Full textParkinson’s disease (PD) is a neurodegenerative disease characterized by motor disorders. However, non-motor symptoms, including cognitive impairment, are also part of the clinical presentation. According to the severity of cognitive impairment, three presentations are usually distinguished in PD: (a) the absence of significant cognitive impairment despite possible symptoms, (b) mild cognitive impairment (MCI) and (c) PD dementia. MCI refers to significant cognitive deficits without global cognitive decline nor impact on activities of daily living. This condition is common in Parkinson’s disease (PD-MCI), can affect one or several cognitive functions and is heterogenous. According to the dual syndrome hypothesis, PD-MCI can be subdivided into two cognitive subtypes: a frontostriatal one, characterized by attentional and/or executive deficits, and a posterior cortical one, characterized by visuospatial, memory and/or language deficits. The latter has been associated with a higher risk of developing dementia earlier. To date, only few studies have considered the cognitive heterogeneity in PD-MCI and no study defined PD-MCI subtypes based on the dual syndrome hypothesis. Besides, in-vivo biomarkers of these cognitive subtypes are lacking.The main objectives of this thesis were (a) to propose a state-of-the-art on neuroimaging outcomes associated with distinct PD-MCI cognitive subtypes, and (b) to identify structural and functional MRI brain changes associated with the frontostriatal and posterior cortical subtypes.Therefore, we performed a systematic review which showed a gap in the scientific literature given that only ten neuroimaging studies considering PD-MCI subtypes were identified. Thereafter, we conducted two studies to identify structural and resting-state functional MRI modifications in PD-MCI subtypes. We used data from non-demented PD patients (n=114) whose cognitive subtype was determined by their cognitive performance at a comprehensive neuropsychological test battery: (a) patients with normal cognition (PD-NC) (n=41), (b) patients with a frontostriatal subtype (PD-FS) (n=16), (c) patients with a posterior cortical subtype (PD-PC) (n=25) and (d) patients with a mixed subtype (PD-MS) (i.e. combination of frontostriatal and posterior cortical deficits) (n=32). For functional analyses, data from 24 age-matched healthy controls were also used.Our results showed (a) more abundant and more extensive structural alterations in patients with posterior cortical deficits (PD-PC and PD-MS), (b) increased functional connectivity within the basal ganglia in PD-PC patients and (c) decreased functional connectivity in various resting-state networks in patients with frontostriatal deficits (PD-FS and PD-MS). Further longitudinal studies are needed to assess the progression of these structural and functional modifications and to determine the predictive potential of these markers regarding the risk of developing dementia

Full textL'auteur se propose d'appréhender et de catégoriser les spécificités des manifestations langagières, discursives et dialogiques de patients schizophrènes dans l'interaction verbale. Nous élaborons des modèles pragmatique et dialogique permettant de mettre au jour le plus objectivement possible les discontinuités qui jalonnent le discours schizophrénique en développant la notion de "pathologie pragmatique". Ces modèles sont confrontés aux spécificités syndromiques de la pathologie schizophrénique tout en contrôlant l'impact des antipsychotiques et leur répercussion sur les spécificités mises au jour. Ces modèles sont particulièrement discrimants pour rendre compte des spécificités associées à la cohérence de la production discursive et dialogique de patients schizophrènes de sous-type clinique paranoïde et désorganisé. L'utilisation des antipsychotiques en tant qu'outils d'investigation se révèle pertinente pour explorer la nature des processus cognitifs élémentaires et complexes intriqués aux troubles. Ces recherches contribuent ainsi à l'élaboration d'instruments diagnostiques qui viennent discuter et compléter les critères utilisés classiquement dans les différentes classifications et laissent entrevoir la possibilité d'opérationnaliser la notion de "Troubles Formels de la Pensée". Ce genre d'investigations permet d'interroger d'autres courants épistémologiques et théoriques contemporains dans la littérature scientifique psychologique ou cognitive, de sorte qu'on parvienne, à plus ou moins long terme, à "d'autres" modèles explicatifs ou interprétatifs des troubles mentaux, voire à poser les prémices d'un nouveau type de théorisation en psychopathologie

Full textThe concept of Mild Cognitive Impairment (MCI) distinguishes between amnestic (a-MCI) and dysexecutive sub-types (d-MCI). These particular ones have been kept out of the research work, and their cognitive profile remains little defined. In addition, some cognition areas are still poorly known, like prospective memory (PM), ability to remember to realize intentions in the future. The aims of this work were to characterize and study the evolution of the neuropsychological impairment in a-MCI and d-MCI and examine their activation profile to cognitive tasks in functional MRI. We tried to differentiate the MCI sub¬types according to their performance profile to a PM task. 1st studies allowed to observe that a-MCI are more impaired at encoding and retrieval processes in episodic memory, and show difficulties in strategies initiation. The d-MCI showed impairments in working memory, in free recall tasks of episodic memory, and in instrumental activities. The 2nd study reveals a more significant progression, at 1 and 2 years, of the MCI than of the controls, the a-MCI developing quite often dysexecutive deficits. The 3rd study reveals that whereas the a-MCI are indeed those with the most trouble remembering the intentions in MP, the d-MCI actually realize less action. The 4th study seems to show an increase in the recruitment of the cerebral areas within MCI subjects compared to a control one. The results of this research result in putting into light the role of the executive processes and of the frontal functions within the MCI, their involvement in the PM performance and the contribution of the PM performance profiles in the early distinction of the a-MCI and d-MCI
