Abstract:
This chapter is divided into three main sections. The first section concerns the patient population that presents for orthopaedic surgery, specifically examining chronic diseases of the musculoskeletal system and the medications commonly used for their management, and the impact this has when these patients present for surgery. Included in this section are the surgical considerations and the anaesthetic implications of orthopaedic surgery, ranging from patient positioning to bone cement implant syndrome. The last part of this first section looks at specific orthopaedic operations, starting with the most commonly performed, hip and knee arthroplasties, and moving onto the specialist areas of spinal deformity, paediatric, and bone tumour surgery that are not usually found outside of specialist centres. The middle section gives a brief overview on analgesia concentrating on pharmacological methods as, although orthopaedic surgery lends itself well to regional anaesthesia, this is covered elsewhere in its own dedicated chapters. No section on analgesia would be complete without mentioning enhanced recovery: the coordinated, multidisciplinary approach that improves the patient experience, increases early mobilization, and reduces length of stay, which should be the standard obtained for every patient. The final section covers the anaesthetic management of in-hospital trauma, giving an overview on initial assessment, timing of surgery, and management of haemorrhage and coagulopathy. This section finishes by covering the orthopaedic-specific topics of compartment syndrome, fat embolism syndrome, and the management of fractured neck of femur and spinal injury.

The chapter begins with a discussion of circle actions and their relation to 2-sphere bundles. It continues with a section on general Hamiltonian group actions and moment maps, then proceeds to discuss various explicit examples in both finite and infinite dimensions, and introduces the Marsden–Weinstein quotient, together with new examples that explain its relation to the construction of generating functions for Lagrangians. Further sections give a proof of the Atiyah–Guillemin–Sternberg convexity theorem about the image of the moment map in the case of torus actions, and use equivariant cohomology to prove the Duistermaat–Heckman localization formula for circle actions. It closes with an overview of geometric invariant theory which grows out of the interplay between the actions of a real Lie group and its complexification.

This chapter describes kidney disorders related to occupational and environmental exposures and addresses prevention and control. Most are randomized controlled trials that have helped shape current ophthalmology practice guidelines. The book is divided into seven sections, corresponding to anatomical segment or disease of the eye (cornea, cataract, glaucoma, vitreoretinal, macula, uveal tract, and orbit/eyelids/extraocular muscles/optic nerve). Each chapter finishes with an illustrative clinical case, and includes a reference to the relevant section of the practice guidelines of the American Academy of Ophthalmology. The book is not aimed solely at ophthalmologists, however. Every practicing physician, no matter his or her specialty, should be familiar with the relation of the eye to the rest of the human body, and with the use and value of the ophthalmoscope. Ophthalmology has a particular relevance to physicians working in primary care, internal medicine, neurology, neurosurgery, pediatrics, and emergency medicine.

Chapter 8 concludes the text with methodical remarks. It defends key assumptions made in the main text and compares them, to the extent they deviate, with more conventional premises. The chapter starts with a comparison of adaptive versus rational expectations. Thereafter, it contrasts infinite planning horizons, finite planning horizons, and overlapping generations models. The third section, which is devoted to modeling money, discusses money-in-the-utility, the transaction costs approach, and more recent theories that derive money demand from a microeconomic framework. The forth section shows that assuming a highly elastic labor supply is empirically unconvincing, whereas a constant labor supply simplifies the model greatly and appears as a reasonable approximation. The final section contrasts behavioral and choice theoretic approaches to price setting.

This article surveys and discusses the core points of contact between notions of information structure and notions of presupposition. Section 1 is devoted to the ‘weak’ presuppositional semantics for focus developed by Mats Rooth, describing its properties with regard to verification and accommodation and showing that it can successfully account for a wide range of phenomena. Section 2 examines the stronger thesis that focus–background structures give rise to existential presuppositions, and finds the counterarguments that have been raised to carry considerable weight. Section 3 looks into the relationship between Givenness and run-of-the-mill presuppositions, finding that this relationship is looser than might be expected, mainly because a presupposition may be in need of focus marking instead of givenness marking.

This chapter introduces a section consisting of four studies of Swedish exceptionalism which focus on aspects of elections and voting behavior. A study of voter turnout shows that Swedes are exceptionally participatory. The second chapter in the section, which presents an analysis of class voting, indicates that Swedes are exceptionally old-fashioned and still vote according to the occupational class they belong to. An examination of ideological voting suggests that Swedes are exceptionally influenced by the classic left–right divide. And the final chapter takes as its subject involvement in election campaigns, and finds that Swedes are in exceptionally little personal contact with parties and candidates.
