Abstract:
AT THE END OF HIS REVIEW OF LABOUR MARKET RIGIDITIES in advanced capitalist societies, Ronald Dore presents us with the classical and highly uncomfortable dilemma between equity and efficiency. There are two kinds of rigidities, he suggests: those that improve economic performance while giving rise to social inequality and societal dualism, and those that promote equity while detracting from efficiency. Unlike neo-classical economics, Dore recognises that to be efficient, advanced production systems require institutions above and beyond the market — such as long-term employment, internal labour markets, and ‘sticky’ mechanisms of wage determination. But these institutions, he insists, are not, or no longer, defensible in terms of the Marshallian tradition of industrial and social citizenship. Social justice and equality, according to Dore, are vested today in a second type of ‘ridigities’ — the redistributive apparatus of the welfare state — which, unlike the first, extracts an ‘efficiency price’ from the economy.

Abstract:
This introductory essay discusses Ronald Dore's academic career and contributions. As an "accidental Japanologist," Dore made enormous contributions to the understanding of Japanese society. As a sociologist, he rigorously employed a sociological approach to the study of comparative political economy with profound moral-philosophical reflections. By doing so, he helped to correct our misunderstanding of education, development, and internationalization—topics of extensive discussion in the following essays. Among his numerous contributions to Pacific Affairs between 1952 and 2008, Dore wrote three essays covering these topics during the 1970s. Amazingly, his ideas as presented in these essays still resonate today.

Abstract:
Ronald Dore's work on education in Japan centred on themes of selection and equality. In his work on Tokugawa education, Dore presaged some of the emphasis he gave in his later work on quality and social and moral content in modern education. The argument of The Diploma Disease concerned the "late development effect" as a tool in understanding the emphasis on qualification and selection that led to Japan's postwar examination hypertrophy, and in understanding the distortions and inequities that ensued. "Late ascription"—tracking and determining one's life chances with a single examination—was one such distortion, narrowing the gate to educational and occupational success, belying the notion that Japan demonstrates a pure "meritocracy."

Abstract:
The pattern of development that emerged in the early stage of industrialization in South Korea, according to the British sociologist Ronald Dore, is closer to the classical (nineteenth-century) small-scale, individualistic entrepreneurial type than the large-scale bureaucratic type that characterizes development in many other countries embarking upon modernization programs in recent years (Dore 1980:292). This arguably stems from the fact that economic growth accompanied by development of a market (in the broader sense of that term) that created opportunities for individual choices and initiatives.

Abstract:
Ronald Dore's 1977 article in Pacific Affairs, "South Korean Development in Wider Perspective," is a rare example of the scholar known for his writings on Japan applying his analytical lens on South Korea. What were some of this article's most notable areas of foresight and elision related to development studies? This essay answers this question by interpreting connections to publications before and after 1977 to analyze areas of insight under the rubric of "discernment" and overlooked subjects under "death." On one hand, Dore's essay was ahead of the curve in its deft foreshadowing of post-developmentalist, varieties of capitalism, and developmental state approaches to economic development. On the other, Dore sidestepped the effects of death on economic development in three forms: literal— effects of changing mortality rates on investments in education and human capital; industries related to death—wars, munitions production and arms expenditures; and the aftereffects of the death of a scholar—the revisiting and renewal of debates that can sometimes emerge as a result.

Abstract:
Ronald Dore's 1979 essay about Japan's "internationalization" tackled one of the defining themes of Japanese politics, society, and culture over the past decades. In his characteristically witty voice, Dore assessed the myriad ways in which a Japan that was well attuned to global cultures was also capable of reaffirming supposed chasms between Japanese society and the world outside, particularly in political and economic matters. In this article, I place Dore's compelling essay in the contexts both of his own changing views on Japan over the course of his distinguished and prolific career, as well as in the currents of a Japan that has been transformed dramatically over the past three decades by transnational flows that fall outside the prevailing use of the word kokusaika (internationalization). Dore's contributions to the field displayed not only his keen engagement with Japanese intellectual and social debates, but also moral judgments regarding the values encoded, reproduced, and sometimes betrayed by institutional environments. By extending the logics of Dore's work, this article suggests that we might think of internationalization as something not only challenging these environments, but also transformed and embedded within them.

In Performing Metaphors: The Singularity of Literary Figuration Derek Attridge redefi nes the work of art in terms of alterity, invention and singularity. For a given work of art to occur, it must be performed, that is, justice must be done to it as a literary event and as the eventness of that event. One of the most vital features of the (process of) performance of literature is metaphor, argues Attridge, adducing various works, from philosophy (David Hume) to poetry (Robert Graves) to everyday spoken speech (quoted by a cognitive linguist, Ronald Carter) to prove his point.

Modern law and economics received much of its impetus from Ronald Coase's analysis in ‘The Problem of Social Cost,’ and a goodly amount of that comes from the Coase theorem, which states that, absent transaction costs, externalities will be efficiently resolved through bargaining. The fact that the analysis that came to be codified in the Coase theorem was (intentionally) an exercise in pure fiction on Coase's part did not deter the erection of a substantial edifice of positive and normative analysis on this foundation, nor, for that matter, has subsequent elaboration of Coase's intent done anything to abate the interest in the theorem and its implications.

AbstractDavid Skarbek argues that qualitative research methods can analyze institutions by exploiting complex evidence not accessible through quantitative methods. He suggests that well-done case studies and process tracing can meet some of the same tests of inference as statistical methods. Although Skarbek's critique and proposals mirror those of many other authors, including Ronald Coase, he nonetheless makes an important contribution. The brief, cogent, and instructive way he presents his advice and his defense of qualitative methods as a complement to mainstream methods rather than a confrontation, may be more persuasive than more confrontational arguments. As ‘datafication’ is quickly turning qualitative observations into quantitative data analyzed through machine learning, Skarbek's excellent advice on how to understand what is happening under different institutional settings could not be timelier.

Georgian and Caucasian historical studies have recently received a stimulus through the publication of Professor Ronald Grigor Suny's important work, The making of the Georgian nation. Professor Suny draws attention to the seminal importance of the first half-century of Russian rule in Georgia, following the 1801 annexation. The old picturesque norms of Georgian social life were increasingly under pressure, as the Russian autocratic military bureaucracy sought to absorb the intransigent Caucasian peoples into their own larger administrative entity. (Dr. Stephen Jones and Professor Laurens Hamilton Rhinelander have done useful work on this phase of Georgian history).2 As we know from recent events in Armenia, Azerbaijan and Georgia, these efforts at assimilation met with mixed results, and serious nationality problems continue today

Abstract:
Abstract. During the early months of the 2015/2016 El Niño event, scientists led by the Earth System Research Laboratory's Physical Sciences Division conducted the National Oceanic and Atmospheric Administration's (NOAA's) El Niño Rapid Response (ENRR) field campaign. One component of ENRR involved in situ observations collected over the near-equatorial eastern–central Pacific Ocean. From 25 January to 28 March 2016, standard surface meteorology observations, including rainfall, were collected at Kiritimati Island (2.0° N, 157.4° E) in support of twice-daily radiosonde launches. From 16 February to 16 March 2016, continuous measurements of surface meteorology, sea surface temperature, and downwelling shortwave radiation were made by NOAA Ship Ronald H. Brown. These were largely done in support of the four to eight radiosondes launched each day as the ship travelled from Hawaii to TAO buoy locations along longitudes 140 and 125° W and then back to port in San Diego, California. The rapid nature of these remote field deployments led to some specific challenges in addition to those common to many surface data collection efforts. This paper documents the two deployments as well as the steps taken to evaluate and process the data. The results are two multi-week surface meteorology data products and one accompanying set of surface fluxes, all collected in the core of the eastern–central Pacific's extremely warm waters. These data sets, plus metadata, are archived at the NOAA's National Centers for Environmental Information (NCEI) and are free for public access: surface meteorology from Kiritimati Island (https://doi.org/10.7289/V51Z42H4); surface meteorology and some surface fluxes from NOAA Ship Ronald H. Brown (https://doi.org/10.7289/V5SF2T80; https://doi.org/10.7289/V58050VP).

Abstract:
ABSTRACTSixty years after William Temple's death, little in the way of constructive theology has been done with the body of writings he left. Part of this is due to the way in which his legacy has been (mis)appropriated by some of the scholars and church leaders who are seen as his heirs and admirers. An over-emphasis on the ‘middle axioms’ approach exemplified in Christianity and Social Order, and later promoted heavily by Ronald Preston, explains much of this lack. Although the ‘middle axioms’ approach is still applicable, the principles set out in Temple's most famous work need to be re-examined and perhaps expanded in the light of a post-Christian plural society. The purpose of this essay is to examine a broader range of Temple's work than is commonly done. By doing so, I will propose that the virtues of intellectual excellence, graciousness, and the welfare of the wider (non-church) society are guiding principles for ecclesial being, speech and action that are fully present in Temple's writings.

Abstract:
Objective: This study aimed to determine the relation of severity of lumbar spinal stenosis and pain improvement after caudal epidural injection. Material and Methods: 70 patients of neurogenic claudication were included from July 2019 to June 2020. MRI lumbosacral spine was done of all patients to categorize the degree of spinal stenosis on T2-W axial. All patients were given caudal epidural steroid injection as a day case procedure. Follow-up was done at 3 & 8 weeks and pain improvement was assessed by using Ronald satisfaction score. Results: Mean age was 37.9 ± 7.8 years. 48.6% patients were male and 51.6% were female. The improvement was observed in 28 patients (40%) at 3 weeks and in 46 patients (65.7%) at 8 weeks. There was no statistically significant relationship between lumbar spinal stenosis and pain improvements at 3 weeks (p value 0.30) and 8 weeks (p value 0.32). Conclusion: The grade of lumbar spinal stenosis has no significant effect on pain improvement after caudal epidural steroid injection. Keywords: LSS (Lumbar spinal stenosis), caudal epidural steroid injection (ESI), neurogenic claudication, MRI (Magnetic resonance imaging), T2W (T2 weighted) images.

Abstract:
Old Norse sagas are simply in today, probably because they contain so much valuable and intriguing information about the world of the Icelanders and Vikings, and because they reflect deeply on mythological concepts and historical events, strongly colored by heroic deeds. This means that publishers of textbooks continue to demonstrate their willingness to produce English translations, such as the present one of The Saga of the Volsungs, which Jackson Crawford accompanies with a translation of The Saga of Ragnar Lothbrok. Henry Halliday Sparling (1890), Margaret Schlauch (1930), Ronald G. Finch (1965), Jesse L. Byock (1990 and 1999), and Kaaren Grimstad (2000), among others, had already offered English translations of the former, but none of them are listed by Crawford in his introduction. Translations into German, French, Italian, or other languages are also not considered, although there is a long tradition of comparable work that has been done many decades ago until today.

"Californians, this is the time for us to do our utmost for the University because it has done its utmost for us,” said Chief Justice Earl Warren at the April 1967 convocation at Berkeley. And what a time it was—on the heels of the Free Speech Movement in 1964, the Vietnam Day marches in 1965, an escalation of anti-war protests in 1966, and, in January of 1967, the dramatic firing of UC President Clark Kerr by Governor Ronald Regan at a meeting of the Board Regents. The following year the University of California would celebrate its hundredth year, and to celebrate this, the UC hired photographer Ansel Adams to take thousands of images of the rapidly expanding UC system. Adams was charged to take photographs of the future. What might these images from futures past tell us about the future for both this university and the state to which it belongs?

Abstract:
AbstractThe size and cost of US public higher education, funded largely by government, grew continuously for nearly twenty-five years after World War II. In the late 1960s, as the nation's economic growth slowed, the question of who should pay for higher education came under fresh political scrutiny. Decades-old no-tuition policies at the University of California and The City University of New York (CUNY) became targets of neoconservative critiques of the proper role of government support for public services. In California, this was done as Governor Ronald Reagan promoted a partisan austerity to win favor with business and other conservative elites. He justified cuts to higher education financing as a rebuke of protesting students and inept administrators and, later, as financially necessary given voters’ reluctance to pay more taxes. In contrast, federal and New York State politicians forced austerity on city leaders to satisfy bond holders during New York City's severe fiscal crisis. Reformers argued that CUNY's no-tuition policy was emblematic of the city's overindulgence of its residents. No-tuition policies became impossible to defend in the context of the stalled economy and growing conservative movement, whose members embraced government austerity.

Abstract:
The advancement of civil engineering plays a major role in determining the nation's development. Generally, geotechnical engineering has a great influence as there is a building on the ground. The facts show that there are still many geotechnical problems that arise during the development process. One of the supporting factors in dealing with these problems is the development of research in the geotechnical sector in various fields. This research aims to determine the development of research related to Geotechnical Engineering through a bibliometric distribution map using the VOSviewer application. Data collection is gathered from the dimension site. The keyword used in this study is "geotechnical engineering" with the publication year of the 2017-2020 articles. This search was conducted on 23 October 2020 which resulted in 79,185 articles. The results show that the most productive writer in geotechnical research is Ronald Kerry Rowe. Moreover, China is the largest country in the field of geotechnical research so that many other countries have established cooperative relations with China. The journal that publishes the most articles on geotechnical engineering is the International Journal of Rock Mechanics and Mining Science. As for keyword analysis, it produces bibliometric maps including network visualizations, overlay visualizations, and density visualizations. Four keywords often appear, namely "Displacement", "Strength", "Stress", and "Earthquake". Through the bibliometric approach, researchers can identify keywords on each topic or research that has been done before, it is, therefore, valuable in determining the novelty of the research that will be conducted in the future.

Abstract:
AbstractIn her early work, the feminist legal scholar Erika Rackley uses the image of Ronald Dworkin's superjudge Hercules to shed light on the experience of the woman judge and on law and adjudication in the liberal legal order. She sees Hercules as representing the judge ‘who inhabits our legal imagination’, and as conjuring up problematic notions of unimpeachable wisdom, detached neutrality and super-humanism. This paper assesses Rackley's argument in light of the feminist judgments scholarship that has emerged in the meantime. It contests Rackley's claim that Hercules, or what he represents, is a patriarchal influence in the real world of law, and argues that he might instead be understood to accommodate, or even to encourage, principled evolutions in law along the lines of those suggested by the feminist judgments literature. This assessment is done mainly through the lens of Stokes v CBS Clonmel, a judgment of the Irish Supreme Court concerning indirect discrimination that was later the subject of a feminist judgment in the Northern/Irish Feminst Judgments volume. The broader aim of this assessment is to interrogate the insights and implications of feminist judgments scholarship. The paper is in four parts. Part 1 places feminist approaches to adjudication in broader theoretical context. Part 2 considers Dworkin's theory of adjudication and Rackley's critique. Part 3 sets out the approach taken by both the real-world and feminist judges in the Stokes case. Part 4 critiques Rackley's take on Hercules in light of the approach adopted in those judgments and draws on preceding analysis to interrogate the insights and implications of feminist judgments scholarship.

Abstract:
Scattered accounts of the treatment of pain by aboriginal Canadians are found in the journals of the early explorers and missionaries. French and English settlers brought with them the remedies of their home countries. The growth of medicine through the 18th and 19th centuries, particularly in Europe, was mirrored in the practice and treatment methods of Canadians and Americans. In the 19th century, while Americans learned about causalgia and the pain of wounds, Canadian insurrections were much less devastating than the United States Civil War. By the end of that century, a Canadian professor working in the United States, Sir William Osler, was responsible for a standard textbook of medicine with a variety of treatments for painful illnesses. Yet pain did not figure in the index of that book. The modern period in pain research and management can probably be dated to the 20 years before the founding of the International Association for the Study of Pain. Pride of place belongs toThe management of painby John Bonica, published in Philadelphia in 1953 and based upon his work in Tacoma and Seattle. Ideas about pain were evolving in Canada in the 1950s with Donald Hebb, Professor of Psychology at McGill University in Montreal, corresponding with the leading American neurophysiologist, George H Bishop. Hebb's pupil Ronald Melzack engaged in studies of early experiences in relation to pain and, joining with Patrick Wall at Massachusetts Institute of Technology, published the 1965 paper in Science that revolutionized thinking. Partly because of this early start with prominent figures and partly because of its social system in the organization of medicine, Canada became a centre for a number of aspects of pain research and management, ranging from pain clinics in Halifax, Kingston and Saskatoon - which were among the earliest to advance treatment of pain - to studying the effects of implanted electrodes for neurosurgery. Work in Toronto by Moldofsky and Smythe was probably responsible for turning ideas about fibromyalgia from the quaint concept of 'psychogenic rheumatism' into the more fruitful avenue of empirical exploration of brain function, muscle tender points and clinical definition of disease. Tasker and others in Toronto made important advances in the neurophysiology of nociception by the thalamus and cingulate regions. Their work continues while a variety of basic and clinical studies are advancing knowledge of fundamental mechanisms, including work by Henry and by Sawynok on purines; by Salter and by Coderre on spinal cord mechanisms and plasticity; by Katz on postoperative pain; by several workers on children's pain; and by Bushnell and others in Montreal on cerebral imaging. Such contributions reflect work done in a country that would not want to claim that its efforts are unique, but would hope to be seen as maintaining some of the best standards in the developed world.

Abstract:
Abstract Background Cabotegravir (CAB) and rilpivirine (RPV) are under development as a novel long-acting (LA) regimen for maintenance of HIV virologic suppression. Pooled data from pivotal phase III trials demonstrated noninferiority of CAB + RPV LA given as gluteal intramuscular injections vs current antiretroviral regimen (CAR) on the primary endpoint of HIV-1 ribonucleic acid (RNA) ≥50c/mL at Week 48, with high levels of adherence. Long-term adherence to dosing visits and outcomes after use of oral CAB+RPV to cover planned missed injections in FLAIR through Week 96 and in LATTE-2 through Week 256 is reported here. Methods Virologically suppressed participants (HIV-1 RNA < 50c/mL) were randomized to switch to CAB+RPV LA or to continue CAR. On-time injections occurred every 4 weeks or every 8 weeks (LATTE-2 only) within a ±7-day dosing window of the projected dosing date. Adherence to LA therapy was calculated as the number of on-time injection visits divided by the number of expected dosing visits through the period of follow up. Injection visits outside the prespecified window and missed injection visits with or without use of oral dosing were characterized. Results Of 6005 expected injection visits through Week 96 in FLAIR, 97% of injections were given within the allowed ±7-day dosing window, with 43% on the projected dosing date. 45 (< 1%) injection visits were early and 107 (2%) were late. Adherence to 9803 expected injection visits in LATTE-2, through Week 256, was similarly high, with 96% of injections given within the allowed ±7-day dosing window and 39% on the projected dosing date. For 31 missed injection visits in 18 participants across both trials, 30 were covered with oral CAB+RPV, with all participants maintaining HIV-1 RNA < 50c/mL through the last study visit. In those participants who used oral CAB + RPV for planned treatment interruptions, 3 had repeat use on ≥2 separate occasions. Conclusion Participants maintained high levels of long-term adherence to CAB+RPV LA, through 2-5 years of follow up, with 97% of injections given within the ±7-day dosing window in the FLAIR and LATTE-2 clinical trials. Oral CAB+RPV to cover planned missed visits provides an effective strategy to maintain virologic suppression during short periods of LA treatment interruption. Abstract:
BackgroundUltrasound (US) has become an established method in the evaluation of joints and is often used in clinical practice to guide management decisions in arthralgia-patients. To date, studies on the prognostic value of MSUS in arthralgia focused on the positive predictive value of subclinical inflammation. However, also the absence of imaging-detected subclinical synovitis is now increasingly used in daily practice to exclude arthralgia-patients from further follow-up. Though, evidence on the value of a negative US in ruling out future IA development in arthralgia-patients (the negative predictive value) is mostly absent. According to the rules of Bayes, predictive values are highly dependent on the prior-risk of developing the disease. The NPV therefore, is strongly related to the prior-risk of not getting IA, which is quite considerable in arthralgia-patients.ObjectivesTo investigate the negative predictive value (NPV) of musculoskeletal ultrasound (MSUS) in arthralgia patients at risk for developing inflammatory arthritis (IA).MethodsAn MSUS examination of hands and feet was performed in arthralgia-patients at risk for IA in four independent cohorts. Patients were followed for one-year on the development of IA. Subclinical synovitis was defined as greyscale≥2 and/or power Doppler≥1. NPVs were determined and compared with the prior risks of not developing IA. Outcomes were pooled using meta-analyses and meta-regression analyses. In sensitivity analyses, MSUS-imaging of tender joints only (rather than the full US-protocol) was analyzed and ACPA-stratification applied, the latter being in line with the use of US in daily care.ResultsAfter one-year 78%, 82%, 77% and 72% of patients in the four cohorts did not develop IA. The NPV of a negative US was 86%, 85%, 82% and 90%, respectively. The meta-analysis showed a pooled non-IA prevalence of 79%(95%CI: 75%-83%) and a pooled NPV of 86%(95%CI:81-89%) (Figure 1). Imaging tender joints only (as generally done in clinical practice) and ACPA-stratification showed similar results.Figure 1.Full US protocol; Prior risks of not developing IA (A) and negative predictive values of MSUS (B) in the four cohorts. For comparison, the pooled prior risk and confidence interval from A are depicted in the red column in B.ConclusionA negative US result in arthralgia has a high NPV for not developing IA, which is mainly due to the high a-priori risk of not developing IA. The added value of a negative US (<10% increase) was limited.ReferencesN.A.Disclosure of InterestsCleo Rogier: None declared, Giulia Frazzei: None declared, Marion Kortekaas: None declared, Marloes Verstappen: None declared, Sarah Ohrndorf: None declared, Elise van Mulligen: None declared, Ronald van Vollenhoven Speakers bureau: Speaker, for which institutional and/or personal honoraria were received: AbbVie, Galapagos, GSK, Janssen, Pfizer, UCB, Consultant of: Consultancy, for which institutional and/or personal honoraria were received: AbbVie, AstraZeneca, Biogen, Biotest, BMS, Galapagos, Gilead, Janssen, Pfizer, Sanofi, Servier, UCB, Vielabio, Grant/research support from:Research Support (institutional grants): BMS, GSK, Lilly, UCBSupport for Educational programs (institutional grants): Pfizer, Roche, Dirkjan van Schaardenburg: None declared, Pascal de Jong: None declared, Annette van der Helm-van Mil: None declared

Abstract:
Abstract Background: Liver metastases develop in 20-50% of colorectal cancer (CRC) patients, being typically resistant to immune checkpoint inhibitors (ICIs) and having a poor prognosis. Our preliminary data showed synergy between IFNα and TLR3 ligands in selectively enhancing the intratumoral production of CD8+ T cell (CTL) attracting chemokines (but not Treg attractants) in ex vivo-treated CRC explants and preferential impact of this combination on liver-metastatic CRC tumor tissues (rather than surrounding liver tissues). Based on these data and previous reports showing the prognostic value of intratumoral CTLs in the CRC outcomes, we hypothesized that systemic infusion of the combination of IFNα2b with rintatolimod (selective TLR3 ligand for i.v. use) can reprogram local balance between the CTL- and regulatory T cell (Treg)-attracting chemokines and the resulting patterns of immune infiltrate in liver lesions. Methods: Recurrent/metastatic CRC patients with unresectable liver metastases amenable to biopsy were eligible. Patients had prior treatment (Rx) with fluoropyrimidine, irinotecan, oxaliplatin, and an anti-EGFR targeted therapy (if RAS wt), or contra-indication to such. Patients received IFNα2b IV (20 million units/m2 IV daily) and rintatolimod (200 mg IV daily) plus oral celecoxib (200 mg twice daily) on days 1, 2, 3, 8, 9, 10, 15, 16, and 17. Response assessment was done via liver biopsies (pre-Rx and on day 24 ± 4 days) and CT imaging (RECIST v1.1) on D46. The primary endpoint was the change in CD8+ T-cells before Rx, with that seen post-Rx (measured by quantitative RT-PCR as a ratio of CD8α to a housekeeping gene, HPRT). With a sample size of N=12 evaluable pts, the study design had a 90% power to detect a 0.77 standard deviation increase (pre- to post Rx) at a significance level of 0.1 (ClinicalTrials.gov Identifier: NCT03403634). Results: Nineteen patients with microsatellite stable (MSS) CRC were enrolled between Apr 2018 and Oct 2020, 15 were treated and 12 were evaluable for the primary endpoint. Most were male (75%, N=9) and Caucasian (92%, N=11). The median age at diagnosis was 65 years. Most had previously received three or more prior lines of therapy (58%, N=7). The study's primary endpoint was met, evidenced by increased CD8a expression post-treatment (p=0.046). An increase in ratios of CD8a/CD4 (p=0.03), CD8a/FOXP3 (p<0.01) and GZMB/FOXP3 (p<0.01) was observed. The expression of CTL-attracting chemokines CCL5 (p=0.08), CXCL9 (p=0.05), and CXCL10 (p=0.06) were increased, while expression of the Treg/MDSC attractant CXCL12 (p=0.07) was decreased post-Rx. Median OS was 10.5 (90% CI 2.2-15.2) months, and the median PFS was 1.5 (90% CI 1.4, 1.8) months. No objective response was seen. The treatment was well tolerated. Of all enrolled patients (N=19), adverse events were noted in 74% of patients, with the most common being fatigue (58%). Grade 3 or higher adverse events were rare (5%). Age, gender, line of treatment, RAS mutational status, and tumor location did not significantly impact CD8a change or PFS. Conclusion: Our study demonstrated that a combinatorial CKM regimen could re-program the immunosuppressive tumor microenvironment in MSS CRC patients with liver metastasis, raising the possibility that the CKM regimen can be used to enhance the effectiveness of ICIs in this group of patients. Abstract:
BackgroundPatients with rheumatoid arthritis (RA) are at an increased risk of venous thromboembolism. Thus far, there have not been any comparative studies investigating the effects of initial antirheumatic treatments in (very) early RA patients.ObjectivesTo assess the effects of different initial treatments on hemostatic parameters in patients with early RA.MethodsNORD-STAR is an international, multicentre, open-label, assessor-blinded, phase 4 study where patients with newly diagnosed RA started methotrexate (MTX) and were randomised 1:1:1:1 to a) conventional treatment (either prednisolone tapered to 5mg/day, or sulfasalazine combined with hydroxychloroquine and intra-articular corticosteroids), b) certolizumab pegol, c) abatacept, d) tocilizumab1. This study is a spin-off from the main NORD-STAR study extensively investigating hemostatic system in 24 per protocol consecutive Dutch participants at baseline, 12 weeks and 24 weeks after the start of the treatment. Statistical analysis was done using paired samples t-test in SPSS version 28.ResultsThe mean age of investigated patients was 51.8 (± 12.7) years and 58.3% were female. At baseline patients had an average DAS28 score of 4.6 (± 0.9) and had elevated levels of investigated coagulation biomarkers: Factor 1 + 2, fibrinogen, D-dimer and parameters of the two global hemostatic assays, i.e. endogenous thrombin potential (ETP) and overall hemostasis potential (OHP). These biomarkers decreased significantly at 12 and 24 weeks in patients in all groups (Table 1). Overall fibrinolytic potential (OFP) was decreased and clot lysis time (CLT) was prolonged at baseline, demonstrating impaired fibrinolytic activity in early RA. The reduction of coagulation parameters was significantly higher in biological treatment arms in comparison to the standard MTX treatment arm. In addition, tocilizumab was more effective compared to certolizumab and abatacept, (Figure 1), which was expected considering the direct inhibitory effect of this drug on the IL-6 synthesis and consequently the coagulation activation as well. After 24 weeks of treatment with methotrexate and tocilizumab, the average fibrinogen of patients was reduced by 63% vs 31% and 36% in the certolizumab and abatacept groups, respectively. The changes in DAS-28 and the changes in fibrinogen had a correlation of 0.385 which did not reach statistical significance.Table 1.Measurements are marked with * if p<0.05, ** if p<0.01 and *** if p<0.001BaselineW12W24Factor 1 + 2 (pmol/L)270.25 (149.4)190.36 (108.6)**179.52 (85.3)***Fibrinogen (g/L)4.64 (1.5)3.61 (1.6)**2.63 (1.2)***D-dimer (mg/L)2.17 (3.0)0.33 (0.23)**0.29 (0.2)**OHP (Abs-sum)157.38 (64.9)120.62 (68.7)*100.49 (53.8)***OCP (Abs-sum)369.52 (58.8)305.04 (101.7)*275.91 (83.1)***OFP (%)57.97 (13.1)63.20 (12.7)*65.25 (11.4)***Lag time (s)304.5 (71.1)306.8 (71.8)312.7 (65.4)Slope0.07 (0.02)0.066 (0.03)0.094 (0.12)Max Abs1.17 (0.3)1.00 (0.4)*0.91 (0.3)**CLT (s)1405 (356)1317 (377)1231 (320)**ETP (nM*min)1480 (471)1395 (395)*1337 (429)*Peak (nM)231 (78)223 (68)223 (74)Lagtime (min)4.06 (2.1)3.28 (1.2)**2.87 (1.0)***ttPeak (min)7.40 (2.2)6.61 (1.5)*6.13 (1.4)**Figure 1.ConclusionOur results indicate an enhanced coagulation and fibrinolytic impairment in newly diagnosed RA patients. Effective antirheumatic treatments reduce this hemostatic imbalance, with significantly more pronounced effects of biologic drugs compared to conventional (MTX+glucocorticoids) treatment.References[1]Hetland M et al.
