Abstract:
Taken together, the articles gathered in this supplement underscore the major role in drug development that single-isomer science can play—a role that will undoubtedly take on a greater importance in the future. While the chirality of certain pharmacologically active molecules has been recognized for almost a century, it is only in recent years that drug synthesis and chiral separation techniques have advanced far enough to allow meaningful comparisons among enantiomers. Now that these methods are broadly available, the potential applications of single isomer drug development are considerable.Numerous examples from a range of therapeutic areas confirm that single enantiomers can enhance clinical efficacy, reduce adverse effects, cause fewer interactions with other drugs, and minimize response variations among patients by offering more predictable pharmacokinetics and greater selectivity. In some cases, these advantages are simply due to the removal of an inactive enantiomer, but in other cases, a given dose of a single isomer offers greater benefits when administered alone than when administered as the racemic mixture, suggesting that the opposite enantiomer (the distomer) actually has detracting effects. As the papers by Drs. Gal and Hurt explain, the different activities of a pair of enantiomers are usually traceable to stereochemical differences in the way they interact with chiral macromolecules such as enzymes, transport systems, and receptors.

This paper describes a bidirectional twisted single-phase single-stage buck-boost dc-ac converter based on an output unfolding circuit. This solution is derived by the combination of an inverting buck-boost dc-dc converter and an unfolding circuit. The operation principle, component design guidelines, along with the control approach are presented. The zero-crossing distortion problem is discussed and solved by a simple approach. The simulation and experimental results confirm all theoretical statements. Loss distribution and achievable efficiency are analyzed. Finally, the pros and cons of the proposed solution, along with the most promising application field, are analyzed and discussed in the conclusion.

There is a need to create the simple, reliable single-piece giving piece dispensers. The information and the attention of researchers and at this issue is not enough. The questions of justification of the parameters substantiation of the conveyor single-piece dispenser are considered. The main conveyor speed positions, shaft diameters, profile configurations, the ways of setting a tuber on the conveyor, etc. are tested experimentally. The conclusion of the possibility and expediency of using the roller version of the single-piece dispenser is made.

This paper adopts the method of multi-phase flow to simulate the extrusion process of single-screw extruder based on different screw speed by using the finite element analysis software, and get the conclusion that the screw speed has a significant effect on the extrusion quality and outlet pressure of the single-screw extruder. With the increase of the screw speed, the extrusion quality of the single-screw extruder gets worse and the outlet pressure of the single-screw extruder increases.

Abstract Background A single atrium is a very rare heart condition, in which the inter atrial septum is missing. These congenital heart defects usually are surgically corrected after birth. Case Presentation We present a successful surgical repair of a single atrium (SA) in a 27-year-old woman, who has a complete missing of the atrial septum, without coexisting valvular pathology. The SA diameter was 9.97 × 6.18 cm and the Systemic blood flow/pulmonary blood flow (Qp/Qs) was 4.1 due to the single atrium. Surgical correction consisted of creating a new atrial septum, using a double-velor patch. At first- and second-year follow up, the patient was in a very good condition and without any symptoms present. We consider that the diagnoses of single atrium, especially in child bearing women, should be done in appropriate time, to avoid miscarriages, preterm births and other complications associated. Conclusion There are many case reports that show good surgical repair of single atrium and without late complications. In our case, we have obtained a very good surgical repair, without AV bundle damage and no residual shunt demonstrated on intraoperative transesophageal echocardiography.

Using Generalized Modus Ponens reasoning, we examine the values of the inferred conclusion depending on the correspondence between the premise of the rule and the observed fact. The conclusion is obtained using Fodor’s implication in order to represent a fuzzy if-then rule with a single input single output and the tnorm with threshold generated by t-norm product, as a compositional operator. A comparison study with the case when the standard t-norm product is used is made. Some comments and an example are presented in order to show how the obtained results can be used.

This conclusion is reached by an extensive examination of shell specimens throughout the range of the species and of the animal itself. This conclusion is corroborated by no genetic differentiation existing between the subspecies at the molecular level (Gruenthal and Burton, 2005; Supernault, et al., 2010). Photoplates of specimens illustrate differences in shell morphology and colorations at various locations throughout the range of the species.

Single Access Laparoscopic Colectomy (SALC) were reported in several studies. The first Single Access laparoscopic right colectomy was descibed by Remzi et Al. We report our experience in SALC describing our approach for Right Colectomy. In our experience we perform as well an extracorporeal side-to-side anastomosis, in contrast wit a conventional laparoscopic right colectomy in which we perform an intra-corporeal anastomosis. We think that an important limit of this approach is represented by the difficult to perform a safe intra-corporeal anastomosis. In conclusion we think that right SALC is a safe and feasible approach. However, many issues will be established, as well as technological, economical and educational aspects, before its introduction in the daily clinical practice.

Abstract The development of EU measures regarding access to employment and Germany’s general attitude to these measures (reluctance) are described briefly. This article mainly concerns Germany’s objections (and the objections of other Member States) to the proposal by the Commission of the Single Permit Directive, which was changed several times during the negotiations. Germany (together with other Member States) managed to downgrade the Directive, in particular with regard to the equality rights of TCNs. The conclusion describes that there are only a few rules that are mandatory, much is left to national legislation. The conclusion mentions further that there is a gap between the rights of EU citizens and TCNs; it refers briefly to the future transposition of the Directive which will mean amending national law.

Background Women with an uncorrected single ventricle heart are at increased risk of adverse maternal and perinatal outcomes. Methods We report our experience of managing pregnant women with uncorrected single ventricles, during the time period 2011 to 2017, in a low-resource setting and compare pregnancy outcome with healthy concurrent controls. Outcomes assessed include the mode of delivery, maternal complications, neonatal death and birth weight. Results There were six pregnant women with uncorrected single ventricles who had a total of 14 pregnancies. There was one maternal death in a woman with atrioventricular-septal defect and Eisenmenger syndrome. Caesarean section rates and preterm delivery were similar, whereas perinatal loss and low-birth weight rates were higher among women with a single ventricle compared to healthy controls. Conclusion Unplanned pregnancies without prenatal counselling/care pose a challenge to physicians especially in low to middle income countries and with the high risk of morbidity/mortality, pregnancy should be discouraged.

Objective: To review the psychometric properties and factor structure of the Bereavement Phenomenology Questionnaire (BPQ). Method: Continuous data were compared using two tailed t-tests and analysis of variance. A confirmatory factor analysis was carried out. Results: Good internal consistency (Cronbach's alpha = 0.93) with a single factor has been found. The BPQ discriminates the intensity of grief between widows and widowers, spouses and offspring, and temporally between acute and later phases of bereavement. Conclusion: Although on the BPQ items are highly correlated and only a single factor has emerged, it remains nonetheless a valid and reliable scale for the measurement of grief.

THIS ARTICLE' PUTS FORWARD SOME GENERAL reflections on the Single European Act, on the conditions in which it was negotiated and on its first consequences. But a detailed description of the modalities of the Single Act, of the entire range of changes which it will introduce and of its potential would be beyond its scope. The Single Act has been extremely controversial, during its negotiation, after its conclusion, and for different reasons, during the ratification process. Some decried it as inadequate, even derisory, but others saw in it a threat to national sovereignty. Two referendums (in Denmark and in Ireland) were needed before it could be ratified. Today, on the contrary, the Single Act and its best-known feature — the achievement of the internal market in the Community by the end of 1992 — are presented in the media as the basis of a new Europe, the foundation of all European policy. The Single European Act does not deserve so much honour, any more than it deserved the past indignity, but it contains important innovations, which might lead to significant changes in the behaviour of the institutions and in the way in which the Community itself will develop. This is what I shall try to put forward by recalling briefly first the background of the Single Act, describing the details of the negotiations and, lastly, by singling out some of the original features of the new Treaty.

The emerging online economy provides consumers with easy access to numerous choices. This article investigates the effects of price promotion framing messages on perceived value and online consumer purchase intention. A 3 (price promotion framing: single discount/multiple discounts- large discount first and small discount last/multiple discounts- small discount first and large discount last) × 2 (price level of product: low/high) experimental design is conducted. The experiment results demonstrated that price framing of single discount has a greater influence on perceived value than multiple discounts. Perceived value has a positive influence on purchase intention. Additionally, price moderates the effects of price promotion framing on consumer purchase intention. This finding offers potential for finding better ways for sellers or managers to present discounts in online shopping. Finally, the conclusion presents implications, limitations, and directions for future research.

Parser is a process of classifying sentence structures of a language. Parser receives a sentence and breaks it up into correct phrases. The purpose of this research is to develop a Malay single sentence parser that can help primary school students to learn Malay language according to the correct phrases. This is because research in Malay sentence parsing has not gotten enough attention from researchers to the extent of building parser prototypes. This research used top-down parsing technique, and grammar chosen was context-free grammar (CFG) for Malay language. However, to parse a sentence with correct phrase was a difficult task due to lack of resources for obtaining Malay lexicon. Malay lexicon is a database that stores thousands of words with their correct phrases. Therefore, this research developed a Malay lexicon based on an article from Dewan Masyarakat magazine. In conclusion, this research can provide help to the primary school students to organize correct Malay single sentences.

Abstract Background Kienbock's disease, in spite of an uncertain natural history, is known to cause lunate compromise, leading to central column collapse, carpal instability, and degenerative arthritis of the wrist. Joint leveling procedures are performed in the early stages of Kienbock's disease to “unload” the lunate. Capitate shortening is the preferred procedure in Kienbock's patients with positive ulnar variance. Description of Technique We describe the rationale and a simplified technique of capitate shortening in early Kienbock's disease. This is a single-cut osteotomy with single-screw stabilization. Patients and Methods We have performed this technique in three cases. We present a case of a 26-year-old male who presented with a 1-year history of pain in his right wrist. Radiology performed demonstrated lunate sclerosis. Diagnostic arthroscopy revealed healthy articular surfaces. Single osteotomy capitate shortening was performed with an oscillating saw and fixed with a single cannulated compression screw. A shortening of 1.5mm was obtained with this technique. Results At 1- to 2-year follow-up, all three patients had considerable pain relief but did not have a complete resolution of pain. There was a significant improvement in function and grip strength. There have been no cases with infection, nonunion, avascular necrosis or a need for a salvage procedure. Conclusion The simplified technique of capitate shortening is easy to perform, less traumatic to the capitate vascularity, and leads to good short-term functional results.

This paper presents an experimental study using Fresnel lens to increase the overall efficiency of the conventional Single Slope Solar Still (SSSS) with single basin. The work aims to increase the performance of single basin conventional SSSS while maintaining the simplicity and usability of the system. Especially in the rural parts of Oman, the device is very simple to use. For the present unit, the conventional single basin SSSS has been fitted with an adjustable Fresnel lens. The adjustment enables the carrying out the experiments with and without the Fresnel lens at our Institute. The equipment was constructed from simple available materials and was placed north-south during the experimentation. The Fresnel lens fitted frame once adjusted in the morning was kept at same position for the entire day without tracking. The depth of water in the basin was maintained constant at 0.02 m during the study. The distillate yield for single basin SSSS fitted with Fresnel lens is observed 3 to 3.5 times higher than the conventional. The overall efficiency of the system has also increased almost 32.19 percent over the conventional. The quality of the distillate was assessed, which was estimated to be within limits in compliance with international standards. In conclusion, Fresnel lens has significantly improved the distillate production output and overall efficiency of conventional single basin SSSS.

Abstract:
In this paper, the single leg hydraulic drive system of the quadruped robot is taken as the research object. Then the nonlinear mathematical model of the hydraulic system is built. According to the real mechanical structure parameters of the single leg, the relationship between position control characteristics of the system and variation of single leg trajectory is investigated. Besides, the tracing accuracy of single leg foot displacement in one trot gait condition is analyzed through combination of kinematics solution and simulation analysis. In conclusion, the research indicates that the nonlinear mathematical model can be used to analyze the position control performance of the single leg hydraulic drive system. This research provides a basic foundation for further research of control method.

AbstractBackground: Of the children with Down syndrome 40–50% have cardiac defects and the majority of these cardiac defects are amenable to biventricular repair. The outcome of single ventricle palliation is improving; nonetheless, there are limited data on Down syndrome patients with associated high-risk factors undergoing single ventricle palliation. Our aim was to study the outcomes of children with Down syndrome and high-risk factors on the single ventricle palliation pathway. Methods: A retrospective study on all patients with Down syndrome on the single ventricle palliation pathway from 2005 until 2011 was conducted. Operative, clinical, echocardiographic, haemodynamic data, and follow-up data were reviewed. Results: A total of 310 patients underwent at least one single ventricle surgical intervention. Of those, eight patients had Down syndrome, five of which had associated risk factors – low birth weight, high pulmonary vascular resistance, pulmonary vein stenosis, significant atrioventricular valve regurgitation, and extracardiac anomalies. Mortality in the high-risk group was 80% (4/5), compared with 33% (1/3) in the non-high-risk patients. Overall, after a median follow-up period of 138 days (8–576 days), only 37.5% (3/8) of patients were alive. Conclusion: Despite many improvements in the care of single ventricle patients, the fate of those with Down syndrome and associated high-risk factors remains poor. Further multicentre longer-term studies are needed to validate and quantify the cumulative effects of negative prognostic factors in this complex group of patients.

The results of this study obtained five themes, namely the transmission of covid-19 occurs due to infection from friends in the work environment and after traveling, feelings when confirmed with covid-19, single parents experience shortness of breath, anxiety, worry, fear, shaking, insomnia, headaches, and anxiety. Was. In self-care, single parents ask to be cared for independently at home by taking medicines, vitamins, herbs to maintain progress and experience changes in excessive eating patterns. This he also applies to his children so that children stay healthy. The conclusion is that single parents experience psychological problems, namely anxiety, with a moderate level when confirmed positive for COVID-19. Background: Double epidural hematomas (EDHs) have a higher mortality rate compared to single EDHs and same Glasgow Coma Scale (GCS), although double EDHs incidence is less common. Case Description: We present the case of a 34-year-old female who underwent single skin incision and frontotemporal and suboccipital craniotomies for fatal traumatic double acute EDHs, then, a literature review was performed. Conclusion: Double EDHs in association with low GCS at presentation and traumatic diastasis of cranial sutures or other maxillofacial injuries are associated to an unfavorable outcome.

NIH 3T3 cells migration activity assay showed that the best combination of the HVCO and fucoidan was 50:50. From COX 2 and VEGF protein expression test results, the combination of HVCO and fucoidan has a higher percentage of expression than single HVCO or single fucoidan Conclusion: The results reveal that the combination of HVCO and fucoidan has better wound healing activity than single HVCO or single fucoidan

The design of this study uses a comparative descriptive method. The results of the study in the group of preterm infants with hypothermia in a single wall incubator with a lid significantly increased mean body temperature changes with a mean value of 36.09 variance 0.152 while for preterm infants with hypothermia in a single wall incubator a mean value of 35.35 variance values 0.859 and obtained t count from the two study groups namely 2.551 and 1.717 t table. Introduction: To minimize the complications related to conventional multiport laparoscopic surgery, the single access laparoscopic surgery has been developed. Some results of case series and case-controlled studies are supporting the feasibility and safety of Single Access Laparoscopic Colectomy (SALC). Materials and Methods: Since January 2009 we performed all kind of colorectal procedure by a single access laparoscopic approach. We began with right colectomy that represent the only procedure in which we did not reproduce the same surgical steps of multiport laparoscopic surgery. In contrast, we reproduce the same surgical technique of multiport colorectal resection during a left or rectal single access laparoscopic procedure as well as total colectomy. Conclusion: About the transferability of SALC, programs of training need to focus on safety and techniques. We believe that only high laparoscopic skills surgeon can perform SALC. It?s mandatory to evaluate outcomes and cost-effectiveness of SALC respect to multiport laparoscopic colectomy using randomized trials.

Objective and Background: Synergistic extraction was introduced to replace single extraction due to enhanced extraction efficiency of heavy metal. Methodology: In order to improve extraction efficiency, di-(2-ethylhexyl) phosphoric acid (D2EHPA) was mixed with isodecanol to provide a synergistic effect together with leaching solution which is thiourea. From the single extraction system, the optimum temperature is found at 30°C producing the highest yield of %E of 91.30%. The single extraction also happened better with the presence of Fe(III) solution and thiourea as the %E is at 91.23%. Results and Conclusion: It was found that the synergistic D2EHPA-isodecanol extraction is successful as it yields higher %E than the single D2EHPA extraction.

Objective: To determine the efficacy of single layer intestinal anastomosis to double layer technique in terms of anastomotic healing. Materials and Methods: Fifty patients who underwent intestinal anastomosis in the Department of Surgery, Western Regional Hospital from June 2014 to May 2016 were taken for this comparative study and divided equally in two groups, 25 each (single layer and double layer). Results: Of the total fifty cases, twenty-five cases included in each group, there was no leakage in single layer group while 1 patient had leakage in double layer group which was statistically insignificant. Conclusion: Single layer interrupted intestinal anastomosis is simple to carry out and is as efficacious as double layer anastomosis in terms of postoperative anastomotic leak.

Abstract Introduction: Psoriasis is a chronic, common immune inflammatory condition of the skin, affecting 2-3% of the population, with regional variability. Classically, psoriasis presents as one of the following types: plaque, guttate, inverse, pustular or erythrodermic psoriasis. Typically, the patient will present with several symmetric psoriatic plaques on typical areas of the body, leading the clinician towards the diagnosis of psoriasis. Case report: The present case report series focuses on an atypical presentation of psoriasis noted in 2 patients who presented to our office with a single large, erythematous plaque located on the lower leg. Due to poor response to previous treatment, a biopsy was performed and upon analysis, revealed a diagnosis of psoriasis. The lesions showed significant improvement under local therapy. Conclusion: In spite of significant research on such a common and seemingly well-understood dermatosis, the present case reports plead for further study with regards to atypical presentations of psoriasis.

Abstract Background Autologous stem cell transplantation (ASCT) is one of the standard treatments of choice for eligible multiple myeloma (MM) patients. Herein, we aimed to analyze MM patients at our center and compare the clinical outcomes of single and double ASCT patients. Materials and methods Patients who were diagnosed as having MM and had undergone single or double ASCT in our clinic between the years 2003 and 2020 were retrospectively examined. Results In this study, the median time of second ASCT is approximately 3.6 years from the first ASCT. Overall survival (OS) duration of the single and double transplanted groups was 4,011 ± 266 vs 3,526 ± 326 days, respectively (p: 0.33). Progression-free survival (PFS) duration of the single and double transplanted groups was 2,344 ± 228 vs 685 ± 120 days, respectively (p: 0.22). Disease assessment after ASCT stable or progressive disease, partial remission, and very good partial or complete remission (CR) in single and double ASCT groups was 62/44/105 and 8/4/5, respectively (p: 0.22). Conclusion The present study points out that the second ASCT treatment option for MM patients may not be effective as suggested, especially in the era of novel MM drugs, since our results come from the past data that novel drugs were not exist. In conclusion, we found no benefit with second ASCT in MM patients in terms of PFS and OS or CR rates, and the novel anti-myeloma drugs might decrease the need for a second transplant.

Surgery can provide excellent results, and in selected patients, it should be considered a first-line treatment in experienced hands.

An analytical method was presented on some pairs of gear teeth contacting each other at the same time. First, the software Pro/E is applied to establish two kinds of gear whose fillet curves are typical double circular and single circular respectively. Second, changes of meshing locations is made to get omni-directional contact analysis results. At last a conclusion was drawn that bending stress on dedendum double circular gear is 1.3~1.5 times bigger than that on dedendum single circular gear,that is dedendum single circular gear has better carrying capacity. The study provides a good method for studying gear bending stress.

We aimed to determine how often Clostridium difficile toxin gene PCR assay (CDPCR)-negative patients were appropriately removed from single room contact isolation. Hospital databases were used to collect information on glutamate dehydrogenase (GDH)-positive, toxin-negative inpatients (February–April 2015). Of 60 CDPCR-negative patients, only two (3%) were removed from single room isolation. At least 36% of 53 CDPCR-positive results did not influence bed management. In conclusion, identification of C. difficile toxigenic status did not impact significantly on decisions whether to continue single room isolation. Cost-benefit analysis should be undertaken before CDPCR testing is introduced.

The aim of this study was to analyze epidemiological data and neurological disorders in single-level lumbar intervertebral disc herniation without confounding factors of root compression. Material and methods. 150 patients with single-level lumbar intervertebral disc herniation operated in the Neurosurgical Department of Gomel Regional Clinical Hospital over 2011-2012 were interviewed, and their medical records and protocols of tomographic studies were studied. Conclusion. The patients with single-level lumbar intervertebral disc herniation males were mostly men (78 people - 52.0 %). 10.0 % of the patients detected motor deficits. Of them, paresis of the flexor muscles of the foot prevailed (6.0 %). Impaired function of pelvic organs was a rare neurological disorder (2.0 %).

Introduction: There is a controversy on single dose or multiple doses of prophylactic antibiotics for prevention of surgical site infection during laparoscopic cholecystectomy in a developing country setting. The objective of this study was to compare single versus multiple doses of prophylactic antibiotics in terms of surgical site infection in laparoscopic cholecystectomy patients. Methods: This was a prospective, comparative, randomized study was conducted in a medical college hospital. Two hundred consecutive patients with symptomatic cholelithiasis planned for routine laparoscopic cholecystectomy were enrolled in the study. Patients were randomly divided in a Single dose (SD) group and multiple dose (MD) group. SD group were given injection ceftriaxone (1gm) before induction of anesthesia and MD group received ceftriaxone (1gm) before induction of anesthesia and continued a total of 3 doses postoperatively for next 24 hours. Results: A total of 200 patients were studied, of which 100 were in single dose (SD group) and another 100 in multiple dose (MD group). The mean age of patients with symptomatic cholelithiasis was 41.76 ± 13.38 years with minimum of 16 years and maximum of 73 years. Of the total patients, 4 patients in single dose (SD) group and 3 patients in multiple dose (MD) group developed surgical site infection of various severity which was not statistically significant. (p=0.500). Conclusion: There is no difference in terms of surgical site infection in patients taking either single or multiple doses of antibiotics in laparoscopic cholecystectomy in a medical college setting in Nepal.
