Raffle, Angela E., Anne Mackie, and J. A. Muir Gray. Screening. Oxford University Press, 2019. http://dx.doi.org/10.1093/med/9780198805984.001.0001.

Full text
Abstract:
Muir Gray, Anne Mackie and Angela Raffle have been at the forefront of achieving improvements in UK screening over recent years, and they bring a wealth of experience to this non-technical introductory guide covering all aspects of screening. As USA expert Gilbert Welch describes it, this book is “A readable yet encyclopaedic guide to screening: its history, its key design elements, its implementation and policy challenges… A must read for clinicians, managers, and policy makers who would like to assist Raffle Mackie and Gray in achieving their goal: ‘to sort out the mess’.” The first four chapters deal with concepts, methods and evidence, explaining what screening is and how it is evaluated. Chapters five to eight describe practical aspects, for example how to make policy, and how to deliver screening to a high standard. The book includes numerous examples and real-life case histories, giving important reminders of the need to be vigilant for the hidden influence of commercial incentives and ‘bad science’ if we are to achieve best value health and healthcare. A comprehensive glossary makes medical terms accessible to all, and each chapter concludes with a summary and self-test questions. Reference is made to the UK National Health Service, a leader in screening, but the book is internationally relevant because the principles of good screening apply in any setting. The controversies, paradoxes, uncertainties, and ethical dilemmas of screening are explained in a balanced way.

Abstract:
Screening has long been portrayed as an inherently beneficial activity that saves lives, rather than as a complex mixture of potential benefits and harms that must be carefully weighed for each modality. The early success of the Pap smear in reducing deaths from cervical cancer may have inadvertently fostered simplistic messaging about unqualified benefits of screening. Over time, large-scale randomized controlled trials (RCTs) of prostate and other cancers have highlighted the potential harms caused by mass screening programs (especially those related to overdiagnosis and unnecessary treatment) and have revealed the counterintuitive elements involved in evaluating such programs. The criteria for evaluation now extend beyond the performance criteria of the test itself to include the net balance of benefits, risks, and costs. PSA screening, widely used in the United States since the late 1980s, has now been removed from the list of routinely recommended procedures, based on evidence from RCTs.

Abstract:
This chapter provides information about a genetic counselor’s role in prenatal screening, including discussing and offering options to a patient, interpreting and providing results, or managing referrals based on abnormal results. It discusses how a screen is evaluated using sensitivity, specificity, positive predictive value, negative predictive value, and personal utility. It provides a detailed description of both maternal serum screening and cell-free DNA testing. The maternal serum screening discussion includes information on multiples of median, calculating risk, timing, pattern association, limitations, and follow-up. The review of cell-free DNA testing includes fetal fraction, methodology, test performance, limitations and considerations for testing, and follow-up. This chapter also provides a list of additional resources to use for cell-free DNA testing.

Mathiesen, Amber, and Kali Roy. Carrier Screening. Oxford University Press, 2018. http://dx.doi.org/10.1093/med/9780190681098.003.0006.

Abstract:
This chapter provides information on carrier screening options available, including testing based on ethnicity, targeted to the family history or clinical situation, or by use of expanded carrier testing panels. It describes who should be offered testing, the timing of testing, how to evaluate the risks of conditions being tested, and how to interpret results, in addition to providing a brief description of newborn screening. The chapter describes testing protocols for specific conditions including cystic fibrosis, spinal muscular atrophy, FMR-1–related conditions, and hemoglobinopathies. It reviews ethnicity-based screening, as in Ashkenazi Jewish and French Canadian populations, as well as screening when there is a family history. It also discusses the use, benefits, and recommendations when offering expanded carrier screening.
