Resumen El tumor fibrohistiocítico plexiforme (TFHP) es una neoplasia mesenquimatosa rara de malignidad intermedia, informada por primera vez por Enzinger y Zhang en 1988. Tiene predilección por niños y adultos jóvenes, pero puede ocurrir a cualquier edad. Suele afectar a extremidades superiores y se presenta como una masa indolora de crecimiento lento. Tiene una alta tasa de recurrencia local y la aparición de metástasis es excepcional. Histológicamente, se caracteriza por una masa dérmica o subcutánea mal demarcada con crecimiento plexiforme multinodular y citomorfología fibrohistiocítica. Se requiere una resección quirúrgica completa de la lesión, preferiblemente con márgenes amplios, para prevenir la recurrencia local. Es necesario un seguimiento a largo plazo para detectar cualquier metástasis nodal o pulmonar. Presentamos una serie de dos casos de TFHP que precisaron de ampliación de márgenes quirúrgicos.

Abstract:
BACKGROUND: Drug poisoning is an important cause of morbidity and mortality in pediatric patients. However, amlodipine poisoning, a widely used dihydropyridine calcium chanel blocker, is not fully documented in Ecuador. Treatment consists of classic measures for shock management and specific measures for this type of intoxication. CASE REPORTS: We present two case reports, both of teenage patients admitted into the pediatric intensive care unit for suicide attempt by taking amlodipine and some other drugs. EVOLUTION: During hospital stay, they presented a different evolutionary course. In both cases vasoactive drugs were needed, dosage was modified according to clinical course. Also in both patients, calcium gluconate was administered along with other support measures described in this paper. Finally, both patients presented a good outcome and were discharged after psychological and psychiatric assessment and follow up. CONCLUSION: The low frequency of amlodipine poisoning and the lack of evidence-based knowledge, constitute it as a diagnostic and therapeutic challenge. Based on our experience, we highlight the importance of early suspicion and prioritizing the use of vasopressors over fluid resuscitation. Additionally, we recommend documenting the exact dose of intake and inquiring about consumption of other drugs to properly classify the severity of the poisoning and stablish the treatment plan. Finally, constant clinical monitoring and support of laboratory tests will guide the conduct.

Abstract Sepsis is currently the leading cause of death in burned patients. There are few studies on sepsis in burned patients at intensive care unit (ICU). To determine demographic profile, clinical presentation, evolution, procedures, and treatments used for burned patients affected by sepsis. Retrospective study in medical records of severe burned adult patients who developed sepsis between November 2015 and May 2018 in a university hospital in Curitiba, Brazil. Patients’ details about hospitalization and sepsis were collected. Were included 44 patients, 75% men, and mean age of 42.1±16.88 years. The median TBSA was 50% that was significantly associated with mortality (P = .013). Outcome of death was observed in 50% of the patients. The median duration of hospitalization was 35 days, and in the ICU was 21.5 days. Orotracheal intubation and tracheostomy were the most prevalent aggravating procedures conducted during the hospitalization (77.2% and 56.8%, respectively). The median time to the first sepsis episode was 7 days, and the average total time in sepsis was 13.2 days. The median length of hospital stay among patients with septic shock who died was significantly lower than that of patients who did not die (P = .031). Blood culture was positive in 79.5% of cases, with the majority being typical ICU bacteria. Sepsis occurs more frequently in patients with higher TBSA and long hospitalization time accompanied by aggravating procedures and complications. Infections were caused by typical ICU bacteria, resulting in 50% patient mortality primarily due to septic shock.

Background: Hepatocellular carcinoma is the most common type of primary liver cancer and is the third cause of cancer related deaths; 80% of the HCC are associated with cirrhotic livers or chronic liver diseases, which constitute the main risk factor. Chronic inflammation, necrosis and regeneration due to these conditions produce genetic mutation and development of tumor cells. Yet, 10% develop in non-cirrhotic healthy livers without precipitating factors. Material and methods: We conducted a retrospective analysis of the characteristics and survival of patients with diagnosis of hepatocellular carcinoma in non-cirrhotic liver and absence of a history of liver cirrhosis or chronic liver disease undergoing surgery in two hepato-pancreato-biliary units between January 2007 and January 2016. Results: Mean age was 65 years and 13 patients were men. Abdominal pain was the most common clinical presentation. Liver panel was normal in 60% of the cases and alpha-fetoprotein was elevated in only 16%. The diagnosis was made by imaging tests in 61% of the cases. Mean tumor size was 110.6 cm. All the patients underwent surgery. Complications were observed in 36.8% of the patients and survival at 5 years was 62.3%. Conclusion: hepatocellular carcinoma is usually diagnosed as a large lesion in imaging tests ordered due to abdominal pain. Surgery provides curative treatment, and large resections can be safely performed, with low perioperative morbidity and mortality and low incidence of postoperative liver failure,since the liver remnant is healthy and liver function is maintained.

Las fracturas de la epitróclea representan el 11 al 20% de las lesiones en codo en población pediátrica y aproximadamente el 50% se asocia a luxación en el codo, siendo la incarceración menos frecuente. El objetivo del trabajo es describir los resultados funcionales de una serie de pacientes con fractura de epitróclea. Se realizó un estudio observacional descriptivo y retrospectivo de pacientes pediátricos con fracturas de epitróclea, atendidos en un hospital pediátrico. Se revisaron las historias clínicas y los datos fueron analizados en Excel. El estudio fue aprobado por el comité de ética del hospital. Se tomaron 42 casos de los cuales 39 cumplieron con los criterios de inclusión. La edad media fue 11.3 años (4-16 años). El 77% fueron operados y la fractura se asoció a luxación del codo en el 25.6%. El 7.6% de los casos tuvieron neuroapraxia de nervio cubital con recuperación espontánea. La fractura de la epitróclea se observa en población pediátrica involucrada en actividades de alto rendimiento. Puede ocurrir falla en diagnóstico debido a la compleja osificación del codo y también a la superposición de imagen de la epitróclea con la tróclea humeral. Se asocia con luxación del codo entre un 25 a 50% de los casos. El tratamiento de esta fractura es ortopédico o quirúrgico. Las tasas de consolidación en ambos tratamientos son similares

Creutzfeldt-Jakob disease (CJD) is an uncommon neurodegenerative disorder with an incidence of 1 per 1,000,000 in humans per year, typically characterized by rapidly progressive dementia, ataxia, myoclonus and behavioral changes. Genetic prion diseases, which develop due to a mutation in the prion protein gene (PRNP), account for an estimated 10 to 15% of all CJD cases. Familial CJD is transmitted with an autosomal dominant inheritance pattern with high penetrance. Worldwide, the most common mutation is E200K (glutamate to lysine). We report four families with CJD assisted in Neuquén Hospital in 2018. Three of the four index cases had family history of neurological and psychiatric illness, though data was not taken into consideration at the moment of evaluation of the new cases. The most significant data recorded for a genetic consultation was when the problem had started, and it was required by a neurologist. The initial symptoms were persistent insomnia and depression with poor response to habitual psychiatric medication. Impoverishment is fast with visual disorder, myoclonias, ataxia, dementia and loss of language. Pedigree analysis allowed the identification of 144persons with the gene potential, who can develop the disease at any time in their adulthood. In all cases, mutation E200K was identified. There is a region of increased frequency of CJD. There must be suspicion on patients with neuropsychiatric symptoms and suspected family history(familiar background). Finding of the mutation confirms the diagnosis in patients and allows the identification on pre-symptomatic individuals. Challenge is posed on gene advice and to avoid iatrogenic disorder transmission. Key words: Familial Creutzfeldt-Jakob Syndrome, PRNP gene, E200K, CJD in Patagonia

BACKGROUND: “Floating knee” describes ipsilateral fractures of the femur and tibia, where the knee is disconnected from the rest of the limb. The mechanism is usually a high-energy trauma. Rates of infection, lack of union, stiffness of the knee, excessive blood loss, fat embolism, are relatively high; leading to functional impairment and often unsatisfactory results. Immediate definitive reduction and fixation of fractures by intramedullary nail reduces the incidence of complications. METHODS: A descriptive study was carried out, a case series report of 7 patientes treated by the SIGN Intramedullary Nail (IMN); to establish the functional results, concomitant injuries, complications and advantages of this nail, in our practice. RESULTS: The majority of the patients were male, with a mean age of 26 years. All injuries were caused in traffic accidents, 50% of the fractures were exposed fractures. According to the Blake & McBryde classification, 6 were Type I and one case Type IIa. Functional results were evaluated according to the Karlström and Olerud criteria; excellent and good results were obtained in 85% of cases. All of the fractures healed; one case of superficial infection was described, one patient presented limb shortening <3 cm and minor limitation of the knee motion range. CONCLUSION: The SIGN intramedullary nail, was efficient and versatile for the treatment of floating knee in adults. It had good functional results (85%) and low incidence of complications. Introduction Multiple myeloma is a neoplasm caused by the proliferation of a clone of plasma cells that in most cases produce a monoclonal protein. Due to its decreased incidence of 4-5 cases per 100,000 inhabitants. Case description The General Hospital San Francisco Quito-Ecuador was diagnosed during this time of pandemic: three cases of Multiple Myeloma, with diverse presentation both in clinical manifestations, laboratories and radiographs, taking us to carry out a series of clinical cases with the objective of differentiating the type of clinic, diagnosis and therapy in times of pandemic in 2020. Finding different diagnoses at admission such as acute coronary syndrome, acute pancreatitis and heart failure, which were later discarded and evidenced in general bone pain, hyperamylasemia and hyperglobulinemia, with image changes both in bone punches and pathological fractures that led to the suspicion of Multiple Myeloma and performance of a medullogram for definitive diagnosis. The subspecialty treatment will run without complications. Conclusion The main characteristics are pathological fractures that can present in the form of acute or chronic pain, as well as related to other pathologies; and since the rest of the clinical manifestations mentioned in the literature were not present. It is also worth mentioning that hyperamylasemia was a factor found in our cases that future studies could be related as a prognostic factor and mortality in multiple myeloma

CONTEXT: Parapharyngeal space tumors comprise less than 0.5% of all head and neck neoplasms.1 The majority of these tumors are benign, but surgery is usually required to establish the diagnosis and treat the patients. We present 26 patients treated surgically for tumors arising in the parapharyngeal space (PPS) at the State University of Campinas Hospital – UNICAMP. CASES SERIE: Of these, 17 (65.5%) had benign and 9 (34.6%) malignant neoplasms. The surgical and pathological data relevant to these cases are highlighted, observing any local recurrence, surgical complications and the five-year survival. Neurogenic tumors and soft tissue sarcomas were, respectively, the most frequent benign (35.3%) and malignant neoplasms (44.5%). Benign tumors accounted for the majority of the cases and involved minimal surgical morbidity with no recurrence during a median follow-up of five years. Malignant tumors had a high rate of recurrence and mortality. Surgery is the treatment of choice for PPS tumors. A knowledge of the anatomy of this site is essential for the safe performance of surgical procedures. Malignant neoplasms have a poor prognosis. Fine needle aspiration was helpful in diagnosis of all tumors.

Abstract:
Purpose: Cervical cancer remains a real public health problem in Togo. This study was undertaken to sensitize women and to determine the prevalence of precancerous and canrous lesions of the cervix in the Togolese woman. Material and methods: This is a retrospective descriptive study of the results of a consecutive series of Pap smears performed in the Laboratory of Pathological Anatomy and Cytology CHU Tokoin Lome 2011-2013. Results: Epidemiological, 512 FCV were interpretable. The average age of women was 37.6 years, extreme 19 and 87 years. By occupation, we noted a predominance of unemployed women (351cas, 68,6%), followed by women traders (103 cases, 20,1%) . The distribution by marital status was possible to observe a predominance of married women (234 cases, 45, 7 %). In terms of reproductive history, we found a mean gravidity and parity respectively 3, 5 and 2. We noted 22 women living with HIV, 77.2% were infected with HIV1. Cytopathological, 512 smears selected, we observed 44 abnormal smears (8,6%). It was indeterminate lesions ASCUS / ASCUS (8 cases), endocervical hyperplasia (3 cases), low-grade lesions (17 cases), high-grade lesions (10 cases) and invasive carcinoma (6 cases). Conclusion: The detection rate of cervical cancer is low among the Togolese woman. The health professionals should take the habit of asking the FCV any woman having a consultation.

Abstract Introduction: in 2015 an increasing number of congenital microcephaly cases were associated to maternal infection due to Zika virus. Some of these patients presented other alterations and arthrogryposis was the most frequently found. Arthrogryposis is defined as congenital joint contractures involving at least two different areas of the body. Description: arthrogryposis was found in 18 patients with congenital microcephaly due to Zika virus. 67% of the cases were vaginal deliveries. 50% of resuscitation performed in the delivery room was necessary. The mean birth weight was 2.371g, gestational age was 39 weeks and the head circumference was 28.3cm, 15 (83%) of these patients presented severe microcephaly. All the neonates resulted in concomitant hip joints and some also had knees, ankles and wrists affected. Nine neonates (50%) presented an early respiratory distress and four (22%) died due to respiratory failure. Discussion: the neurological result found in patients with Congenital Zika Syndrome seems to be associated to the maternal infection period. During the early stages of embryogenesis, in addition to microcephaly, could be related to the peripheral motor nerves leading to fetal akinesia, joint stiffness and arthrogryposis. These neonates tend to present greater morbimortality with the worst prognosis.

Introducción: El objetivo de este estudio es analizar nuestros resultados clínico-radiológicos con el uso de copas de tantalio trabecular para la reconstrucción de acetábulos complejos, tanto en cirugías primarias como en revisiones, y com- pararlos con series similares de otros autores. Materiales y Métodos: Se llevó a cabo una revisión retrospectiva de una serie de 42 cirugías (29 revisiones y 13 artro- plastias primarias) en 40 pacientes, entre marzo de 2010 y marzo de 2016. La serie incluyó a 20 mujeres y 20 hombres, con una edad promedio de 60 años (rango 27-93). El seguimiento promedio fue de 37 meses (rango 12-84). Todos los pacientes fueron tratados con el mismo tipo de metal trabecular. Resultados: Al promedio de seguimiento mencionado la supervivencia del componente acetabular fue del 97,6%. Un paciente fue tratado con artroplastia de resección debido a una infección persistente. La tasa de complicaciones totales fue del 12% y el promedio del Harris Hip Score posoperatorio fue de 81,54 (rango 63,25-92,75). Conclusiones: Aunque se necesita un seguimiento más prolongado, el uso de copas de metal trabecular logra resultados prometedores para el tratamiento de acetábulos complejos.

OBJECTIVES: To determine the prevalence of Her-2/Neu-cerbb-2 in the gastric mucosa of patients with gastric adenocarcinoma in a brazilian patient group. METHODS: The immunohistochemical expression of Her-2/Neu was studied in 37 formalin-fixed paraffin-embedded tissue sections. RESULTS: The immunohistochemical reaction produced by the anti-HER-2/Neu antibody was positive in two cases (5.4%). CONCLUSION: The low prevalence of Her-2/Neu observed in these southern brazilian cases is probably due to the great number of poorly differentiated cancers in this serie.

Introduction Evaluation of a single injection of botulinum toxin A in infantile esotropia as an alternative choice to surgery in a retrospective cases serie. Methods Patients (65 consecutive children aged 9 to 26 months with infantile strabismus) underwent botulinum toxin A injection in both medial recti Botulinum toxin A injection. Clinical datas including measurement of angle of deviation before and after injection. visual acuity, stereoscopy and side effects were observed in a 24 months period of follow-up. Results Stable satisfactory result (angle equal to or less than 8 dioptres) was obtained in 33 cases (50.7%). Excellent result (-2 to + 4 dioptres) were obtained in 17 children (26%). Additional surgery was required in 32 cases for a residual (albeit smaller angle than the initial deviation) or recurrent deviation and/or DVD (49,2%). Conclusions Botulinum toxin A reduces the tonic spastic motor component of strabismus and, should additional surgery be required (49% in our series), allows the procedure to be carried out on a smaller angle. A reduction in the maximum angle of deviation is thus achieved sooner, and allows the development of anomalous binocularity in a stable microtropia.

Sin embargo la técnica de distracción osteogénica mediante transporte óseo alveolar intraoral ha demostrado ser muy predecible. Objetivo: Describir los resultados del cierre de la Hendidura alveolar, con la técnica de transporte óseo alveolar intraoral bifocal y trifocal en pacientes labio fisurados. Diseño: Estudio retrospectivo de serie de casos. Materiales y métodos: Fueron tratados 4 pacientes con hendidura alveolar unilateral, manejados con la técnica de transporte óseo alveolar intraoral bifocal y trifocal en el año 2008 al 2009, con un seguimiento hasta el año 2014. Todos los pacientes fueron tratados con un aparato Hyrax® (Dentaurum, Alemania) modificado. Resultados: El promedio de distancia del transporte óseo requerido fue de 9,75mm (rango de 15 mm a 9 mm). Se colocaron 5 implantes dentales rehabilitados con prótesis dental fija. Conclusión: El estudio de los casos clínicos demuestra una predictibilidadelevada así como un rango de éxito alto, en los individuos tratados conesta técnica. Por tal motivo el transporte óseo alveolar intraoral es una alternativa eficaz para el tratamiento de hendiduras alveolares extensas.Introduction: The management of alveolar cleft patients, allows an adequate facial growth in cleft lip and palate patients. Bone grafts have been the gold standard treatment. However, the technique of osteogenic distraction by intraoral alveolar bone transportation has proven to be highly predictable. Objective: To describe the results of alveolar cleft management with the intraoral bifocal and trifocal alveolar bone transportation technique in cleft palate patients. Design of study: Retrospective case series study. Materials and methods: 4 patients with unilateral alveolar cleft were treated with the bifocal and trifocal intraoral alveolar bone transportation technique from 2008 to 2009. The cases were followed up until 2014. All patients were treated with a modified Hyrax® (Dentaurum, Germany) device. Results: The average distance of bone transportation required was 9.75mm (range from 15mm to 9mm). 5 dental implants with fixed dental prosthesis were placed. Conclusion: Clinical cases study demonstrated a high predictability, as well as a high success ratein individuals treated with this technique. Therefore, intraoral alveolar bone transportation is an effective alternative for the management of extensive alveolar clefts.

Background: Penetrating brain injury is a rare entity. Furthermore, clinical presentation can be highly variable and management difficult, especially when the foreign body is retain in the skull Methods: We present a serie of three recent cases of penetrating brain injury that happened at our institutionm including clinical and radiological data. We discuss management of those challenging cases and present a brief review of the literature. Results: Our cases (3) encompassed different mechanisms: bilateral nail gun injury, knife and aircraft propeller. All patients were male, with a median age of 37 years old. Work-up was negative for intracranial vascular injury. All patients were treated with initial craniectomy (bilateral in one case) and a course of antibiotics. Cranioplasty was later performed. All patients survived and evolved in favorable fashion. Conclusions: Penetrating brain injury is a rare injury, requiring individualized surgical and medical management. A few recommendations may be found in the literature but are often based on literature from blunt traumatic brain injury or war-related injuries.

Abstract We used DNA fingerprinting to document serial polyandry in a double-brooded female Long-eared Owl (Asio otus) in western Montana. We also determined that an extra male that helped provision offspring at a nest was a close relative of the female parent. The nest had split paternity, and it is not clear whether the helper was the sire or brother of two of the four nestlings. These data constitute the first known cases of serial polyandry and alloparenting in Long-eared Owls and leave open the question of whether cooperative polyandry also occurs in this species. Poliandría en Serie y Alopaternidad en Asio otus Resumen. Empleamos huellas dactilares genéticas (i.e., DNA fingerprinting) para documentar poliandría en serie en una hembra de Asio otus que presentó dos nidadas, en el oeste de Montana. También determinamos que un macho extra-pareja que ayudó a abastecer a los pichones en un nido era un pariente cercano de la madre. El nido presentó paternidad compartida y no está claro si el ave que ayudaba era el padre o el hermano de dos de los cuatro polluelos. Estos datos representan el primer caso conocido de poliandría en serie y alopaternidad en Asio otus, y deja abierta la pregunta si en esta especie también ocurre poliandría cooperativa.

Realizou-se série temporal de casos cumulativos de COVID-19 por 28 dias, após o 100º caso confirmado de cada país (baseado nas estatísticas do Worldometer 2020). Modelos de tendência linear, exponencial, potencial e logaritmo foram testados, sendo escolhido o melhor coeficiente de determinação (R²). No Brasil, a linha de tendência foi segmentada em 1º-14º dia e 15º-28º dia. Resultados: no 100º dia, os EUA possuíam maior número de casos e o Brasil, o menor. Houve linha de tendência em sua maioria exponencial, com maior velocidade de crescimento nos EUA. No Brasil, houve tendência de crescimento mais lento no segundo período. Conclusão: as linhas de tendência calculadas demonstraram pior prognóstico para os EUA. No Brasil, o crescimento do número cumulativo de casos foi mais lento na no segundo período do estudo.ABSTRACTObjective: to examine cumulative cases of COVID-19 in Brazil, Spain, Italy, China, and USA. Transforaminal lumbar interbody fusion (TLIF) is a relatively new technique of lumbar arthrodesis via posterior transforaminal approach to the disc, indicated mainly in cases of degenerative disc disease, low grade spondylolisthesis and reoperation for disc herniation, specially when there is indication for interbody fusion and posterior decompression. The main advantage of TLIF is that it allows the complete removal of the intervertebral disc through the vertebral foramen, decompression of the spinal canal and vertebral foramen with minimum risk of neural lesion, due to the access being lateral to the nerve roots. In this study, we describe the first 24 cases of TLIF that we have done, wich shows to be very safe and efficient in our serie, with an relief of pain in 83.3% of patients, great improvements in the life quality in 75% of cases and satisfaction with the surgery in 79.1% of patients.

Purpose To describe patient-reported outcomes, radiological results, and revision to total hip replacement in patients with hip dysplasia that underwent periacetabular osteotomy as isolated treatment or concomitant with hip arthroscopy. Methods Case series study. Between 2014 and 2017, patients were included if they complained of hip pain and had a lateral center-edge angle ≤ of 20°. Exclusion criteria included an in-maturate skeleton, age of 40 or older, previous hip surgery, concomitant connective tissue related disease, and Tönnis osteoarthritis grade ≥ 1. All patients were studied before surgery with an anteroposterior pelvis radiograph, false-profile radiograph, and magnetic resonance imaging. Magnetic resonance imaging was used to assess intraarticular lesions, and if a labral or chondral injury was found, concomitant hip arthroscopy was performed. The non-parametric median test for paired data was used to compare radiological measures (anterior and lateral center-edge angle, Tönnis angle, and extrusion index) after and before surgery. Survival analysis was performed using revision to total hip arthroplasty as a failure. Kaplan Meier curve was estimated. The data were processed using Stata. Results A total of 15 consecutive patients were included; 14 (93%) were female patients. The median follow-up was 3.5 years (range, 2 to 8 years). The median age was 20 (range 13 to 32). Lateral center-edge angle, Tönnis angle, and extrusion index correction achieved statistical significance. Seven patients (47%) underwent concomitant hip arthroscopy; three of them (47%) were bilateral (10 hips). The labrum was repaired in six cases (60%). Three patients (15%) required revision with hip arthroplasty, and no hip arthroscopy-related complications are reported in this series. Conclusion To perform a hip arthroscopy concomitant with periacetabular osteotomy did not affect the acetabular correction. Nowadays, due to a lack of conclusive evidence, a case by case decision seems more appropriate to design a comprehensive treatment.

Abstract Background: Juvenile myelomonocytic leukemia (JMML) is a rare malignant myeloid disorder in childhood, for which stem cell transplantation is currently the only curative treatment option. Although unrelated donor umbilical cord blood transplantations (UCBTs) have been described, series of HLA-identical sibling donor UCBTs in JMML are not available. Methods: From the European Working Group on Childhood MDS (EWOG MDS) registry the 5 JMML patients who underwent a fully HLA matched sibling UCBT are described. Results: The median age at diagnosis was 18 months (range 15–30 months). In one case no mutation analysis was performed, in one case a PTPN11 mutation was found. In the other cases no mutation in RAS or PTPN11 was found. None of the patients had clinical signs of NF1. The conditioning consisted of busulfan, cyclophosphamide and melphalan in four cases and of cyclophosphamide, etoposide and total body irradiation in one case. All patients engrafted slowly. In 3 patients mild acute graft versus host was noticed (grade 1 and 2), no chronic graft versus host was reported. Two patients relapsed after the initianal transplantation and underwent a second transplant with marrow of the initianal donor. One of them is in second complete remission and the other died after a second relapse. One patient developed increased donor chimerism from day 42 without any clinical sign of relapse. She was treated with DLI, 6 mercaptopurine and 13-cis retinoic acid respectively. She is still in complete remission 5 years after transplantation. Conclusion: This EWOG serie illustrates that, although this needs to be confirmed in larger series, relatively immunologically naive HLA-matched sibling UCBT is feasible in selected cases of JMML.

El feocromocitoma es un tumor productor de catecolaminas. Se describe la experiencia en el diagnóstico, manejo y seguimiento de pacientes con este diagnóstico en una institución de alta complejidad en Cali, Colombia, en los últimos 10 años. Se encontraron 11 casos con diagnóstico de feocromocitoma llevados a cirugía, dos de estos extraadrenal y uno maligno. Fueron asintomáticos 18%, 81% presentaron hipertensión arterial. La evolución posquirúrgica de los pacientes fue favorable, 33% persistieron con hipertensión arterial (HTA) luego de la resección del tumor, requiriendo menor dosis de antihipertensivos. El único caso de malignidad tuvo una respuesta no satisfactoria. A pesar de ser un diagnóstico infrecuente, su abordaje es importante debido a la morbilidad que representa. Las características de nuestra población son similares a lo reportado en la literatura, con baja frecuencia de complicaciones y adecuada respuesta al tratamiento.Abstract Pheochromocytoma is a catecholamine-producing tumor. We describe the experience in the diagnosis, management and monitoring of patients with pheochromocytoma in a highly complex institution in Cali, Colombia, in the last 10 years. We identified 11 cases with diagnosis of pheochromocytoma, who were taken to surgery, being two of these extraadrenal and one of these malignant. 18% were asymptomatic, 81% had hypertension. The postoperative course of patients was favorable, 33% persisted hypertensive after tumor resection, although less doses of blood pressure medication were required. The only case of malignancy had an unsatisfactory response. Despite being a rare diagnosis, the approach is important because morbidity in our population is similar to those reported in the literature, with a low frequency of complications and adequate response to treatment.

e20721 Background: To report the epidemiologic, clinical, extension, treatment and results of CEP in a Tunisian population from a department of medical oncology. Methods: We collected retrospectively cases of CEP (> 70 years) within a cohort treated in a medical oncology department in Tunisia. We analyzed the following data : age, sex, delay to diagnosis(DD), site, histologic type, stage, co-morbidities, treatment and evolution. Results: These CEP collected from 2002 to 2007 represented 16.7% of the 1507 patients treated for cancer during this period. Mean age was 77.7+/- 4.2 years(70 to 86), SR 0.93 and delay to diagnosis 7.8+/6 5,28 months(1 to 48). Lung cancer (49 cases), digestive (46 cases) and Breast cancer (29 cases) represented the most frequent anatomic sites and patients presented co-morbidities in 75 cases. 11 patients (4.5%) developed metachronous second cancers and 40 pts (16.4%) have initial metastases at diagnosis. Treatment included CT in 148 pts (61.3%), S in 85 pts (34%) and RT in 40 pts (17%). Median survival was 24 months for the whole population. Conclusions: CEP represented more than 15% of patients treated for cancer in a Tunisian serie, this age group representing presently around 6% of the Tunisian population where aging modified in the last 20 years the ages pyramid aspect. CEP are characterized by late diagnosis/consultation, frequent co-morbidities, locally advanced and/or metastatic cases and probably less aggressive treatment compared to younger patients.

The prevalence, morbidity and costs of asthma care have increased worldwide. This study describes the effect and safety of an outpatient medical treatment with traditional medicine for asthma, through a retrospective case series with patients admitted to outpatient consultation from 1995 to 2015 in Cota, Colombia. Analyzing 26 cases with a clinical diagnosis of asthma, during the treatment 34.6% (9/26) did not present episodes of uncontrolled asthma and 61.5% (16/26) were classified as controlled asthma in their last consultation of control. 88.4% (23/26) of the patients reported no adverse reactions and the three reported were mild. The results allow to generate hypotheses about the effectiveness and safety of an outpatient treatment based on the incorporation of resources from traditional medicine. These observations could be explored with experimental studies to determine their long-term effectiveness, safety and low cost.

17570 Background: More than 20,000 new cases of HD are diagnosed each year all over Europe, Canada and the United States. Nevertheless, there are few reports in current literature showing data from large series in developing countries. Methods: The medical records of adult pts with an oncohaematologic diagnosis in two large oncological centers (HMC and IOHM) were reviewed. A period spanning the past 10 years was considered with the objective of measuring the incidence of HD. Oncologists in charge were asked to fill a form with the relevant clinical data. Results: Three hundred and fifty eight out of 1,884 medical records of pts with HD (19%) were retrieved (M: 55.2%/F: 44.8%). Seven pts were HIV positive. Bulky disease was present in 7,4%. The following table shows the main topics (See table ): Conclusions: Incidence of HD in our serie was similar to the published report. However, two important differences arose: it was not possible to notice neither the double curve described nor the usual histological subtype distribution (more MC than the expected frequency). According to previous reports, prevalence of NS was associated with mediastinal locations and younger pts. Actuarial survival must be presented. AbstractJosua Bruyn's article 'Towards a Scriptural Reading of Seventeenth-Century Dutch Landscape Paintings' (1987) elicited a great deal of criticism for both its method and its occasionally sweeping conclusions. To a certain extent this criticism is understandable. It does not however mean that recently initiated, cautious attempts to peer below the surface of the painted landscape should be aborted. It is still highly unlikely that the landscape was the only Dutch 'genre' without any intentions other than to beguile the eye. Following Wiegand and Falkenburg, each of whom has researched and interpreted the work of a single artist (Ruisdael and Patinir respectively), the author, too, focuses on one artist. Claes Jansz. Visscher is generally regarded as the publisher and artist who decisively influenced the acceptance of the landscape as an autonomous work of art without a narrative or moral tenor. One of his first publications of his own work was the series Plaisante Plaetsen of about 1611, consisting of an allegorical title print, a view ofZandvoort with the list of contents, followed by ten small landscapes in the environs of Haarlem. The author offers an iconographic analysis of the first two sheets, comparing them with Visscher's religious views, as far as these can be deduced from his life and work. Visscher was an orthodox Calvinist, and his ideas about the place of art and the artist in society were presumably formed by John Calvin's dogma. There are two ways of looking at this. In the first place Calvin, obedient to the Second Commandment in Mosaic Law, purged public worship of Divine or human representations. He did see a task for art outside the church, but only if it had a didactic, edifying character. However, another aspect of Calvin's teachings suggests that art and religion are compatible. His dogma hinges on a view of earthly reality which, unlike that of mediaeval theology, is not negative but positive: a visible reflection of the invisible divine presence. Accordingly, instead of shunning the world and nature, man should enjoy and indeed investigate them in order to gain knowledge of God's creation and thus of God Himself. This idea of creation and the concomitant mission to investigate were of great significance for the development of empirical science. The same now applied to art, inasmuch as it pursues the visual examination of nature and its registration on the flat surface. This implies works of art done 'from life' rather than 'from the mind', and generated the tradition of the empirical, 'topographical' landscape art which flourished in seventeenth-century Holland alongside the landscape which was a mental invention composed of separate elements. Seen against that background, Visscher's two representations may be interpreted as follows: 'This series is intended as a monument to Haarlem. The city boasts not only a glorious and devout past but also most pleasant surroundings. They can compare with Classical landscape, but have a character of their own, and may therefore be praised both in Latin and Dutch. The city may bask in the knowledge that God directs the radiant light of his mercy on her, as the sun shines upon Haarlem's dunes. But Haarlem's glory does not render her haughty: the thorntree in her coat of arms is a reminder that all earthly things are transient. Let the sight of this city and the knowledge of her history thus incite the beholder to sobriety and diligence. Should this mean that you have no time to visit the pleasant spots in the surroundings of Haarlem, these pictures offer you a walk on paper. Be mindful that your own conduct in life match the tenor of this print. 'I, Claes Janszoon Visschcr, the printer of these views, am an educated and versatile artist and a God-fearing man. My work as an artist may be seen as the portrayal of what 1 have read in the book of creation. With my art I open a window on God's nature as it were, not only in the form of these lifelike memories of my walks around Haarlem, but on God's creation as a whole, as its chief elements are condensed in this panorama which also contains a reference to my own name and emblem.' The moment at which these two representations were published suggests that they were intended as a visual programme, not only for this modest series of prints but for Visscher's entire activities as an artist and publisher of prints. His approach to nature, incidentally, is wholly in keeping with that of the poets of his day, who presented their pastoral verses as paeans to creation and the Creator. The notion of a pious walk on paper stayed alive throughout the seventeenth century. In 1685, for instance, a book of meditations on God's nature was published, and reprinted many times; it took the form of walks around Haarlem, illustrated with six landscapes done 'from life', including a view of Haarlem in the manner of Vermeer's celebrated panorama. The above interpretation does not preclude a particular didactic or other associative value in individual landscape motifs. Even then, however, and perhaps first and foremost, they are depicted as the object of (pious) enjoyment. In all these cases a message is conveyed. It is the artistic formulation of the message that determines the work's quality. Seen in this light, the painted landscape in the seventeenth century was not intended primarily for artistic enjoyment but was meant to inspire personal meditation, even if for art-lovers the latter tended to recede into the background in practice.

ObjectiveTo present a seven-cases serie of Mowat-Wilson syndrome (MWS).MethodAll patients with positive mutation for the ZEB2 were evaluated by a geneticist and a neurologist, with clinical and laboratorial characterization.ResultsA peculiar facies and mental retardation were present in all patients. The Denver II scale showed intense delay in all aspects, especially fine motor and adaptive. Acquired microcephaly was observed in five patients. Only one patient did not present epilepsy. Epilepsy was focal and predominating in sleep, with status epilepticus in three patients. The initial seizure was associated with fever in most patients (4/6). The EEG showed epileptic focal activity (5/7). The imaging studies revealed total agenesis (4/7) and partial agenesis of the corpus callosum (1/7).ConclusionPhysicians who care for patients with mental retardation and epilepsy should be aware of SMW.

Las letras del alfabeto constituyen un escenario de posibilidades gráficas que combinan el potencial visual con aquellos significados verbales e icónicos que introducen los signos empleados. Debido al poder comunicativo de las letras, las obras realizadas posibilitan una múltiple lectura, ya que imprimen un carácter polisémico que transita entre lo visual y lo verbal. Para organizar cada idea utilizo la serie como esquema de conjunto, algo que viene influido decididamente por mi formación como grabador. En la mayoría de ocasiones la serie está constituida por un alfabeto de veintiuna letras. En este artículo hago un repaso a mi trayectoria como artista visual destacando el uso del alfabeto como material compositivo y organizativo. También incluyo mi faceta como educador en artes, una componente necesaria para llevar a cabo mis actividades como creador y como docente. Graphic creation in visual arts with conceptual ideas woven with lettersDuring three decades, I have always used the Latin alphabet as creative structure in my artistic production. The letters of the Alphabet constitute a scenario full of graphic possibilities, which combine the visual potential with the verbal and iconic meanings of the characters being used. Due to the communicative power of letters, my artwork enables multiple readings, for its polysemic personality travels between the visual and the verbal world. I use serials as a scheme, as a way to organize ideas, something that comes from my training as printer and engraver. In most cases the series consist of a twenty one letter alphabet. In this article, I review my career as a visual artist, emphasizing the use of the Alphabet as a compositional and organizational material. I also mention my role as an art teacher, a decisive role to carry out my activities as an artist. 14657 Background: Most patients with disseminated germ-cell tumor have an excellent prognosis with cisplatin-based combination chemotherapy, although there have been described certain subgroups with a worse prognosis. The presence of liver metastases (LM) represents an independent cause of poor prognosis. This study reviews the clinical course and treatment results of patients treated at Instituto Nacional de Cancerología de México (INCan-Mex). Methods: The records of all patients with germ-cell cancer and LM between 1992 and 2002 were reviewed. Age, primary site, metastases site, number of metastasis sites, histopathology type, serum tumor markers (STM) levels, liver functional assay, number of LM and used chemotherapy were examinated. The overall survival (OS) and disease free survival (DFS) were analized with Kaplan-Meier method and Log Rank test. Results: Of 32 reviewed patients, median age was 24 (range 18–42 y). The primary site was testis in 27 patients and retroperitoneal in 5 patients. The number of metastasis sites was > 3 en all cases. All patients had nonseminomatous component, predominating choriocarcinoma (82%) and seminoma (78%). The 60% of patients had STM of poor prognosis. 50% of the patients had abnormal liver functional assay. 25 cases (78%) had multiple LM. First line chemotherapy had complete response (CR) in 2 patients, partial response with STM negative (PRM-) in 12 cases, partial response with STM positive (PRM+) 8, and progression in 9 patients. All patients were treated with cisplatin-based chemotherapy. No prognostic factors of chemotherapy response were determined. Only 11 cases underwent to resection surgery of retroperitoneal or pulmonary residual. No patients underwent to liver surgery. Five year OS of 32 cases were 50%, 34% with DFS, between the patients with superior response to chemotherapy (RC + PRM-) the OS and DFS was better in relation with minor response or progression (p < 0.05). The cases with RC in liver (11) were 5-year OS 100% and DFS 66% (p < 0.05). Conclusions: The clinical course and results of treatment in the 32 cases of the INCan-Mex were similar to the literature. This study represents the greatest individual serie reported. These patients with poor prognosis are candidates to innovative treatment modalities. No significant financial relationships to disclose.

Abstract:
A revision of literature was done with the objective of tracing an epidemiologic profile of neurocysticercosis (NCC) in Brazil. The prevalence was 0.12-9% in autopsies. The frequency was 0.03-7.5% in clinical series and 0.68-5.2% in seroepidemiological studies. The disease corresponds to 0.08-2.5% of admissions to general hospitals. Patient origin was rural in 30-63% of cases. The most involved age range (64-100%) was 11 to 60 years, with a predominance (22-67%) between 21 and 40 years. The male sex was the most affected (51-80%). In the severe forms there was a predominance of urban origin (53-62%) and of the female sex (53-75%). The period of hospitalization ranges from 1 to 254 days and 33 to 50% of patients suffer 1.7 ± 1.4 admissions. The clinical picture was variable, with a predominance of epileptic syndrome (22-92%) and intracranial hypertension (19-89%). Psychiatric manifestations were associated in 9-23% of patients. Lethality was 0.29% in terms of all diseases in general and 4.8-25.9% in terms of neurologic diseases. The asymptomatic form was detected in 6% of patients in clinical serie and in 48.5% of case from autopsies. The racemose form and ventricular localization also was observed as asymptomatic form. Among the patients with cutaneous cysticercosis 65% of them showed neurologic manifestations.
