Abstract:
Purpose The purpose of this paper is to examine the content and style of clinic letters written by psychiatrists and to compare these with national guidelines and standards. To then consider the impact that writing directly to patients and carers has on their feeling of inclusion and understanding via a questionnaire. Design/methodology/approach Two audits were completed, the first was carried out in 2012 and the second during 2014 with both being over a three-month period. The first 50 clinic letters sent out during these periods were examined using an audit tool that was developed using national standards from the Department of Health and the Royal College of Psychiatrists. A questionnaire was then devised in 2016 and sent to patients and carers regarding their views on the simplified clinic letters that were written directly to them. Findings In the original audit none of the letters were simplified and written to the patient whereas in the re-audit 66 per cent were simplified. The questionnaire sent out to patients and carers revealed that 50 per cent of patients felt that the simplified letter helped them to feel more included and gave them a better understanding of their care. Originality/value This paper highlights the potentially positive impact of writing simplified clinic letters directly to patients with intellectual disability and their carers. It also includes a clinic letter format designed so that medical information is not lost in the written communication and so that the service’s workload is not impacted on by having to write two separate letters to the patient and to their GP.

Background: Bronchiolitis is a viral lower respiratory infection of young infants. It requires monitoring by outpatient health care providers and rarely needs hospitalization. Scoring of severity in community helps in early referral. A simplified bronchiolitis scoring for rural health care providers with no facility for measurement of oxygen saturation (SpO2) and chest X-ray (CXR) is a long-felt need. This study proposes a simplified score.Methods: The proposed bronchiolitis severity score (PSS) has to be validated against the standard bronchiolitis severity score (SSS). The PSS was administered by a physician, nurse and researcher. The reliability was measured by the comportment of internal consistency and inter-observer agreement.Results: The physicians rating of severity by SSS and PSS was similar in 97% of cases. The internal consistency of 0.72 and the kappa index of 0.86 were obtained. The inter-rater agreements between physician, nurse, researcher was 0.94, 0.94 and 0.93 respectively.Conclusions: The PSS is reliable, valid and can be administered by nurses in peripheral health care settings.

Abstract:
Obstructive sleep apnea (OSA) is a prevalent disease. Often resources to provide care for OSA are inadequate, leading to long patient waiting times. Simpler validated methods of care are needed. In the first study in Chapter 2, the utility of a new high-sampling rate oximeter to diagnose OSA was explored. The home oximetry data collection was robust, with few failures and the data allowed the “rule in” or “rule out” of moderate-severe OSA with high degree of certainty. It is concluded that home oximetry could replace polysomnography (PSG) as a diagnostic test in a significant proportion of patients, thus allowing limited resources available for the care of those with OSA to be re-directed e.g. towards providing therapy. In Chapter 3, the diagnostic information from the oximeter was used to underpin a study designed to demonstrate that a nurse-led model of care could produce health outcomes in moderate-severe OSA not inferior to physician-led care. A randomised controlled multi-centre non-inferiority clinical trial was performed. 1,427 patients referred to 3 sleep medical centres with possible OSA were assessed. 195 patients were randomised to 2 models of care. Model A, a simplified model, involved home oximetry to diagnose moderate-severe OSA, auto-titrating constant positive airway pressure (APAP) to set a therapeutic constant positive airway pressure (CPAP), with all care supervised by an experienced nurse. Model B involved 2 laboratory PSG’s, to diagnose OSA then titrate CPAP, supervised by a sleep physician. The primary endpoint was change in Epworth Sleepiness Score (ESS) measured before and after 3 months of CPAP. A range of other outcome measures were collected. The change in ESS for nurse-led management (Model A) was not inferior to the physician-led service (Model B) since the lower limit of the two-sided 95% CI did not include -2, the margin of equivalence (difference 0.13, 95% CI -1.52 to -1.25). 11 patients in Model A and 10 in Model B were lost to follow up during the trial. There were no significant differences between Model A and Model B after 3 months of CPAP in any of the other outcome measures, including CPAP adherence at 3 months. It is concluded that a simplified nurse-led model of care can produce non-inferior results to physician-directed care in the management of moderate-severe OSA. In Chapter 4 the efficacy of CPAP in normalising or improving subjective and objective sleepiness, quality of life and selected neurocognitive measures was explored. It was shown that only a proportion of patients (60% on ESS, 35% on FOSQ) normalised their scores after 3 months of CPAP therapy. This is important information. As new health care delivery strategies evolve as a result of the data presented in Chapter 3 and elsewhere, it will be crucially important to train new health care professionals in the complexities of OSA management, such that they are aware that the symptoms of patients presenting for OSA investigations can have multiple aetiologies, and may not always resolve by simply applying CPAP. The data presented in this thesis add to the evidence base in treatment of moderate-severe OSA and will help further evolve health care delivery for this important disease.

ENGLISH ABSTRACT: The management of physical assets has become a popular eld of study over recent years and is being acknowledged in multiple disciplines world wide. In this project, research on Physical Asset Management (PAM), maintenance and decision making are presented. PAM is a complex subject and requires the participation of multiple disciplines in order to successfully manage physical assets. Moreover, the management of maintenance makes a big contribution in achieving successful PAM. Decision making is a core element to manage maintenance e ciently, both on strategic and operational level. Various methods and techniques can be used to aid the decision making process such as, using past experience, xed decision making techniques and techniques involving numerical calculations, to mention only a few. However, using numerical calculations to make decisions are not very popular. This is due to various reasons, for example the inherent complexity of the mathematics and the time required to execute such calculations are disliked. People tend to avoid complex numerical calculations and rather rely on past experience and discussion of circulating opinions to make decisions. This is not ideal and can lead to inconsistent and inaccurate decisions. In this project, the importance of numerical decision making is researched, especially in maintenance related decisions. The focus is placed on the simpli cation of numerical decision making techniques with the aim to make it easy and quick to use to support operational PAM decisions. Di erent decisions regarding PAM, especially decisions with regards to managing maintenance in order to achieve PAM, are discussed by means of a literature study. This is done to clarify the applicability of using numerical decision making techniques to support this type of decisions. A few di erent available numerical techniques are highlighted that can be used to support the decision making process. The decisions together with numerical decision making techniques are evaluated in order to combine the most appropriate techniques in a simpli ed manner. The purpose of this is that it can be used by anyone with the necessary knowledge of a speci c system or operation. As a result a simpli ed numerical decision making toolbox is developed that can support maintenance related decision. This toolbox is applied to a real life situation by means of a case study, made possible by Anglo American Platinum Limited (Amplats). An evaluation and validation of the toolbox is done through the case study to conclude wether it has value in practice or not.
AFRIKAANSE OPSOMMING: Die bestuur van siese bates het die afgelope paar jaar 'n gewilde studieveld geword en word erken in verskeie dissiplines reg oor die w^ereld. In hierdie projek word navorsing gedoen oor Fisiese Bate Bestuur (FBB), instandhouding en besluitneming. FBB is 'n komplekse onderwerp en vereis die deelname van verskeie dissiplines om sukses te behaal. Die bestuur van instandhouding maak 'n groot bydrae tot suksesvolle FBB. 'n Kern element van doeltre ende instandhouding is besluitneming, beide op strategiese en operasionele vlak. Verskillende metodes en tegnieke kan gebruik word om die besluitnemingsproses te ondersteun soos byvoorbeeld om gebruik te maak van ondervinding en vorige gebeurtenisse, vaste besluitnemingstegnieke, tegnieke wat numeriese berekeninge gebruik en nog meer. Die gebruik van numeriese metodes om besluite te neem is nie baie gewild nie. Dit is as gevolg van verskeie redes soos byvoorbeeld die inherente kompleksiteit en ingewikkeldheid van die wiskunde en ook die tyd wat benodig word om sulke berekeninge uit te voer. Mense is geneig om ingewikkelde numeriese berekeninge te vermy en eerder staat te maak op vorige ervaring en die bespreking van menings om besluite te neem. Dit is nie ideaal nie en kan lei tot onkonsekwente besluite, of selfs verkeerde besluite. In hierdie projek is die belangrikheid van numeriese besluitneming nagevors, veral in die onderhoudsverwante besluite. Die fokus word geplaas op die vereenvoudiging van die numeriese besluitnemings tegnieke. Die doel is om dit op so 'n manier te vereenvoudig dat dit maklik en vinnig is om te gebruik vir operasionele FBB besluite. Verskillende besluite oor FBB, veral besluite met betrekking tot instandhouding om suksesvolle FBB te bereik, word bespreek deur middel van 'n literatuurstudie. Die literatuurstudie ondersoek die toepaslikheid van die gebruik van numeriese besluitnemingstegnieke vir hierdie soort besluite. 'n Paar verskillende beskikbare numeriese tegnieke wat gebruik kan word om die besluitnemingsproses te ondersteun word uitgelig. Die besluite, saam met numeriese besluitnemingtegnieke, word ge evalueer om die mees gepaste tegnieke te kombineer in 'n vereenvoudigde manier. Uiteindelik moet dit deur enige iemand met die nodige kennis van 'n spesi eke stelsel of proses gebruik kan word. La transition énergétique est un enjeu majeur actuel et des années à venir. Parmi les défis qu’elle va soulever figure la limitation des impacts environnementaux de la production d’électricité. Pour cela, des outils d’aide à la décision, simples d’utilisation et suffisamment précis, considérant les aspects environnementaux et permettant d’optimiser les choix énergétiques futurs, doivent être mis en place. L’analyse environnementale d’une filière de conversion d’énergie est un sujet complexe. Elle comporte en effet deux niveaux. Le niveau « filière », caractérise le profil global d’impacts environnementaux des systèmes, et le niveau « système » caractérise leurs impacts, permettant ainsi une analyse intra-filière. Pour répondre à ce besoin de caractérisation en deux niveaux, nous proposons une méthodologie générique permettant de développer des modèles d’estimation des profils environnementaux de chacune des filières et d’estimer simplement ceux des systèmes qui la composent sans avoir à réaliser une étude détaillée. Cette méthodologie repose sur la définition d’un modèle d’Analyse de Cycle de Vie paramétré prenant en compte, par Analyse Globale de Sensibilité, un large échantillon de systèmes représentatif des configurations observées en pratique au sein des filières. Dans un second temps, des modèles simplifiés estimant les performances environnementales des systèmes sont définis, en fonction de quelques paramètres clefs identifiés comme expliquant la plus grande part de variance des impacts environnementaux de la filière. The energy transition debate is a key issue for today and the coming years. One of the challenges is to limit the environmental impacts of electricity production. Decision support tools, sufficiently accurate, simple to use, accounting for environmental aspects and favoring future energetic choices, must be implemented. However, the environmental assessment of the energy pathways is complex, and it means considering a two levels characterization. The “energy pathway” is the 1st level and corresponds to its environmental distribution, to compare overall pathways. The “system pathway” is the 2nd level and compares environmental impacts of systems within each pathway. We have devised a generic methodology covering both necessary characterization levels by estimating the energy pathways environmental profiles while allowing a simple comparison of its systems environmental impacts. This methodology is based on the definition of a parameterized Life Cycle Assessment model and considers, through a Global Sensitivity Analysis, the environmental impacts of a large sample of systems representative of an energy pathway. As a second step, this methodology defines simplified models based on few key parameters identified as inducing the largest variability in the energy pathway environmental impacts. These models assess in a simple way the systems environmental impacts, avoiding any complex LCAs. This reduction methodology has been applied to the onshore wind power energy pathway in Europe and the photovoltaic energy pathway in France

En Union Européenne les accidents frontaux font 28% des accidents routiers et sont responsables de 49% de mortalité, les fractures thoraciques étant la cause principale de décès. Les modèles en éléments finis du corps humain sont un outil important pour la simulation de chocs réels et la prédiction des risques d'endommagement. Cette thèse a permis de développer un modèle simple en éléments finis de la cage thoracique suffisamment souple d'utilisation et facilement paramétrable. Ce modèle est validé expérimentalement avant d'être utilisé pour une étude paramétrique. Cette étude a permis de caractériser l'influence de différents paramètres structurels et géométriques sur le comportement de la cage thoracique sous chargement dynamique. Le travail réalisé au cours de cette thèse est divisé en trois parties : Modélisation de la cage thoracique entière avec des éléments finis de type poutre dont les propriétés mécaniques sont déterminées à partir d'essais de flexion trois points sur des segments de côtes et complétées par des éléments de la littérature, Validation du modèle dont les résultats sont suffisamment proches des résultats des essais de chargement dynamique antéro-postérieur menés par Vezin et Berthet [Vez09], Etude paramétrique sur l'influence paramètres, géométrie des sections droites et géométrie globale de la cage thoracique (inclinaison des côtes, forme et taille globale de la cage thoracique). A partir de cette étude nous trouvons que le module d'Young et l'épaisseur du cortical ont une influence identique sur la raideur globale de la cage thoracique ainsi que sur la rotation et la déformation des côtes. Avec l'augmentation de ces deux paramètres la rigidité du thorax augmente et le taux de compression maximal diminue. D'autre part les côtes tournent plus et se déforment moins. La raideur des liaisons costo-verterbales a une influence directe sur la rotation latérale qui diminue avec l'augmentation de cette raideur alors que les déformations augmentent ; tandis que la raideur globale de la cage thoracique est légèrement modifiée. L'inclinaison des côtes est le facteur ayant la plus grande influence sur la déformation des côtes et donc sur le risque d'endommagement : plus les côtes sont proches de la direction de chargement la raideur de la cage thoracique augmente et la déformation des côtes augmente
In the European Union, 28% of road accidents are frontal impacts which provoke 49% of fatalities where the thoracic fractures are the main cause of death. The finite element models of the human body are an important tool for the simulation of real impacts and the prediction of damage. This thesis has led to develop a rib cage simplified finite element model sufficiently flexible and easily customizable. First, this model is experimentally validated and then used in a parametric study. This study allowed us to characterize the influence of different structural and geometric parameters on the behavior of the rib cage under dynamic loading. This work is divided into three parts : Modeling the rib cage using beam elements whose mechanical properties are determined by three-point bending tests on rib segments and supplemented from literature, Validating the model by simulating the anteroposterior dynamic loading tests led by Vezin and Berthet [Vez09], Performing a parametric study on the influence of the mechanical parameters (Young modulus, stiffness of costo-vertebral joints), the geometry of the rib sections and the overall geometry of the rib cage (ribs slope, shape and overall size of the rib cage). This study permitted to find that Young modulus and the thickness of the cortical have the same influence on the overall stiffness of the chest as well as on the rotation and deformation of the ribs. By increasing these parameters, the stiffness of the chest increases and the maximum compression ratio decreases. Besides, we'll find more rotation and less deformation of the ribs. The stiffness of the costoverterbal joints has a direct influence on the lateral rotation : it will decrease by increasing of the stiffness while deformation will increase. However, the overall stiffness of the chest is slightly modified by modifying the costovertebral joint stiffness. The initial inclination of the ribs accordingly to the load direction has the greatest influence on the deformation of the ribs and therefore on the damage risk. When the ribs are closer to the loading direction, the stiffness of the rib cage and the deformation of the ribs increases

Purpose: To introduce the simplified therapeutic intervention scoring system (TISS-28), the original therapeutic intervention scoring system (TISS-76) and simplified acute physiological score (SAPS) version II in critically ill adult patients, in order to describe the validity and reliability of TISS-28 as a suitable measure of quantifying nursing workload in the adult intensive care units (ICU) of a public sector hospital in Johannesburg. Objectives: To describe the profile of patient admissions to the intensive care units, to investigate the impact of the patients’ profile on the requirements for nursing workload and to validate the use of TISS-28 as a measure of quantifying nursing workload in this setting. Design: A non-experimental, comparative descriptive, correlational and prospective two-staged design was utilized to meet the study objectives. Stage I involved face and content validation of TISS-28 by a panel of ICU nurse experts (n=6). Stage II involved assessment of concurrent and construct validity as well as inter-rater reliability of TISS-28 using participants (n=105) drawn from trauma, cardiothoracic and multidisciplinary ICUs. Data necessary for the calculation of TISS-28, TISS-76 and SAPS II were recorded for each patient in the ICU at 24 and 48 hours after admission and in the wards after discharge within 24-48 hours. Descriptive and inferential statistics were used to analyze data. Results: Content Validity Index (CVI) of 0.93 was found for TISS-28. A significant positive correlation was found between TISS-28 and TISS-76 scores (r = 0.7857, p = 0.0001) as well as TISS-28 and SAPS II scores (r = 0.2098, p = 0.0317). A significant difference was found between TISS-28 scores among patients in the ICU and patients in the ward (t = 25.59, p = 0.0001; t = 21.48, p = 0.0001) respectively. A significant correlation was found between the data collected from a sample of patients by the researcher and the expert assistant researcher with an intra-class correlation coefficient of 0.99 and a p-value of 0.0001. Conclusions: The findings support validity and reliability of TISS-28 hence its feasibility for use in South African ICUs. Recommendations for nursing education, practice, management and research are proposed.

The purpose of this study was to explore “the effects of Simplified Tai-Chi Exercise Program (STEP) on the physical and psychological health of the wheelchair-bound elders in the long-term care facilities”. A time series quasi- experimental design was used in this study. Using convenience sampling, selected the wheelchair-bound elders aged 65 and over in the long-term care facilities in Tainan area. 82 subjects completed their pretest. And 51 subjects completed their posttest. The experimental group (n = 25) received STEP training 50 minutes each time and three times a week for 14 weeks;on the contrary, subjects in the control group (n = 26) maintained regular activities of daily living. The variables in the physical health were the measurements of resting heart rate, blood pressure, pulmonary function, shoulder range of motion, muscle strength, and flexibility. The variable in the psychological health was the measurement of depression status. Results show that variables were significant difference in the shoulder flexion (F = 3.864, p = .028) and shoulder abduction (F = 4.894, p = .012) had better improved after STEP training. However, the variables in the physical and psychological health were no significant difference between the experimental group and the control group in the 2-week posttest and 14-week posttest. Therefore, the study reveals the fact that test the STEP further could spread it.

En Union Européenne les accidents frontaux font 28% des accidents routiers et sont responsables de 49% de mortalité, les fractures thoraciques étant la cause principale de décès. Les modèles en éléments finis du corps humain sont un outil important pour la simulation de chocs réels et la prédiction des risques d'endommagement. Cette thèse a permis de développer un modèle simple en éléments finis de la cage thoracique suffisamment souple d'utilisation et facilement paramétrable. Ce modèle est validé expérimentalement avant d'être utilisé pour une étude paramétrique. Cette étude a permis de caractériser l'influence de différents paramètres structurels et géométriques sur le comportement de la cage thoracique sous chargement dynamique. Le travail réalisé au cours de cette thèse est divisé en trois parties : Modélisation de la cage thoracique entière avec des éléments finis de type poutre dont les propriétés mécaniques sont déterminées à partir d'essais de flexion trois points sur des segments de côtes et complétées par des éléments de la littérature, Validation du modèle dont les résultats sont suffisamment proches des résultats des essais de chargement dynamique antéro-postérieur menés par Vezin et Berthet [Vez09], Etude paramétrique sur l'influence paramètres, géométrie des sections droites et géométrie globale de la cage thoracique (inclinaison des côtes, forme et taille globale de la cage thoracique). A partir de cette étude nous trouvons que le module d'Young et l'épaisseur du cortical ont une influence identique sur la raideur globale de la cage thoracique ainsi que sur la rotation et la déformation des côtes. Avec l'augmentation de ces deux paramètres la rigidité du thorax augmente et le taux de compression maximal diminue. D'autre part les côtes tournent plus et se déforment moins. La raideur des liaisons costo-verterbales a une influence directe sur la rotation latérale qui diminue avec l'augmentation de cette raideur alors que les déformations augmentent ; tandis que la raideur globale de la cage thoracique est légèrement modifiée. L'inclinaison des côtes est le facteur ayant la plus grande influence sur la déformation des côtes et donc sur le risque d'endommagement : plus les côtes sont proches de la direction de chargement la raideur de la cage thoracique augmente et la déformation des côtes augmente.

This rapid review synthesises the literature on how community-based management of acute malnutrition (CMAM) programmes could be adapted in settings of conflict and fragility. It identifies multiple factors affecting the quality and effectiveness of CMAM services including the health system, community engagement and linkages with other programmes, including education, sanitation, and early childhood development. Family MUAC (Mid-Upper Arm Circumference) is a useful tool to increase community participation and detect early cases of moderate acute malnutrition (MAM) and severe acute malnutrition (SAM) more effectively and less likely to require inpatient care. The literature does not say a lot about m-Health solutions (using mobile devises and applications) in data collection and surveillance systems. Many of the above-mentioned issues are relevant for CMAM programmes in settings of non-emergency, emergency, conflict and fragility. However, there are special circumstance in conflict and fragile settings that need adaptation and simplification of the standard protocols. Because of a broken or partly broken health system in settings of conflict and fragility, local governments are not able to fund access to adequate inpatient and outpatient treatment centres. NGOs and humanitarian agencies are often able to set up stand-alone outpatient therapeutic programmes or mobile centres in the most affected regions. The training of community health volunteers (CHVs) is important and implementing Family MUAC. Importantly, research shows that: Low literacy of CHVs is not a problem to achieve good nutritional outcomes as long as protocols are simplified. Combined/simplified protocols are not inferior to standard protocols. However, due to complexities and low funding, treatment is focused on SAM and availability for children with MAM is far less prioritised, until they deteriorate to SAM. There is widespread confusion about combined/simplified protocol terminology and content, because there is no coherence at the global level.

Abstract:
This report documents the development of an integrated hydrodynamic and ecological model to test assumptions about island resilience. Swan Island, a 25-acre island in Chesapeake Bay, Maryland, was used as a case study. An interagency, interdisciplinary team of scientists and engineers came together in a series of workshops to develop a simplified resilience model to examine the ability of islands to reduce waves and erosion and the impacts to nearby habitats and shorelines. This report describes the model development process and the results from this first key step: model conceptualization. The final conceptual model identifies four main components: vegetative biomass, island elevation, waves/currents, and sediment supply. These components interact to form and support specific habitat types occurring on the island: coastal dunes, high marsh, low marsh, and submerged aquatic vegetation. The pre-and post-construction field data, coupled with hydrodynamic ecological models, will provide predictive capabilities of island resilience and evaluations of accrued benefits for future island creation and restoration projects. The process and methods described can be applied to island projects in a variety of regions and geographic scales.

The Sustainable Development Goal 6 targets are all dependent on capacity development as outlined in SDG 6a “Expand international cooperation and capacity-building support to developing countries in water- and sanitation related activities and programmes “. Massive Open On-line Courses (MOOCs) and distance learning in general have a significant role to play in this expansion. This report examines the role that MOOCs and similar courses could play in capacity development in the water sector. The appearance of MOOCs in 2010/11 led within 4 years to a huge increase in this type of course and in student enrollment. Some problems with student dropout rates, over-estimating the transformational and disruptive nature of MOOCs and uncertain business models remain, but less “massive” MOOCs with more engaged students are overcoming these problems. There are many existing distance learning courses and programmes in the water sector designed to train and/ or educate professionals, operators, graduate and undergraduate students and, to a lesser extent, members of communities dealing with water issues. There are few existing true MOOCs in the water sector. MOOCs could supply significant numbers of qualified practitioners for the water sector. A suite of programmes on water-related topics would allow anyone to try the courses and determine whether they were appropriate and useful. If they were, the students could officially enroll in the course or programme to gain a meaningful qualification or simply to upgrade their qualifications. To make MOOCs more relevant to education and training in the water sector an analysis of the requirements in the sector and the potential demand for such courses is required. Cooperation between institutions preparing MOOCs would be desirable given the substantial time and funding required to produce excellent quality courses. One attractive model for cooperation would be to produce modules on all aspects of water and sanitation dealing with technical, scientific, social, legal and management topics. These should be produced by recognized experts in each field and should be “stand-alone” or complete in themselves. If all modules were made freely available, users or mentors could assemble different MOOCs by linking relevant modules. Then extracts, simplified or less technical versions of the modules could then be used to produce presentations to encourage public participation and for other training purposes. Adaptive learning, where course materials are more tailored to individual students based on their test results and reactions to the material, can be an integral part of MOOCs. MOOCs efficiently provide access to quality courses at low or no cost to students around the world, they enable students to try courses at their convenience, they can be tailored to both professional and technical aspects, and they are very suitable to provide adaptive learning courses. Cooperation between institutions would provide many course modules for the water sector that collectively could provide excellent programmes to address the challenges of capacity development for SDG 6 and other issues within the water sector.
