Abstract:
Hypomagnesemia is a relatively common electrolyte abnormality that may produce little to no significant clinical manifestations in patients. Commonly used medications such as proton-pump inhibitors and antidepressants can cause magnesium deficiency. The primary cardiac effect of hypomagnesemia is a prolongation of the Q-T interval. It is exposure to other drugs in the perioperative period and physiologic changes caused by anesthesia and surgery that can further alter cardiac electrophysiology and lead to serious ventricular dysrhythmias. Hypermagnesemia is generally iatrogenic from excessive ingestion, renal failure, or therapeutic administration for preeclampsia. Adverse effects of hypermagnesemia include somnolence, muscle weakness, and slowing of cardiac conduction.

Sleep disorders are an increasingly common reason for referral to the respiratory clinic, and our understanding of the different aetiologies is increasing. The commonest sleep disorder is sleep apnoea, but other sleep disorders can cause similar symptoms. Narcolepsy is a neurological disorder that affects the brain’s ability to regulate the normal sleep-wake cycle and often presents with similar symptoms to obstructive sleep apnoea, daytime hypersomnolence, and disturbed night-time sleeping but is largely underdiagnosed. This chapter discusses a patient who presented with symptoms of daytime somnolence and witnessed apnoeas and details the investigations required for a diagnosis of narcolepsy. It covers the more specialized sleep studies required for a clinical diagnosis and the treatment options available for patients with this condition.

Abstract:
Obstructive sleep apnoea (OSA) is caused by the repetitive closure of the pharynx during sleep, leading to sleep fragmentation and, often, daytime somnolence. Traditionally, it is defined as either the number of apnoeas (complete cessation of breathing for longer than 10 seconds) or hyponoeas (reduction in air flow by >50%) per hour in an overnight sleep study. However, it must be remembered that this definition is arbitrary, and OSA is better viewed as a spectrum with trivial snoring at one end and severe, almost continuous obstruction at the other. In addition to the sleep-study findings, if the patient is sleepy during the day, as defined by the Epworth Sleepiness Scale, then this condition is termed ‘obstructive sleep apnoea syndrome’. This distinction is important, as patients with this syndrome usually warrant treatment.

Abstract:
This book chapter reviews the most common sleep disorders in older adults and their treatment. It begins with a brief review of sleep physiology and then gives an outline on how to take a comprehensive sleep history. Sleep is commonly defined as a periodic temporary loss of consciousness with restorative effects. There are physiological sleep changes related to ageing, but sleep disorders are not part of normal ageing and are often associated with mental or physical disorders, pain and neurodegenerative disease. The most common sleep disorders include insomnia, obstructive sleep apnoea, restless legs syndrome, REM sleep behaviour disorder, excessive daytime somnolence and circadian rhythms disorders. An in depth clinical history, including if possible bed-partner’s information, is the key to diagnosis. Patients need to be informed about the physiological sleep changes and the principles of sleep hygiene. They can benefit from pharmacological and non-pharmacological treatment strategies.

Abstract:
The landmark paper discussed in this chapter is ‘Endosonography-guided celiac plexus neurolysis’, published by Wiersema and Wiersema in 1996. Pain is one of its most distressing complaints of pancreatic cancer, affecting more than 80% of patients with advanced disease. However, the use of opioids and other drugs is often limited by undesirable side effects, which include somnolence, confusion, lethargy, and decreased cognitive function. Intrathecal drug delivery systems, although effective, are often deemed impractical in pancreatic cancer, due to its poor prognosis and the fact that it is often diagnosed late. Tumour infiltration of the coeliac plexus results in pain in the abdomen and back; thus, this area has often been targeted for analgesia via a neurolytic coeliac plexus block. The paper by Wiersema and Wiersema examines the efficacy of an approach that uses ultrasound to guide needle placement in celiac plexus neurolysis, in a study of 30 patients.

Abstract:
The impact of poor sleep in the developing child is still tremendously underrated and virtually unexplored. The normative, age-appropriate sleep needs during development remain undefined to a large extent. Furthermore, it remains crucial to realize that relations between sleep duration and cognition, effect, and health are all multi-directional. The most consistent result of insufficient or disrupted sleep is increased daytime somnolence. Thorough sleep history-taking by the healthcare provider and the use of objective sleep assessments are potential first steps and should include the five sleep variables (bedtime, rise time, total sleep duration, variability of sleep over time, and alignment of sleep with other circadian factors). We need to explore the beneficial effects of healthy sleep and how disrupted, shortened, or insufficient sleep may adversely impact on our children’s cognitive and affective functioning, may compromise their health, and may therefore impose a heavy price on our socio-economic structure.

Abstract:
Epilepsy is the commonest serious chronic neurological condition, affecting 0.5% of the population in the UK. Subjective sleep disturbance and excessive daytime sleepiness are reported to be 50% more frequent in those with epilepsy than in controls. Causes are multiple. Both poor seizure control and nocturnal attacks are known to contribute to such sleep disorders. Epilepsy also increases the risk of associated sleep disorders, and additional neurological conditions, such as dementia, learning disability, and depression. These all affect sleep hygiene. Prescribed anti-epileptic drugs will further aggravate the problem. Side-effects will include drowsiness. Sedating benzodiazepines and barbiturates are considered worst offenders. Others affect sleep architecture to varying degrees and/or cause insomnia. While hyper-somnolence in patients with epilepsy will raise the possibility of any of the above issues, sleep deprivation is one of the commonest seizure triggers. This chapter will shed more light on the intricate relationship between sleep and epilepsy.
