Abstract:
A syringe driver, or pump, represents a simple and cost-effective method of delivering a continuous subcutaneous infusion (CSCI). A CSCI provides a safe and effective way of drug administration and can be used to maintain symptom control in patients who are no longer able to take oral medication. There have been several developments in this field since the third edition of this highly successful book. The text in this edition has been completely revised, including a new chapter describing the compatibility and stability of drugs in addition to incorporating new treatment options and an extensive list of new compatibility data. This book serves as a valuable reference source, providing a comprehensive review of syringe pump use and administration of drugs via CSCI. The first chapter provides an overview of syringe pumps and CSCIs, including a useful array of frequently asked questions and reference to needlestick injuries. The new second chapter discusses the reasons why drugs in solution are at risk of compatibility and stability issues. The third chapter incorporates revised and referenced information relating to most drugs likely to be administered via a CSCI using a syringe pump. The fourth chapter briefly discusses the control of specific symptoms that are often encountered when CSCIs are required. The fifth and final chapter contains an extensive, referenced (where possible) list of physical and chemical stability data relating to drug combinations administered via CSCI.

Abstract:
This chapter describes methods to analyze the scalp electric field recorded with multichannel electroencephalography (EEG). With advances in high-density EEG, systems now allow fast and easy recording from 64 to 256 channels simultaneously. Pattern-recognition algorithms can characterize the topography of scalp electric fields and detect changes in topography over time and between experimental or clinical conditions. Methods for estimating the sources underlying the recorded scalp potential maps have increased the spatial resolution of EEG. The use of anatomical information in EEG source reconstruction has increased the precision of EEG source localization. Algorithms of functional connectivity applied to the source space allow determination of communication between large-scale brain networks in certain frequencies and identification of the directionality of this information flow and detection of crucial drivers in these networks. These methods have boosted the use of EEG as a functional neuroimaging method in experimental and clinical applications.

Abstract:
The focal point of this chapter is the notion of “drive” (Trieb), akin to “instinct,” which becomes a primary explanatory concept in the eighteenth and nineteenth centuries, especially in the work of Blumenbach, Spencer, Schopenhauer, and Nietzsche. Drive plays a central role in three distinct areas: embryology, ethology, and metaphysics. In embryology, it describes a force, inaccessible in itself but whose results are visible and susceptible to scientific and philosophical study, governing organic development. In ethology, drives are the sources of seemingly deliberate, highly articulated, yet nonconscious activities, which are directed at ends of which the animal is ignorant. In metaphysics, drive describes the human essence. This chapter focuses on the way in which the emergence of the drive concept undermines the idea that there is a sharp distinction between humans and animals, and concludes by examining how the blurring of this line reshapes ethical theories.

This chapter explores some of the strategies used by event-driven hedge funds, namely merger arbitrage, trading distressed securities, special situations, and activism. This broad category within the hedge fund space attracts about a quarter of the capital deployed to this part of the alternatives world. Investors are drawn to the idea of uncorrelated returns that can act as a source of diversification for their portfolios as well as the ability to follow the news flow related to their investments. In essence, such trades should have identifiable catalysts and time frames. The chapter offers illustrative examples of historical trades, providing some context of the types of positions funds may take and time frames involved. Various skill sets should be sought in an event-driven manager. Managers dealing in distressed securities are likely to benefit from a legal expertise, whereas activists need to be able to influence management and campaign publically.

For the past 13,000 years Indians in the North American Great Plains hunted bison (Bison bison and B. antiquus) in large communally organized drive operations. This chapter briefly describes the taxonomy of fossil and living bison, the behaviour of modern bison, and what is known from ethnohistoric and archaeological sources about the ways that Indians conducted these drives, including the use of foot surrounds, cliff jumps, arroyo traps, and pounds (corrals). The chapter concludes by considering whether such drives were conducted annually in the late fall and/or early winter as a means of winter provisioning; or instead were conducted periodically, but not necessarily annually, and at many different times of year, as a socio-political mechanism for integrating otherwise widely dispersed and highly mobile hunting bands.

The Drive-In meaningfully contributes to the complex picture of outdoor cinema that has been central to American culture and to a history of US cinema based on diverse viewing experiences rather than a select number of films. Drive-in cinemas flourished in 1950s America, in some summer weeks to the extent that there were more cinemagoers outdoors than indoors. Often associated with teenagers interested in the drive-in as a ‘passion pit’ or a venue for exploitation films, accounts of the 1950s American drive-in tend to emphasise their popularity with families with young children, downplaying the importance of a film programme apparently limited to old, low-budget or independent films and characterising drive-in operators as industry outsiders. They retain a hold on the popular imagination. The Drive-In identifies the mix of generations in the drive-in audience as well as accounts that articulate individual experiences, from the drive-in as a dating venue to a segregated space. Through detailed analysis of the film industry trade press, local newspapers and a range of other primary sources including archival records on cinemas and cinema circuits in Arkansas, California, New York State and Texas, this book examines how drive-ins were integrated into local communities and the film industry and reveals the importance and range of drive-in programmes that were often close to that of their indoor neighbours.

Abstract:
In this chapter I provide a methodological framework for the rest of the book to help explain Poland’s long-term economic backwardness, its unprecedented economic success since 1989, and its development prospects. I argue that the existing economic literature largely focuses on the proximate causes of economic growth, but underestimates the fundamental causes of growth—institutions, culture, ideologies, leaders, and luck—which drive economic policies and are ultimately the reasons why some countries are rich while others are poor. I then focus on the role of institutions and culture in explaining economic development. Following Acemoglu, Johnson and Robinson (ARJ), I distinguish between ‘extractive societies’ and ‘inclusive societies’, and argue that it is rare to move from one type of society equilibrium to another. I extend the ARJ institutional framework and show the conditions necessary for the shift from extractive to inclusive societies and vice versa.
