<p>Society expects that buildings and other structures are safe for the people who use them or who are near them. The failure of a building or structure is expected to be an extremely rare event. Thus, society implicitly relies on the expertise of the professionals involved in the planning, design, construction, operation and maintenance of the structures it uses.<p>Structural engineers devote all their effort to meeting society’s expectations effi ciently. Engineers and scientists work together to develop solutions to structural problems. Given that nothing is absolutely and eternally safe, the goal is to attain an acceptably small probability of failure for a structure, a facility, or a situation. Reliability analysis is part of the science and practice of engineering today, not only with respect to the safety of structures, but also for questions of serviceability and other requirements of technical systems that might be impacted by some probability.<p>The present volume takes a rather broad approach to safety and reliability in Structural Engineering. It treats the underlying concepts of safety, reliability and risk and introduces the reader in a fi rst chapter to the main concepts and strategies for dealing with hazards. The next chapter is devoted to the processing of data into information that is relevant for applying reliability theory. Two following chapters deal with the modelling of structures and with methods of reliability analysis. Another chapter focuses on problems related to establishing target reliabilities, assessing existing structures, and on effective strategies against human error. The last chapter presents an outlook to more advanced applications. The Appendix supports the application of the methods proposed and refers readers to a number of related computer programs.<p>This book is aimed at both students and practicing engineers. It presents the concepts and procedures of reliability analysis in a straightforward, understandable way, making use of simple examples, rather than extended theoretical discussion. It is hoped that this approach serves to advance the application of safety and reliability analysis in engineering practice.<p>The book is amended with a free access to an educational version of a Variables Processor computer program. FreeVaP can be downloaded free of charge and supports the understanding of the subjects treated in this book.

Patient safety suddenly burst into public consciousness in the late 1990s and became a “celebrated” cause in the 2000s. It has since gradually faltered, and little improvement has been noted over almost 20 years. Both the rise and fall of patient safety demand explanation. Medical harm had been known long before the 1990s, so why did it suddenly become popular? And why were safety efforts ineffective? The authors propose that this rise was due to a discursive shift that reframed “medical harm” into “medical error” in the setting of anxiety about industrialization and great change in healthcare. The “error” framing, with its inherent notion of agency, was useful in advancing the agenda of a technocratic, managerial group of health professionals and diminishing the authority of the old guard based on clinical expertise. The fall was due to this “medicalization” of safety. Health professionals and managers with little knowledge of safety science came to dominate the patient safety field, crowding out expertise from the safety sciences (e.g., psychology, engineering) and thus keeping reform under the control of the healthcare establishment. Operating with a sort of delusional clarity, this scientific-bureaucratic cabal generated a great deal of activity but made little progress because they failed to engage with expertise in the safety sciences. Twenty years after sudden popularity, there is general agreement that little of value has been achieved. The future of patient safety is in doubt, and radical reform in approaches to safety will be required for progress to be made.

One of the essential components of the safe vaccination system is the surveillance of events supposedly attributable to vaccination or immunization (ESAVI). This surveillance is aimed at early detection of any adverse events that may occur following immunization, in order to monitor and classify risks related to a vaccine, the manufacturing process, transportation, storage, administration, and any preexisting condition in the vaccinated person, and to rule out an association between the event and the vaccine. This manual has been adapted for the Region of the Americas from the Global Manual on Surveillance of Adverse Events Following Immunization, published by the World Health Organization in 2014. It provides a comprehensive technical review of all processes and procedures for applying and implementing high-quality ESAVI surveillance systems. It brings together the expertise of vaccine safety specialists from the Region and from around the world, experts from national immunization programs, national regulatory bodies, and other institutions that have developed relevant knowledge on surveillance of these events. It is hoped that this document will serve as a guide to provide national immunization program managers, pharmacovigilance officers of national regulatory authorities, and other institutions responsible for monitoring vaccine safety with tools to facilitate their task, enabling them to apply international standards to issues such as event detection, event investigation, causality assessment, management of ESAVI data, and risk communication.

The term ‘formal methods’ in computer science refers to a suite of techniques drawing on mathematical notions of sets, logic, and functions or precise diagrammatic notations, most of which are currently primarily focused on safety-critical applications in the aerospace or nuclear industries. While research into broader use of these methods could be regarded as a theoretical interest, the early development of formal methods was driven as much by practical considerations as theory. Reviving Rationality: Saving Cost-Benefit Analysis for the Sake of the Environment and Our Health explains how Donald Trump destabilized the decades-long bipartisan consensus that federal agencies must base their decisions on evidence, expertise, and analysis. Administrative agencies are charged by law with protecting values like stable financial markets and clean air. Their decisions often have profound consequences, affecting everything from the safety of workplaces to access to the dream of home ownership. Under the Trump administration, agencies have been hampered in their ability to advance these missions by the conflicting ideological whims of a changing cast of political appointees and overwhelming pressure from well-connected interest groups. Inconvenient evidence has been ignored, experts have been sidelined, and analysis has been used to obscure facts rather than inform the public. The results are grim: incoherent policy, social division, defeats in court, a demoralized federal workforce, and a loss of faith in government’s ability to respond to pressing problems. This experiment in abandoning the norms of good governance has been a disaster. Reviving Rationality explains how and why our government has abandoned rationality in recent years, and why it is so important for future administrations to restore rigorous and even-handed cost-benefit analysis if we are to return to a policymaking approach that effectively tackles the most pressing problems of our era.

Since the mid-1980s pioneering research about daily coercive interactions in relationships has shed light on the development and treatment of multiple mental health problems and school safety issues. This handbook brings together the expertise and the perspective of leading scholars and intervention developers in an effort to interpret and clarify coercive dynamics and discuss interventions that reduce coercion and improve health and adjustment. Researchers examine social, physiological, and genetic correlates of coercion dynamics from multiple perspectives, including an evolutionary framework. Coercion is explored with respect to the etiology of aggression, antisocial behavior, violence, anxiety disorders, suicidal behavior, and academic adjustment. Interventions are described that effectively reduce coercion in families, romantic and peer relationships, committed adult intimate relationships, and schools, and among youths and families with autism. Several chapters illustrate methodological, measurement, and conceptual issues that enhance the scientific understanding of how daily coercive interactions influence adjustment over time. Also offered are prospects for prosocial cooperation free of coercive dynamics and strategies for disseminating interventions across global communities for promoting public health and well-being.
