Abstract:
Individuals with alcohol problems have well-described disturbances of sleep, but the development of these disturbances both before and after the onset of problem drinking is poorly understood. This chapter first discusses sleep physiology and its measurement in humans. It then examines the functions of sleep and its role in development. Next, it reviews recent research on the relationship between sleep and alcohol use and related problems. Whereas sleep problems (e.g., difficulties falling or staying asleep) increase the risk of early onset of alcohol use and related problems, sleep rhythmicity promotes resilience to the development of alcohol use disorder and problem substance use. Based on existing research, this chapter proposes a theoretical model of sleep and alcohol use, highlighting the role of self-regulatory processes as mediators of this relationship.

Babor, Thomas F., Jonathan Caulkins, Benedikt Fischer, David Foxcroft, Keith Humphreys, María Elena Medina-Mora, Isidore Obot, et al. The legal market: prescription and diversion of psychopharmaceuticals. Oxford University Press, 2018. http://dx.doi.org/10.1093/oso/9780198818014.003.0006.

Full text
Abstract:
The growth of modern medicine, is paralleled by substantial growth in psychopharmaceutical medications designed to treat psychiatric disorders, pain, cognitive dysfunction, mental distress, and sleep disorders. These medications, many of which have high dependence potential, are primarily distributed through a prescription system. Diversion of psychopharmaceuticals from this system for non-medical use constitutes a substantial part of the illicit drug market in a growing number of countries. While there is considerable criminal or organized diversion from the prescription system, much of the leakage happens informally, often at the consumer/patient end of the distribution or availability chain. Modern technologies, including Internet-based market elements and new synthetic medications, are blurring the boundaries between medicines and illicit drugs, especially in affluent countries. The pathways of availability and distribution for non-medical use of ‘illicit drugs’ and of psychopharmaceuticals are thus increasingly converging and overlapping.

Wijdicks, Eelco F. M., and Sarah L. Clark. Agitation and Delirium. Oxford University Press, 2018. http://dx.doi.org/10.1093/med/9780190684747.003.0003.

Full text
Abstract:
Delirium involves abnormalities in perception and in attention with failure of the caregiver to correct or recognize behavior. Marked agitation disturbs the patient’s orientation, attention, and sleep. The effects of uncontrolled agitation are manifold and not in the least causing bodily harm. In intensive care units, agitation and delirium occur in the sickest patients. Treatment of delirium is necessary and needed early after recognition. In this chapter, the best pharmacologic approaches and indications are discussed, including neuroleptics. Atypical antipsychotics are the first-line treatment for delirium. Recognition of delirium starts with the nursing staff and the identification of medications that are associated with triggering delirium.

Full text
Abstract:
Experienced neurologists work fast. They ask few questions, maybe perform a brief examination, and they come up with the right answer. Sometimes they do neither and their conclusions are accurate—but how do they do it? This book holds the answers. The book is divided into 14 chapters which, for the most part, focus on a particular neurologic condition, namely: demyelination, headache, epilepsy and sleep, myopathy and motor neuron disorders, movement disorders, stroke, peripheral neuropathy, cerebellar ataxia, and dementia. The remaining chapters are concerned with the clinician’s initial impressions (first encounters), cranial nerves, limbs and trunk, spinal lesions, and cerebrospinal fluid. At the end of each chapter is a summary of the salient points and a few key references. The final chapter relates to the fast neurological examination. Most diagnostic clues or “Handles” are illustrated by a table, figure, or video clip to reinforce a particular message, and the text is marked with Red Flags that the clinician must be alert for.
