RODER, EMANUELA. "The complexity of therapeutic action in DBT: preliminary studies on process and outcome." Doctoral thesis, Università degli Studi di Milano-Bicocca, 2019. http://hdl.handle.net/10281/241311.
Full textDialectical Behavior Therapy (DBT; Linehan, 1993, 2014) is a structured, complex and comprehensive cognitive-behavioral treatment program for patients with Borderline Personality Disorder (BPD) and severe dysfunctional behaviors (repeated suicidal attempts, self-harm behaviors, relational instability, other impulsive behaviors). Up to now, DBT proved its effectiveness in several studies: suicide attempts and dysfunctional behaviors decreased, while emotional regulation and general functioning improved. The present research aimed at assessing the effectiveness of DBT and its mechanisms of action, evaluating both outcome and process dimensions. The first part of the thesis is dedicated to the presentation of the DBT model, examining its theoretical foundations, the agreements underlying the treatment, the modalities of therapeutic program, and the strategies of intervention. The second part of the thesis is an evaluation of the effectiveness of DBT, examining the course over time of the target variables. The study is longitudinal, single-blind, with a two-arm parallel design, conducted following the international guidelines for the outcome studies on intention-to-treat samples. DBT was compared with another treatment program comparable by patient type, objectives, and complexity of interventions. The sample was comprised by 95 outpatients, assigned to groups with the minimisation procedure and assessed every three months. Since the individual variability was expected to be consistent, Hierarchical Linear Models with random effects were used. Results showed that suicidality, self-harm, emotional and behavioral dysregulation decreased in both groups after one year; unconditional growth models indicated that subjects differed in the elevation and in the rate of change. Moreover, results on the completers’ subsample suggested that the group setting and the intensity of treatment could represent specific therapeutic mechanisms. The third part of the thesis is composed by process studies with a single-case design, in the strand of the process-outcome research: the empirical evaluation of two therapeutic couples, one with a favorable outcome and one with a partial outcome, was conducted. The patients were two young women with a diagnosis of Borderline Personality Disorder, different for personality profile and dysfunctional behaviors at the beginning of treatment; they followed a DBT standard program with the same therapist, a male experienced clinician. Sessions over the first year of treatment were examined (N1 = 38; N2 = 37). The technical and the relational dimensions of the therapeutic process were assessed and examined through a macroanalytic and microanalytic perspective. Results showed that some aspects are present in both couples: namely, the adherence to the treatment model and the attitude of the therapist oriented towards collaboration. On the other hand, specificities relating to each therapeutic couple emerged. In the treatment of the patient with positive outcomes, there was a globally positive relational climate; furthermore, therapist and patient can deal even with episodes of misunderstanding. Instead, in the treatment of the patient with partial outcomes, therapist and patient struggled to find an attunement and to work in synergy, remaining trapped in problematic relational patterns and without fully achieving the therapeutic objectives previously agreed. Taken together, results confirmed the effectiveness and the complexity of DBT. More specifically, they shed light on overlaps and differences between DBT and other theoretical models, in particular interventions promoting reflective functioning. Furthermore, the importance of a collaborative relationship between therapist and patient was confirmed. Overall, results suggested that mechanisms of action in DBT can be understood only in light of the dynamics of the therapeutic process in which they occur.

Full textThe purpose of this work was to identify the characteristics of filterability of beer throughout different filtration tests on a laboratory scale and on a pilot scale, using different types of filter systems and experimenting with different membrane materials. The preliminary characterization of beer filterability with the particle size analysis was conducted at the laboratories of Campden-BRI - The Brewing Research International, United Kingdom, while the pilot-scale filtration tests were conducted at the CERB – The Italian Brewing Research Centre, Italy. The filtration tests and the analysis on the samples allowed to verify the performance of the different membranes used in response to the variation of the main process parameters of cross-flow filtration. Several quality parameters were monitored on both filtered and unfiltered beer samples, to highlighting the effects of the different technologies used. With regard to the laboratory tests for the characterization of beer filterability, the analysis of the PCS instrument (Photon Correlation Spectroscopy) was used to verify the influence of various enzymatic treatments (Protease and Cellulase) on the efficiency of the filtration process and their influence on the colloidal stability of filtered beer The results showed that the PCS instrument is a reliable tool for determining the size distribution of suspended particles in beer and can be used to predict the colloidal stability of the filtered beer and to monitor the process filtration in real time.

Full textThis dissertation describes the role of the specialized translator in the process of the composition of medical research articles. The dissertation investigates the revisions of two specialized translators, respectively Italian and English native speakers, in five medical research articles written by Italian doctors in English and subsequently published in international specialised medical journals. An analysis of the revisions performed by the native and non-native speakers is undertaken using qualitative and quantitative research methods. The results of this analysis describe the respective changes that each translator makes to the texts, and explains how the final text is mediated differently by native and non-native speaking translators. The research highlights that the Italian translator introduces lexical and phraseologic features that are less frequently used in medical discourse. I conclude that this has implications for understanding the way in which English is used in multi-lingual research communication, and suggest that the study of the intervention of the translator in the process of medical research article composition is a useful way of analysing the developing profile of the use of English in multi-lingual research communities.

Full textThe present dissertation refers to the theoretical background known in the literature as Empirically Supported Therapy Relationships, a research tradition focused on the relationship and its nature, beyond the outcome and process research opposition. The three studies included in this work focused on different portions of treatment: respectively the initial intake session, the first two sessions, and the overall treatment. In the first study different outcomes were compared in order to examine if the effects of interpretations were connected to specific factors of psychotherapeutic process. The second study aimed to assess clinical cases of premature termination, differentiated on the basis of the contract attainment: the purpose was to depict specificities due to the different phase of dropping out. Finally, the third study was a single-case research examining alliance ruptures within an entire clinical process of forty sessions.

Full textThe thesis is a contribution to the study of the economic implications of fertility intentions and desires and of the economic aspects of the fertility decision making process. Precisely, it is based on three autonomous chapters and it aims at understanding: a) for women and for individuals within the couples the interplay between fertility and labour market preferences (with a particular focus on the role of women's sector of employment), and the influences that the (un)balanced division of domestic duties play on the intentions of becoming parents for another time in Italy; b) for couples living together the determinants of their (dis)agreement about the intention of becoming parents again; and c) for couples that disagree on future fertility plans, the determinants of female’s contrasting attitude towards her partners’ positive fertility intention. The dissertation uses a mixed-method design- a strategy based on survey data analysis as well as in-depth interviews analysis: this approach allows investigating the topic from different perspectives, by means of both quantitative and qualitative research methods in order to provide new insights into the phenomenon of interest.

Full textStrong inequalities in the labour market characterize the current European scenario. In particular, many scholars maintain that young people are the losers of the globalization process. In this context, the research aims at deepening the relationship between young people and labour market policies addressed to youth entrepreneurship. This phenomenon has been observed along some specific vectors related to the entrepreneurial attitude expressed in terms of perceived opportunities, business skills, fear of failure and intent to undertake, and the relevance of social ties in the dynamics of self-employment and business creation. In order to collect the relevant information the research has addressed a survey to a sample of young aspiring European entrepreneurs, involved in active labour policies, i.e. education and training courses, vocational guidance, coaching measures and services supporting entrepreneurship. Considering variables regarding human capital, social capital and family status, and their influence in the different entrepreneurial paths of youth entrepreneurship, the research tried to design some possible social trends. The research has highlighted that young people coming from family contexts with high levels of human and social capital, tend to express an entrepreneurial approach characterized by self-confidence, openness in terms of innovation, thus using the full potential of weak ties. By contrast a low family status appears as correlated to a low self-confidence, lack of innovation, limited social networks, and less prospects in terms of entrepreneurial success.

Full textBy situating in the critical framework of platform capitalism and adopting the socio-material approach, we investigate the capital-work-nature contradiction embodied in the digital. Thus, this research aim to highlight the socio-materiality of the digital, with a critique to the dematerialisation of the productive process as a result of digital technologies. On the contrary, this thesis make explicit the exploitation of work and nature along the global value chains of the platform capitalism. This hypothesis was developed on the basis of the case of platformisation of work in the urban spaces: the food delivery in Bologna and the ride-hailing in Lisbon. Both these cases show the persistance of contradictory and ambivalent process, for which the public actor and policy-makers (in our case the Municipality of Bologna and the Portuguese parliament) have tried to regulate the gig economy. But the process of platformization in the urban spaces are, at once, cause-and-effect of two more elements of the socio-materiality of the digital: the degradation of work in the productive nodes in the extra-urban spaces and the ecological impact. Thus, we identified the "digital propagation" as the socio-material infrastracture of platform capitalism, since it is a direct contribution to the increment of the exploitation of work and nature in the contemporary phase of capitalism. On the basis of a multi-situated etnography, we adopt a qualitative methodology, in particualar the in-depth interview to qualified witness (specifically workers, policy-makers, union representatives, activists). The field research was conducted between 2019 and 2020.

Full textThe aim of this study was to understand better the role of middle managers in lombard Child Protection services. This managers are the first organizational and methodological reference for social workers, but at the same time they have a responsability towards top management (and communities) in terms of a good service trend. There isn’t so much knowledge about their work and it seems that they have built their role in practice. The study explored the current characterization of managers that are active in lombard Child Protection system and collected their opinions about the role. The study consisted in a preliminary investigation about Child Protection services in Lombardy After this first investigation, the first step was a survey on-line addressed to all the managers that actually works in Lombardy; the second step consisted in semistructured interviews addressed to a sample of these managers (18). The aim of the first part of the research was descriptive (socio-demographic characteristics, qualification and career of the managers) while that of the second part was exploratory (managers opinion about their role in terms of competences requested, needs and resources). The research has combined two theoretical approaches, cultural studies of organizations/management (Alvesson e Berg, 1993; Mintzberg, 1980, 2010; Gagliardi e Monaci, 2011) and Relational Social Work (Folgheraiter, 1998, 2011, 2017): the integration of them allowed the identification of specific characteristics of child protection middle managers and their point of view about their job.

Full textThis thesis, carried out under the mandate of CTA Milan, shows the results of a participatory research on the process of reconstruction of pre-adoptive history by Italian adult adoptees. According to the participatory model, four experts by experience were involved in a research group (steering group), and work in tandem with the researcher on all the research project. Integrating researcher’ s technical skills with experiential skills of co-researchers, we defined the research question and decided to use semi-structured interviews to collect data. Text analysis of the transcripts of the 16 interviews carried out, was also made in the steering group, according to the literature review and to the personal experience of co-researchers. The interviews analysis shows the whole complexity of the origins’ research process: what is the meaning of origins’ research in the view of adoptees; how Italian adoptees try to find the identity of their birth family; by whom they have been supported during this research process; the relationships with adoptive family, with other people who live adoptive experiences, with the Juvenile Court and social workers. With these results we could learn more about origins’ research and provide significant suggestions on how adopted people could be supported in their experience of research.

Full textEconomic, social and cultural transformations of these times have produced ambivalent results while giving rise to a newfound and widespread form of distress where economic, social and relational fragility and urban suffering intertwine (Saraceno, 2010): this grey area of distress (Iori and Rampazi, 2008; Tramma, 2015) does not exceed standard thresholds of overt malaise that would place it within the traditional boundaries regarding severe marginality and exclusion – areas often touched upon by educational services and policies. The elements that contributed to the spread of social vulnerability (Ranci, 2002) – including the precariousness of living and working conditions, the weakening of the systems in place offering social protection and the erosion of the social fabric – have played a role in altering some traits of contemporary poverty: the concept of new poverty aims to account for new elements of a population dealing with uncertain borders, grey poverty (Dovis and Saraceno, 2011), meaning that those who were not previously considered at risk of poverty are now implicated: a risk related to events increasingly frequent and increasingly linked to those paths of life seen as “normal”. The pedagogical research on the educational implications of impoverishment processes focuses, through biographical methods (Merril & West, 2012), on the life stories and educational biographies of fifteen men and women who have recently become impoverished. The theoretical framework concerning social pedagogy (Tramma, 2010) has directed the exploration of these biographical trajectories in and around both formal and informal educational dimensions: these contribute to falling into fragile situations, but also preventing and/or reducing their eventual impact. The pedagogical analysis focuses on the vulnerability areas around which life’s critical aspects are concentrated, and on the representations and meanings through which impoverished people live and rework their own story and condition, highlighting the key role of a contemporary educational climate. Such an educational climate promotes increasingly individualised and notably hyper-responsible representations of one’s own life and impoverishment, while at the same time hindering the critical understanding of one’s life path, losing focus on the dynamics of the present – something indispensable to promote with the subjects changes for the improvement of individual and collective living conditions. The transformative tension (Baldacci, 2001) as part of this pedagogical research aims to contribute to the pedagogical reflection on new poverty in order to identify theoretical and methodological orientations for educational actions able to promote fresh paths for subjects, both in terms of preventive interventions with people at risk of poverty, and in terms of paths out of poverty.

Full textThe research is focused on the process of gender and ethnic identity construction in migration, on multiple axes that represent migrant’s belongings: ethnicity, gender, and class. The methodological approach is based on the action research perspective and proposes an engaged, emotional and relational way of doing research, in order to promote individuals and groups in social empowerment processes. The privileged method is ethnography, in a transnational context, based on building relationships of trust with participants and combining visual methods, biographical interviews, and peer research in order to narrate the complex picture of migrant youth experiences and to reach all the actors involved. Young Roma’s migratory experience connects the South West of Romania to Milan, in transnational circuits and kinship networks. The participants are involved street economies: they beg, play music, or pickpocket on a daily basis. Some of the practices, in the encounters between groups in migration, come to symbolize tradition and mediate difference between Roma and non-Roma, or between different groups of Roma. Such are the practices related to the virginity of young girls, matrimonies and the control over the bodies and sexuality of young girls. By maintaining strong distinctions between gendered life paths, stigma afflicted groups reclaim valued identity attributes related to an essentially different family order, discursively used in order to re-state ethnic and status differences between “us”, the “majority” group, and other Roma groups. Young men and women contextualize these scenarios, the traditional as well as occidental “stylistics of existence”, and separate their gendered performances in the various spaces of their migrant life. They define new belongings able to construct valued identities and permanently challenge the systems of practice, in a continuous dialogue between “us” (in-group) and “I” (individual identities).
