Full textSelf-esteem is defined by the « feeling of self-appreciation ». It is built in childhood then evolves with the social environment and life events. College years corresponding generally to the period of young adulthood and is a particular developmental period due to changes in the social environment and frequent problems of mental health and substance use. It is also a period of vulnerability for self-esteem. College years could therefore be an interesting period to investigate the specific links between self-esteem and health. The purpose of this thesis was to conduct epidemiological studies to estimate the links between self-esteem and health behaviors, physical and mental health among university students.First, a systematic review of the literature synthesized empirical evidence on the relationships between self-esteem and health-related behaviors (i.e. substance use, sexual behavior, physical activity, and nutrition). High self-esteem was associated with more physical activity and less extreme health-related behaviors (extreme alcohol consumption, use of condoms). However, mixed results regarding alcohol consumption and number of sexual partners and a lack of research for smoking, drug use and nutrition have been highlighted. Next, two studies were conducted on a sub-sample of the i-Share cohort; a prospective French longitudinal study following nearly 15,000 volunteers studying at the university since 2013. Thus, the second study of this thesis aimed at estimating the association between self-esteem and self-rated health one year after. Of the 1,011 participants included in the study, 20% reported having average or poor health. Students with higher self-esteem were more likely to have a better perception of their health (odds ratio = 1.4, 95% confidence interval = 1.2 to 1.7). A third study aimed to investigate the mediating role of self-esteem in the link between ADHD symptoms and suicidal thoughts. Of the 2,631 participants included in the study, 17% reported having suicidal thoughts at one-year follow-up. ADHD symptoms were associated with suicidal thoughts (beta = 0.2, 95% confidence interval = 0.1 to 0.2). Self-esteem mediated 44% of the total effect and was involved in 60% of the total effect when depression was also a mediator.Self-esteem is a factor of interest in youth health and interventional research could target this psychological factor. Furthermore, because the concept of self-esteem is complex and the study population was specific, further researches could be conducted to confirm these results with different measurement tools of self-esteem (especially to reveal multidimensional aspect) and representative populations

Full textThis thesis aimed at emphasizing the connections between sports club participation, performance, and health as well as studying the extent of mindfulness meditation inside these interrelations. At first, the interrelations between sports club participation, performance, and health have been observed among a large sample of young people. It is demonstrated that: a) sports club participation would lead to a better level of performance and health, and b) some sports practices (more than others) are associated with greater benefits to physiological and psychological factors that influence performance and health. Then, through a systematic review, the effects of mindfulness short periods on a health psychophysiological biomarker (heart rate variability) have been evaluated. Our results underline the lack of randomised controlled trials to be able to state that the autonomic nervous system would benefit from mindfulness. Finally, we have tested the effects of mindfulness short periods meditation combined with high intensity running training on performance and health. Our randomised controlled trial highlights that: a) mindfulness meditation would produce differences (compared in a control group) in cardiorespiratory endurance, and b) mindfulness meditation would improve certain psychological skills in sport and in athletes' well-being. Our results also show that mindfulness meditation would not affect heart rate variability, ratings of perceived exertion, and other components of physical fitness

Full textAn epidemiological survey included 1505 women from 26 healthcare departments of "assistance publique" (public hospitals) in paris and an ergonomic study included 10 of these departments have shown the real impact of personal and occupational risk factors on employee's health and on the running of institutions : work places should be defined according to factors which determine the workload, in these work places, the most frequent pathologies should be identified, actual work should be studied, indicators of pathologies realated to working conditions should be recognized and may be used to evaluate working condition arrangement programmes. From both approaches, the nursing staff's comments were better understood and proposals were exposed to improve nursing staff's work activity as well as to arrange strategies that reconcile a bearable workload with a correct quality of healthcare

Full textIn this study, PE teachers’ views have been collected. These teachers’ standpoints have been analysed with regard to one of their special assignments, considered as the third objective in the official syllabus of physical education and thus defined : “access to a good knowledge of how to manage and keep us physical life”. These semi-guided interviews revealed how difficult it was sometimes for the teachers to combine EP and health education in their classes. Furthermore, most of them say that health education cannot be dissociated from their function. Nevertheless, they wonder about how to provide a sufficient range of activities and which requirements they should meet in this domain. They acknowledge that they are in need of teaching aids to take the job on. We held group interviews in the third and fourth forms of a secondary school. The transcript of the tapes shows that students recognize someone is in good health thanks mainly to what he does, what he actually is and what he communicates. Contrary to a generally accepted idea, teenagers have a nodding acquaintance with health education and they have a pretty accurate picture of what course of action they have to take in order to preserve or improve health. This first part underlines the importance of not just focusing the health education activities on the acquisition of a large body of knowledge but rather on personal commitments and specific training conditions capable of encouraging teenagers to adopt a healthy way of life. This is precisely how we envisaged mounting a major coherent operation for health at school. We devised suitable experimentation in EP classes and this is what the second part is all about

Full textBackground: Non-medical staff in nursing homes for the elderly are exposed to high levels of physical and psychological stress related to managing increasingly dependent residents with multiple pathologies. Our research approach is based on Green and Kreuter's public health model. This is intended to describe the physical and mental health (epidemiological diagnosis) and to explore working conditions as experienced by nursing home staff (behavioral and environmental diagnosis). Methods: A cross-sectional descriptive survey was conducted by 78 occupational physicians on 2,649 employees (706 housekeepers, 1,565 nursing assistants and 378 nurses) in 105 nursing homes for the elderly in the Rhône-Alpes Region of France. Employee data were collected on several validated questionnaires. All statistical analyses were performed on SAS software, version 9.3. Results: Respondents related elevated hardship and desired more continuous training in palliative care. They were highly exposed to psychosocial stress (strong effort, low reward, effort/reward imbalance, overcommitment). Neck and upper limb musculoskeletal complaints and signs of psychological distress were significantly associated with exposure to psychosocial stress. Conclusions: The next phase of the research plan should consist in establishing an educational diagnosis by assessing the organizational factors associated with psychosocial stress in nursing home staff

Full textHematopoietic stem cells allograft, or bone marrow transplantation, is one of the treatments proposed to stop the course of hematopoietic malignancies such as acute leukemia. This procedure consists of replacing the cells in the diseased blood with healthy cells from a compatible donor to maximize the chances of success. Although injecting the graft appears to be a relatively simple procedure, it is nonetheless part of a whole process that begins well before the procedure and has consequences in the months and even years following the allograft. Thus, the patient's care pathway can be divided into three stages: the pre-transplant, hospitalization in a protected sector, and the post-transplant period with the return home. At these different stages, an alteration in quality of life, the presence of symptoms of psychological distress and even a state of post-traumatic stress disorder (PTSD) can be observed. However, these psychological consequences may differ from one individual to another, suggesting that certain factors, other than medical ones, could influence both the physical and psychological consequences of the allograft. It is in this perspective and based on the Transactional Integrative and Multidimensional Model of Bruchon-Schweitzer and Boujut (2014) and the Complete State Health Approach of Keyes and Lopez (2002) that four studies were conducted in this thesis work in health psychology. The objective was to highlight the different protective and deleterious psychological factors at the different stages of allograft on the mental and physical health of patients, focusing on factors little explored to date and from the third wave of cognitive and behavioural therapies such as acceptance and commitment therapy. After validating a French version of the AFQ scale (article 1), the first study found that the level of happiness before the transplant contributes to a better quality of life before the procedure (article 2). The second highlighted the prospective effect of anxious preoccupations during hospitalization on the occurrence of post-traumatic stress disorder and the positive effect of fighting spirit on the level of resilience and happiness five months post-transplant (article 3). The third one revealed that the more people tended to be in experiential avoidance, the more likely they were to develop symptoms of PTSD, while patients with high non-judgmental scores (a facet of mindfulness) were less likely to develop such symptomatology (article 4). Finally, the fourth study shows that the experience of positive life changes in patients' lives after transplantation is far from systematic. Rather, patients show a decline in their psychological health that is related to a low level of acceptance (article 5). All of these results led us to reflect and propose an intervention adapted to the needs of patients (i.e. a digital platform) based on effective interventions in psycho-oncology and web-based therapies. In this perspective, a first device (i.e. DESP task) aimed at remedying the attentional biases involved in anxiety and depression has been developed and tested (article 6). The DESP has been the subject of a declaration of invention (declaration of invention n°0487-UCADESPTASK). Proposing a continuity of care and trying to overcome the obstacles inherent to the specificities of the allograft course is essential to improve the care of patients and accompany them during this pathway
