Abstract:
There are many misconceptions about sexual-offender treatment, which are particularly magnified when it comes to understanding and managing the risk of sexual offending in the community. Many of the misconceptions are based on faulty information about the types of treatment, treatment outcomes, and sexual-offence recidivism. Additionally, public misconception of sexual-offending behaviour in sexually deviant individuals creates stigma and fear. This is despite the large number of studies on sexual-offender treatment, recidivism, and treatment outcome available in the scientific literature. In fact, various actuarial risk-assessment instruments can be used to estimate the probability that a sexual offender will recidivate. Risk can be managed through treatment. The aim of this chapter is to review the evidence-based studies on the efficacy of sexual-offender treatment, as well as how the risk management of sexual offenders is currently implemented in sexual-offender treatment programmes.

Crime-preventing neurointerventions (CPNs) are increasingly being used or advocated for crime prevention. There is increasing use of testosterone-lowering agents to prevent recidivism in sexual offenders, and strong political and scientific interest in developing pharmaceutical treatments for psychopathy and anti-social behaviour. Recent developments suggest that we may ultimately have at our disposal a range of drugs capable of suppressing violent aggression, and it is not difficult to imagine possible applications of such drugs in crime prevention. But should neurointerventions be used in crime prevention, and may the state ever permissibly impose CPNs as part of the criminal justice process? It is widely thought that preventing recidivism is one of the aims of criminal justice, yet existing means of pursuing this aim are often poorly effective, restrictive of basic freedoms, and harmful. Incarceration, for example, tends to be disruptive of personal relationships and careers, detrimental to physical and mental health, highly restrictive of freedom of movement and association, and rarely more than modestly effective at preventing recidivism. Neurointerventions hold the promise of preventing recidivism in ways that are more effective and more humane, but the use of CPNs in criminal justice raises several ethical concerns. CPNs could be highly intrusive and may threaten fundamental human values, such as bodily integrity and freedom of thought, and humanity has a track record of misguided, harmful, and unwarrantedly coercive use of neurotechnological ‘solutions’ to criminality. This collection brings together original contributions from emerging scholars and internationally renowned moral and political philosophers to address these issues.

This chapter reviews the topic of problematic sexual behaviors committed by elderly men and women. Given the dearth of research done on problematic sexual behaviors in older people, references in this chapter include studies involving people as young as 55. Age-relevant findings regarding criminal sex offenses committed by elderly sex offenders and recidivism are considered along with aspects of this population that are similar to and different from younger men and women. Important ethical and sexual boundaries and their implications are discussed. Patterns of sexual offenses committed by elderly offenders against children are reviewed. Specific risk factors, particularly the role of cognitive impairment and psychiatric illness, are identified and described. Approaches to assessment and treatment are also presented. The chapter concludes with suggestions for future research.

This essay considers the challenges associated with managing and treating sex offenders within the prison setting. What is known from scientific research about the most effective approaches to treating sex offenders is reviewed, followed by the major rehabilitation theories. The role of assessment with sex offenders (e.g., interviews, composite risk and need assessments for both sexual and general recidivism, penile plethysmography) as well as the challenges and limitations of conducting assessments with incarcerated sex offenders are also discussed. The various approaches to sex offender treatment are critiqued, including physiological strategies, behavioral strategies, cognitive strategies, and relapse prevention. Each phase of treatment (preparation, addressing criminogenic needs, planning for the future) is considered separately, and directions for future research are considered.

Abstract:
This essay discusses various treatments for sexual offenders and their success in reducing reoffending. Overall, research reveals a positive treatment effect that indicates up to 25 per cent less recidivism in treatment versus control groups. Cognitive-behavioral therapy, relapse prevention, and programs based on the Risk–Need–Responsivity model have the strongest evidence base, although the studies and findings are heterogeneous and outcomes vary depending on many factors. Most promising are programs that involve treatment in the community and in forensic hospitals, delivered in a partly individualized mode, implemented with sound integrity, targeting medium- to high-risk offenders, addressing young individuals, and being evaluated in well-documented small studies. In contrast, programs in prisons, delivered merely in a group format, including low-risk offenders, and evaluations in large samples show smaller or no effects. Recent developments aim to modernize and widen standard programs toward more differentiated interventions, but more sound evaluation research is needed.

Juvenile sexual offending is often regarded as a precursor of serious and continued sexual offending in adulthood, but there has been little empirical evidence supporting this assumption. Could juvenile sexual offending be just a ‘passing phase’? The study discussed in this essay follows the criminal career about 1,600 juvenile sex offenders from early adolescence into adulthood. A comprehensive view of the entire criminal career is presented to establish whether juvenile sexual offending is a precursor of continued (sexual) offending in adulthood or if (sexual) offending is non-chronic for most. The sexual recidivists in the sample are identified, and this group is used to establish the risk factors associated with continued sexual offending. These risk factors are compared to the ones used in risk assessment instruments for (juvenile) sex offenders. This study holds crucial information for policy and theory regarding juvenile sex offenders.
