Sepsis is a clinical syndrome defined by the presence of both infection and a systemic inflammatory response with or without organ damage. The pathogenesis of sepsis is complex and may differ according to the infecting microbe, the site of primary infection, and the host’s immunological and physiological status prior to infection. The term ‘systemic inflammatory response syndrome’ refers to the clinical manifestations of a dysregulated host immune response, while ‘bacteraemia’, in contrast, refers to the presence of viable organisms that can be cultured from blood.

Abstract:
Sepsis is a clinical syndrome characterized by systemic inflammation in the presence of infection. The source of infection may be occult. One must be aware of the epidemiology, presenting features and complications, diagnostic considerations and tests, and the organisms involved. Bacteria (gram positive and negative) are most commonly associated with sepsis, although fungi, viruses, and parasites can cause sepsis. Infections in the lungs, urinary tract, abdomen, skin, brain, and other areas can cause bacteremia and lead to sepsis. Treatment includes airway, breathing, and circulation (ABCs) management; aggressive fluid resuscitation; early administration of broad-spectrum antibiotics; and early goal-directed therapy and severe sepsis resuscitation bundle. Diagnosis can be challenging in pediatric and geriatric populations.

Sepsis is common and has been the focus of recent large-scale, multi-centre trials internationally. Mortality rates have improved, largely due to a focus on early identification, key interventions, and close monitoring. Setting goals and resuscitation targets coupled with frequent reassessment is the essence of modern sepsis care. Retrieval services bring the intensive care unit to the patient. Specific conditions such as infective endocarditis, central nervous system infections, respiratory, gastrointestinal, abdominal, obstetric, and necrotizing soft tissue sepsis require special consideration. With newly emerging infections and increasing air travel, the importance of a travel history is emphasized. Sepsis in the immunocompromised host gets a special mention due to the complexity of patient, organism, and drug interactions. The chapter is completed by a table to guide empirical antibiotic choice.
