Introduction to the Land code of the Russian Federation the concept of "zones with special conditions of use of territories" is one of the promising solutions to the above tasks and is aimed at ensuring sanitary and epidemiological welfare of the population, industrial safety, safety in operating all types of transport, defence and state security, environmental protection etc. The Handbook describes the concept and the legal nature of the zones with special conditions of use of territories as a new category, which should become a full part of fur- the mechanism of the land law regulation. Omphalocele and gastroschisis are the most common types of abdominal wall defects encountered in the neonatal population. Both conditions result in the herniation of abdominal viscera through a defect in the upper or lower abdominal wall. These neonates can present with herniated organs, intestinal obstruction, impaired blood supply to visceral organs, and major fluid deficits. The primary goal for a neonate with an abdominal wall defect is surgical closure. Prior to achieving this goal, maintenance of perfusion of the herniated viscera and minimization of evaporative fluid losses secondary to the exposed surface area are paramount. This chapter discusses the distinctly different etiologies of these conditions, comorbidities, and prognoses, as well as the surgical and anesthetic management strategies and commonly encountered perioperative complications.

This chapter discusses omphalocele, which is a neonatal abdominal wall defect through which abdominal visceral content herniates. Although omphalocele is similar in gross appearance to gastroschisis, it is more frequently associated with chromosomal abnormalities such as trisomies 13, 15, 18, and 21, and syndromes such as Beckwith-Wiedemann, Rieger’s, and prune belly syndrome. It is also more frequently associated with anomalies in other organ systems. Management of infants with Beckwith-Wiedemann syndrome and omphalocele can be challenging due to the concurrent risk of difficult airway and aspiration. Associated comorbidities also need to be addressed to avoid secondary insults in the perioperative period.

ST-elevation myocardial infarction (STEMI) is generally caused by a ruptured plaque that triggers local thrombus formation, which occludes the coronary artery. STEMI should be diagnosed rapidly, based on the combination of ST-segment elevation and symptoms of acute myocardial infarction. The main treatment objective is myocardial tissue reperfusion as quickly as possible. The preferred method of reperfusion is primary percutaneous coronary interventionif transport time is below 2 hours, and thrombolysis if longer STEMI patients with acute onset cardiogenic shock should be evaluated by echocardiography to exclude mechanical complications, such as flail mitral insufficiency, ventricular septal defect or tamponade. Secondary prevention includes aspirin, adenosine diphosphate receptor antagonists, statins, beta-blockers, angiotensin-converting enzymeinhibitors, and lifestyle changes.

In this chapter we consider defences that are usually deployed during, or just before, contact between a prey and its predator: so-called ‘secondary’ defences. Secondary defences are found right across the tree of life and therefore come in very many forms, including: 1.) chemical defences; 2.) mechanical defences; and 3.) behavioural defences. Here we review selected examples that provide useful illustrations of the ecological and evolutionary characteristics associated with secondary defences. We discuss costs of secondary defences, placing emphasis on the consequences of such costs, especially as they relate to forms of social interaction. We show also that the acquisition of secondary defences may modify niche, life history, and habitat range of prey animals and review a well-known and significant study of predator–prey co-evolution of defensive toxins of prey and resistance to those toxins in predators. We include a small selection of examples and ideas from the plant and microbe defence literature where we think a broader perspective is helpful. We begin the chapter by considering the evolutionary mechanisms that favour secondary defence evolution.

Myelomeningocele (MMC) is a spinal birth defect associated with significant morbidity directly related to the exposure of meninges and neural structures. Further neurological dysfunction may occur secondary to Chiari II malformation and hydrocephalus. MMC repair is typically performed postnatally within the first 24 to 48 hours of life due to the concern for infection. Prenatal MMC correction is performed in select cases after studies showed improved neurological outcomes. Anesthesia for MMC repairs can be challenging, and appropriate screening should be performed preoperatively. During postnatal repair, care must be taken when positioning the infant to avoid any pressure on the MMC sac. Anesthesia can be maintained with a combination of inhalational agents and intravenous opioids. Prenatal MMC repairs must consider both fetal and maternal safety outcomes. They can be performed through both open and fetoscopic routes, with anesthesia focused on maintaining maternal blood pressure, optimizing uterine relaxation, and adequate pain control.

This chapter provides an overview of skin conditions affecting the hand, including nail pathology, benign and malignant skin tumours, and Dupuytren’s disease (DD). Although distortion of the nail occurs most commonly after trauma, nail changes may indicate other systemic causes (e.g. psoriasis), and may occasionally be due to underlying malignancy. Hands are exposed to sunlight and other occupational hazards (chemicals, radiation), and are vulnerable to skin cancer, most commonly squamous cell carcinoma. DD is often familial, commoner in men, and can affect the feet (plantar fibromatosis) and penis (Peyronie’s disease). Discreet areas of DD are now treatable by collagenase injection. Surgery is still indicated to restore function, either by fasciectomy (excision of DD) or dermofasciectomy (fasciectomy plus full thickness skin graft) where skin is involved or there is a secondary skin defect following fasciectomy. Patients should be counselled realistically about the post-operative recovery to full function, and that DD is not curable by surgery.

The term Neurotansmitter disorder, in the area of metabolic disease, focuses particularly on inborn errors affecting monoamine (dopamine & serotonin), pyridoxal phosphate (B6) and folate metabolism. Whilst there has been considerable focus on these disorders with regards to the paediatric population, it is clear that an increasing number of adult patients are being identified. Adult neurologists need to be aware of the clinical presentation of such patients and the appropriate tests that need to be requested to ensure a correct diagnosis is achieved. CSF profiling, by a specialist laboratory, is often required. This has the ability to very often identify the nature of a primary defect with regards to implementation of appropriate treatment. For some of these disorders, treatment can be effective. This may be in the form of monoamine/vitamin replacement. However there are exceptions, e.g. aromatic amino acid decarboxylase and dopamine transporter deficiencies. There also needs also to be an awareness of the growing list of secondary factors that can cause impaired dopamine and serotonin metabolism.

Neutrophils are an important component of the innate immune system, forming the first line of defence against bacterial invasion. Abnormalities in either neutrophil numbers or function lead to immunodeficiency disorders affecting the innate immune system, with a predisposition towards developing serious and often life-threatening infections. Alterations in neutrophil numbers and function may also be noted secondary to systemic diseases, where they may act as markers for ongoing disease processes. Most of the primary neutrophil disorders discussed in this chapter will present in childhood. In adults, acquired neutropenia is the commonest neutrophil abnormality encountered in clinical practice, although, rarely, some primary neutrophil defects may present.

Histologic bone abnormalities begin very early in the course of chronic kidney disease. The KDIGO guidelines recommend that bone disease in patients with chronic kidney disease should be diagnosed on the basis of bone biopsy examination, with bone histomorphometry. They have also proposed a new classification system (TMV), using three key features of bone histology—turnover, mineralization, and volume—to describe bone disease in these patients. However, bone biopsy is still rarely performed today, as it involves an invasive procedure and highly specialized laboratory techniques. High-turnover bone disease (osteitis fibrosa cystica) is mainly related to secondary hyperparathyroidism and is characterized by increased rates of both bone formation and resorption, with extensive osteoclast and osteoblast activity, and a progressive increase in peritrabecular marrow space fibrosis. On the other hand, low-turnover (adynamic) bone disease involves a decline in osteoblast and osteoclast activities, reduced new bone formation and mineralization, and endosteal fibrosis. The pathophysiological mechanisms of adynamic bone include vitamin D deficiency, hyperphosphataemia, metabolic acidosis, inflammation, low oestrogen and testosterone levels, bone resistance to parathyroid hormone, and high serum fibroblast growth factor 23. Mixed uraemic osteodystrophy describes a combination of osteitis fibrosa and mineralization defect. In the past few decades, an increase in the prevalence of mixed uraemic osteodystrophy and adynamic bone disease has been observed.

Aposematism is the pairing of two kinds of defensive phenotype: an often repellent secondary defence that typically renders prey unprofitable to predators if they attack them and some evolved signal that indicates the presence of that defence. Aposematic signals often work to modify the behaviours of predators both before and during attacks. Warning coloration, for example, may increase wariness and hence improve the chances that a chemically defended prey is released unharmed after an attack. An aposematic signal may therefore first tend to reduce the probability that a predator commences attack (a primary defence) and then (as a component of secondary defence) reduce the probability that the prey is injured or killed during any subsequent attack. In this chapter we will consider both the primary and the secondary effects of aposematic signals on prey protection. We begin first by describing the common features of aposematic signals and attempting to show the wide use to which aposematic signalling is deployed across animals (and perhaps plants too). We then review the interesting evolutionary issues aposematic signals raise, including their initial evolution and their integration with sexual and other signals. We also discuss important ecological, co-evolutionary, and macroevolutionary consequences of aposematism.

The term “spinal dysraphism” encompasses the broadest array of the conditions known as the neural tube defects. The open neural tube defects (spina bifida aperta and cystica) include both disorders of primary and/or secondary neuralation and are best defined as myelomeningocele complex (MMC) due to their protean nervous system manifestations beyond the spinal lesion. Closed spinal dysraphisms (so-called spina bifida occulta) include lipomatous lesions, forms of tethered spinal cord, sinus tracts, and forms of split spinal cord (diastematomyelia). Both genetic and environmental etiologies have been identified. Gene-environment and gene-gene interactions are also important in the pathobiology of these conditions.

Insects have evolved a wide range of behavioral traits to avoid predation. Frequently, these behaviors are deployed to augment the effectiveness of a primary defence such as crypsis or mimicry, but they are also sometimes elicited as a secondary defence when a primary defence fails. Anti-predator behaviors in insects include adaptations to avoid being detected by predators, adaptations rendering the insect unattractive to consume, warning behaviors, and behaviors to enhance the effectiveness of mimicry. This chapter reviews many of these behavioral anti-predator adaptations, emphasizing when they are elicited and highlighting their adaptive significance. We argue that some of the inter-specific variation in behavioral defences can be explained in terms of defensive portfolios: if a physical defence is sufficient, then behaviour to augment or back-up this defence is unnecessary. As the use of comparative methods increases, researchers will be better placed to understand variation in the suites of defences that evolve.

The property which perceptual experiences of a given phenomenal kind enable the subject to track is not always identical with the property objects appear to have (in a phenomenally manifest manner) in the kind of experience at issue. Tracked properties and apparent properties come apart in colour perception but they are identical in the perception of three-dimensional shape. In order to defend these claims, this chapter proposes a criterion for the identity of tracked property and apparent property in a given experience. The view developed leads to an account of the traditional distinction between primary and secondary qualities. In order to capture what it is to instantiate a given secondary quality, one must specify the phenomenal kind of perceptual experience an instantiation of the property would render veridical.

Brain serotonin deficiency is a heterogeneous condition whose etiology remains unknown in the majority of cases. Strong evidence supports a major role for brain serotonin deficiency in common conditions such as depression and other psychiatric and cognitive disorders, which are probably due to interactions between genetic and environmental factors. Mendelian monogenic conditions leading to brain serotonin deficiency have also been identified, but they are rare. These diseases are associated with defects in other neurotransmitters (primarily dopamine), and it is difficult to link serotonin deficiency with specific neurological syndromes. Secondary serotonin deficiency is also common. In adults, when serotonin deficiency is thought to contribute to neurological symptoms such as sleep disturbance and alterations in behavior, treatment with serotonin precursors may be useful.

Abstract:
The papers presented in this volume cover topics, such as the “phenomenal concept strategy,” to defend materialism from anti-materialist intuitions, the doctrine of representationalism about phenomenal character, the modal argument against materialism, the nature of demonstrative thought, and cognitive phenomenology. On the one hand, I argue that the phenomenal concept strategy cannot work and that representationalism has certain fatal flaws, at least if it’s to be joined to a materialist metaphysics. On the other, I defend materialism from the modal argument, arguing that it relies on a questionable conflation of semantic and metaphysical issues. I also provide a naturalistic theory of demonstrative thought, criticizing certain philosophical arguments involving that notion in the process. I argue as well that the peculiarly subjective nature of secondary qualities provides a window into the nature of the relation between phenomenal character and intentional content, and conclude that relation involves a robust notion of acquaintance.

Since the end of the bipolar era, Italy has regularly undertaken military interventions around the world, with an average of 8,000 units employed abroad in the twenty-first century. Moreover, Italy is one of the principal contributors to the UN operations. The end of the cold war represented a turning point for Italian defence, allowing for greater military dynamism. Several reforms have been approved, while public opinion changed its view regarding the armed forces. This chapter aims to provide a comprehensive perspective of the process of transformation that occurred in post-cold-war Italian defence, looking at the evolution of national strategies, military doctrines, and the structure of forces. After a brief literature review, the study highlights the process of transformation of Italian defeshnce policy since 1989. Through primary and secondary sources, the chapter illustrates the main changes that occurred, the never-ending cold-war legacies, and key challenges.

Abstract:
This chapter provides an analysis of the changes that German defence has undergone since 1990. Two developments shaped German defence after the end of the cold war: First, the German armed forces changed owing to a radical downsizing, the end of conscription, and the modernization of equipment. Secondly, the style of German defence policy changed from reactive and norm-based in the 1990s to assertive, interest-based, and executive in the 2010s. The culture of military restraint—for decades a characteristic trait of German defence—waned. German military deployments abroad became more frequent and more serious. The leadership position Germany had taken in Europe during the Euro crisis and during the Ukraine conflict, as well as a domestic window of opportunity stemming from a favourable majority for the government parties in the parliament, serve as explanations for a more assured formulation of national security interests.

Abstract:
This chapter explores the initial conjunction and subsequent disentanglement of social and nationalist revolutions in Lithuania by focusing on the impact that war and various mobilizations had on the local population in 1918–19. Despite the explosion of social and nationalist unrest all over the country in late 1918, in a matter of several months the Bolsheviks lost their case. The key reasons for their failure were their military defeat by German, Lithuanian, and Polish troops, but also economic mismanagement, the refusal to distribute land to peasants, and an inability to present their revolution as “native.” Following the Leninist doctrine of “proletarian revolution” that relegated peasantry to a secondary position, the Bolsheviks failed to forge an alliance with the largest population group of Lithuanian society, which resulted in their downfall.

Due to the significance and ubiquity of state capture in Serbia, the BCSP has embarked upon a pilot research project that aims to establish the main factors and conditions, as well as the mechanisms and actors, involved in state capture in the security sector. The capturing of the police, the security services and the defence sector in Serbia is analysed in this early study. Furthermore, it was necessary to situate the analysis in the country’s political context, and special chapters cover parliamentary oversight and control and the security sector financing. The research was conducted from May 2019 to April 2020 and is based on insights gleaned from both primary and secondary sources.

Abstract:
This chapter highlights communication failures in conservation. The environment is a secondary consideration to most of the American public, with the fraction of the US population that identifies itself as environmentalists shrinking year by year. This reflects the ineffectiveness of the conservation community at communicating why people should care about the environment, much less what they should do about it. This chapter outlines five failures in environmental and conservation communications. Three failures are well enough understood that they could and should be avoided. These failures are assuming the problem is simply an information deficit, misreading the audience’s values, and using fear to motivate. Two challenges remain that no one yet knows how to overcome: addressing psychological distance and overcoming compassion fade.

This chapter describes reproductive disorders and developmental disorders, with a focus on their recognition and prevention. Certain hazardous exposures at sufficient doses during preconception among men and women have been shown to increase the risk for infertility, miscarriage (spontaneous abortion) and birth defects. Women’s exposures during pregnancy can cause fetal death, congenital anomalies, and neurodevelopmental disorders. Fathers’ occupational exposures can secondarily expose pregnant women through contamination of the home environment. Numerous chemical pollutants readily cross the placenta and are transmitted into breast milk. Prenatal and perinatal exposures have been linked to abnormal development of the immune system, childhood cancer, and learning disabilities. Specific examples, such as lead and dibromochloropropane, are provided in the chapter to illustrate general concepts. A final section addresses evaluation and control of risk.
