Full textBACKGROUND: In Italy, practice experience provides up to 50% of the educational Midwifery student’s curricula. It is recognised as an important part of clinical practice for preregistration students and is accepted as the preceptor’s responsibility, once in practice, to provide opportunities for effective learning. However, little is known about the impact that preceptors have on Midwifery students and their effectiveness on students’ perceptions of learning. OBJECTIVES: This study is aimed at creating and validating a tool to explore the perceptions of Midwifery students on their clinical preceptors and to gather opinions on how to improve the effectiveness of clinical preceptors. METHODS: A multicentric research has been conducted at Northern Italy Bachelors in Midwifery. Focus groups consisted of 88 second and third year students. A survey tool has been created, validity and reliability have been tested through gold standards and students assessors. RESULTS: From the focus groups, it emerged that midwifery clinical preceptors should bridge the gap between theory and practice, improve evidence based learning, give useful feedback and correct students mistakes without belittling them. A 33 items survey tool has been created. It has internal consistency. Reliability has been tested through Cronbach’s Alpha: 0,97 for gold standards assessors and 0,94 for students assessors. CONCLUSIONS AND IMPLICATIONS FOR PRACTICE: These results imply that, from a student perspective, the ideal clinical teaching experience is heavily influenced by the preceptors’ behaviours. A survey tool specific for Midwifery students has been created and validated. The use of this tool allows for a uniform and country wide method of assessment of preceptors’ behaviours. Therefore, this gives headmasters the required tool to objectively measure a preceptor’s level of competence.

Full textThis study is intended to present the Camperio Family Photographic Collection and to highlight the specific relevance of the travel photography it contains as a key both to the interpretation of the whole Collection and to the ethics of vision of the members of this 19-20th century upper middle class French-Italian family. To the members of this family, travel and travel photography had been primary tools both to frame the world and to relate themselves to their own time. The photographic eye, focusing on natural resources as well as on natural, technological and artistic wonders and on wartime, unveils the ethics of vision of the collectors, and paradigmately reflects the cultural climate of their age. The ideological transmission from parents to sons that emerges from the analysis of both photographic and related archival materials is also a fil rouge that goes through the collection, and gives it a coherence seldom met in other family funds.The Camperio Photographic Collection is housed in the Biblioteca Civica di Villasanta (Villasanta (MI), Italy). It has been catalogued by the author for the Cultural Heritage Information System of Cultural Department of Lombardy, and can be found online (Beta version): http://www.lombardiabeniculturali.it/percorsi/camperio/1/ _________________________ The Camperio Family Photographic Collection (Fondo Fotografico Camperio) is part of the Camperio Family Fund (Fondo Camperio), a 19-20th century family fund housed in the Biblioteca Civica di Villasanta (Villasanta Civic Library) that holds together the Family Archive, the Family Library, and the Photographic Collection itself. The Photographic Collection was gathered over one century by the members of the family who took, bought and ordered photographs to document their enterprises. The collection contains pictures made by renowned photographers, scholars, explorers and by the Camperios themselves. It hosts several photographic genres, but it is expecially devoted to travel photography as the greatest part of the pictures was made or acquired during the many journeys undertaken by the family members in Africa, Egypt, Far East, Australia, Russia. This relevant historical photographic collection came to us in exceptionally good conditions, as it is integrated in the undivided, original archival context which is in almost intact state and provides useful material to determine the collection’s stratigraphy. The family Archive holds a wide range of documents (travel diaries, letters, essays…) that give account of the interests and activities in which the Camperios were involved, among which travel held a front-rank position. Also, it often provides straightforward reference to picture taking and collecting. The family Library is mainly composed by geographical and military literature, thus reflecting the great interest of the family members in geographical exploration and travel, and giving further evidence (in the many illustrated magazines and books) to the importance of photographic image as a tool to classificate, reduce and take possession of the world. During the second half of the 19th century and until World war 2, travelling had been a fundamental activity for to the members of this family. They interpreted journey in all its nuances, from exploration to tourism to war campaigning, and travelled extensively all around the world with a sense of essential necessity and an effortless attitude, in times when such an activity was no ordinary matter. The relevance of travel as an experience by means of which the Camperios related themselves to the world and to their own time is highlighted not only by the prominence that this activity had in the education of all the family members, but also in the importance that it had in the pursuing of their goals and in the developement of their professional careers. Italian Unification patriots, explorers, colonial entrepreneurs, philanthropists, professional soldiers, amateur photographers, the Camperios, both men and women, were extremely active personalities permeated with positivist tought and committed to international promotion, modernization and progress of Italy. Following their father’s steps, the young Camperios travelled extensively, collected slices of world through photography, and pursued the accomplishment of projects in line with their parent’s ideals. Taken as a whole, the Camperio Fund goes beyond the boundaries of the local or strictly personal experience, providing visions of the social, economical and political international history of the times. It also shows how ideological transmission from generation to generation worked as a leitmotiv declined in various ways in the personal choiches and cultural orientation of the members of the family. This ideolgical continuum is mirrored in the Photographic Collection and represents its strongest unifying element, its coherence being not merely provided by the thematic element (the travel) or by the collectors family membership.

Full textPost-transplant active immunization with HBsAg vaccine is a potential prophylaxis strategy against HBV-recurrence after liver tranplantation due to HBV-related disease. Previous studies showed conflicting results using standard vaccines, whereas the use of the new adjuvant 3- deacylated monophosphoryl-lipid-A (MPL) significantly increased patient’s immunization rate through an high anti-HBs titre increase. Aim: We investigated the efficacy of a long-term (12 months) accelerated (monthly doses) reinferced vaccination schedule using the MPL-adjuvanted recombinant S vaccine administered with and without concomitant HBIg. Methods: 18 patients (M/F:13/5) transplanted for HBV-related cirrhosis 73±38 months earlier were recruited. All were HBsAg and HBV DNA negative in serum and cccDNA negative in liver tissue; 5 (27.7%) were coinfected with HCV and 5 (27.7%) with HDV. Study protocol consisted of 12 consecutive monthly intramuscular vaccine doses (HBsAg 20μg plus MPL 50μg) given together with lamivudine (100 mg/daily). Each of the initial 6 doses (first cycle) was administered within 7 days after 2000 IU HBIg i.v. infusion, while the last 6 doses were given after complete HBIg withdrawal (second cycle). HBsAb titre was determined before each vaccine dose and during the follow-up. All patients were maintaiened on low-level immunosuppression. Results: all patients completed the whole vaccination program, receiving 12 adjuvanted vaccine doses (first and second cycles) and were monitored during 3 months follow-up after vaccination end. No side effects occurred, nor evidence of HBV recurrence. At the end of first cycle 17/18 (94.4%) patients achieved an anti-HBs titre greater than 100 IU/L (mean 389±445 IU/L) and 2 (11.1%) a titre greater than 500 IU/L. At the end of follow-up 9/18 (50%) and 3/18 (16,6%) had an anti-HBs titre greater than 100 (mean 510±427 UI/L) and 500 IU/L, respectively. Conclusions: nine months after HBIg withdrawal half of the patients reached and maintained a protective anti-HBs titre (>100 IU/L). This intensive schedule using the MPL-adjuvanted recombinant S vaccine, given in combination with HBIg and lamivudine, seems to be more effective than previous HBV vaccination protocols, although a longer follow-up is needed to assess its final effectiveness.

Previous studies have reported that repeated administration of psychomotor stimulants such as amphetamine (AMPH), cocaine, and morphine results in an augmentation of related behaviors and neurotransmission in the central nervous system (CNS).This phenomenon is termed behavioral sensitization. This sensitization has been the subject of considerable investigation due to its proposed relevance to drug addiction and psychosis, but at the present time its mechanisms are unknown. When food - deprived rats are exposed to schedules of intermittent presentations of small amounts of food many of them exhibit excessive drinking, which is called schedule - induced polydipsia (SIP). Evidence suggests that excessive drinking behavior is a kind of displacement which belongs to defense mechanisms. Thus. SIP serves as a coping of stress and arousal reducing function. SIP is recently concerned with schizophrenia, and is used as a model of drug abuse, Hence, we investigate the SIP sensitization phenomenon induced by AMPH in this animal model with a series of experiments. We firstly studied the interaction of AMPH and arecoline, and its effect on SIP behavior. Secondly, We examined whether the SIP rats develop behavioral sensitization. The rats were treated with AMPH (1 mg/kg bp) for 1-5 days before abstinence of 3 and 14 days, and then challenged with low dose AMPH (1mg/kg )in SIP rats. Thirdly, the effects of pretreatment with SCH23390 (D1 dopamine receptor antagonist ) haloperidol (D2 dopamine receptor antagonist) or MK-801 (a non- competitive NMDA receptor antagonist) on the AMPH-induced SIP sensitization were explored. In addition, we used in vivo microdialysis to measure extracellular glutamate concentrations in the nucleus accumbens(NAc). The results indicated. 1.AMPH(0-2 mg/kg i.p.) decreased lick numbers and water intake in SIP rats dose-dependentaly without affecting number of nose pokes and pellets. 2.Arecoline (0-0.8 mg/kg) did not significantly affect SIP. The effects of AMPH on the reduction of the SIP were not altered by co-administration of arecoline and AMPH. 3.Repeated administration of low doses of AMPH(1 mg/kg)signififcantly induced SIP sensitization, which was evident with a withdrawal period of 3 or 14 days. 4.Pretreatment with MK-801 (0.1mg/kg i.p.) significantly blocked SIP sensitization after either withdrawal period. 5.Pretreatment with SCH23390 (0.04 mg/kg i.p.) can not block SIP sensitization on either 3 or 14 days withdrawal. 6.Pretreatment with haloperidol (0.05 mg/kg i.p.) blocked SIP sensitization at long-term withdrawal period. 7.The initial results revealed that extracellular glutamate concentrations in the NAc did not increase significantly on either 3 or 14 days withdrawal as detected by microdialysis. The results suggest that there was no direct interaction between arecoline and AMPH on the modulation of SIP. SIP sonsitization occurs in rats following chronic administration of low doses of AMPH, and these effects at least in part are mediated via the activation of NMDA receptors. In addition, these effects may involve the D2 receptor of long withdrawal periods.
