Abstract:
Abstract Background: Over the last years, the management of patients with node positive early breast cancer has gone through important innovations. On the medical side, new targeted therapies such as olaparib and abemaciclib have been introduced, with promising results on the invasive disease-free survival. Moreover, sparing axillary lymph node dissection has proven to be noninferior in terms of overall survival. However, no tools are currently available to predict lymph node involvement before definitive surgical evaluation. The aim of the study was to analyze clinical and pathological characteristics of patients with node positive early breast cancer to explore potential risk profiles associated with a ≥3 nodal involvement. Methods: The study retrospectively analyzed 335 node-positive breast cancer patients treated at the Breast Unit of the CRO Aviano National Cancer Institute, between 2017 and 2021. Data regarding primary tumor biological features, lymph node involvement and surgical approach were collected. Associations between clinico-pathological characteristics and ≥3 lymph node involvement were tested through stepwise logistic regression and the gradient boosting machine learning algorithm (GBM). Results: Among the 335 analyzed patients, 87.0% had a primary tumor < 5 cm, with a single positive lymph node in 73.3% of cases. Hormone receptors were mainly positive (respectively 93.5% and 83.4% for estrogen and progesterone receptors). Tumor grade was most frequently well differentiated (Grade 1 in 60.7%), with a Ki67 < 20% (59.5%). After multivariable logistic regression, a tumor size ≥ 3 cm (OR 3.24, CI95% 1.47-7.17, p = 0.004), the presence of massive lymphovascular stromal invasion (OR 2.50, CI95% 1.02-6.14, p = 0.045) and 2 or more positive sentinel lymph nodes at surgical evaluation (OR 6.08, CI95% 3.34-11.05, p < 0.001) were associated with a higher risk of identifying ≥ 3 positive lymph nodes after subsequent axillary dissection. Similar results were observed in the luminal-like cohort. A GBM machine learning model was then developed with a 0.77 Area Under the Curve. Features with the highest relative importance (RI) were single sentinel node involvement (RI 16.1873), followed by tumor size ≥ 3 cm (RI 10.2024), ≥2 positive sentinel lymph nodes (RI 8.5050) and lymphovascular stromal invasion (4.0217). Consistently, number of positive sentinel lymph nodes and tumor size were the predominant features in all top 20 GBM models. Conclusions: The present study explored the definition of risk profiles linked to 3 or more positive lymph nodes based on clinical and pathological features. It, moreover, tested the feasibility of developing machine learning classifiers to support future clinical decision-making. Due to the growing complexity of the adjuvant setting, finding a balance between minimally invasive surgical and staging approaches and risk definition for treatment personalization will become increasingly critical. Civilna družba v demokratičnih državah zahteva, da so oborožene sile profesional- ne. Torej morajo oborožene sile teh družb imeti jasno predstavo o tem, kaj vojaška profesija sploh je. Predvsem pa morajo imeti jasno izoblikovano predstavo o tem, kakšen je profesionalni častnik, saj je prav častniški zbor tisti, ki je odgovoren za vodenje oboroženih sil. Znanstvene razprave o vojaški profesiji in profesionalnem častniku so se pojavile po koncu druge svetovne vojne z deli Samuela Huntingtona, Morissa Janowitza, Bengta Abrahamssona in drugih. Njihova razmišljanja so odsevala predvsem okoli- ščine v času hladne vojne, te pa so se s padcem berlinskega zidu v devetdesetih letih prejšnjega stoletja močno spremenile. Večina avtorjev tako ugotavlja, da je nujno preveriti in posodobiti koncepte vojaške profesije in vojaškega profesionalca. V članku poskušam odgovoriti na to, kaj danes pomenita pojma vojaška profesija in profesionalni častnik ter kako bi k razvoju vojaške profesije in profesionalnega častnika morali pristopiti v Slovenski vojski (SV). Civil society in democratic countries expects its armed forces to be professional. Therefore, the armed forces in those countries need to have a clear idea of what pro- fession of arms is. They should especially have a clear idea of what a professional officer is, since the officer corps is responsible for leading the armed forces. The subjects of profession of arms and professional officer were first scientifically discussed after the Second World War by authors such as Samuel Huntington, Morris Janowitz, Bengt Abrahamson and others. Their theories reflected the circumstances of the Cold War, which, however, significantly changed with the fall of the Berlin Wall in the 1990’s. Most of the authors today have thus identified the need for revising and updating the traditional concepts of the profession of arms and military professional. The article attempts an answer to what the terms profession of arms and professional officer mean today, and how the SAF should approach the development of military profession and professional officers.

Abstract:
The examination of the final chapters of the books of Samuel offered here proceeds initially from an unease about the frequent pejorative appraisal of them as an unsuitable, late addition that is out of place in its context. Taking a cue from the recent interest in initial and concluding texts in the context of literary and "Canonical" methods, the work attempts to describe this text complex, with its six chiastically arranged units, in terms of its literary function as a concluding text in relation to the rest of the preceding book. Following remarks of W. Brueggemann and J. Flanagan, the ring structure is further compared with other groups of texts in the Samuel corpus. The specific, overarching macrostructure which is thus perceived is structured not according to linear-chronological principles but according to patterns of parallelism and chiasmus. This observation of a concept of order that is distinct from modem western convention is understood, following Emma Brunner-Traut, as deriving from an "aspectival" perception of reality. By means of this kind of reading, many inner relationships open up, binding the closing chapters to the other parts of the book in such a way that it is shown to be a unified literary work. The two poetic texts are shown to stand in a complex relationship with the four other songs of the books of Samuel. The allusion to the prophet Gad belongs in a series of six encounters between David and prophets, arranged as a set of three pairs. The two lists of soldiers are interpreted by analogy with the double lists of the sons and ministers of David. The final contrast, in the closing chapter, between the two kings, Saul and David, and the polarity - expressed in the tension between centre and periphery - between rule of Yahweh and sin of the kings, both mirror and finally draw together the main themes of the book. These relationships suggest that the appraisal of the closing chapters as a late addition is in need of revision. It will be argued that they should be interpreted in close connection with the rest of the book.

Le cercle montre des caracteres symboliques pour beckett, la lettre m de mot et la lettre w de word pour le bilinguisme, le m et le w comme initiales de personnages innommables ou fantomes, le chiffre 3 d'une trilogie. L'observation des lettres sam sur le plan phonique, graphique et semantique permet l'etude d'une voix secrete dans cette microtrilogie. Le jeu de lettres et de langues montre l'expression d'une voix. Le signe linguistique dans sa totalite se trouve engage dans l'histoire de sam. Cette histoire est a la fois transcendanteet immanente. L'approche pour raconter l'histoire de ce discours est theologique, philosophique et psychanalytique. L'heritage de la question sur un << sujet litteraire >> a partir de proust, kafka et joyce va jusqu'a l'epuisement dans la derniere trilogie de beckett. Cette ecriture apporte des questions fondamentales pour la litterature.

This study intends to analyze the contributions of Samuel Harman Lowrie and Dina Lévi-Strauss to the Department of Culture of São Paulo during the period of administration of Mário de Andrade, between the years 1935 and 1938. Both were directly or indirectly representative of the theoretical thoughts and trends which provided a synthesis of the influences for Brazilian Social Sciences along the years: the North-American and the European lineages. The North-American sociologist Samuel Harman Lowrie was hired by the Escola Livre de Sociologia e Política and by the São Paulo City Administration, and developed a study in the area of Sociology and its practical applications, whereas the French ethnologist Dina Lévi-Strauss, with the support and trust from Mário de Andrade, conduced actions in the area of Ethnography, aiming at the possibility of obtaining reconnaissance in her professional life. During a short period, these two scholars allowed the simultaneity of theoretical reflections and practical actions in the universe of public policies, since they believed that Brazil was a favorable place for the development of scientific research
Este trabalho analisa as contribuições de Samuel Harman Lowrie e Dina Lévi-Strauss ao Departamento de Cultura de São Paulo na gestão de Mário de Andrade, entre 1935 e 1938. Foram representantes na época, direta ou indiretamente, do pensamento e das linhas teóricas, que ao longo do tempo conferiram o retrato síntese das influências das Ciências Sociais no Brasil: a norte-americana e a européia. Ele, norte-americano contratado pela Escola Livre de Sociologia e Política e pelo município de São Paulo, desenvolve trabalho voltado para a Sociologia e sua aplicação prática. Ela, etnóloga francesa conduz, sob o estímulo e confiança de Mário de Andrade, ações no campo da Etnografia, tendo em vista a possibilidade de obter reconhecimento em sua trajetória profissional. Durante um curto período, estes dois intelectuais tornaram viável a simultaneidade entre a reflexão teórica e a execução prática no universo das políticas públicas, por acreditarem ser o Brasil campo fértil para o desenvolvimento de estudos científicos

For each passage, the chapter considers the amount of evidence (strong, moderate, or weak) that it provides for the book’s thesis, that God intends that sin occur. Also, for each passage the chapter considers alternative interpretations, interpretations on which God does not intend that sin occur. In 1868 the theatrical context in Venice was affected by local abut also statal economic situation, due by the development of the new Italian State, formed after the Peace of Vienna that, signed on 23 October 1866, marked the end of the Third War of Independence and the annexation of Venice to the unitary state. Venice therefore became less important for theatrical production, and the city was no longer among the capitals of theatre and music, as it could have been considered until in the first half of 19th century. A new trend throughout Europe was moreover the ‘grand opera’, while in Venice the dominant show was still the traditional musical opera, in addition to opera and ‘opera buffa’, created by great Italian composers. The theatres active in 1868 in the city are La Fenice, Teatro San Benedetto then called Rossini, Teatro Apollo, Teatro Malibran and Teatro San Samuele. Through the reviews of the Gazzetta di Venezia and documents such as sketches of the scene made by famous set designers, that interesting moment is reconstructed.

THE STORY OF Samuel, the last and possibly the best of the judges (1 Sam 7:15), provides the framework through which we view the rise and fall of Israel’s first king. The opening section of the book focuses on Samuel’s birth, childhood, and lifelong work as a circuit-riding judge (7:16–17), and then in his old age, his reluctant anointing of a monarch so Israel can fulfill its ambition to be “like all the nations” (8:20). Saul’s last important exchange before his own death is with the disgruntled ghost of Samuel, summoned back from Sheol by a medium, through whom Samuel delivers, not the guidance for which Saul longs, but rather a last rehearsal of Saul’s royal failures (28:16–19); hence, Samuel renders the final judgment on Saul’s kingship. But the book’s first and most comprehensive statement about how God characteristically disturbs human power arrangements comes many years before that, in an exultant song (2:1–10) uttered by Samuel’s mother, the once-barren Hannah. Beckett’s translations of his own poems from French into English puzzle both his admirers and his devoted scholars. Marjorie Perloff was the first to draw attention to the disparity between the French originals and their subsequent rendering in English, on the example of “je suis ce cours de sable qui glisse” and “my way is in the sand flowing.” As is the case of many works by Beckett first written in French and then translated by himself, the French original and the English version are never exact counterparts or analogues of each other. Beckett’s translating practice goes to prove the thesis advanced in translation studies that self-translators usually take advantage of poetic license and rewrite “their” originals. In fact, Beckett’s bilingual creations invite rather a study of semantic interplay between the two linguistic versions. This chapter deals with the three poems first published in Transition 48.2, June 1946: “je voudrais que mon amour meure,” “je suis ce cours de sable qui glisse,” and “que ferais-je sans ce monde sans visage sans questions”, and their English counterparts. Finally, some attention is given to “Comment dire” and “what is the word.” The last two mentioned texts merit special attention as they are Beckett’s final works, “what is the word” appearing in print shortly after his death.

Temporaland spatial properties of a stimulus visual can be used to isolate different visual pathways (sustained and transient) which are thought to be mediated by different neuronal sub-populations (M- and P-cells) in humans. Temporal modulation has been used to detect early visual dysfunction by way of flicker modulation thresholds or frequency doubling (Casson et al., 1993: Johnson & Samuels, 1997). In these cases it has been assumed that flicker detects losses by targeting sub-populations of neurones.

Abstract In today’s fast growing world, the economy — especially the field of technology and production — are developing very rapidly. Engineering design that would predict the results of this rapid development and equip the society with tools to control them, faces a big challenge. Rapidly developing technology brings many benefits to humanity and makes life easier, friendlier and more comfortable. This has been the case for thousands of years as new branches of engineering were born and came to serve society. One might say that engineers have the privilege of creating a bloodless and peaceful revolution resulting in easier and happier lives for people. At the same time, such fast developing technology creates traps and dangers, and may cause harm. The inventions of Alfred Nobel, Samuel Colt and Eliphalet Remington, for example, or nuclear research have all brought significant technological progress to nations and societies but have also brought harms and disasters affecting both societies and individuals. The role of engineering design is to predict these harmful actions and plan to neutralize or eliminate them, or even change them from harmful into friendly. Such actions follow the way recommended by BTIPS (Brief Theory of Inventive Problem Solving) procedures [1], especially those using the Prediction module [2], [3]. When developing Prevention Engineering a system approach should be observed and hierarchy of systems established and defined. All systems should be designed in such a way that prevents harm to humans and the natural world. Recommendations for introducing Prevention Engineering as a branch of engineering practice, and as an educational and research discipline, should be created as soon as possible, and directions for introducing courses in Prevention Engineering design and practice should also be developed [4]. For example, personal protective equipment for individuals and groups as designed by ME and MEM engineering students in their courses might be considered as Prevention Engineering developments [5]. Defining and formulating Prevention Engineering as a new branch of engineering is necessity in our times. In every step of our lives we face the challenge of preventing harms and destruction that can be done by the contemporary surrounding world. The goal of Prevention Engineering [PE] is to make the world safe. Prevention and safety are connected, prevention is an action, while safety is the condition or state that we are trying to achieve. Preventative actions can be based on the recommendations of BTIPS - Brief Theory of Inventing Problem Solving - and may use BTIPS’s approach [4], [5]. The reasons for the development of PE have already been described [6]. Each of these should be pointed out and preventative measures should be found. Adding these preventative measures to the contemporary engineering research, practice and education, and especially reflecting them in the engineering curriculum would be useful now and will also be necessary in the future [7], [8].
