Abstract:
Retention of airway mucus is one of the major problem that confronts post-operative and critically-ill patients, as well as the caregivers that address it. Retained secretions increase the work of breathing and promote hypoxaemia, atelectasis, and pneumonia. The airway-intubated patient is at particular risk of retaining mucus, as the presence of the tube interrupts normal flow of airway secretions toward the larynx by the mucociliary escalator and coughing effectiveness is degraded by a glottis that is stented open and cannot close effectively. Clearance of mucus is aided by using sufficient gas stream and total body hydration to reduce sputum viscosity and lubricate secretion plugs. Airway suctioning, a routine, but inherently traumatic experience for the patient, may clear the central airway, but leave peripheral airways unrelieved of their secretion burden. Prone positioning appears to confer an advantage regarding secretion drainage and clearance. Physiotherapy techniques may be useful in re-establishing and maintaining airway patency.

Abstract:
This chapter discusses pituitary disorders, focusing on prolactinoma, acromegaly, and pituitary apoplexy. A prolactinoma (prolactin-secreting pituitary tumour) is defined as a macroprolactinoma when it is >1 cm in diameter, and as a microprolactinoma when it is <1 cm in diameter. Acromegaly occurs as a result of excessive secretion of growth hormone (GH) and, consequently, of insulin-like growth factor 1 in adults. It is termed pituitary gigantism in the paediatric setting when excessive GH secretion occurs prior to epiphyseal plate fusion. Pituitary apoplexy is infarction of the pituitary gland, via haemorrhage or ischaemia, with resulting hypopituitarism. The etiologies, symptoms, diagnosis, investigation, prognosis, and treatment of these pituitary disorders are also discussed.

Abstract:
Critically-ill patients retain respiratory secretions. Toilet bronchoscopy is applied to aspirate retained secretions and revert lung atelectasis. Toilet bronchoscopy is particularly indicated when retained secretions are visible during the procedureand air-bronchograms are not present at the chest radiograph. Yet, toilet bronchoscopy should only be applied when other less invasive methods of secretion removal have failed. Ventilatory settings during the intervention, the inspiratory fraction of oxygen should be increased to 100%. In volume control ventilation, the pressure limit alarm needs to be increased; during pressure-controlled ventilation, the set inspiratory pressure should be increased. The external PEEP should be decreased to at least 50% of the baseline values, to prevent barotrauma. The use of sedatives, analgesics, and topical anaesthetics is mandatory to achieve favourable procedural condition. Toilet bronchoscopy is also feasible and safe in critically-ill patients undergoing non-invasive ventilation.

Abstract:
The image is Francesca Woodman’s, a New York artist who worked in the 1970s and early 1980s, until she killed herself at the age of twenty-two. It is a photographic self-portrait. She is a woman holding a mirror up to her face – a fairly common trope in our artistic tradition, and one with which Woodman often engaged and manipulated throughout her tragically brief career. But the reflective side of the mirror is not directed at her, as we often see in such pictures. Rather, it is directed at us, or whoever happens to be looking at the image. In the mirror, we should see our face, or us looking back at ourselves, narcissistically, no doubt, or confidently self-aware. But we do not see ourselves. Rather, we see her back and the back of her head. In other words, and paradoxically, she faces us with her back turned towards us. She looks out at us looking at her refusing to look at us. Or is that the best and most felicitous interpretation? I am not sure. For, obviously, with the back of the mirror pressed up against her nose, she cannot see us either. We look at her not looking at us, facing forward, with her back to us. Or, perhaps, we somehow occupy her position, turning her back on herself, even as she, or we, are still able to watch her do so.
