Mental illness and substance abuse are separate and additive risk factors for HIV infection. A comprehensive approach in the treatment of those with the dual diagnosis of mental illness and substance abuse is very important because these patients are inherently at a higher risk of contracting or transmitting sexually transmitted diseases, including HIV. Problems with adherence to medical treatment seem to be additive in this group. In addition, substance abuse is associated with a host of medical sequelae (liver disease, infection, diabetes, cardiovascular disease, and neurocognitive changes), complicating treatment of the virus in a population already at risk for these problems and leading to increased disease progression.

Having presented the role of upacāra in Asaṅga’s critique of an essentialist theory of meaning, this chapter now turns to examine additional aspects of the concept of metaphor as it appears in other Yogācāra-related Buddhist sources. Concluding the book’s survey of the Buddhist context of the Yogācāra, the text explores the possible ways in which a wide variety of Buddhist sources—including Vasubandhu’s Abhidharmakoṣabhāṣya (and Sthiramati’s commentary on these sections), the Yogācāra-related Laṅkāvatārasūtra (LAS), and Dignāga’s Pramāṇasamuccaya (PS)—contributed to Sthiramati’s full-fledged theory of metaphor. Here, the reconstruction of the context of the Yogācāra understanding of metaphor becomes more specific, tracing not only the broad common presuppositions underlying figurative usage, but also the possibility of a more concrete intertextual exchange that helped shape Sthiramati’s claims—some of them highly innovative—on this topic.

This chapter outlines some of our more effective demonstrations for teaching sampling. Part I of this book contains many activities related to sampling that we also use in our more advanced courses on the subject (e.g., see chapter 6 for an activity on estimating family size, and chapter 9 for a candy weighing activity). This chapter describes additional student activities that we have developed for the advanced undergraduate survey sampling class. These include provocative questionnaires to demonstrate question bias and statistical literacy packets for dissecting news stories about surveys. In addition, this chapter contains sample handouts used for teaching particular topics, techniques for encouraging student participation, and materials to organize student projects on complex surveys.

Façade technology of the 20th century is related to the dissolution of the massive wall into a separation of structure and façade. Looking at the development of façade technology, after 60 years of curtain wall systems, 30 years of element-façade systems and ten years of experience with the integration of environmental services in double façades, it can be concluded that the peak of optimisation has been reached. No further technical developments can be expected by continuing to apply extra layers for each additional technical function. Understanding façades - or better envelopes - as part of an integral building, we have to see that creating the future envelope has to be done on a ’network’ basis: employing systems - but also methods of thinking - which provide the possibility to develop different aspects simultaneously and combine them as required. The envelope has to be seen as a functional part of the entire building, serving a part of the demand by providing the necessary technologies and qualities. In this regard, we have to withdraw from material and structure-orientated thinking and construction – we have to develop the envelope as a hybrid system: materials, technologies and production processes have to be integrated into the summation and combined into an all-encompassing result. Façades comprise various themes covering strategic, material and technological developments. Aspects such as function integration, networking of elements, new structures and materials as well as the addition of functions to existing structures will be investigated and explained in 85 or so concrete ideas.

This chapter discusses challenges to arbitrators in International Centre for Settlement of Investment Disputes (ICSID) and ICSID Additional-Facility arbitrations, challenges to arbitrators in UNCITRAL arbitrations, and challenges to counsel and experts in ICSID and UNCITRAL arbitrations. Disqualification proposals have become a routine procedural mechanism used by both claimants and respondents in ICSID and UNCITRAL investment arbitrations. During the period 1982–2017, parties filed 121 disqualification proposals in ICSID proceedings. These included not only original arbitration proceedings, but also post-award proceedings, such as rectification, interpretation and annulment proceedings. In addition to arbitrators, disqualification proposals now target counsel, party-appointed experts, and tribunal-appointed experts. The chapter examines the relevant articles and rules that apply when challenges are against ICSID arbitrators, the timeliness of challenge application, the potential waiver of the right to seek disqualification, and arbitrator resignations in response to disqualification proposals. It also considers the procedure for adjudicating challenges to arbitrators in UNCITRAL arbitrations.

The case of East Germany raises the question of why religion and church, which had fallen to an unprecedentedly low level after four decades of suppression, have not recovered since 1989. The repressive church politics of the SED were undoubtedly the decisive factor in the unique process of minoritizing churches in the GDR. However, other external factors such as increasing prosperity, socio-structural transformation, and the expansion of the leisure and entertainment sector played an important role, too. In addition, church activity itself probably also helped to weaken the social position of churches. The absence of a church renaissance after 1990 can be explained by several factors, such as the long-term effects of the break with tradition caused by the GDR system, the political and moral discrediting of the church by the state security service, and people’s dwindling confidence in the church, which was suddenly seen as a non-representative Western institution.

Borderline personality disorder (BPD) is still seen in many settings as a chronic disorder. This book details the ways that it is symptomatically a “good prognosis” diagnosis. This is particularly seen in the high rate of remissions of BPD and its constituent symptoms. The rate of suicide is also good news, as it is half the expected rate from four follow-back studies conducted in the 1980s. Areas with a more guarded prognosis, particularly for those who have not recovered, are poor physical health and poor vocational adjustment. In addition, rates of other deaths are increasing and have surpassed the rate of deaths by suicide. This book covers the following topics: History of the borderline diagnosis, models of the core features of BPD, earlier studies of the longitudinal course of BPD, the McLean Study of Adult Development (MSAD), the symptoms of BPD assessed in MSAD, the long-term course of the symptoms of BPD, symptomatic remissions and recurrences of the borderline diagnosis, prevalence and predictors of physically self-destructive acts over time, additional symptom areas over time, psychosocial functioning over time, recovery from BPD, predictors of time-to-remission and recovery, co-occurring disorders over time, mental health treatment over time, physical health and medical treatment, adult victimization over time, sexual issues over time, defense mechanisms over time, and new directions.

High courts differ in the processes for choosing who sits on the court, the size and composition of panels, and the cases the court hears. Once the case is before the court, a judge may also be influenced by the parties that appear in the appeal. This chapter examines this influence. The government, for example, tends to be more successful in many cases than other parties. This success may be due, for example, to the government having more resources than other parties or being a repeat player in front of the court. Further, judges seem to at least be modestly influenced by interveners, who are not actually the parties to the appeal but seek to provide additional information. Judges appear to consider the information provided by the interveners in some contexts, though the presence of interveners also appears connected to an increase in the probability of a judge dissenting.

The following is an update to a book entitled, “A Historical Review of Enzymatic Debridement: Revisited”, which I wrote in 2003. Since its publication, while the relevant clinical evidence has remained consistent, the amount of biochemical research and knowledge gained has been impressive. In the first chapter a sampling of the typical topical enzymatic debriding agents that have been used in wound care are reviewed and interestingly enough only one remains on the market. The FDA has removed all others from the marketplace and an explanation is provided in chapter one along with descriptions of the use and mode of action (MoA) of these agents. Chapter two is a review of the many different types of collagen found in the body, including their structure, form, and function as so much additional insight into this molecule has been gained since 2003. In chapter three we see an account depicting the many advances in understanding matrix metalloproteinases (MMPs) reviewed in detail. Form, function, tissue orientation and preferred substrates are addressed. Finally, in chapter four we see the history of the MoA of MMPs as compared to bacterial collagenase starting in the early ‘80s to the time of this current publication. In addition we see the level of complexity of bacterial collagenases compared to MMPs, helping us to better understand why bacterial collagenase is much more efficient at removing necrotic tissue from wounds than are our own (endogenous) MMPs. I hope the reader finds this review useful from an academic standpoint, but more importantly from a clinical framework helping to understand the role of these types of therapies in wound care.

In addition to reasoning from the valuational condition we are already in, it is possible to reason toward value. This form of value-learning is aspiration. Its neglect in the philosophical literature on rationality, moral psychology, and responsibility is visible in certain subtle ways in which the space of possibility has been narrowed in each area. But aspiration must be possible, since it is actual. We see it in Alcibiades’ speech in Plato’s Symposium. Plato shows us the tortured condition of someone who can almost see what it would be like to see things differently: Alcibiades’ conventional honor-loving values move him to flee from and hate Socrates, even as he struggles to recognize that the honor-loving life is not worth living. He can almost see his values as bankrupt—but he can’t quite, because they won’t go away.

Intelligence can be considered a process, a product, and an institution. Institutions in particular point toward the idea of national security, since intelligence services are curiously bound up with both state sovereignty and the core executive. Preemption is perhaps the most important idea that has served to enhance the importance of intelligence. One of the most enduring definitions of intelligence is that it is a special form of information that allows policy makers, or operational commanders, to make more effective decisions. Quite often this intelligence is secret in nature, consisting of information that an opponent does not wish to surrender and actively seeks to hide. And although it is widely accepted that intelligence studies as a field is under-theorized, some areas have received more attention than others. Perhaps because policy makers have seen warning against surprise attack as one of the highest priority intelligence requirements, this area has been the most fully conceptualized. In addition, intelligence agencies themselves have frequently advanced the claim that their ability to lend a general transparency to the international system improves stability. Also, these agencies not only gather intelligence on world affairs but also seek to intervene covertly to change the course of events. Another controversial aspect of intelligence involves the cooperation between intelligence and security services.

When individuals experience distress, they try to make sense of this and, in the first instance, may seek help from personal, folk, or social sectors. If these interventions do not work, they will contact the professional sector. It is likely that the healthcare system will direct their help-seeking behaviour. In addition, the explanatory models they have will direct them into help-seeking accordingly. Once therapeutic interaction has started, the explanatory models of the individuals, their families, carers, and those of the clinician will affect therapeutic engagement. Race, gender, social status, education, and economic status will all affect explanatory models and where individuals seek help. If different from that of the patient, the culture of the clinician will affect therapeutic alliance. Working with interpreters requires training if the primary language of the patient differs from that of the healthcare professional.

The chapter uses autoethnography and personal interviews to illustrate the experiences of white and black women in taiko. Given that a majority of taiko players in the United States are Asian American, taiko is a rare site in which white bodies are seen not as normal but rather as remarkable. Some black women, however, are seen as more American than their Asian and Asian American groupmates. In addition to the impact of racial identity, white and black women also experience taiko as open to a range of gender expressions and as an empowering art form. This chapter examines the psychology of advice recipients, focusing on research predominantly conducted using the Judge Advisor System, in which a participant “judge” receives advice from one or more advisors but has ultimate responsibility for making the decision. First, it reviews methods of typical Judge Advisor System experiments. Next, it surveys the research to explore why decision makers often do not seek out advice, focusing on the costs of advice and decision-maker overconfidence. It then examines why decision makers underutilize the advice they receive due to factors like confirmation bias, egocentric discounting, and power. In addition, factors that increase the utilization of advice, such as trust, advisor confidence, and advisor expertise, are considered. Finally, the influence of advice-recipient power and reception to computerized advice are examined in depth. Finally, advice to decision makers about how to seek and utilize advice to make better decisions is provided.

This book collects the scholarly work that Sam See had completed as of his death in 2013. It includes essays that have been previously published in leading journals as well as materials that remained unpublished. Its parts represent the two book projects that See hoped to complete: Queer Natures: Feeling Degenerate in Literary Modernism and Queer Mythologies: Community and Memory in Modern Literature. The first reinterprets the key term nature, central to so many discussions of literature and sexuality. For See, nature is no longer an unchanging substrate or a philosophical given. Relying on a thorough reading of Darwin, See argues instead that nature is constantly and aimlessly variable. Since it makes room for the aesthetic, by way of what Darwin called sexual selection, nature is also affected by feeling. On these grounds, See argues that nature itself might be considered queer. The second project proposes that, understood as queer in this way, nature might be made the foundational myth for the building of queer communities. See looks at the ways in which queer community has been imagined in literary works from a wide range of authors, and he analyzes the role that literature has played in providing significant aesthetic versions of that community. Locating the various failures of these myths is a way, he hopes, of approaching another, more successful communal story. In addition to his reading of Darwin, See provides new interpretations of modern writers including Langston Hughes, Virginia Woolf, Oscar Wilde, Hart Crane, and T. S. Eliot.

This book is one of the most controversial horror films ever made. Despite not achieving huge success when it was first released, the Italian production found an audience on home video in the 1980s and became a 'must-see' for connoisseurs of extreme cinema. Indeed, Cannibal Holocaust's foremost legacy is in the United Kingdom, where it obtained its reputation as one of the most harrowing and offensive 'video nasties' — a term used to refer to a group of films deemed to be 'obscene' by the Department of Public Prosecutions. However, as the years have progressed, Cannibal Holocaust has been re-evaluated, mainly as the forefather of the 'found footage' film, and recent home video re-releases have added some valuable perspective to the onscreen violence with extensive cast and crew interviews. What is missing from this contemporary activity is contextualization of Cannibal Holocaust's style, affirmation and discussion of its locations and any extensive discourse about its representation of third world inhabitants (i.e. as 'primitives'). In addition, and also amiss from previous dialogue on the production, is that Cannibal Holocaust can be seen as one of the key post-Vietnam films. It is the spectre of war — and an explicit warning about Western involvement in civil conflict — which progresses Deodato's story of jungle adventurers in peril. By approaching the film from a more formalist position, this book provides an insightful discussion of this groundbreaking film.

As will be seen, those putatively thin premises in fact depend on a number of deeply controversial assumptions. Although at least some of those assumptions are very likely false, this chapter does not need to establish their falsity. Instead, the point will be to reveal that liberal neutralism in one of its most prominent and perceptive instantiations is fundamentally non-neutral. In addition, the chapter will expose the oddly asymmetrical reasoning through which Gaus argues against anarchism and in favor of private property.

Obsessive-compulsive and related disorders (OCRDs) have received considerable attention over the past two decades, culminating with the inclusion of a new classification category of “Obsessive-Compulsive and Related Disorders” in the DSM-5. This group of conditions includes obsessive-compulsive disorder along with two newly minted conditions (hoarding disorder and excoriation disorder) and others previously classified as somatoform disorders (body dysmorphic disorder) and impulse control disorders (hair-pulling disorder). In addition, other conditions that are not discussed in the DSM-5 have received attention, such as misophonia and orthorexia nervosa. The implications for research on these conditions, as well as their relations with one another, are significant since their aggregation is based on putative central mechanisms with limited empirical support to date. Indeed, the past decades have seen a dramatic surge in research on OCRDs across several domains, including clinical phenomenology, assessment, and psychological therapies. With these issues in mind, this comprehensive text addresses recent advances in the field of OCRDs, highlighting psychosocial theoretical and intervention approaches. As researchers and clinicians will be increasingly focused on this topic in light of the changes to DSM-5, this book is a timely addition to the literature in guiding clinicians in advances in OCRDs that will impact their practice.

In the past 50 years, classification systems for renal neoplasms have become increasingly complex as distinctive morphologic patterns in renal neoplasms have been recognized and correlated with clinical findings. In addition to classic histopatology, more sophisticated diagnostic tools, including electron microscopy, immunohistochemistry, cytogenetics, and molecular diagnostic techniques have greatly influenced distinctions between various types of renal neoplasms. The current World Health Organization classification of renal neoplasms encompasses nearly 50 distinctive renal neoplasms categorized as malignant or benign tumours. These categories have been expanded during recent years to incorporate newer histotypes, thus suggesting that the next revision of this classification will incorporate some recently recognized entities. In this chapter, we examine clinicopathologic and genetic features of the renal tumours most often seen in clinical practice.
