Full textIn the first part, we were interested in the criteria of oxygen supply/demand imbalance involved in the occurrence of a type 2 infarction. We explored in 610 patients the association between the parameters of these criteria and the occurrence of acute myocardial injury and type 2 infarction, as well as the correlation between these parameters and the extent of myocardial injury. Our results did not show any association between the importance of oxygen mismatch and the occurrence of acute myocardial injury. There was also no correlation with the magnitude of such injury. Therefore, we could not define strict restrictive thresholds that could be considered a significant myocardial stressor. In the second part, we compared the short-term and the long-term outcomes of patients admitted with an oxygen supply/demand imbalance condition according to the presence of myocardial injury or type 2 infarction and assessed the association of these pathological entities with mortality and major cardiovascular events. In this population of 824 patients, the occurrence of myocardial injury or type 2 infarction led to high in-hospital mortality of more than 20% and was significantly associated with it after adjustment for patient characteristics. In the follow-up of survivors, the outcome was dependent on comorbidities without the involvement of the occurrence of these initial myocardial injuries, with mortality rates of 27 to 35% and major cardiovascular events of 23 to 40%. We proposed to compare these results in another study, conducted prospectively, with a standardized 6-month follow-up of patients admitted for oxygenation failure, the methods of which are detailed here. This cohort consists of 670 patients whose data are currently being analyzed. Finally, in the third part, we focused on the 675 elderly patients, who represent more than 80% of our cohort, to determine the factors associated with the occurrence of these myocardial injuries and type 2 infarction according to age classes. We found very dependent patient profiles in these classes, linked to the epidemiological changes of aging. However, the individualization of type 2 myocardial infarction within acute myocardial lesions was not obvious, nor was the impact on mortality, which was essentially based on the burden of comorbidities

Full textUpper and lower respiratory infections are a frequent reason for seeking treatment. Among the suspicions of lower respiratory infections in emergency rooms, respiratory viruses are found in 30 to 50% of cases. Their precise and rapid diagnosis is necessary for the proper use of anti-infectives, for hospitalization, for the management of the flow of patients, and the establishment of the necessary precautionary measures. We evaluated the impact of the implementation of a multiplex PCR allowing a rapid and delocalized syndromic diagnosis in an emergency department of the viruses responsible for respiratory infections. First, we carried out a narrative review of the available microbiological tests, then we:- Studied the impact of the response time of the centralized virology laboratory (median 18h) on room-only placement strategies for patients with influenza virus infection,- Conducted a feasibility study on the implementation of a rapid syndromic PCR technique relocated to the emergency room, -Prospectively evaluated the impact of the rapid virological response on the management of patients with suspected lower respiratory infections using a controlled clinical trial, - Studied, at the start of the SARS-CoV-2 pandemic, the clinical and biological characteristics which made it possible to distinguish patients infected with SARS-CoV-2 or another respiratory virus such as influenza viruses, RSV, or rhinovirus.We have thus shown:- that the long turnaround time from the centralized laboratory did not allow effective consideration of the PCR result for the single room assignment of influenza-positive patients - that delocalized viral diagnosis in the emergency room is possible and allows more room-only hospitalization for influenza-positive patients during the randomized study (74% vs 50%), but no benefit was found from delocalized diagnosis throughout antibiotic therapy or hospitalization. - that fever, age, male sex, and absence of sputum were more frequently associated with the diagnosis of SARS-CoV-2 than with any other respiratory virus, and co-infection with SARS- CoV-2 with another respiratory virus was not associated with a more severe prognosis. It now seems necessary to integrate mPCR into a broader strategy, including CT scan and inflammation markers, for optimal management in emergencies and impact the consumption of antibiotics and the length of stays

Full textScreening is essential to controlling the HIV epidemic as it ensures early treatment initiation, which limits morbidity, mortality and secondary transmission. In France, thousands of individuals remain unaware of their HIV status and a quarter of diagnoses occur at a late stage of infection. Nurse participation in HIV screening is widely discussed as a means by which to get a greater number of actors involved. Through a systematic review we suggested that, in countries with concentrated epidemics such as France, the United States and the United Kingdom, nurses could increase the number of screening tests being offered and performed. In these countries, Health authorities have recommended non-targeted systematic HIV screening in non-specialized health care settings. While this recommendation has gradually been questioned due to its limited effectiveness and associated increased workload, the DICI-VIH trial was designed to explore a new targeted method of HIV screening, performed by nurses in the Emergency Departments of the Paris metropolitan area, where HIV prevalence is high. A large-scale trial with a cluster-randomized two-period crossover design showed that this screening strategy is a compelling approach. By limiting the use of resources allocated to screening in Emergency Departments and by targeting a limited number of patients, this strategy is shown to be feasible and effective in identifying individuals who are unaware of their HIV status. It is also associated with limited costs. These results contribute new data to the discussion on how to best adapt HIV screening strategies in contexts where the epidemic is concentrated and to the evaluation of the contribution of the nursing profession to screening programs
