Abstract:
This chapter on laryngotracheal reconstruction (LTR) describes and reviews the preoperative, intraoperative, and postoperative course of a virtual case of a patient undergoing double stage laryngotracheal reconstruction (ds-LTR). The discussion section reviews the etiology and pathogenesis of subglottic stenosis (SGS) and describes the Myer-Cotton classification system of SGS. The chapter also reviews the medical management of gastroesophageal reflux disease, endoscopic treatment of granulations, anterior cricoid split, LTR with cartilage augmentation (single and double stage), and cricotracheal resection. The differences in the technique and patient selection of single stage LTR (ss-LTR) and ds-LTR are reviewed. The intraoperative and postoperative complications of LTR and postoperative sedation management are also discussed.

Abstract:
Foreign investors invest in a host state via a contract between the foreign investor and an entity or instrumentality of the host state. Disputes between investors and host states under investment treaties often arise out of breaches of these contracts. In such cases, international tribunals must assess whether the asserted claims rise to the level of a breach of a state’s international obligations. More than a decade ago, the decisions on jurisdiction in SGS v Pakistan and SGS v Philippines brought this issue into the spotlight. These decisions, often perceived as contradictory, deal with the jurisdiction of treaty-based tribunals over claims for a breach of contract. This chapter reviews the seeming confusion regarding the interplay between treaty claims and contract claims and discusses how to dispel any confusion.

Abstract:
Pediatric airway reconstruction is undertaken to correct airway obstruction, which may be congenital or iatrogenic. Congenital airway obstruction can be located at any level of the respiratory system, from the nares to the bronchi. It can be complete obstruction, such as choanal atresia in a newborn, or incomplete, such as adenotonsillar hypertrophy or tracheomalacia. Iatrogenic obstruction is often the result of prolonged intubation in a premature infant, which results in subglottic stenosis (SGS) after extubation. As these children grow, their airway growth may not keep pace, resulting in restrictive symptoms limiting their activity. Laryngotracheoplasty (LTP) or tracheal resection procedures may be needed to reconstruct and enlarge the airway. If the premature infant has concurrent lung disease, long-term intubation is often followed by tracheostomy placement. The timing of decannulation depends on the patient's airway patency and respiratory reserve. Understanding the process of evaluation and treatment of patients with SGS helps the anesthetist provide safe anesthesia for this challenging group of children.

The main types of mathematical structuralism that have been proposed and developed to the point of permitting systematic and instructive comparison are four: structuralism based on model theory, carried out formally in set theory (e.g., first- or second-order Zermelo–Fraenkel set theory), referred to as STS (for set-theoretic structuralism); the approach of philosophers such as Shapiro and Resnik of taking structures to be sui generis universals, patterns, or structures in an ante rem sense (explained in this article), referred to as SGS (for sui generis structuralism); an approach based on category and topos theory, proposed as an alternative to set theory as an overarching mathematical framework, referred to as CTS (for category-theoretic structuralism); and a kind of eliminative, quasi-nominalist structuralism employing modal logic, referred to as MS (for modal-structuralism). This article takes these up in turn, guided by few questions, with the aim of understanding their relative merits and the choices they present.

Abstract:
This book assesses the role of the UN Secretariat in the Rwandan genocide. With the help of new sources, including the personal diaries and private papers of the late Sir Marrack Goulding, it situates the Rwanda operation within the context of bureaucratic friction existing at Headquarters in the early 1990s between the Department of Political Affairs (DPA) and the Department of Peacekeeping Operations (DPKO). The book argues that these units clashed not only over resources (a classic symptom of bureaucratic pathology) but also over the scope of peacekeeping and the role of the Secretary-General (SG) within it. This rivalry also reflected a split between a strong-willed SG determined to leave his mark on international affairs and to use his ‘political’ department independently of states, and Washington and the politico-military apparatus of the Pentagon, which in Somalia, Rwanda, Bosnia, and Haiti found itself increasingly at odds with Boutros-Ghali. Although the book looks at how this bureaucratic and power-political confrontation impacted on the Rwanda mission, it identifies the conceptual reasons for the DPA–DPKO split in the grey area that separates peacebuilding and peacekeeping. The difficulty of distinguishing these two key UN functions, coupled with the creative tension between SGs and states, explains why six decades after the birth of the UN, it has still not been possible to demarcate the exact roles of DPA and DPKO. Far from being dull and irrelevant, the book concludes that the UN bureaucracy is an intriguing barometer of the role of the Secretary-General in world politics.
