Abstract:
There are many uses of radiation outside healthcare, and this chapter gives an overview of sources both natural and man-made. Radon which is the single largest source of public exposure is considered in some detail. In the event of an incident involving radiation sources whether in a hospital or in the community, medical physicists with their expertise in radiation use may be called upon to provide advice. In the event of a major accident, such as one involving a nuclear facility, it is likely that radiation monitoring and decontamination of individuals will be required, and in some institutions medical physicists may have an important role to play here as well. The chapter sets out practical information that may be of use in dealing with such situations.

Abstract:
Gangs are a fact of life in jails and prisons. The extent and impact of gang activity on a facility will depend upon the size and geographic location of the facility. Smaller jails and prisons, or facilities in rural areas, are more likely to be involved with local or regional groups, also known as street gangs, while large facilities in urban areas will be affected more by nationally known or connected gangs. One survey of Florida prisoners found that inmates who were suspected or confirmed gang members were 35% more likely to commit violent acts than non-members. In a study of 2,158 male inmates in the Arizona Department of Corrections, gang-affiliated inmates were more than twice as likely as nonaffiliated inmates to commit an assault during the first three years of confinement Since institutional management often involves restriction of privileges, placement on long-term segregation, or transfer to a control unit prison, advocacy groups and individual inmates have filed suit against these policies based on First and Eighth Amendment, religious freedom, and anti-discrimination claims. Gang validation procedures themselves have been challenged as arbitrary and inaccurate, leading to inappropriate segregation or restrictions on prisoners who have exhibited no institutional violence. Psychiatrists need to be aware of the dynamics of gang leadership, membership or involvement when working with any gang member, as that will affect their ability and interest in collaborative treatment. These issues and best practices for intervention will be presented in this chapter.

This introduction sets the scene by recognising that more recent reinvestments in the ‘political’ facility and prospects of psychoanalysis, frequently routed through debates around Lacanianism, display a tendency to dismiss ‘poststructuralism’ (and, indeed, any vestige of a ‘poststructuralist’ Lacan), as they attempt sharp distinctions between, say, a materialism supposedly capable of addressing ‘political’ issues today versus unwanted traces of a longstanding idealism with which ‘deconstruction’ can now be historically aligned. Reviewing some of Derrida’s writing on Lacan, the introduction moves on to assess the dismissal of ‘poststructuralism’ that one finds, for example, in Žižek’s recently published doctoral thesis, originally from 1982. Since the ‘political’ implications of this dismissal are far from simply ‘progressive’, the introduction asks questions about the ‘politics’ of the performative in both Derrida’s and Žižek’s texts on Lacan.

“I sort of ‘think in music’ in the same way Temple [Grandin] says she ‘thinks in pictures,’ ” states the autistic professor, social activist, and singer Elizabeth J. “Ibby” Grace. “Music was the nexus between my self and language for a long time. . . my communicative access,” she explains. “When I relax among myself there are not words going on in my head. There are intervals, tones . . . sometimes in order to think, I structure the thoughts into more like music, or they do themselves like that.” Thinking in music also enables Ibby to function socially “in ways I would have no chance of access to without it . . . . I think I can hear people’s own music sometimes, [which is] how I classify what their soul sounds like to me, [and] I can use this facility to predict if people will be liable to get along with one another.”

Abstract:
This chapter explores the trends of corruption and neglect in the administration of the San Francisco pesthouse. Both city authorities and the public at large had considered the San Francisco pesthouse an unjust burden imposed on them, particularly since a fair number of the patients came from other parts of the state or abroad. Repeated official complaints, tinged with antipathy toward strangers and hatred of Chinese immigrants, sought to justify the policy of penury. Moreover, given California's failure to develop a coherent policy for medical segregation, responsibility and funding for a stigmatized facility like the San Francisco pesthouse remained subject to the whims of local politicians and businessmen. Hence, this chapter illustrates the challenges of achieving a proper political and economic balance during this period, attested by decades of seemingly erratic municipal decisions concerning pesthouse finances.
