared from the site, and building a team of extension and research personnel that would provide information and act as reviewers for web publications. Assistance in the form of graphics and general know how were supplied by departmental specialists. After development, the web site was maintained and refined on a regular basis through the utilization of the HTML editor and a web file transfer program.

Objective: to characterize assistance of 88 elderly with venous ulcers attended in the tertiary and primary levels of health service in Natal city. Methods: integrative and descriptive study, from quantitative approach, with retrospective data of patients with venous ulcers who were studied in two dissertations presented at the Post-Graduate Program in Nursing/UFRN and a search report of CNPQ. Data was carried out from 2005 to 2007 through structured interview, physical examination and observation of the exchange of dressing, being organized in Excel and transported to the SPSS 14.0. These studies were approved by the Committees of Ethics in Research of the Federal University of Rio Grande do Norte (55/05; 54/05; 196/06). Results: we studied 88 elderly, mainly female sex (68,2%), literate (81,8%), outcomes two basic salary (69,3%), (52,3%) with edema (69,3%) and pain (86,4%). The main mismatches in assistance were: dressing kit (71,6%), angiologist consultation (60,2%) and monitoring specialist (56.8%). Conclusion: the assistance was considered inappropriate, mainly in the tertiary level, fragmented, with low resolution level and integration between the levels of complexity of the Public Health System. The research highlights current aspects of the institute of special knowledge in the detection and investigation of felonies. Based on a thorough analysis of the views of prominent forensic scientists, based on current legislation, the concepts of special knowledge, forms and types of their use. Given the different professional competencies, the concept of special knowledge of an expert and a specialist is considered separately. Among the typical forms of use of special knowledge, attention is focused on those that are essential for the investigation of serious crimes, adhering to their classification into procedural and non-procedural. It is emphasized that one form of use of special knowledge passes into another, in particular, after forensic examinations, an audit is performed. Appointment and conduct of forensic examinations are covered depending on the type of serious crime (mercenary-violent, economic, etc.). Attention is paid to the choice of expert institution, the sequence of appointment and conduct of comprehensive forensic examinations. Some issues of specialist participation in the conduct of investigative (search) actions and measures of criminal proceedings are considered. The need for professional assistance during the inspection, search, investigative experiment, temporary access to things and documents at the preparatory stage, to record the progress of the investigative action, to work with traces and other evidence, during the evaluation of the results of the investigative action. The involvement of a psychologist as a consultant or specialist to compile a search psychological portrait of a criminal is considered. The significant role of departmental inspections in the investigation of serious crimes in the economic and financial spheres has been noted. Attention is paid to the expediency of involving a group or «team» of various specialists to participate in investigative (search) actions, the use of innovative technical developments for forensic purposes. Examples of practical activities and interview data of practical staff are given.

The papers captured 43 diagnoses, 26 interventions and 78 nursing results depicting human responses in their singularities. Final considerations: the validated forms fill a gap by enabling the capture of human responses from people on hemodialysis and by subsidizing the planning of nursing care on a scientific basis.

From the perspective of modern pedagogy, the appreciation of each person and, consequently, the inclusion with him, should constitute a principality not so much of theory as of teaching practice. Entity-oriented concepts direct attention among others on the issue of implementation of the idea of inclusive education. The theories of inclusive education demand bold pedagogical concretization. Inclusion is not just a slogan idea of education, but a series of activities, ranging from social attitudes towards people with disabilities, through the construction of an individualized teaching process to specialist student support. The article presents the ideas of inclusions in education, in confrontation with the reality of modern school. The overarching goal became to popularize knowledge about the model of education of a student with disabilities in a public school, with the implementation of the idea already at the planning stage, and then responsible specialist assistance and the implementation of an individualized teaching-learning process.

AbstractIntroduction:Large substernal goitres present a challenge to the otolaryngologist due to their size and location. Predicting which patients will require sternotomy can enable planning of surgery in a specialist centre, with the assistance of a thoracic surgeon. Our aim for this study was to establish clinical and radiological predictors which would aid the otolaryngologist in the pre-operative planning of such cases.Methods:A retrospective medical record review was conducted for all patients undergoing thyroidectomy for substernal goitre who required sternotomy in our institution over a 10-year period.Results:During the study period, 140 patients with substernal goitres underwent thyroidectomy. Three patients (2 per cent) required sternotomy. These cases are described.Conclusions:Radiological evidence of extension of a substernal goitre to the aortic knuckle, or loss of tissue planes on computed tomography, should raise suspicion that the patient may require sternotomy for safe delivery of the gland. The otolaryngologist should plan surgery for these patients in a specialist centre, with the help of a thoracic surgeon.

Abstract:
One hundred patients with active rheumatoid arthritis, uncontrolled by symptom relieving agents, were allocated randomly to intramuscular gold or oral penicillamine therapy. Their general practitioners (97 in all) were requested to assist with blood, urine, skin and mucous membrane monitoring. After four years, 16 patients in each treatment group remained on their original therapy, and three-quarters of these continued attending their family doctor for toxicity monitoring. Most GP monitoring problems occurred in the first six months of treatment. Sustained improvement in haemoglobin and ESR was seen in both treatment groups. Second-line drug toxicity monitoring by general practitioners allows specialist time to be diverted to rheumatological problem solving but may make it more difficult to relate rare but serious adverse adverse events to drug therapy.

Objectives: We described the duration from symptom debut to assessment at specialist healthcare outpatient clinics for dementia in Norway and explored whether educational level was associated with time from symptom debut to dementia assessment. Methods: The study comprised 835 persons from a register for individuals with cognitive symptoms (NorCog). The outcome variable was time in months from symptom debut to assessment. The main independent variable was the number of years of education. Also age, gender, marital status, cognitive function, neuropsychiatric symptoms, assistance and location were assessed. Results: In an adjusted linear mixed model, a higher educational level was associated with a longer duration from symptom debut to assessment, where 5 additional years of education increased the time from symptom debut to consultation by 10%. Conclusion: The findings may perhaps be explained by the hypothesis that highly educated people may be able to compensate better for cognitive impairment, which is in line with a hypothesis of cognitive reserve.

Abstract:
The article analyses the levels of communication skills of future social workers at the final stage of the research. It is stated that implementation of communication and speaking training is conditioned by the necessity of forming a competent specialist who has deep theoretical knowledge, communication skills necessary for establishing constructive interpersonal interaction and providing assistance to different groups of clients, and operates innovative technologies methods of assistance. It is possible to achieve a high level of professional competence during training, through a specially organized educational process aimed at the development of professional, communication and speaking skills that form the basis of future work of a social worker. The results of the study showed positive changes in the students of the experimental group, which made it possible to conclude on the efficiency of the proposed experimental method of forming the communicative and speaking competence of future social workers in the process of professional training.

The problems of category of population at social risk - orphaned children and social orphans - are viewed. According to statistics, their number continues to grow, driven by a complex set of social, economic and psychological factors. Adoption is the most humane way to support these children, but it isnt the only way, for a number of objective and subjective reasons. Specialist agencies are still required for the provision of social, psychological and medical assistance to certain categories of children. Existing institutions are obsolescent, and therefore an urgent review of their typological structure and functional planning is demanded.

This paper suggests that Government plans to increase childminding provision in areas of deprivation may be seen as an entrepreneurial opportunity for lone parents in receipt of welfare benefits. But if it is to offer a route out of poverty, they will need integrated, specialist and continuing assistance to negotiate the fragmented systems currently associated with business and personal finances, and to understand the inter-connections between them. The findings from a small-scale feasibility project are presented as the starting point for the development of a recruitment and support strategy which foregrounds the financial aspects of childminding.
