L’Herpèsvirus Humain 8 (HHV-8) est l’un des sept virus humains reconnus à caractère oncogénique. Les principales pathologies tumorales associées au virus HHV-8, à savoir la maladie de Kaposi (KS), le lymphome primitif des séreuses (PEL) et la maladie de Castleman multicentrique (MCD), représentent encore à ce jour un véritable défi clinique, étant donné le peu d’options thérapeutiques disponibles et efficaces pour y faire face. Au cours de nos études in vitro nous avons pu démontrer l’effet cytotoxique et pro-apoptotique de l’antipaludéen primaquine diphosphate, spécifique aux lignées cellulaires de PEL infectées par l’HHV-8. Nous avons également pu mettre en évidence que l’induction d’un stress oxydatif, ainsi que des voies pro-apoptotiques de l’UPR liées à un stress sévère du réticulum endoplasmique, prenait part aux mécanismes d’actions mis en œuvre dans l’effet pro-apoptotique de la primaquine. Enfin, des études in vivo ont permis de montrer une efficacité anti-tumorale très encourageante de la primaquine, d’une part chez un modèle de souris PEL et d’autre part, au cours d’une étude pilote chez des patients présentant une forme sévère du KS. Aucuns effets secondaires manifestes n’ont été observés chez les souris et les patients traités à la primaquine. Ces études précliniques et cliniques permettent donc de considérer la primaquine diphosphate comme un nouveau candidat très prometteur pour le traitement des cancers associés à l’HHV-8
The Human Herpesvirus 8 (HHV-8) is one of seven recognized human cancer causing viruses. HHV-8 is a principal causative agent of several human cancers including Kaposi’s sarcoma (KS), multicentric Castleman’s disease (MCD) and primary effusion lymphoma (PEL), which represent important and difficult to treat clinical problem, with very few therapeutic options. In the present in vitro study, we demonstrated the specific cytotoxic and pro-apoptotic effect of the antimalarial primaquine disphosphate in HHV-8-infected PEL cells, without any toxicity on different uninfected cells. We also found that primaquine-induced oxidative stress and pro-apoptotic UPR activated by excessive reticulum endoplasmic stress, to be part of the pro-apoptotic mechanisms of action of primaquine in PEL cells. Finally, PQ treatment had a clinical positive effect on tumor growth in a in vivo NOD/SCID xenograft PEL mouse model, as well as in a pilot clinical study in human harboring very severe KS. Importantly, primaquine was well tolerated and none adverse event and overt toxic effects were observed in both primaquine-treated mice and patients. The combination of preclinical and clinical observations and results from our analysis thereby raising the possibility that primaquine may be used as a promising targeted agent in treatment of HHV-8-associated cancers
