O objetivo deste estudo foi avaliar a capacidade de preenchimento de canais laterais simulados pela guta-percha e Resilon, empregando diferentes técnicas termoplastificadoras de obturação: Condensação Vertical Aquecida (CVA), Compactação Termomecânica (CT) e Sistema Obtura II (SO). Foram utilizados 135 dentes humanos unirradiculados extraídos. Após preparo biomecânico, foram confeccionados seis canais laterais simulados em suas raízes, de acordo com a técnica proposta por Venturi et al. (2005). O diâmetro desses canais foi correspondente aos instrumentos # 08 e 10, sendo estabelecidos a 2, 5 e 8 mm do comprimento de trabalho (CT). Os espécimes foram divididos em 9 grupos (n=15), sendo: Grupo I (CVA + guta-percha Dentsply), Grupo II (CVA + Resilon), Grupo III (CVA + guta-percha Odous), Grupo IV (CT + guta-percha Dentsply), Grupo V (CT + Resilon), Grupo VI (CT + guta-percha Odous), Grupo VII (SO + guta-percha Obtura Flow 150), Grupo VIII (SO + Resilon) e Grupo IX (SO + guta-percha Odous Flow). Após a realização das obturações, os espécimes foram analisados pelos métodos radiográfico e diafanização, em função do percentual de preenchimento nos canais laterais, nos três terços. Os resultados foram submetidos ao teste de Kruskal-Wallis, com 5% de nível de significância, de acordo com avaliações específicas intra-grupos: CVA (Grupos I, II e III), CTH (Grupos IV, V e VI), SO (Grupos VII, VIII e IX) e Resilon (Grupos II, V e VIII). As avaliações CVA e CTH mostraram que o Resilon apresentou maior capacidade de preenchimento nas duas análises, em relação aos demais materiais, nos terços apical e médio (p<0,05). The aim of this study was to evaluate the ability of gutta-percha and Resilon to fill simulated canals using different thermoplastic obturation techniques: Warm Vertical Compaction (CVA), Thermomechanical Compaction (CTE) and Obtura II System (SO). It was used 135 extracted human teeth uniradicular. After biomechanical preparation, six simulated lateral canals were prepared in the roots, according to the technique proposed by Venturi et al. (2005). The diameter of these canals was equivalent to # 08 and 10 instruments and are set at 2, 5 and 8 mm to working length (WL). Full textForam utilizados 115 dentes pré molares inferiores de humanos, que foram divididos em 5 grupos de 16 dentes cada; sendo 5 grupos com 5 dentes cada, utilizados como controle positivo e, 5 grupos de 2 dentes como controle negativo. Em todos os dentes após a abertura coronária, foi realizado o desbridamento foraminal com uma lima tipo Kerr no 15 até 1mm além do forame apical e o preparo biomecânico a 1mm aquém comprimento real do dente. Foram utilizados para a instrumentação dos canais radiculares, os sistemas de limas rotatórias de níquel titânio ProFile 0.4/.06 e Quantec, e a técnica Clássica Modificada; sendo o batente apical preparado até a lima 45 de cada técnica de instrumentação. A cada troca de lima, foi utilizado o hipoclorito de sódio à 1% como solução irrigadora. Posteriormente os dentes foram obturados pelos sistemas de guta-percha termoplastificada Thermafil, Microseal e pela técnica da Condensação Lateral Ativa; utilizando como cimento obturador; AH Plus. Após as obturações os dentes foram impermeabilizados com esmalte de unha, exceto, o 1mm ao redor do forame apical; imersos no corante de azul de metileno à 2% sob vácuo e fraturados de maneira a serem obtidas duas hemi-partes. A penetração do corante foi medida com auxílio de um perfilômetro, e os dados, após análise estatística mostraram que, o grupo de dentes instrumentados pelo sistema ProFile 0.4/.06 e obturados com o sistema Thermafil, apresentaram menor infiltração apical, quando comparado com os demais grupos.
It was evaluated 115 mandibular premolars of humans, that were divided in 5 groups of 16 teeth each; 5 groups with 5 teeth each, used as positive control and 5 groups of 2 teeth as negative control. After coronary acess, the apical patency was accomplished with a file type Kerr no 15 up to 1mm after the apical foramen and prepared to 1mm short of working length. Instrumentation wasdone, using ProFile 0,04 and 0,06 and Quantec, stotary files and the Modified Classic technique; The apical preparation used a file 45 as memory file. After each file change, sodium hypochlorite was used at 1% as irrigation solution. The teeth were filled using systems of thermoplasticized gutta-percha Thermafil, Microseal and the technique of the Active Lateral Condensation. The sealer used was the AH Plus root canal sealer. After obturation the teeth were waterproof with fingernail enamel, except, the 1mm short of the apical foramen; immerged in the coloring of methylene blue at 2% under vacuum and fractured in a way where it was obtained two hemi-parts. The penetration of the coloring was measured with aid of a Nikon Profile Projector, and the data, after statistical analysis showed that, the group of teeth under the system ProFile 0,04 and 0,06 and filled with the system Thermafil, presented lower leakage apical, when compared with other groups.

Resumo: O objetivo deste estudo in vitro foi avaliar o selamento apical após obturação endodôntica, em dentes humanos preparados com e sem ampliação do forame apical, com o Sistema Rotatório Mtwo® (VDW, Munique, Alemanha). Vinte prémolares inferiores foram divididos em dois grupos. No Grupo I, os canais radiculares foram preparados 1mm além do forame apical e, no Grupo II, 1 mm aquém do CRC. Os dentes foram preparados para avaliação no MEV (JEOL, Modelo JSM 5600LV, Tókio, Japão) e as imagens do forame apical obtidas em aumento de 75x, em posições padronizadas, permitindo a mensuração da área foraminal antes, após o preparo e após a obturação. Por meio do processo do Sistema de Subtração de Imagens (Emago® Sistema de Diagnóstico Oral, Louwesweg, Amsterdam, Holanda), foi possível identificar as variações de forma e transporte do forame apical assim como o nível de preenchimento de sua circunferência pelo cimento obturador Pulp Canal Sealer (Sybron Endo, USA) após o preparo. Os resultados encontrados, submetidos à análise estatística, permitiram concluir que os dentes do Grupo I apresentaram maior diâmetro foraminal que os do Grupo II (Teste Anova, p<0,05) e que houve diferença estatisticamente significante quanto ao transporte do forame (Teste Exato de Fisher, p=0,0108); quanto ao selamento apical houve diferença estatisticamente significante entre os grupos I e II (Testes t de Student e Exato de Fisher, p=0,0007) sendo que, no Grupo I, ocorreram selamento em 100% dos espécimes analisados
Abstract: The aim of this in vitro study was to evaluate the apical root sealing after endodontic filling, in human teeth that were prepared with and without apical foramen enlargement by Mtwo® Rotary System (VDW, Munich, Germany). Twenty mandibular premolars were divided in two groups. Group I had root canals prepared 1mm beyond apical foramen, and in Group II, 1mm below RCL. Teeth were prepared for an electronic microscopy analysis (JEOL, Model JSM 5600LV, Tokyo, Japan). Apical foramen images were 75 times magnified, at standardized positions, allowing measurements from apical foramen area before and after root canal preparation, and after root filling. By using the Image Subtraction System (Emago®, Oral Diagnostic System, Louwesweg, Amsterdam, Netherlands) apical foramen shape modifications and transportation as well as the level of its circumferential filling by Pulp Canal Sealer cement (Sybron Endo, USA) after root canal preparation were accessed. After statistical analysis, results allowed to conclude that Group I teeth showed larger foraminal diameter than Group II (Anova Test, p<0,05) and that foramen transportation difference was statistically significant (Fisher Exact Test, p=0,0108); apical foramen sealing also differed statistically between groups I and II (Student t Test and Fisher Exact Test, p=0,0007) and that 100% of Group I specimens showed apical root sealing

Quinhentos e vinte incisivos inferiores foram avaliados por meio de radiografias digitais nos sentidos vestíbulo lingual (VL) e mesio distal (MD), sendo obtidos a partir da relação de diâmetro VL/MD: 23.3% canais achatados; 41.3% ovais; 27.3% arredondados; 4.5% redondos e 3.6% com achatamento VL (Publicação 1). Abstract: This study evaluated the morphology, preparation and filling using different techniques and materials in oval-shaped canals. Five hundred and twenty mandibular incisors were evaluated using digital radiography in the buccolingual (BL) and mesio distal (MD) directions, being obtained from the VL / MD diameter ratio: 23.3% flattened canals; 41.3% ovals; 27.3% rounded; 4.5% round e 3.6% with flattening BL (Publication 1). O objetivo desse estudo é avaliar in vitro, por meio dos testes de push-out e tração, a adesão de diferentes cimentos endodônticos à dentina e aos componentes sólidos da obturação, guta-percha e resilon. Sessenta dentes humanos uniradiculares tiveram suas coroas removidas e foram divididos em três gupos de vinte dentes. Cada grupo foi instrumentado pela técnica rotatória até uma lima tipo Profile 40.04 e alargados com uma broca de preparo de pino com 1,5 mm de diâmetro, sob irrigação com hipoclorito de sódio 2,5% (NaOCl) seguido de irrigação final com EDTA 17%; gel de clorexidina 2%(CHX) alternado com soro fisiológico e seguido por EDTA 17% ou apenas com soro fisiológico, como controle. A seguir, foram obturados com os cimentos AH 26, Endo Fill, Endo Rez e Epiphany. Após 48 horas de incubação a 37 graus Celsius e 100% de umidade, os dentes foram cortados tranversalmente em discos de 2 mm de espessura. As amostras foram submetidas à força de compressão pelo teste de push-out. Para o teste de tração, discos de guta-percha e resilon foram criados e os cimentos AH 26, Endo Fill, Endo Rez e Epiphany foram colocados em contato com o material sólido da obturação por meio de anéis metálicos contendo os cimentos em seu interor. Após incubação as amostras foram submetidas ao teste de tração. Ambos os testes foram realizados em máquina de ensaios (1mm/min) e os resultados expressos em Mpa. O teste Anova e Newman-Keuls evidenciaram que cimento Epiphany apresentou maior adesão à dentina no teste de push-out, seguido pelos cimentos Endo Rez e AH 26 que foram semelhantes entre si e, por último o cimento Endo Fill. O uso da CHX aumentou a adesão à dentina de todos os cimentos testados. No teste de tração aos componentes sólidos, o cimento AH 26 mostrou ter uma maior adesão que os demais cimentos, que foram semelhantes entre si.
The aim of this in vitro study was to evaluate the bond strength of different root canal sealers to dentin and to the filling core material, using push-out and tensile tests. Sixty extracted single-rooted human teeth were divided into three groups and evenly instrumented with the crown-down technique up to a file #55 followed by a root canal enlargement using a 1,5 mm diameter post drill. Teeth were prepared under irrigation with 2.5% NaOCl + EDTA 17 %, 2% CHX associated to a saline solution + EDTA 17% or just saline solution, as control. All teeth were completely filled with AH 26, Endo Rez, Endo Fill and Epiphany sealers. After 48 hours of storage in 100% humidity at 37°C, all samples were sectioned transversally into 2mm thickness disks and the push-out tests were performed. For the tensile tests, gutta-percha and resilon disks were created and metallic rings with the sealers were positioned over the core material disks, in the same time/temperature conditions. Both push-out and tensile tests were performed using the universal testing machine (1 mm/min) and the maximum load at failure expressed in MPa. Two-way ANOVA and Newman-Keuls tests showed that Ephipany sealant presented the highest bond strength to dentin followed by Endo REZ and AH 26 that were similar to each other and Endo Fill with the lowest values of bonding to dentin. 2% CHX irrigation increased the bonding strength of all sealers to dentin. Tensile tests to the filling core materials showed that AH 26 presented the highest values of bonding. No statistical differences were found between the other sealers.

Resumo: O objetivo deste estudo foi avaliar a microbiota presente no sistema canais radiculares antes e após o preparo biomecânico; e as condições histopatológicas da reparação apical e periapical após preparo biomecânico dos canais radiculares de dentes com reação periapical crônica. Foram utilizados 78 canais radiculares de dentes de cães, os quais, após indução de lesão periapical, foram submetidos ao preparo biomecânico utilizando as seguintes soluções irrigadoras: solução de digluconato de clorexidina a 2%, de hipoclorito de sódio a 2,5% ou soro fisiológico. Um grupo controle não recebeu preparo biomecânico. Foram realizadas culturas microbiológicas antes e após 30 dias do preparo biomecânico. Após este período os animais foram mortos para processamento e análise histopatológica. O estudo microbiológico demonstrou que houve redução dos microrganismos nos grupos que utilizaram soluções antimicrobianas (p<0,05), com melhor resultado para a solução de clorexidina. Os grupos que utilizaram solução fisiológica controle apresentaram aumento de microrganismos. No estudo histopatológico, foi observado, de um modo geral, infiltrado inflamatório periapical severo e em grande extensão, severo espessamento do ligamento periodontal e grandes áreas de reabsorção óssea e apical em todos os grupos (p>0,05). Concluiu-se que o emprego de soluções irrigadoras antimicrobianas durante o preparo biomecânico promove redução da microbiota endodôntica, sem a sua eliminação do sistema de canais radiculares em dentes de cães com lesão periapical. A solução de clorexidina a 2% proporcionou maior redução da microbiota que a solução de hipoclorito de sódio a 2,5% (p<0,05). Porém, os resultados histopatológicos demonstram que somente o preparo biomecânico não foi capaz de criar condições para reparação dos tecidos apicais e periapicais.
Abstract: The aim of this study was to evaluate the microorganisms in the root canal system before and after biomechanical preparation and the apical and periapical repair after this biomechanical preparation. The study was performed in 78 root canals of dog's teeth with induced chronic periradicular lesion. There were used the following endodontic irrigating solutions: 2% chlorhexidine digluconate solution; 2.5% sodium hypochlorite solution, saline solution and a control group without biomechanical preparation. The microbiological sampling were performed before and 30 days after the biomechanical preparation. After this period the animals were killed to histological analysis. In the microbiologic study, the results showed that the antimicrobial solutions used reduced the number of microorganisms (p<0.05). The chlorhexidine solution showed better performance. The group that used saline solution and the control group presented an increased number of microorganisms. The histological results showed a severe periapical inflammation, severe thickness of periodontal ligament, apical and osseous resorption in all groups (p>0.05). It was concluded that the antimicrobial endodontic solution reduced but not eliminated the microorganisms present in root canals of dogþs teeth with necrosis and periapical lesion. The chlorhexidine solution was more effective in reducing the number of microorganisms than sodium hypochlorite solution (p<0.05). Although, the histopathological results showed that the biomechanical preparation alone was not capable to promote an adequate condition for the apical and periapical tissues repair.

Full textGutta-percha, in combination with a variety of sealers, is the most commonly used material for root canal filling. This association presents no adhesiveness to the root structure. Resilon cones and resinous cement (Epiphany) were introduced on the market that, in association with a self-conditioning primer, would alow a solid monoblock to be obtained. The Resilon/Epiphany system forms a monoblock within the canals that bonds to the dentin walls. It has potential to strengthen the walls against fracture. The material was proposed as an alternative to gutta-percha. The purpose of this in vitro study was to evaluate the fracture resistance of root canals filled with gutta-percha/ AH Plus and root canals filled with Resilon/Epipanhy system. Sixty extracted single rooted teeth were selected. The teeth were randomly categorized into 6 groups of 10 teeth each as follows: Group I: teeth without endodontic treatment; Group II: teeth were instrumented but without endodontic obturation; Group III: teeth were instrumented and obturated with gutta-percha in conjunction with AH Plus sealer and obturated with lateral condensation technique; Group IV: teeth were instrumented and obturated with gutta-percha in conjunction with AH Plus sealer and obturated with hybrid Tagger`s techinique; Group V: teeth were instrumented and obturated with Resilon/Epiphany system and obturated with lateral condensation technique; Group VI: teeth were instrumented and obturated with Resilon/Epiphany system and obturated with hybrid Tagger`s technique. The roots were submited to the compressive test. A Kratos Universal Testing Machine was used to apply a load to each specimen at a crosshead speed of 0,5mm per minute until the root fracture. The results were submited to ANOVA test ( 5%) and Tukey test. With respect to the fracture resistance there was no statiscally significant difference between the group filled with gutta-percha and the Resilon/Epiphany system. However, the group VI showed statistically significant difference when compared with group I and group II. The presence of the endodontic material is very important to maintenance the good fracture resistance, because the group II showed the worst results. The thermoplastic technique presented better results, but within statistically significant differences.

Full textThe main aim of endodontic therapy is the cleaning and shaping of the root canal and the complete filling of the entire root canal system. Adhesion is defined as the atomic or molecular attraction between two surfaces that touch, caused by the interfacial force of attraction between the molecules or atoms of both materials. The adhesion can be chemical, mechanical or a combination of both. The perfect adhesion of the filling to the canal walls is a constant search through the improvement and introduction of new materials used in this phase of treatment, with emphasis on the sealer that promotes the union of the solid part of the filling, gutta-percha, to the walls of the root canal. This study aimed to evaluate the bond strength of three root canal sealers by the push-out test, that is quite reliable in view of the accuracy and reproducibility of results. Thirty human upper canines teeth were selected and sectioned at the cementoenamel junction to remove the crown and 2 mm from the apex. All canals were enlarged with Gates-Glidden drills #1-5 and finished the preparation with Peeso drill #6. Between every drill exchange, 1 ml of 2.5% sodium hypochlorite was used as irrigating solution. The roots were sectioned in 1 mm slices and filled according to the root canal sealer, subdividing each group according to the root third (n=25). They were then filled with the sealer as follow: Group 1, AH Plus, Group 2, MTA Fillapex and Group 3 Pulp Canal Sealer. The results showed statistically significant superiority of the AH Plus compared to the other two sealers and Pulp Canal Sealer in relation to the MTA Fillapex. Regarding the root thirds, only the Pulp Canal SealerÂs group showed statistically significant differences, with superiority of the apical third compared to cervical and middle thirds. It is concluded that AH Plus showed the best values of bond strength, regardless of the root third.
Os principais objetivos do tratamento endodÃntico sÃo a limpeza, a modelagem do canal radicular e a completa obturaÃÃo de todo o sistema de canais radiculares. Uma das propriedades exigidas do material obturador à a adesÃo, que à definida como a atraÃÃo molecular ou atÃmica entre duas superfÃcies que se tocam, causada pela forÃa de atraÃÃo interfacial existente entre as molÃculas ou Ãtomos de dois materiais. A adesÃo pode ser quÃmica, mecÃnica ou a combinaÃÃo de ambas. A perfeita adesÃo da obturaÃÃo Ãs paredes do canal à uma busca constante atravÃs do aprimoramento e lanÃamento de novos materiais utilizados nessa fase do tratamento, com Ãnfase no cimento endodÃntico, responsÃvel pela uniÃo da parte sÃlida da obturaÃÃo, a guta percha, Ãs paredes do canal radicular. Este estudo teve como objetivo avaliar a forÃa de adesÃo de trÃs cimentos endodÃnticos atravÃs do teste de push-out, que à um teste bastante confiÃvel, tendo em vista a precisÃo dos resultados e sua reprodutibilidade. Trinta caninos superiores humanos extraÃdos foram selecionados e seccionados na junÃÃo cemento-esmalte, descartando-se a coroa e removendo-se 2mm do Ãpice radicular. Em seguida, os canais radiculares foram preparados com brocas de Gates-Glidden #1 a 5 e finalizados com a broca de Peeso #6, gerando um preparo do canal radicular de forma cilÃndrica e com 1,5 mm de diÃmetro. Logo apÃs, as raÃzes foram seccionadas transversalmente em fatias de 1mm de altura, sendo logo apÃs divididas em 3 grupos de acordo com o cimento utilizado e 3 sub grupos, de acordo com o terÃo radicular (n=25). Os espÃcimes foram entÃo preenchidos com o cimento da seguinte forma: Grupo 1, cimento AH Plus, Grupo 2, cimento MTA Fillapex e Grupo 3, cimento Pulp Canal Sealer. Os resultados mostraram superioridade com diferenÃa estatisticamente significante do cimento AH Plus em relaÃÃo aos outros dois cimentos e do Pulp Canal Sealer em relaÃÃo ao MTA Fillapex. Em relaÃÃo aos terÃos radiculares, somente no grupo do Pulp Canal Sealer foram encontradas diferenÃas estatisticamente significantes, com superioridade do terÃo apical em relaÃÃo aos terÃos cervical e mÃdio. Conclui-se que o cimento AH Plus apresentou os melhores valores de forÃa de adesÃo, independente do terÃo radicular.

Full textThe aim of this study was to evaluate the chemical, biological and antimicrobial properties of the Endodontic solvents, Citrol, Eucalyptol, d-Limonene, Xylene and Endosolv E. First chemical properties analysis were evaluated by dentin microhardness, where we used bovine dentin blocks that were exposed to the solvents in 2 periods, 5 and 15 min and subjected to the microhardness test. Second test was to evaluate the ability of endodontic solvents to dissolve root-filling materials by immersion test in, 2, 5 and 15 minutes. Specimens of sealers, AH Plus, Acroseal, Sealer 26, Endofill, MTA Fillapex an RealSeal SE and guttapercha Dentsply (DP), ProTaper and Resilon points were prepared and evaluated in the three periods. Next test was conducted to evaluate the ability of solvents to desobturate root canals filled and analyse the effect of the ultrasonic passive agitation of solvents after canal unfill procedure. Sixty maxillary central incisors were prepared and obturated by same technique and randomly divided into 6 groups, one for each solvent (n=10) and a control group (saline solution). Teeth were scanned in Microcomputed Tomography (Micro-CT), unfilled with ProTaper Retratament/Protaper Universal with one of the solvents and scanned again. The residual root-fillings volumes were analysed and all teeth root canal were filled with the same solvent used in unfilling process and were passive-ultrasonically agitated (PUl) for 1 min. All teeth were scanned again in Micro-CT and data were analysed. Biological properties assessments were evaluated by cytotoxicity test with NIH-3T3 mouse cells by MTT assay. Cell cultures were subjected to diluted solvents at concentrations of 0.5 to 2.5%, and cell viability were analysed. Last evaluation in our study was the antimicrobial test. A total of 60 bovine dentin specimens infected intraorally were exposed for 5 min in direct contact to one of the solvents evaluated. After that, the dentin blocks with biofilms were stained with Live/Dead and the proportions of dead and live bacteria assessed by Confocal Laser Scanning Microscopy were analysed. The results revealed that that all solvents caused physical changes in dentin and were able to compromise the dentin microhardness, and this action was increased when the contact time was higher. In the dissolving test, Xylene and Endosolv E were more effective to dissolve AH Plus sealers and Endofill in the 15 min period, compared to other solvents. The highest dissolution capacity of Endosolv E was observed on guttapercha cones DP also in the period of 15 minutes an this dissolving capacity was more effectively then other solvents. In all other root-fillings and gutta-percha and Resilon points and in other periods (2 and 5 min), the solvents showed similar dissolution capacity. In Micro-CT evaluation all solvents were able to remove the root canal filling materials, including saline solution, but none could remove it completely. Solvents PUl, showed an increase in root-filling material left from unfilling procedure, but also provided incomplete removal. When solvents were PUl, they removed the remaining materials significantly compared to control group (saline solution). In biological test all solvents showed high cytotoxicity, however only the Xylene was able to maintain more than 50% of viable cells. According to the antimicrobial evaluation, Xylene and Eucalyptol showed best antimicrobial action while the Citrol showed the worst.

Full textPersistent root canal pathogens are one of the main causes of endodontic treatment failure. These pathogens are usually isolated in areas within the root canals that are inaccessible to mechanical instrumentation, chemical irrigants and medicaments resulting in incomplete sterilization of the root canal system. Furthermore, the development of resistant microbial species renders it difficult to disinfect the root canal system using commonly available root canal irrigants and intra-canal medicaments. Intra-canal medicaments are antimicrobial agents that are placed inside the root canal system in order to eliminate the remaining microorganisms that persist after mechanical instrumentation and irrigation. However, their antimicrobial efficacy is effective only against some of the root canal pathogens. Furthermore, the presence of tissue inhibitory factors such as dentine powder and serum albumine within the root canal system inhibits their antimicrobial activity. The use of nanoparticles as antimicrobial agents has recently attracted considerable attention especially in the medical field as a result of their unique antibacterial properties. These properties include their ability to use multiple mechanisms to eradicate microbial cells and their low potentiality to produce microbial resistance. A evolução da endodontia ocorre continuamente de modo que sistemas, técnicas e instrumentos endodônticos têm sido criados e estudados por diversos autores por meio de diferentes métodos analisando seu desempenho. O presente estudo teve como objetivo avaliar aspectos relacionados ao preparo do canal radicular assim como também analisar os métodos de estudo empregados para este fim. Foram empregadas 10 raízes mésio-vestibulares de molares superiores humanos extraídos, as quais foram incluídas em blocos de resina e seccionadas transversalmente nos terços médio e apical. Para a instrumentação foi empregado o sistema Race. A análise do preparo foi feita por meio da Plataforma Radiográfica, do Método de Bramante e do Método Histológico. Foi observado que, diante de uma padronização dos métodos de estudo empregados, é possível obter resultados coerentes entre cada um, mesmo quando se avalia aspectos diferentes do preparo do canal radicular. Pesquisas que avaliam o mesmo aspecto do preparo podem apresentar resultados diferentes e estas diferenças podem estar associadas a variáveis inerentes a cada sistema de avaliação empregado. Em relação ao método de estudo concluiu-se que todos forneceram informações importantes para a análise do preparo do canal radicular, no entanto, não devem ser considerados individualmente. De acordo com o aspecto avaliado, um método pode complementar o outro e, desta forma, atingir resultados fidedignos. Em relação aos aspectos do preparo concluiu-se que o sistema Race mostrou ausência de desvio apical, ação sobre todas as paredes dentinárias, deslocamento do preparo em direção às paredes externa da curvatura e vestibular do canal radicular, ampliação da área nos terços médio e apical, superfícies dentinárias regulares, formato irregular e pequena quantidade de resíduos na luz do canal radicular.
There is an usually endodontic evolution thus rotary systems and techniques has been created and evaluated by several authors by different methods. The purpose of this study was to investigate root canal aspects and the methods employed to evaluate the root canal preparation. Mesiobuccal canals of ten maxillary molars extracted human were included in blocks of resin and split across the middle and apical thirds. The root canals were prepared by RaCe rotary files. The prepare analysis was done through the Radiographic Platform, Bramante method and Histological method. It was observed that before the methods standardization it can get consistent results between each one even when assessing different aspects of the root canal preparation. It was concluded that studies that evaluate the same aspect of the preparation may show different results and these are associated with variables inherent in each methodology employee. All the methods supplies important information about root canal preparation. However, it is not should be separately considered. In accord to the evaluated aspect a method can supplies the other one and get reliable results. In relation to the aspects of the preparation it was concluded that the Race system showed apical deviation absence, action on all the walls, displacement to the external curvature walls and vestibular, magnifying of the area, regular surfaces, irregular format and small amount of residues inside the root canal.

Resumo: Este trabalho teve por objetivo avaliar o poder de dissolução pulpar da solução de hipoclorito de sódio (NaOCl) em função da concentração e tempo de contato por meio de uma nova metodologia. Foram utilizados 120 dentes incisivos recém extraídos de bovinos armazenados em freezer à -20oC. O dente foi seccionado de modo a manter 15 mm da raiz a partir do ápice e, então inserido em alvéolo artificial confeccionado com silicona de adição. Removeu-se o dente do alvéolo e seccionou-o em duas partes. No fragmento apical (5 mm) a polpa foi mantida. No fragmento coronário (10 mm) a polpa foi removida e o canal radicular regularizado com brocas largo #3 e #4, onde seria preenchido com a solução irrigadora testada. Os dentes foram remontados no alvéolo artificial e os fragmentos unidos por meio do cimento resinoso autoadesivo RelyX U100 (3M ESPE). Foi utilizado o NaOCl 1% (n=30), 2.5% (n= 30) e 5.25% (n=30) em tempos de 10, 30 e 60 minutos de contato com a polpa do fragmento apical, renovando-a em intervalos de 10 minutos. Solução fisiológica foi utilizada como controle (n=30). Após os tempos, os dentes foram removidos dos alvéolos e os fragmentos separados. O fragmento coronário foi descartado e o apical colocado em tubo de ensaio contendo solução de formol 10% e submetido a processamento histológico onde cortes de 7mm foram obtidos. As lâminas foram fotografadas em microscópio óptico binocular em aumentos de 40X e com auxílio do programa ImageTools 3.0 analisou-se à area da polpa dissolvida. Análise estatística foi realizada utilizando o teste de Kruskal-Wallis (p<0,05) que mostrou não haver diferenças estatísticas significativas entre as concentrações de 1% e 2.5% nos tempos de 10 e 30 minutos, com dissolução pulpar de 8,31% (±4,69%) e 11,22% (±6,75%) e de 34,33% (±25,53%) e 32,92% (±33,02%) respectivamente. Observou-se que, tanto o NaOCl 1% quanto o 2.5% no tempo de 10 minutos, não apresentou diferenças estatísticas significativas em relação ao soro fisiológico (2,53% ± 2,10%). No tempo de 60 minutos é que foi observada diferença estatística significativa entre o NaOCl 1% e 2.5%, com dissolução pulpar de 64,23% (±24,46%) e 95,24% (±5,76%) respectivamente. Para o NaOCl 5.25% a dissolução pulpar foi maior que o NaOCl 1% e 2.5% nos tempos de 10 e 30 minutos com diferenças estatísticas significativas, porém, em 60 minutos não apresentou diferenças estatísticas signifativas em relação ao NaOCl 2.5%. Quando o NaOCl 5.25% permaneceu por 60 minutos de contato houve 100% de dissolução pulpar. Concluiu-se que a metodologia utilizada mostrou-se adequada em analisar a dissolução tecidual, e que esta foi maior à medida em que se aumentou a concentração e o tempo de contato do NaOCl
Abstract: This work aimed to assess the pulp dissolution capacity of sodium hypochlorite solution (NaOCl) according to concentration and contact time using a new methodology. One hundred and twenty recently extracted mandibular bovine incisors, stored in a freezer at -20oC, were used. Teeth were cut at 15 mm from the root apex and inserted in an artificial socket made of addition silicone impression material. The teeth were then removed from the socket and sectioned in two parts. Pulp was kept intact in the 5 mm apical fragment while in the coronal fragment (10 mm) the pulp was removed and the canal walls enlarged using # 3 and # 4 largo drills, so that it could be filled with the tested solutions. Teeth were rebuilt in the artificial socket and fragments bonded using the self-adhesive resin cement RelyX U100 (3M ESPE). NaOCI solution at 1% (n=30), 2.5% (n=30) and 5.25% (n=30) were tested at 10, 30, and 60 minutes contact with the pulp of the apical fragment, renewing the solution every 10 minutes. Saline solution irrigation was used as control. The teeth were then removed from the socket and the apical and coronal fragments separated. The coronal fragment was discarded and the apical fragment put into a test-tube containing formaldehyde solution 10% followed by histological processing. Sections of 7?m were obtained. Slides were photographed under an optic binocular 40X microscope and, the area dissolved by the action of irrigation solutions was analyzed using ImageTools 3.0 software. Statistical analysis was performed employing Kruskal-Wallis (p<0.05). Results showed that there were no significant statistical difference between 1% and 2.5% NaOCl in 10 and 30 minutes of contact, with pulp dissolution of 8,31% (±4,69%) and 11,22% (±6,75%) and 34,33% (±25,53%) and 32,92% (±33,02%), respectively. It was also observed that there were no significant statistical differences between both 1% and 2.5% NaOCl and saline solution (2,53% ± 2,10%) at 10 minutes of contact. Only at 60 minutes was there significant statistical difference between NaOCl 1% and 2.5%, with the dissolution of pulp at 64,23% (±24,46%) and at 95,24% (±5,76%), respectively. Pulp dissolution was greater for 5.25% than 1% and 2.5% NaOCI in 10 and 30 minutes contact with significant statistical difference, but at 60 minutes there were no difference for 2.5% NaOCl. Only when the NaOCl 5.25% remained in contact for 60 minutes there was 100% pulp dissolution. It is possible to conclude that the methodology employed was appropriate to examine bovine pulp tissue dissolution using NaOCl. Tissue dissolution increased with concentration and contact time

Full textThe aim of this clinical study was to compare the effects of passive ultrasonic irrigation (PUI) and intracanal medication with calcium hydroxide in bacteria and endotoxins from root canal. Fifty teeth with apical periodontitis were randomly divided into two groups according to the irrigation protocol: PUI (n = 25) and conventional irrigation (CI) (n = 25). The root canal preparation of all the teeth was carried out with reciprocating files and 2.5% NaOCl during preparation; and 17% EDTA for smear layer removal. The root canals were medicated with calcium hydroxide for 14 days. Microbiological sampling were performed before (S1) and after the preparation (S2) after irrigation protocols (S3), and after intracanal medication (S4 and S5). During the processing of the samples, five cases were lost for several factors. The samples were analyzed by real time PCR, for the detection and quantification of bacteria, and the turbidimetric LAL assay, for the detection and analysis of the endotoxin levels. Bacteria and endotoxins were observed in 100% of the initial samples. In both groups, there was a significant decrease in the concentration of endotoxins between one step and the subsequent step of treatment (p < 0.05). The same was observed for the number of bacteria, with the exception of the reinstrumentation after the removal of the medication (S5). The intergroup analysis showed no significant differences between groups in endotoxin reduction ( p > 0.05 ). PUI was able to reduce the number of bacteria significantly better than CI (p < 0.05). No significant statistical difference was observed between groups regarding the occurrence of cases wielding positive results for bacteria or endotoxin. It was concluded that PUI was more effective than CI in reducing the number of bacteria but not the amount of endotoxin in the root canal. Furthermore, each step of the endodontic therapy was effective in reducing both the number of bacteria as the amount of endotoxin.
