Abstract:
Abstract:
The article analyzes the regional features of the provision of medical personnel in Russia in the context of various subjects and federal districts. P u r p o s e of the study is to analyze the features of staffing of medical organizations in the constituent entities of the Russian Federation in modern conditions and the main directions of policy to improve the staffing situation at the regional level. M a t e r i a l s a n d m e t h o d s . According to the official statistics, an analysis of the staffing of medical and paramedical personnel was carried out in the context of the constituent entities of the Russian Federation; in the course of the study, methods were used such as: analytical, statistical, study and generalization of experience. R e s u l t s . The analysis showed that the provision of doctors and the provision of emergency medical services for different regions of the Russian Federation is different. The lowest values of the indicator of the provision of doctors in 2019 were recorded in the Ural FD, the Southern FD and the North Caucasian FD, while in the constituent entities of the Russian Federation the most pronounced tendency towards a decrease in the level of provision nursing staff was noted in Moscow, Tyumen region, Chukotka Autonomous Okrug, Republic of Kalmykia, Udmurt Republic, as well as in Yaroslavl, Murmansk, Novgorod and Kirov regions, which emphasizes the need to take appropriate management measures, both at the local and federal levels. F i n d i n g s . The existing imbalance in the country between the provision of the population with doctors and nurses in various regions (the difference between low and high rates is more than 2.3 times), as well as between the provision of medical workers in the city and rural areas is the main problem of staffing the system. To overcome this situation, further implementation of new measures of social support and moral incentives for medical personnel in Russia is required.

R e l e v a n c e : The variety of methods and approaches to the financing of medical care accumulated by the world experience determines the importance of classification. However, the existing capabilities of classification systems seem to be limited and unsuitable for practice and management decision-making. P u r p o s e : development of a new classification of health care financing systems to improve the efficiency of managerial decisions taken when modeling long-term strategies for the development of health care in the Russian Federation. M e t h o d s : A method was used based on a functional and structural analysis of the main links of the technological cycle of financing public (free at the place of delivery) medical care: coverage, determination of sources of revenues, their redistribution, consolidation, payment for medical care and management expenses for creating a new classification of health care financing systems. R e s u l t s a n d c o n c l u s i o n s : A new classification has been developed and tested to analyze the national health systems of different countries and model the most relevant changes in the health financing system. One of the most important characteristics of the labour market is labour mobility that allows assessing the economic efficienc y o f labour . A comparativ e analysi s i s necessar y fo r determinin g th e degre e o f mobility . I n term s o f spatia l and sectoral characteristics, the paper assesses the degree and dynamics of mobility in the Russian labour market based on previously published studies, as well as the authors’ findings. To determine the degree of mobility, the research uses various approaches, applying both direct (mobility costs, transition matrices) and indirect indicators (structural unemployment, wage differentiation, unemployment rate, gross regional product (GRP)). The analysis uses the data of the Russia Longitudinal Monitoring Survey — Higher School of Economics (RLMS-HSE) and Federal State Statistic Service (Rosstat) for 2000– 2016. The obtained results demonstrate a relatively low intersectoral and interregional mobility in Russia compared to Organisation for Economic Co-operation and Development (OECD) countries. Low intersectoral mobility may indicate weak exchangeability of the sectors and high mobility costs. The largest number of transitions is observed in trade, where employees do not need any specific knowledge. Generally, other transitions are made between related sectors that require similar knowledge from employees. The lowest intersectoral mobility is characteristic for the education and health sectors. According to the Shorrocks index, in Russia, interregional mobility is lower than intersectoral mobility. Low spatial mobility is explained by high migration costs, including those associated with “poverty traps”, the peculiarity of statistical accounting of migrants and the size of Russian regions. The obtained results are correct for the examined period and the applied criteria. The changes in labour mobility in Russia caused by global digitalisation of the economy and the transition to remote working require a separate study.

Abstract:
Abstract Objectives To compare the patterns of thermal food processing methods in patients with and without the syndrome intestinal bacterial overgrowth (SIBO). Methods It was retrospective single-center study. Data of patients with lactulose breath test GastroCheck Gastrolyser (Bedfont, UK) and diet assessment (24-hours dietary recall) were enrolled. Those with excessive production of hydrogen, methane, both or neither gases were identified. All dishes and products consumed by a patient we divided to 6 categories depending on thermal food processing method: those that were not processed (raw) (Cat-r), boiled (Cat-bl), fried (Cat-f), stewed (Cat-s), baked (Cat-bk) in the oven or grilled (Cat-g). To analyze the structure of thermal food processing, we divided the weight of all products or dishes that underwent specific method of processing to the total weight of the food consumed. The pattern of thermal food processing within each group was obtained as a quotient of the total percentage (by weight) of the food processed with the certain method by the number of patients in the group. Results The data of 1200 patients were available for the final analysis: 640 patients in the SIBO-H2 group, 162 in the SIBO-CH4 group, 270 in the SIBO-CH4-H2 group, and 128 patients in the control group. The distribution of thermal food processing categories was as follows: in patients with SIBO-H2 (Cat-r: 45.3 ± 17.2%, Cat-bl: 31.8 ± 15.6%, Cat-s: 3.5 ± 7.7%, Cat-f: 6.5 ± 10.5%, Cat-bk: 12.4 ± 10.3%, Cat-g 0.3 ± 3.3%); with SIBO-CH4 (Cat-r: 47.2 ± 16.6%, Cat-bl: 29.5 ± 15.1%, Cat-s: 4.3 ± 7.4%, Cat-f: 5.8 ± 10.0%, Cat-bk: 12.4 ± 10.3%, Cat-g: 0,4 ± 3,9%); with SIBO-CH4-H2 (Cat-r: 45.5 ± 16.3%, Cat-bl: 31.0 ± 16.1%, Cat-s: 4.0 ± 8.0%, Cat-f: 5.4 ± 9.9%, Cat-bk: 13.4 ± 11.0%, Cat-g: 0.3 ± 2.2%); in the control group (Cat-r: 44.8 ± 16.8%, Cat-bl: 32.4 ± 16.4%, Cat-s: 3.0 ± 6.5%, Cat-f: 5.2 ± 7.7%, Cat-bk 14.1 ± 10.4%, Cat-g: 0.2 ± 1.9%). No significant difference was found between the study groups and the control group by mean percentage of raw, boiled, stewed, fried, baked and grilled food intake. Conclusions We found no association between thermal food processing patterns and SIBO. Funding Sources The study was supported by Russian Scientific Foundation grant No. 19-76-30014.

Abstract:
Competent management is crucial condition for the development and functioning and requires qualified managers. In the Russian Federation over the past few years Federation there were many rotations in management of regional health authorities. These changes will be followed by lack of continuity in management decision-making, high degree of uncertainty and a low level of population and professional community trust. T h e a i m o f t h e s t u d y was to give a detailed information about regional health authorities managers in Russian Federation. M a t e r i a l s a n d m e t h o d s . This research was designed as observational study with the use of questionnaire which included the characteristics of the age and gender of the respondents, the level of their basic and additional education, additional competencies, occupational background in leadership position. R e s u l t s . We achieved a response from 81 respondents. We founded that rotation of managers and extremely short occupation period had significant impact on managers’ qualifications. The culture of purposeful formation of the professional skills by the respondents was poor. Only 7% of managers paid attention to their education. About 40% did not have professional retraining for management in healthcare. C o n c l u s i o n s . It is necessary to develop a standard set of Russian region health managers competencies and responsibilities.

Abstract:
The Soviet diplomacy in the second half of the 1940s included the Russian Orthodox Church and its institutions of international presence in its sphere of activity. At that time the Russian Orthodox Church (ROC) began to play a significant role in the Soviet Union's foreign policy. The Middle Eastern direction becomes one of the most significant areas of “church diplomacy”. The first visit of Patriarch Alexy (Simanskiy) to the Holy Land in 1945 was part of a “package” of diplomatic steps made by Soviet diplomacy in partnership with the Moscow Patriarchate in 1945–1955 to restore the property of the ROC in Palestine. The analysis of the documents on the ROC (State Archive of the Russian Federation, F. R-6991) and the materials on the foreign policy of the USSR Council of Ministers (State Archive of the Russian Federation, F. R-5446), as well as the extensive historiography of historical relations between Russia and the Holy Land, allows the authors to consider joint efforts to consolidate the presence of the ROC in the region. The research allows tracing the “birth of tradition” of foreign policy mission of the Moscow Patriarchate and its foreign structures, which became points of influence of the USSR in the post-war world. It allows one to reconstruct the social image of Moscow's “agents of influence” in the Middle East, both the new emissaries and the traditional agents of Russian influence in the region – the pilgrims and nuns of the Russian monasteries of the Holy Land.

Abstract:
With age, the risk of stroke increases significantly, but at the same time, there are tendencies for an increase in its prevalence in young and middle-aged people, which determines the urgency of the problem of rehabilitation treatment after stroke both in the world and in Russia. The use of digital technologies in neurorehabilitation can be very multifaceted and extensive. Telerehabilitation as an innovative direction of telemedicine is an important aspect within the framework of the national project «Healthcare» and the program «Digital Economy of the Russian Federation». P u r p o s e of the study is to analysis of the Russian experience of organizing and conducting telerehabilitation of post-stroke period in the context of the digital transformation of the healthcare system in Russia. M a t e r i a l s a n d m e t h o d s . The article used technologies for searching and selecting information using search engines (www.google.ru, www.yandex.ru), specialized and bibliographic databases (PubMed, eLIBRARY.RU), methods of analysis and synthesis, analytical data processing. R e s u l t s . The study showed that telerehabilitation has a high clinical value of adherence to the treatment of patients of post-stroke period and is not a separate type of medical activity, but an innovative therapeutic and prophylactic method that expands the possibilities of therapy and improves the process of interaction in the «doctor-patient» and «doctor-doctor» communication systems. This technology of restorative telemedicine makes it possible to increase the level of scientific knowledge, skills and abilities of healthcare organizers and medical workers in order to strengthen the national healthcare system of Russia, as well as to rationally use the scientific and practical potential of digital solutions in interdisciplinary interaction of specialists dealing with medical neurorehabilitation, significantly expanding the coverage of the population telerehabilitation services after a stroke. F i n d i n g s . Despite the positive experience of some medical organizations in Russia in the use of telerehabilitation, this direction is still in its infancy and is likely to develop in the coming years for the rehabilitation of many stroke patients, which will help healthcare organizers, doctors and patients and other interdisciplinary specialists effectively and interact seamlessly in the digital health ecosystem in the context of the development of patient-centered medicine.

The implementation of the healthcare development strategy, effective management of the healthcare system is impossible without monitoring and comprehensive comparative analysis at various levels of the state and development of the network, resources and activities of medical organizations, individual services and the healthcare system as a whole. P u r p o s e of the study is scientific assessment of the state of the network of medical organizations providing medical care in outpatient and inpatient settings, as well as analysis of the provision of beds for the population of the Russian Federation in 2012–2019. M a t e r i a l s a n d m e t h o d s . Within the framework of the conducted research, official statistics data were analyzed, modern trends and trends in the provision of beds and the state of the network of medical organizations in Russia at the regional level were determined. Statistical data processing was carried out using SPSS, Excel‑2007 packages. Such methods as analytical, statistical methods, study and generalization of experience, comparative analysis were used. R e s u l t s . The analysis has shown that the unevenness and the level of high differentiation of indicators of provision of beds by subjects of the Russian Federation remains. Thus, over the period 2012–2019, the provision of specialized beds for all the main profiles of beds is decreasing, the largest reduction affected socially significant beds – dermatovenerological – by 1,5 times, as well as therapeutic – by 32,5%, gastroenterological – by 28,2%, pediatric – by 24,4%, and infectious – by 20,3%. F i n d i n g s . The results of this study confirm the reduction in the number of beds of the main profiles, as well as the conversion of part of the bed stock, especially in the context of a pandemic of a new coronavirus infection, which showed the need for structural and functional transformations of health resources in general and the bed stock in particular.

Abstract:

Abstract:

Abstract:

Abstract:
Hypothesis: the child grappling with his/her infantile neurosis acts according to his/her perception of his;/her Other, a person for whom or a place for which his/her speech is meant. Ln the work of the first psychoanalysts for children, we check whether or not a stumbling block of the act is that the analyst comes to conform with what the child infers from his/her perception of his/her Other. Fantasy becomes reality. Defences are reinforced. The misunderstanding : the analyst would know what is good for the child, his/her nature and would act accordingly: as a father, a mother, a child, an. . . Ideal analyst. M. Klein once again invokes the good Mother of certain phenomena of transference. A. Freud builds, according to the fantasy of the child in his/her oedipian age, laws to his/her psychosocial development. D. W. Winnicott, playing, frustrates the child's actings and outlines the symbolical presence of the analyst in the city (the demand of speech). F. Dolto becomes the Other of the Truth, gives the child the knowledge on his/her fantasy. Symbolical castration is, for everyone, the goal of the cure : the analyse delivers it while fulfilling the so called "psy" needs. R et R. Lefort do not check this collage. The subject opens to his/her desire, not of a knowledge of himself/herself, but of his/her access to his/her speech. The relationship of the subject with the signifiers, with the phallic deficiency of the drive, is put into function. The hypothesis is confirmed. The analyst for children is not the imaginary or real images of the Other. The history of concepts confirms that unconscious knowledge is on the child's side- and not on the analyst's. The end of the cure: the child becomes a subject to the science discourse of which he/she is the object of the discontent of civilisation. The definition is no longer given in opposition with the social but according to the aim of the questions which founds it on the invocation of the subject. This confirmation shows a difference with ail "ideologization" of psychoanalysis, a questioning of the social act : its response to invocations and questions asked

Abstract:

Abstract:
Paul Wittgenstein (1887-1961) made significant contributions to the piano literature for the left hand through numerous commissioned works as well as his own transcriptions. In the transcriptions, Wittgenstein preserved the texture of two-hand music, aiming for the simulation of the original works. This requires special techniques in the performance by the left hand alone. This dissertation investigations technical means and performance problems associated with the transcriptions as well as Wittgenstein's own recordings of selections from his works. Chapter 1 serves as an introduction, providing a historical overview of the role of the left hand in two-hand piano literature. Chapter 2 gives biological information on Paul Wittgenstein and discusses the commissioned works. Chapter 3 investigates special techniques in the transcriptions, in the areas of arpeggios, widespread chords, fingering, pedaling, and others. Chapter 4 discusses Wittgensteins's performance style based on his recordings. Chapter 5 presents a conclusion pointing to the benefits of performing left-hand music in two-hand piano playing.

Abstract:
This dissertation addresses the intensifying global issue of food insecurity within our country and the contributing factors thereof. It investigates the role architecture can play as a systemic tool (in the form of a gastronomic quarter) to facilitate the re[grow]th of a highly contested site. By identifying and drawing from global and local issues, new solutions, developed through the integration of socio-economic systems, act as points of acupuncture. These points aim to improve contextual conditions through food network empowerment. Through a newly informed architectural approach various people within society are enabled through local means of support. It is food and the accessibility thereof that forms the basis of the socially inclined space. Local support is essential for community growth and empowerment, which in turn, positively impacts the overall urban fabric through the provision of a more inclusive environment.

Eliot." Thesis, Strasbourg, 2014. http://www.theses.fr/2014STRAC005/document.

Full text
Abstract:
Connecting T. S. Eliot's Waste Land to R. F. Burton's Kasidah produces a literary theory. The founding principle of this theory is the Unity of Consciousness (Wahdat al Wayy), after the exegesis of Ibn Arabi (Wahdat al Wujud and Wahdat al Adyan). It also postulates that any life is but a stream of consciousness. Action is thus the way by which consciousness writes in the world, and experience is the way the world writes in consciousness. The expression of consciousness in perspective is in turn a profound literary invariant, connecting The Waste Land and The Kasidah but also Poe's Al Aaraaf, Baudelaire's Voyage, Villon's Testament or Leopardi's Canto Notturno. Another invariant, based on the precedent, is the invariant of the wasteland, which can be summed up by the myth of the Ortolano Eterno : Homo : locatus est, damnatus est, humatus est, renatus est : in Horto. Now the seventh surah of the Quran is a notable expression of the invariant of the wasteland. In the same way that there is a connectomics of the human brain, there is a connectomics and also a biology of literatures. A sample of its corpus callosum, connecting the Western and Eastern literatures, is the "chain of the wasteland", a lineament of texts which leitmotiv is the interaction between consciousness and the world. Regarding Eliot his direct sufi influences range from Omar Khayyam to Guénon and Schuon, and his indirect ones regard the known sufi influence over the troubadours. In turn Eliot has been influencing the contemporary poetry of the muslim area since at least 1950

Abstract:
Contemporary Russian socio-cultural, cultural and philosophical, socio psychological, artistic and aesthetic practices actualize the Russian tradition of rejection, criticism, undisguised hatred and fear of power. Today, however, power has ceased to be a subject of one-dimensional denial or condemnation, becoming the subject of an interdisciplinary scientific discourse that integrates cultural studies, philosophy, social psychology, semiotics, art criticism and history (history of culture). The article provides theoretical substantiation and empirical support for the two facets of notions of power. The first facet is the unique, not only political, but also mental determinant of the problem of power in Russia, a kind of reflection of modus vivendi. The second facet is the artistic and image-based determinant of problem of power in Russia designated as artis imago. Theoretical grounds for solving these problems are found in F. Nietzsche’s perceptions of the binary “potentate-mass” opposition, G. Le Bon’s of the “leader”, K.-G. Jung’s of mechanisms of human motivation for power. The paper dwells on the “semiosis of power” in the focus of thoughts by A. F. Losev, P. A. Sorokin, R. Barthes. Based on S. Freud’s views of the unconscious and G. V. Plekhanov’s and J. Maritain’s views of the totalitarian power, we substantiate the concept of “the imperial unconscious”. The paper focuses on the importance of the freedom motif in art (D. Diderot and V. G. Belinsky as theorists, S. Y. Yursky as an art practitioner). Power as a subject of influence and object of analysis by Russian creators is studied on the material of perceptions and creative experience of A. S. Pushkin (in the context of works devoted to Russian “impostors” by numerous authors). Special attention is paid to the early twenty-first century television series on Soviet rulers (Stalin, Khrushchev, Brezhnev, Furtseva). The conclusion is made on the relevance of Pushkin’s remark about “living power” “hated by the rabble” for contemporary Russia.

Abstract:
The article is devoted to the analysis of the problems of inequality in modern education from the point of view of the world-system approach. The authors establish links between educational inequality and in- 153 equality of societies in the «core – semiperiphery – periphery» structure. The authors attempt to consider the mechanism of the formation of educational inequality in peripheral societies in which social contradictions are most clearly observed from the perspective of the world-system approach. The authors use the theoretical constructions of the world-system approach of I. Wallerstein, S. Amin, F. Cardozo, the ideas of the representatives of the world-system paradigm in comparative education of R.F. Arnove, T. Griffiths, and the concept of a closed circle of inequality in education by R. Flecha. The authors believe that changes in the education system of a society that is integrated into the world-system through the specialization of its economy correspond to those specific transformations that are caused in this society by the innovation spread by the global hegemon. The authors of the article show that the reform of the education system proceeds in the general direction of integrating society into the world system of the division of labor, when the elite forms priority consumption patterns in a peripheralized society (including patterns of knowledge and education), borrowing they from the countries of the core and the hegemon of the world system. The formation of priority patterns leads to the displacement of their own educational culture, the imitation of the masses of the elite and the uneven spread of the patterns. Since full compliance with the priority patterns is unattainable for the majority of the population, its imitations are spreading.

Full text
Abstract:
The main problem of wheat immunity to leaf rust is the loss of efficiency of most Lrgenes. The decrease in efficiency is associated with microevolutionary processes within the population and the emergence of new virulent phytopathogen races that can overcome previously efficient resistance genes. The article presents the results of the phytopathological test and marker analysis of the selected material of winter wheat for resistance to the leaf rust pathogen (Puccinia recondita Rob.ex Desm f. sp. tritici.). The object of the research was 20 cultivar samples of various ecological and geographical origins. DNA was isolated from the leaves of 10-day-old wheat germs. Molecular markers were used for the following genes: Lr9 (SCS5), Lr10 (Fi.2245/Lr10-6/r2), Lr19/Sr25 (SCS265), Lr20/Sr15 (STS638), Lr24/Sr24 (Sr24#12), Lr34/Sr57 (csLV34), Lr37/Sr38/Yr17/Pch2/Cre5 (Ventriup/LN2), Lr41 (GDM35), Lr47 (PS10). Using molecular markers, the studied wheat varieties did not reveal the highly and partially effective genes Lr9, Lr19/Sr25, Lr24/Sr24, Lr41, and Lr47 in Russia, and the ineffective gene Lr20/Sr1. As a result of molecular screening, it was found that the List 25 variety had Lr37 genes; the Mif variety had Lr10 genes; the Eltan variety had Lr10 genes; the Markola variety had Lr34 genes; the Malvina variety had Lr26 genes; the Tvorets variety had Lr10 genes; the DB 1/05 variety had Lr10 genes; the Evklid variety had Lr10 genes; the Sumai aut variety had Lr34 genes; the Lebidka odes'ka variety had Lr34 genes; the Solara variety – Lr34; the Zhiva variety – Lr10, Lr34. When comparing the results of marker analysis with field resistance to leaf rust, the resistant type of reaction to infection (R) was shown by the cultivars: Battum, Eltan, Evklid, Areal, and Solara; the susceptible type of reaction (S) was noted in the cultivars Markola and Mallyska; the medium susceptible type of reaction (MS) – in the cultivars Lebidka odes'ka and Tvorets.

Abstract:
The paper discusses the possibility of a potential reception between F.-R. Chateaubriand and S. S. Uvarov in pamphlets on Napoleon in 1814. The author (in the process of comparative comparison of political and cultural glossaries and rhetorical strategies of both figures) draws on the methodological tools of the Cambridge School of the “History of Concepts” (Q. Skinner and J.G.A. Pocock). Based on this analysis, a conclusion is drawn about the similarities and differences between rhetorical strategies, political, and cultural glossaries in the designated pamphlets about Napoleon.

Abstract:

Abstract:
ASME B31.8 states that "Dents that contain stress corrosion cracking or other cracks are injurious to the pipeline" and therefore, requires immediate attention by the Operators. Dent containing crack fields (colonies) are often observed in liquid pipelines. The recently completed PRCI research project MD-1N "Study of the Mechanism for Cracking in Dents in a Crude Oil Pipeline" showed evidence of a mechanism for fatigue cracking. The crack growth rate as a function of stress intensity factor was estimated using the measured spacings of fatigue striations from fracture surfaces based on the assumption that the formation of fatigue striations on a cycle-by-cycle basis. However, due to the lack of full-scale fatigue crack growth data, the success was limited. This gap prompted PRCI to launch a full-scale experimental investigation of crack growth rates of cracks in dents under cyclic pressure load in the simulated groundwater NS4 environment (PRC-328-133702, MD-1Q). The objective of the study was to determine the crack growth rate as a function of stress intensity factor, the number of cycles to failure, and the failure modes of cracks in dents. The test results would be used to evaluate the validity of cycle-by-cycle based assumption for crack growth rate estimation from the measured fatigue-striation-spacing. The investigation was also aimed at establishing a framework for remaining fatigue life prediction of cracks in dents in liquid pipelines. This framework would benefit liquid pipeline Operators to manage better the integrity of dents associated with corrosion fatigue cracking in groundwater. A total of six pipe samples containing cracks in shallow dents excavated from a retired 24-inch diameter liquid transmission pipeline were available and used for the full-scale fatigue tests. The test system developed under the project consisted of four components: (1) a computer-controlled hydraulic pressure cycling system, (2) an environment chamber containing a simulated groundwater NS4 solution mounted on the pipe in around the dent region to provide a simulated field environment condition; (3) real-time crack growth monitoring systems including direct cur-rent potential drop (DCPD), Clip gage and Strain gage; (4) data acquisition system. The cyclic pressure range used in the fatigue tests was 78 to 780 psig (72%SMYS) with R=0.1, which was based on historical operational pressure data and the Rain flow analysis. A constant frequency of 0.0526 Hz was selected for the testing to ensure the frequency requirement for corrosion fatigue is met. The remaining fatigue life of cracks-in-dents and failure modes were evaluated using the full-scale fatigue test results. Further, fatigue crack growth rates were established. Finally, a framework was developed for the life prediction of cracks in shallow dents based on the findings from six full-scale fatigue cyclic tests. This framework will assist liquid pipeline operators to estimate the remaining fatigue life for cracks in shallow dents utilizing inputs from ILI and pipeline's historical operational pressure fluctuation data and to mitigate the threat of cracks in dents in a timely manner.
