Full textThe accelerated aging of the Chinese population, a consequence of the “one-child policy” and China’s rapid economic development, has prompted the government to deeply transform its welfare system. These transformations affect all aspects of social action related to aging: pensions, health insurance, long-term care insurance, and housing. This thesis explores the government’s efforts at all institutional levels to adapt long-term care and housing supply to a society grappling with the decline of Confucian familialism. Locating at the intersection of the geography of aging and geographical gerontology, this thesis particularly examines the production of yanglaoyuan (nursing homes and retirement homes) and policies for adapting existing housing to the elderly. It draws on the case of two metropolitan areas with similar demographic size but differing economic maturity: Hangzhou, a prosperous coastal city, and Zhengzhou, an inland city in the midst of development. The theoretical framework connects debates on the characterization of social protection regimes to the literature on housing policies and asset-based welfare. The empirical contribution is substantial, based on surveys of 70 public and privatetakehold stakeholders and an ethnographic analysis of 50 residential projects. The results highlight a strong emphasis on aging in place within a home ownership- oriented doctrine, with the goal of providing 90% of residential options with minor local variations. Municipalities oversee the requalification of existing buildings and, as part of urban renewal projects, establish a dense network of community-based services for basic elderly assistance, in coordination with local hospital infrastructure. These initiatives are made possible by the significant public assets inherited from the socialist era, which can be repurposed, and more generally by the continued state ownership of land. This advantage also allows local governments to disengage from the operation of public yanglaoyuan by attracting private operators through the provision of free or low- cost public facilities. Everywhere, this “asset-light” business model is promoted to develop a wide supply of non-profit yanglaoyuan. However, support for this type of housing also involves public subsidies, leading to significant regional inequalities in long-term housing supply based on the financial capacity of local authorities. Even when subsidies are substantial, as in Hangzhou, they are insufficient to significantly reduce housing costs and ensure adequate quality of care, rendering the supply of yanglaoyuan inadequate in terms of medicalization and pricing. These dynamics contribute to reinforcing social and spatial stratification in the living conditions of the elderly, driven by differences in financial capacity of local governments as well as households due to notable disparities in pension amounts based on former employers

In recent years, senior villages have drawn more attention because of population aging. Moreover, as various deadly and highly infectious diseases have continued to emerge, and the elderly usually have weaker resistance to disease, the outbreak of infection may easily get out of control in senior villages. To perform infection control, medical personnel use the manual method of tracking, investigating, and interviewing patients to acquire patient contact list. This can neither find people who have high infection risk nor to distinguish the distance between patients and other residents. This study proposed a visual infection control positioning system for senior villages using the RFID (Radio Frequency Identification) technology. In order to effectively calculate the distance, this study combined the KNN (K-Nearest Neighbor) algorithm and the location fingerprinting method. KNN algorithm is used to fix errors caused by signal fluctuation to improve the positioning accuracy and effectively calculate the target position. Location fingerprinting method is used to establish fingerprinting database in order to reduce the number of deployed RFID equipment and to reduce the cost of establishing the system. The system records the indoor movement path of the residents in real-time and visually displays the distance between the patients and other residents. Using the proposed system, medical personnel can identify residents with potentially high risk of infection and adopt proper medical measures.

In this study, we use the principle and theory of social capital to study the quality of life of seniors living in senior villages; we hope to explore the relationship between social capital and the quality of life of seniors living in senior villages, as well as to understand the diverse and dynamic lifestyles within senior villages. The advancing and continued evolving healthcare system, science and technology, society and education have all contributed to the increasing life expectancy of the human population. The aging population has created the need to build senior villages. Today, both non-profit government organizations and private companies have started to fund and construct senior villages to accommodate the aging population - offering better quality of life and living environment. Much current research regarding the operation and management of senior villages focuses their studies on the quality of foods and housing within senior villages. For example, most research focuses on the reasons why residents decide to live in senior villages, some research studies the structure of the healthcare system, while other research studies the relationship between living environment, leisure activities, voluntary signup and satisfaction. However, the effects of social capital on the quality of life have, yet, been explored. Therefore, this study uses seniors who are residing in senior villages as research subjects, and investigates the impact of social capital’s Internet on the quality of life of seniors. We study past literatures, recognize the possible spiritual beliefs and values among seniors in the near future; we then explore the operation and management of well-established domestic senior villages. This allows us to study the relationship between social capital and the awareness and appreciation of quality of life among seniors living in senior villages. This study has its academic uniqueness and industrial practicality.

The objectives of this research are to have better understanding of the current long-term care operation and system at three villages: Fuyuan, Fumin and Fuxing in Ruisui Township of Hualien County in the hope of discussing and analyzing the obstacles of suburbs and tribes from a grass-roots perspective. To achieve the above-mentioned objectives and to understand the long-term care issues in these areas, the in-depth interview (first step) is adopted. In the meantime, depending on relevant literatures, the questionnaires are designed to find out solutions, and the specialists in long-term care services in Ruisui County as well as in Guangfu County are considered as the main survey population. After the questionnaires are released for the second time (the sample size was 43 for the second step, and 38 for the third step), the Delphi method is used to evaluate consensus among a panel of experts. The acquired data will be analyzed by One Sample t-Test, Independent Sample t-Test and Analysis of Variance via the software SPSS 20.0. The conclusions are as follows: 1. The expert consensus is focusing on four aspects: "Take the initiative in controlling potential target groups in favor of counseling applications in the future", "Cultivate local manpower to provide services and reduce the personal losses", "Raise the salary and improve the workfare to increase caregivers' willingness to stay" and "Simplify the redundant application procedures". 2. After tested by the software SPSS 20.0, the above-mentioned four factors turned out to be significantly important. What drew our attention the most is the dimension of "Course Diversity": it wasn't highly focused in the expert consensus; however, the significant data were shown by different tests afterwards. It shows that this dimension is approved and recognized by the experts as the potential improvement. Based on the results of research, the following suggestions are made as reference: 1.Take the initiative in controlling potential target groups and provide relevant services in good time. 2.Cultivate local manpower and increase employment opportunities in suburbs and tribes. 3.Raise the salary and improve the workfare to hire talent to work in suburbs and tribes. 4.Simplify the long-term care application procedures to avoid the delay of document process. 5.Plan courses in accordance with different locations and people to increase the production by using talent in an efficient way. 6.Currently, the long-term care 2.0 can be divided into Level A, B and C. Yet, how to make different levels in suburban and tribal areas? The researchers in the future might be able to continue the research on such direction. 7.It is suggested that quantitative research can be added in the study in order to understand deeper the problems, which may increase the reference value of research.

The development of leisure agriculture is an important policy transformation of agriculture in Taiwan. Many farmers and farmland owners have invested in competition to set up new leisure farms. Most of them imitated each other, and were unable to highlight the characteristics of individual leisure farms. It may be a forward-looking development direction to transform into a health village that combines leisure farm and intelligent technology. This research is a mixed study of quantitative and qualitative. The research subject is the leisure farm. By using the in-depth interview method and the SWOT analysis matrix as the basis, the leisure farmer's opinions on the introduction of the concept of senior intelligent health village are discussed. Then using the questionnaire survey method, the technology acceptance model as the theoretical basis, it evaluated the acceptability and willingness to stay of the seniors to the leisure farm with intelligent health village concept. The results showed that the research model can effectively explain the perceived ease of use, perceived usefulness, expectation, and willingness to stay of the seniors for the leisure farm with intelligent health village concept. Finally, the feasibility assessments of introducing leisure farms concept into the senior intelligent health village are proposed based on the results. And specific management implications and research recommendations are also provided.

Due to the fact that the progress of modern medical technology and the popularize of health care system, the average of life expectany in Taiwan has increased steadily. Taiwan has turned into an aging society, and the rate of the seniors group rised steadily since 1993. Family patterns and living patterns have changed because of the low birth rate, and the pressure on children, to take care of their elders, is getting exacerbating. Although aging population is the trend of the world, Taiwan is one of the fastest aging country. To adapt living pattern in a short time, the demand for the seniors housing and seniors nursing might be increased for the upcoming future. The emphasis of this research is to discuss the management of Senior Health Village, in the aspects of human resources, marketing, financial affairs, expansibility, quality of service and so on. Through the literature review, field visiting, and the analysis of senior health village from experts, we can tell the difference between domestic and other countries. Therefore, this research not only can find out the weakness for manage the Seniors Health Village in domestic but also can make suggestions for the problem that we don''t realize at first. Elderly welfare issues need to be treated with a pluralistic and innovative way. In my opinion, seniors living environment should contain professional health care quality, safety spacious living space, elegant environment and leisure facilities. Seniors Health Village not only include all the elements above-mentioned, but also offer a high quality living space which can make seniors live independently.

Taiwanese society has become an aging society since 1993. Due to changes in societal structure and traditional concepts, phenomenon such as aging people started to emphasize more on their personal life after retirement and increasing ratio of aging population, either single or with domestic partner, move into specially designed retirement apartment have brought up to attention. In order to effectively encourage aging resident to exercise beneficial recreation and socialization activities, safe and proper common activity areas in retirement apartments are necessary and thus, the design and planning of the common recreation areas has become an essential issue. Questions regarding whether the retirement apartments will fully satisfy the needs of their aging resident and whether the aging residents actively utilizing those facilities are two of the most essential regards for planning, building, and improving retirement apartments; this is also the motivation for conducting this research. Our research focuses four main issues regarding as utilization and planning of public area such as “public service facility”, “in-door recreation facility”, “out-door activity space”, and “shared service space” in Chang Gung Health and Culture Village which possesses varieties of in-door and out-door public facilities and public areas; the research itself is conducted by data investigation, on-site observation, detailed interview, and paper-based survey to insightfully investigate the degree of satisfaction of aging residents toward existing planning of shared area and real feeling and reaction toward existing planning of public area. Suggestions for planning of shared areas, as results from analysis of conducted surveys, are provided for the reference of future planning of shared areas in aging resident village. The suggestions are as followed: 1.Out-door forest trails and healthcare centers are the two types of facilities that are listed among the highest degree of satisfaction by aging people. Health-oriented recreation facilities are still regarded as the most essential element in future planning of retirement apartment construction. 2.The degree of satisfaction in retirement apartment is directly associated to three factors which include “degree of education received”, “physical condition of the aging person”, and “the frequency of family visits.” 3.The current issues for public area in retirement apartments are low degree of attention and easily neglected or sacrificed. 4.Issues regarding the planning of public areas in retirement apartments includes: A.Cafeteria is one of the most important socializing space for aging residents in retirement apartment and an important space for family visits. Degree of comfortableness and spaciousness of cafeteria and whether the cafeteria offers socializing opportunities to aging residents are the two main concerns of aging residents. B.Utilization degrees in lobbies that provide newspaper are higher than lobbies that do not provide newspaper. The lighting and management of such lobbies are needed to be emphasized. C.Socialization opportunities decrease due to the facts that lighting and aeration in central hallway are not quite satisfactory. D.Recreation areas that involve physical activities such as basketball and table tennis are only for family members that visit the aging resident and are not satisfactory to the demand of aging residents. 5.Plan the resident area for each aging resident by utilizing principles of universal design is suggested. 6.The public space of retirement apartment for daily live, such as social room or laundering room, shall be disposed fitting Cluster of Living Style for accessibility to increase the using of the elderly; As for the common space of leisure for all elderly, Such as learning room, recreation room, conference room, it is suggested be allocated together for the convenience of management.

The population of seniors will account for one-fifth of the total population in Taiwan in 15 years, and the living arrangement and housing demand of senior citizens will change with the aging phenomenon and the transformation of social structure. Meanwhile, the autonomy and independence of senior citizens will be relatively elevated, and they will demand not only medical care during convalescence, but also maintain better and healthier quality of life. The Executive Yuan has proposed a related long-term care system and promotion program for seniors' housing construction in the hope that senior citizens can enjoy a physically and mentally healthy retired life. Past literatures related to caring institutions mainly researched into the service quality and management strategies of the institutions. Although there were quite a few discussions over factors for customers' selection of the institutions, they did not deeply explore factors that senior citizens valued the most. Through literature analysis, this study divided factors that senior citizens selected to stay in health villages into five major aspects including health care, software and hardware equipment in a health village, recreational activity and learning, geographical environment, management method and price, and twenty evaluation criteria. 130 questionnaires were distributed to senior citizens, and 122 of them were retrieved. The study adopted analytic hierarchy process (AHP) to organize factors for seniors’ selection of health villages and analyzed the appropriate weight of each aspect and criterion to propose concrete suggestions for experts, service providers and governmental units when they establish health villages in the future. The results of this study found that the order of aspects that seniors valued in choosing health villages was “health care,” “management method and price,” “recreational activity and learning,” “software and hardware equipment in a health village” and “geographical environment.” Among criteria, the most valued one was “reliable goodwill guarantee,” followed by “reasonable charge,” “friendly attitude and emergency handling abilities of service personnel” and “healthy diet with balanced nutrition.” The least valued ones were “sufficient recreational green areas outdoors” and “course arrangement for lifelong learning.” Moreover, this study discovered that recreational green areas and courses for lifelong learning emphasized by renowned health villages were not the factors that senior citizens valued the most. Good public reputation, reliable goodwill and reasonable charge were the primary factors that they gave thought to. The factor of “people” in a health village was certainly a key, and thus, the cultivation of the abilities and service attitudes of service personnel should not be neglected. Furthermore, senior citizens also have certain demands for a healthy diet. Therefore, service providers of health villages must pay particular attention to the nutrition of daily diet and the freshness of ingredients. Only if a healthy environment for retirement that meets the expectations of senior citizens is constructed to reduce the time of long-term care in the future can our aging society be healthy and energetic.

Social values change through time. In the past, the social welfare organizations of the elderly were only for the minority or the poor. However, nowadays, due to the need of family and one’s self, more and more elderly choose to live in these institutes. In recent years, many of our domestic enterprises have seen the potential of this market and start to develop senior housing. Special guidelines need to be followed when designing senior housing so as to assist senior’s citizen in daily life. Users’ perception toward the environment proposed in Place Theory is used in this research to conduct survey and interviews so as to explore and set out the principles for designing senior housing. Follow-up survey on how senior citizens respond to the facility and Post-occupancy evaluation are also conducted as a reference for future senior housing designing projects. The results are as follow: 1. Citizens in The Linkou Chang-Gung Health and Cultural Village, in average, have higher education background. More than half of the citizens hold a college degree or above and most of them originally live in northern Taiwan. It’s important to note that when the elderly choose the location of their senior housing, places near their original houses are always their priority. 2. In general, it is thought that the elderly move into senior housing because the need of the facility and the service it provided. However, the more realistic concerns for them to accept the quasi formal caring system are that their children are too busy to care for them, or that they live alone, or that they have lost their spouses, and that they could still take care of themselves in their daily life. 3. The facility, barrier-free design, and open space arrangement within The Linkou Chang-Gung Health and Cultural Village are among the best. During the interview, the interviewee gave high credits to the living environment and the entertainment the village provided. Residences here on average spend minimum 1~2 hours and maximum 4~5 hours on using this facility. The research shows that, due to the decreasing mobility of the elderly, the closer the facility is to the indoor, the higher the users’ rate is.

The population aging is a phenomenon in Taiwan society caused by the well developed technology of today. The proportion of aged population is getting higher due to the late marriage and more and more people choosing not to bear child. Therefore, the concept of elderly residence emerges and the ideas of barrier free environment design, suitability, and lifelong design become the goal in designing such community. The government responds to the phenomenon and starts to construct the elder friendly environment and residence, publishing the “Act for Promotion of Private Participation in Elderly Residence” in 2004 to promote the private participation in elderly residence. The elderly residence gradually shakes off the negative stereotype and offers secure, cozy, and health living quality for elder citizen. The study finds out that the arrangement for outdoor environment of the elderly residence should use natural elements to create the artificial view and focus on the principle of generic design, planning traffic flow from the residence to the public facility with high accessibility and setting up anti-skidding surface, deploying outdoor seats and rehabilitation equipment fitting the elderly ergonomics. The study probes into the spatial and barrier free requirements for elderly residence, simulating the on-site environment to make the arrangement, developing a beautiful health living community with high security. The study may serve as a useful reference for designer making the arrangement for elderly residence.

Abstract The literature shows that the meal together program is a positive activity that can contribute to successful aging, and has a positive effect on solving the health problems of seniors. The purpose of this study is to understand seniors’ motivations for participation the meal together programs, and explores more influences on motivations. Both qualitative and quantitative methods are used in this study. First, the semi-structured interview of qualitative study is conducted. According to the proportion of men and women, the subjects are 12 seniors, including 5 males and 7 females living in the Haiguang Village, Kaohsiung. After interviews, the transcript data is gathered and coded. Triangulation is used for the content validity and reliability. Second, based on the pre-test qualitative research, questionnaire survey of the quantitative method is designed to investigate 417 seniors’ motivations for meal together participation. Independent-sample T test and one-way ANOVA are used to explore the differences among background variables in participation motivations. The results of this study can be conducted as: First, the distance of the meal location significantly affects the willingness to participate. Second, the limitation of meal together programs do related to the motivation of seniors participants who would have a need to dine with his family. Third, the meal leftovers in family affect meal together participations. Abstract Background: The National Development Council predicts Taiwan will become an ultra-aging society by 2025, when the proportion of the senior population rise to 14%. Facing the double crises of population aging and childless, the long-term care for the senior citizens will become a critical issue. Therefore, it is essential to conduct an in-depth study on the behavioral intention and selection factors of senior citizens to live in a retirement home. Purpose: This study aimed to explore factors affecting seniors’ intentions and the important selection factors to them to reside in a retirement village. Design/method: This research utilized a mixed method of quantitative and qualitative procedures. Three hundred and forty valid questionnaires have been collected via in-class surveys of Tainan and Kaohsiung Senior Citizens Learning Camps. In addition to quantitative research, this study also conducted focus group interviews with senior’s citizens and semi-structured interviews with one retirement village owner and the faculty representatives. Results: Quantitative analysis results showed that attitude, subjective norm and perceived behavioral affect participants’ behavioral intention significantly. Among which, attitude presents the greatest impact. The higher the income, the greater the willingness to stay in the village, and can bear the risk of health management at the village. In addition, the most important selection factor to choose a retirement village is the brand image. Conclusions: All hypotheses were found supportive. Based on the research results, the implication and recommendations to the government and retirement village owners are provided. For follow-up researchers, since the views of the elderly in different areas may be different, it is recommended to continuely conduct other studies to the older people who live in different counties and cities. Donations and government grants are important financial sources of non-profit organizations. However, the supply of external support are instable due to the impact of economy fluctuations. Moreover, as more and more newly established non-profit organizations fighting for the limited resources, the non-profit organizations will collapse or may erode their mission to maintain operations, which may led to a “common tragedy” result. But in the practice, we still can find some non-profit organizations operating in a low dependence, low social cost, and high satisfaction situation. To explore the strategies of these success stories, as well as their internal designs to achieve such results, this study selected the Pu-Li Senior Village, and the Bunun Tribe as the case study targets. Business models of non-profit organizations are complex, dynamic, and involve numerous causal related factors and feedback relations. This study used system dynamics to identify the causal relationships between the system behavior and the internal mechanism which contributes to the efficient and self-sustained operation of that organization. A qualitative causal loop diagram is constructed to illustrate the business model for each organization, and the key designs and their implications in building a self-sustaining business model are also discussed.

Abstract The purpose of the research was to explore the influence of environmental satisfaction level on the quality of life, focusing on residents of self-supported senior housing communities. The cross-sectional descriptive correlation method was adopted. The participants (N=76) of this study were the residents of a senior housing community in the northern part of Taiwan. The age of the participants ranged from 67 years to 93 years, with an average of 78.34 years. They are all cognitive intact and physically independent. Structured questionnaires including demographic information, environment satisfaction scale (Environment), self-care abilities scale (ADL, IADL) and quality of life scale (WHOQOL-BREF Taiwan version) were administered. Three subjects, however, were excluded due to missing out two questions on the “Quality of Life- Social Domains”. The number of valid subjects, therefore, was 73 only. The research results showed that, within the environment satisfaction scale, the residence quality domain and neighborhood status domain had positive correlation with the seniors’ quality of life. Through hierarchical regression analysis, we further identified five variables which were the predictive factors for quality of life, respectively, gender, original living status, self-care abilities (IADL), residence quality domain, and environmental/personal security. The residence quality domain was the best predictive factor which accounted for 15%-50% of the variance in the overall quality of life and various domains. The total variance can be explained by the entire model is 32% to 64%. The seniors would enjoy a better quality of life if they were not living alone; had good self-care ability (IADL) and better environmental satisfaction levels in the “Residence Quality Domain”. In terms of gender, male seniors have a better quality of life in the “Psychological Domain”; while female seniors have a better quality of life in the “Social Domain”. Based on the research results, we recommend environmental assessment can be included in the routine assessment for residents of senior housing communities. Nursing personnel need to be enabled and encouraged to evaluate the influence of environments on the seniors’ quality of life; identify and prevent any hazardous factors to the senior group; and elevate the life quality satisfaction level of the elderly. The purpose of this study was to investigate the quality of sleep and its influences on quality of life and objective health indicators for senior residents in a retirement community. This study used cross-sectional descriptive correlational approach. The participants aged 65 or older, and were cognitive intact without disability. Instruments used in this study included demographic information, self-care abilities scale (Activities of Daily Living, ADL; Instrumental Activities daily Living IADL), Geriatric Depression Scale (GDS), Pittsburgh Sleep Quality Index (PSQI) and quality of life scale (WHOQOL-BREF Taiwan version), and the objective indicators included physical body composition and peripheral circulation. A total of 76 participants were recruited. However, 3 of them were excluded due to missing out two questions on the “Quality of Life- Social Domains.” Therefore, a total of 73 participants were analyzed in this study. The results found that the average score of sleep quality was 6.52 (SD = 3.52), and 52.06% of them had poor sleep quality. Higher sleep quality score was related with lower IADL, more depression symptoms, lower overall quality of life and lower physical and psychological QOL, and higher venous efficacy (left). Stepwise regression analysis found that predictors of better quality of life were older age, female, better ADL, less depressive symptoms and better sleep quality. These predictors can explain the variance in overall and all aspects of quality of life (ranged from 19% to 40%). The depressive symptoms and sleep quality could explain 31% of the variances in overall quality of life that more depressive symptoms and poorer sleep quality predicted poorer overall quality of life. At the same time, more depressive symptoms, poorer ADL, and poor sleep quality predicted lower score for physiological aspect of quality of life and these predictors were able to explained 40% of the variance in physiological quality of life. The results of this study suggest that the sleep quality is important to be assessed for elderly persons in the community. Related intervention, management and health education which aim to enhance sleep quality can be provided to promote the quality of life and decrease depressive symptoms for this population.
