Abstract:
Background: Perinuclear Antineutrophil Cytoplasmic Autoantibodies (pANCA) have been found in patients with rheumatiod arthritis. Cathepsin G was the major target antigen.The present study was to investigate the unknown target antigen of ANCA (Cathapsin G ) in patients with rheumatiod arthritis Objective: This study is to investigate the prevalence of anti-Cathapsin G and rheumatoid factor in Iraqi patients with rheumatiod arthritis.Patienties&Methods: From1st January until 30 June of 2011 fourty five rheumatiod arthritis patients referred to the immunological department in the teaching laboratory of medical city and twenty five apparently healthy individual used as a control group were investeged to rheumatic factor IgG, IgM ,IgA isotypes and anti-Cathapsin G were measured by using enzyme immunoassay technique.Result: This study showed that the age of patients with rheumatiod arthritis ranged between eigthen years and sixty six years with mean age of (36.08) , while the mean level for RF screen, RF IgG, RF IgM, RF IgA isotypes and anti-Cathapsin G were (160.71, 91.7, 136.64, 95.03 and > 9.2 ) respectively. There were 17(37.8%) males and 28(62.2%) females of RA patients. Immunologically, this study revealed that the serum RF screen was found more than 25 U/ml in all rheumatiod arthritis patients (100%, P: 0.0001 HS). On the other hand RF isotypes IgG ,RF IgM and IgA level were elevated more than 20 U/ml and it was detected in 31 (68.9%, P: 0.0001 HS), 31 (68.9%, P : 0.0001 HS) ,and 32 (71.1%, P: 0.0001 HS) patients respectively, while the rheumatiod factors (IgG,IgM, and IgA)were normal in healthy control group . Statistically RF screen has a highly significant association with RF IgG, IgM, and IgA (P= 0.0001). Anti-Cathapsin G was found in 12(26.7%, P value: 0.019) of patients with rheumatic arthritis patients. This study showed a association between the positive result of anti-Cathapsin G with RF IgG and RF IgM (35.5%, P: 0.047) the results were the same for both, while no association found with RF IgA (25%, P: 0.692).Conclusion: A significant correlation was found between anti-Cathapsin G and patients with rheumatiod arthritis and has a significant association with RF IgG and RF IgM while RF IgA had no relation with anti-Cathapsin G.

While anti-Lactoferrin Abs showed no significant correlation with RF screen. This study showed a association between the positive results of anti- Cathapsin G Abs with RF IgG and RF IgM only .Anti- Elastase Abs, Anti-Lactoferrin Abs and Antilysozyme Abs showed neither a significant correlation with RF IgG, RF IgM nor with RF IgA.Conclusion:A significant correlation was found between Elastase antibodies, Cathapsin G antibodies , lysozyme antibodies and patients with rheumatiod arthritis Cathapsin G antibodies has a significant association with RF IgG and RF IgM

Abstract:
Objective: To determine the performance of patient reported outcome measurement information system inpatients with rheumatoid arthritis in our setup. Study Design: Cross sectional study. Place and Duration of Study: Division of Rheumatology, Fatima memorial hospital, Lahore Pakistan, from May2019 to Jul 2019. Methodology: A total of 191 patients of either gender, aged more than 16 years with sero +ve rheumatoid arthritis were included in the study. Results: Majority of patients 156 (81.67%) were female and male 35 (18.32%) with mean age 38 ± 12.19 years.Diagnosed as rheumatoid arthritis and compliant with csDMARDs. Out of all these, majority were in low diseaseactivity (LDA) 72 (38%), remission 62 (32%), moderate 42 (22%) and high disease activity 16 (8%). Mean T-score of all measures showed variations of scores as disease progresses. For all measures, mild change was noted between low disease activity and moderate disease activity whereas high difference was seen in remission and high disease activity. Highest correlations were seen among similar constructs of physical health, mental health, and social health. Fatigue was strongly correlated with social role and social activity. Conclusion: There was a considerable impact of rheumatoid arthritis on physical, social and mental healthcalculated with patient reported outcomes measurement information system-29 (PROMIS-29).

Abstract:
Nonsteroidal anti-inflammatory drugs are used for pain and to slow the progression of rheumatoid arthritis. Accordingly, the discovery of rheumatoid arthritis active compounds from the ethanol-water fraction Coleus atropurpureus (EWC) was conducted to characterize the isolated compounds as well as the anti-rheumatic effects of the EWC induced Complete Freund's Adjuvant (CFA). We conducted in vivo study in rats which were randomly divided into 5 groups. Group 1 was only given CFA as a negative control. Group 2 as positive control was orally exposed to diclofenac potassium (9 mg/BW). Three groups were given different EWCs orally as follows: 11.25 mg/BW, 22.5 mg/BW, and 45 mg/BW, respectively. Rheumatism rates were then compared with positive controls using a visual arthritic scoring system. The compounds identified by isolation of the EWC of Coleus atropurpureus predicted forskolin. The ethanol-water fraction Coleus atropurpureus did not act as an anti-rheumatic arthritis agent in CFA-induced rats.

Rheumatism (Rheumatoid Arthritis) is a chronic of systemic inflammatory the disease, systemic inflammation that can be affect tissues and organs, especially attacking flexible (synovial) joints (WHO, 2016). Based on the American of college Rheumathology (2013) states that 52.5 million or approximately 23 % of the population of United States suffer from rheumatoid arthritis. It is estimated that at least 355 million of the world population suffer from rheumatism, which means that 1 in 6 of the world's population of the experiences rheumatic disease. The results of the survey on the European continent in 2004 showed that rheumatic disease was the most common chronic disease. Approximately 50% of Europeans aged over 50 years experience musculoskletal pain complaints (North Sumatra Province Health Service, 2010). All the types of rheumatism cause disruptive pain so that one's of the ability to move can be disrupted by rheumatic disease (Riskesdes, 2007 in Maj. Medicine Indonesia, 2009). One of the non-pharmacological interventions that nurses can do independently in reducing the scale of rheumatoid arthritis pain is by compressing warm ginger (Sentoso, 2013). The design of the reseach used Pre-Experiment using One Grop design. The design of theTest is Pre-Post. The sample of the Reseach was 13 people. the results of the test is t test statistical test the effect of ginger compresses on the decrease in pain scale is known that the value of p = 0.001 is p <0.05 thus Ho is rejected which means there is the effect of ginger warm compresses on the reduction in rheumatoid arthritis pain scale.

Arthritis or commonly called rheumatism is a disease that attacks the joints and surrounding structures. Rheumatic disease in the community is often considered a trivial disease because it does not cause death, but if not treated quickly rheumatism can make limbs function abnormally, starting from bumps, stiff joints, difficulty walking, even lifelong disability. Objective: to find out there is a relationship between rheumatoid arthritis pain and independence of Activity daily living (ADL) in the elderly. Research Methods: This study used a quantitative research design with a cross sectional approach to reveal the relationship between independent variables (Rheumatoid Arthritis pain) and the dependent variable of Independent Activity daily living (ADL) at the same time and once a measurement. Statistical tests using chi-square consisted of 33 respondents. Results: From the results of the chi-square statistical test obtained p value = 0.047 (p <0.05) there was a relationship between rheumatoid arthritis pain and the independence of the elderly in the puskesmas classaman city of Sorong. Conclusion: There is a relationship between Rheumatoid Arthritis pain and the degree of independence in carrying out daily life activities in the elderly in the working area of ​​the Sorong City Community Health Center (p value = =, 047) Keywords: Rheumatoid Pain Arthritis; Elderly ADL Bibliography: 2002 – 2017

Rheumathoid Arthritis is a disease that occurs due to abnormalities in the joints, causing pain and stiffness in the musculoskeletal system such as skeletal muscles, joints, ligaments and bones and generally attacks clients over the age of 40 years (Syapitri, 2018). Rheumatoid Arthritis has typical symptoms in sufferers, one of which is pain. If not handled properly, pain in rheumathoid arthritis can disrupt the activities of sufferers, paralysis and impaired balance. Therefore, pain in reumathoid arthritis must be treated. The aim of writing this literature review is to provide information about the non-pharmacological management of pain in clients with rheumatoid arthritis. The method used in this study is to use the Literature review method. Literature review is carried out by analyzing articles relevant to the title taken, namely the management of non-pharmacological pain in clients with Rheumathoid arthritis. The stages in conducting a Literature review are based on 5 stages of literature review that cover the eligibility criteria, information sources, selection of literature, data collection and selection of data items. With pain rheumatic arthritis include: back massage, cupping, benson relaxation and warm compresses, cinnamon compresses, giving of the gnome, ginger compresses, lemongrass compresses, Swedish massases, giving keroncong music, grated ginger grater, tai chi exercise, and yoga interventions.

Abstract Rheumatoid arthritis is an autoimmune disease when a person’s immune system attacks the body’s cells. Signs and symptoms of rheumatoid arthritis include joint inflammation and joint deformity. In most cases, patients with rheumatoid arthritis experience joint pain. Rheumatic Gymnastics is an alternative therapy that has been proven to reduce joint pain in rheumatic patients. The purpose of this case study is to describe the use of rheumatic exercise therapy in arthritic patients. The purpose of this case study is to examine therapeutic gymnastic in reducing pain among patients with rheumatoid arthritis. Two patients were taught to exercise therapeutic gymnastic. The research instrument is a pain scale observation sheet (Numerical Rating Scale). Two patients reported that there was a decreasing intensity of joint pain after doing exercise. This study concludes this particular exercise may reduce joint pain. Nurses are suggested to implement therapeutic gymnastics exercise in reducing pain among patients with Rheumatoid arthritis.Keywords : rheumatoid arthritis; pain; therapeutic gymnastics exercise Abstrak Rematik merupakan penyakit auto imun ketika sistem imun pada tubuh seseorang menyerang sel-sel tubuhnya sendiri. Gejala rematik yaitu inflamasi, deformitas, dan nyeri sendi yang paling dirasakan oleh penderita rematik. Senam Rematik merupakan terapi alternative yang sudah terbukti dapat menurunkan nyeri sendi pada pasien rematik. Tujuan studi kasus ini adalah untuk menggambarkan penggunaan terapi senam rematik pada pasien rematik. Metode yang digunakan adalah asuhan keperawatan dengan menerapkan terapi senam rematik. Instrumen penelitian berupa lembar observasi skala nyeri (Numerical Rating Scale). Hasil yang didapatkan pada klien 1 maupun 2 yaitu mengalami penurunan nyeri. Kesimpulan pada studi kasus ini bahwa senam rematik dapat menurunkan skala nyeri. Saran bagi perawat diharapkan dapat menerapkan tindakan senam rematik untuk menurunkan skala nyeri pada pasien rematik.Kata kunci : Rematik, nyeri, senam rematik

Background:The time from first symptom to diagnosis (= diagnostic delay) is considered as key factor for better outcome in many chronic inflammatory rheumatic diseases, especially for rheumatoid arthritis (RA) and vasculitides like giant cell arteritis (GCA). A longer diagnostic delay may cause pain, reduced functionality, reduced life-quality and increased morbidity, as well as structural damages of the organs linked with higher mortality. This retrospective study assessed the diagnostic delay in consecutive Middle-European outpatients and compared results with those of a systematic literature review (SLR).Objectives:To compare disease-specific diagnostic delays of consecutive rheumatic patients with international data from a systematic literature review.Methods:Charts of a single-centre cohort with consecutively recruited patients were retrospectively reviewed for patients’ and diseases’ characteristics at a Middle-European university outpatient clinic for rheumatology. A SLR was performed according to PRISMA guidelines.Results:The average mean ± SD time from first symptom to established diagnosis was 7.9 ± 11.7 (0.02-56.7) years. Spondyloarthritis patients showed the longest diagnostic delay with 13.1 ± 14.2 (0.1-56.7) years, whereas polymyalgia rheumatica-patients had the shortest diagnostic delay with 1.5 ± 0.4 (0.3-18.0) months. In the SLR, most data for diagnostic delays are comparable to the Innsbruck cohort, but the diagnostic delay for psoriatic arthritis in Innsbruck is longer than in the Danish DANBIO registry (p<0.001). Abstract Background: Rheumatoid arthritis is a chronic disease affecting the immune system and it particularly targets synovial joints, bringing about inflammation and causing functional limitations. Objective: To explore the evidence showing the influence of early menopause in women with rheumatoid arthritis. Methods: In this systematic review, all the literature from 2010 to 2019 was searched with specific keywords “menopause, premature menopause, early menopause, rheumatoid arthritis, rheumatic diseases” with the use of Boolean operators. Databases that were thoroughly searched included Google Scholar, PubMed and HEC digital library. After careful screening, 10 studies that met the inclusion criteria were included in the review. The quality of these articles was assessed using the AXIS tool for that cross-sectional studies. Results: These studies indicated that post-menopausal women are at higher risk of developing rheumatoid arthritis and they are more likely to have early or premature menopause (in ≤45 years). At baseline, the post-menopausal women had high health assessment questionnaire scores than pre-menopausal women. Conclusion: This review states that menopausal women are highly at risk of developing rheumatoid arthritis, especially those experiencing this at an early age and this condition can also worsen arthritic symptoms. The purpose of this study was to determine the effect of Warm Lemongrass Compress Against Pain Intensity in Rheumatoid Arthritis in Elderly People at Tresna Werdha Social Home Budi Luhur Jambi. The research design used was a Pre-Experiment design with One Group Pre-Post Test Design. The study was conducted at the Budi Luhur Nursing Home in Jambi. The sampling method was carried out using purposive sampling, with 15 respondents consisting of the Serai compress. Analysis of the data will be used univariate analysis and bivariate analysis with the t-test Dependent. The results were obtained. by pain intensity in elderly Rheumatoid Arthrtitis before the average treatment 6.90 and after the pain is reduced to an average of 4.13. There is an effect of giving warm lemongrass compresses on decreasing the intensity of Rheumatoid Arthritis pain in the elderly at Tresna Werdha Social Home in Jambi City with p-value = 0,000. It is recommended to the room nurse or the person in charge of the room to be able to try to implement or demonstrate of warm lemongrass compresses so as to reduce the intensity of pain in the elderly.

Arthritis is a major cause of disability. Currently available anti-arthritic drugs, such as disease-modifying anti-rheumatic drugs (DMARDs), have serious side-effects associated with long-term use. Triterpenoids are natural products with known anti-inflammatory properties, and many have revealed efficiency against arthritis both in vitro and in vivo in several animal models, with negligible cytotoxicity. However, poor bioavailability due to low water solubility and extensive metabolism upon oral administration hinder the therapeutic use of anti-arthritic triterpenoids. Therefore, drug delivery systems (DDSs) able to improve the pharmacokinetic profile of triterpenoids and achieve sustained drug release are useful alternatives for targeted delivery in arthritis treatment. Several DDSs have been described in the literature for triterpenoid delivery, including microparticulate and nanoparticulate DDSs, such as polymeric micro and nanoparticles (NPs), polymeric micelles, liposomes, micro and nanoemulsions, and hydrogels. These systems have shown superior therapeutic effects in arthritis compared to the free drugs and are similar to currently available anti-arthritic drugs without significant side-effects. This review focuses on nanocarriers for triterpenoid delivery in arthritis therapy, including osteoarthritis (OA), rheumatoid arthritis (RA) and gout that appeared in the literature in the last ten years.

Plectranthus amboinicus(P. amboinicus) is a folk herb that is used to treat inflammatory diseases or swelling symptoms in Taiwan. We investigated therapeutic efficacy ofP. amboinicusin treating Rheumatoid Arthritis (RA) using collagen-induced arthritis animal model. Arthritis was induced in Lewis rats by immunization with bovine type II collagen. Serum anti-collagen IgG, IgM and C-reactive protein (CRP) were analyzed. To understand the inflammation condition of treated animals, production of TNF-α, IL-6 and IL-1β from peritoneal exudates cells (PEC) were also analyzed.P. amboinicussignificantly inhibited the footpad swelling and arthritic symptoms in collagen-induced arthritic rats, while the serum anti-collagen IgM and CRP levels were consistently decreased. The production of pro-inflammatory cytokines TNF-α, IL-6 and IL-1β were also decreased in the high dosage ofP. amboinicusgroup. Here, we demonstrate the potential anti-arthritic effect ofP. amboinicusfor treating RA, which might confer its anti-rheumatic activity. This differs the pharmacological action mode of indomethacin.

Rheumatoid arthritis penyakit yang menyerang pada bagian sendi sehingga mengakibatkan struktur ataupun jaringan terganggu sehingga mengganggu aktifitas sehari-hari. Untuk mengurangi nyeri sendi yang dialami penderita dapat dilakukan dengan non farmakologi yaitu dengan senam rematik. Kegiatan pengabdian masyarakat bertujuan untuk memberikan pemahaman dan pencegahan penyakit Rheumatoid arthritis pada lansia di Posyandu Mugi Sehat. Metode yang digunakan dalam kegiatan ini yaitu penyuluhan kesehatan dengan metode ceramah, diskusi, tanya jawab, dan praktik senam rematik. Pasca pengabdian masyarakat diketahui bahwa peserta mengalami peningkatan pengetahuan terkait dengan Rheumatoid arthritis. Kegiatan pengabdian masyarakat penting dilakukan untuk meningkatkan pengetahuan masyarakat terkait berbagai penyakit dan meningkatkan kesadaran masyarakat akan pentingnya pencegahan penyakit rheumatoid arthritis sehingga dapat mengurangi tingkat kesakitan dan menurunkan prevalensi rheumatoid arthritis di Indonesia sendiri. To reduce joint pain experienced by sufferers, it can be done non-pharmacological, namely rheumatic exercises. Community service activities aim to provide an understanding and prevention of rheumatoid arthritis in the elderly at Posyandu Mugi Sehat.The method used in this activity is health education using lecture, discussion, question and answer methods, and rheumatic exercise practice. After community service, it was discovered that participants experienced increased knowledge related to Rheumatoid arthritis. It is important to carry out community service activities to increase public knowledge regarding various diseases and increase public awareness of the importance of preventing rheumatoid arthritis so that it can reduce the level of morbidity and reduce the prevalence of rheumatoid arthritis in Indonesia itself. Intolerance to Methotrexate is a common cause of non-compliance2&3.Objectives:To investigate the effect of adding caffeine orally as Methylxanthines (Caffeine), act as adenosine receptor antagonists4to reduce symptoms of moderate to severe methotrexate intolerance in patients with Rheumatoid Arthritis5.Methods:A prospective, randomized controlled study conducted at Aswan University Hospital, Egypt from Jan 2018 till may 2019. Sixty patients with Rheumatoid arthritis who have had experienced moderate to severe methotrexate intolerance was enrolled in the study. The methotrexate intolerance severity score (MISS)6was evaluated at base line before initiation of study then at the next three months consecutively. Patients were randomly assigned by closed envelope method into 2 groups each containing 30 patients:Group (A); 30 patients was prescribed caffeine (coffee or dark chocolate) as an antidote to methotrexate intolerance7.Group (B); 30 matched patients acted as control group that included who will continue methotrexate regimen without addition of any extra caffeine.Results:Twenty four patients (80%) at time three follow up visit showed full improvement of symptoms of methotrexate-intolerance compared to ten patients (33.3%) at 2nd month follow up visit and seven patients (23%) at 1st month follow up visit with statistically significant difference all over the study period (P=0.005). half of study group patients discontinued anti-emetic and other drugs while none in control group did.Conclusion:Adding caffeine to management regimen can reduce the symptoms of severe methotrexate-intolerance in Rheumatoid Arthritis patients.References:[1]Friedman, B., & Cronstein, B. Rheumatoid arthritis (RA) is the most common autoimmune inflammatory arthritis in adults. Pharmacological treatment is quite effective to overcome it, but in some cases, especially in elderly RA sufferers, pharmacological treatment such as non-steroidal anti-inflammatory drugs (NSAIDs), corticosteroids and DMARD (Disease Modifying Antirheumatic Drugs) have an impact on drug side effects, namely gastrointestinal disorders that cause toxic effects on liver and kidney. The developed natural medicine treatment has been supported by several previous studies that provide anti-inflammatory benefits, namely, ginger (Zingiber Officinale Rosc.). Ginger (Zingiber Officinale Rosc.) The method used is descriptive. The purpose of this study was to determine the characteristics of respondents who suffer from rheumathoid arthritic therapy with essential ginger oil and resisted active movement. The results of the study showed that as many as 20 respondents who suffered from rheumatoid arthritis, based on their characteristics, the most were female, namely 16 people (80%, ages 60-75 years, as many as 15 people (75%), the most part of the joints experiencing rheumathoid arthritis. occurred in the knee as many as 9 people (45%), work history, at most not working as many as 8 people (40%), history of injury, there were most injuries as many as 12 people (60%)

Prior studies in Latin America had reported adherence of 16.4% on Rheumatoid Arthritis (RA) and 45.9% in Systemic Lupus Erythematous (SLE). There is evidence that these patients believe their treatment increases the risk and gravity of COVID-19 and therefore, suspending the treatment could reduce this risk. All the materials were peer-reviewed full-text papers, letters, reviews, or book chapters obtained through a literature search of the PubMed database using the keywords ocular manifestations; pathogenesis; systemic inflammatory rheumatic diseases; rheumatoid arthritis; osteoarthritis; fibromyalgia; systemic lupus erythematosus; seronegative spondyloarthritis; ankylosing spondylitis; reactive arthritis; enteropathic arthritis; psoriatic arthritis; systemic sclerosis; polymyalgia rheumatica and covering all years available. Some statements articulated in this paper reflect the clinical experience of the authors in their tertiary-referral center. Results Ophthalmic disorders are categorized by anatomical subgroups in all rheumatic diseases. The most common ocular manifestations are diverse types of inflammations of different tissues and dry eye disease (DED). Conclusion The eye could be a responsive marker for the onset or aggravation of an immune reactivation in many rheumatic diseases, furthermore, ocular findings can antedate the diagnosis of the underlying rheumatic disease. By recognizing ocular manifestations of systemic rheumatic diseases it might be possible to avoid or at least delay many long term sequelae.

Some patients develop genuine classic rheumatic and musculoskeletal diseases, but a number of rheumatic immune-related adverse events mimic rheumatic and musculoskeletal diseases with atypical features, mainly polymyalgia rheumatica, rheumatoid arthritis and myositis, as well as several systemic conditions, including sicca syndrome, vasculitis, sarcoidosis, systemic sclerosis and lupus.

The pediatrician frequently encounters children and adolescents with musculoskeletal complaints that raise the possibility of rheumatic disease. The purposes of this article are: to review an approach to the evaluation of "joint" symptoms and to review the pharmacology, use, and adverse effects of nonsteroidal anti-inflammatory drugs. RHEUMATOLOGIC HISTORY Systematic collection of the historical details is fundamental in the evaluation of "joint" complaints (Table 1). Age and Sex The child's age and sex provide initial clues. For example, suspicion that a young girl (less than 5 years of age) with knee swelling might have monoarticular juvenile rheumatoid arthritis is heightened simply because of her age and sex. (At onset of their disease, almost 20% of all patients with juvenile rheumatoid arthritis are young girls with pauciarthritis, most commonly involving the knee.) Suspicion that an older boy (10 years of age or older) with axioskeletal complaints might have an enthesopathy syndrome is increased, in part, because of his age and sex. of the age and sex predilections of various rheumatic conditions is, therefore, helpful. Chief Complaint The chief complaint is often directive. For example, the complaint "his knees hurt every night" or "his legs hurt at night" is not characteristic of children who have juvenile rheumatoid arthritis or other well-defined inflammatory arthritides.

ObjectivesTo evaluate the prevalence and type of rheumatic immune-related adverse events (irAEs) in patients receiving immune checkpoint inhibitors (ICIs), as well as the correlation with tumour response.MethodsThis was a single-centre prospective observational study including all cancer patients receiving ICIs. The occurrence of irAEs and tumour response was assessed on a regular basis. Patients who experienced musculoskeletal symptoms were referred to the department of rheumatology for clinical evaluation and management.ResultsFrom September 2015 to May 2017, 524 patients received ICIs and 35 were referred to the department of rheumatology (6.6%). All but one of the rheumatic irAEs occurred with anti-programmed cell death protein 1(PD-1)/PD-1 ligand 1(PD-L1) antibodies, with a median exposure time of 70 days. There were two distinct clinical presentations: (1) inflammatory arthritis (3.8%) mimicking either rheumatoid arthritis (n=7), polymyalgia rheumatica (n=11) or psoriatic arthritis (n=2) and (2) non-inflammatory musculoskeletal conditions (2.8%; n=15). One patient with rheumatoid arthritis was anti-cyclic citrullinated peptide (anti-CCP) positive. Nineteen patients required glucocorticoids, and methotrexate was started in two patients. Non-inflammatory disorders were managed with non-steroidal anti-inflammatory drugs, analgesics and/or physiotherapy. ICI treatment was pursued in all but one patient. Patients with rheumatic irAEs had a higher tumour response rate compared with patients without irAEs (85.7% vs 35.3%; P<0.0001).ConclusionSince ICIs are used with increasing frequency, knowledge of rheumatic irAEs and their management is of major interest. All patients were responsive either to low-to-moderate doses of prednisone or symptomatic therapies and did not require ICI discontinuation. Furthermore, tumour response was significantly higher in patients who experienced rheumatic irAEs.

Temporomandibular joint (TMJ) is commonly involved in various immune-mediated rheumatic disorders accounting for significant disability and impaired quality of life. The aim of our study was to assess inflammatory and immune parameters in patients with TMJ arthritis related to rheumatoid arthritis (RA), juvenile idiopathic arthritis (JIA), ankylosing spondylitis (AS) and psoriatic arthritis (PsA) and to identify potential relation with severity and dysfunction of TMJ pathology. We performed a cross-sectional study in a cohort of 433 consecutive RA, 32 JIA, 258 AS, and 103 PsA. Only patients presenting with clinically significant TMJ involvement (273) related to their rheumatic condition were included in the final analysis. TMJ involvement is traditionally described in chronic inflammatory rheumatic disorders, particularly in patients with higher levels of inflammation as detected in rheumatoid arthritis and psoriatic arthritis. Disease activity and severity, as well as biological and positive serological assessments (rheumatoid factor, anti-cyclic citrullinated peptide, IL-1) remain significant determinants of the severity of TMJ arthritis.

Various factors can cause a high incidence of rheumatism, one of which is a behavioral factor that is not in accordance with health principles. Behavioral factors are very important to pay attention to so that they can reduce the incidence of disease. The purpose of counseling is to understand rheumatism as a factor, signs and symptoms, and the importance of rheumatism treatment. Participants who attended were 5 people consisting of 2 men and 3 women. The place is used according to the plan made using the tools during the socialization, such as flipcharts and leaflets. The medical records of all new referral to rheumatology clinic Hospital Sultan Ismail and Hospital Pakar Sultanah Fatimah from 1st January 2020 to 31th November 2022 were reviewed. The referral diagnosis and final diagnosis were identified and analysed.ResultsThere were total of 927 new cases referral throughout the 35 months during Covid-19pandemic. Majority of them were diagnosed to have rheumatoid arthritis (217/927)followed by systemic lupus erythematosus (190/927), psoriatic arthritis (147/927),gout (62/927), osteoarthritis (58/927), systemic sclerosis (25/927), ankylosing spondylitis (25/927), soft tissue rheumatism (24/927), Sjogren syndrome (24/927),mixed connective tissue disease (14/927), vasculitis (11/927), fibromyalgia (10/927),polymyositis (7/927) and miscellaneous (39/927).45 out of the new cases were diagnosed as unlikely rheumatic diseases. Polymyalgia rheumatica (PMR) is an age-related chronic inflammatory disease with rheumatic features at the fore. In addition to the high-grade systemic inflammation, it is characterized by typical “polymyalgic” musculoskeletal symptoms, including diffuse and severe pain and prolonged morning stiffness of the shoulder girdle, pelvic girdle, and neck. PMR is a member of the so-called giant cell arteritis complex; however, in spite of the marked systemic inflammation in PMR, the local vasculitis process aborts. The pathological background is synovitis, with a predominant inflammation of the extra-articular synovial structures. Synovitis of PMR is mild, transient, and non-erosive. Distal musculoskeletal symptoms are also observed but are more variable and less recognizable than the predominant proximal polymyalgic syndrome. PMR often overlaps with elderly-onset seronegative arthritides, elderly-onset rheumatoid arthritis, late-onset seronegative spondylarthritis, and the RS3PE1 syndrome. Although glucocorticoids are the cornerstone of PMR therapy, considerable hope is attached to tocilizumab, an IL-6 receptor inhibitor.

The purpose of this work is to perform a general analysis of relevant literature on the issue of kidney damage in patients with rheumatoid arthritis. Kidney damage in patients with rheumatic diseases is potentially dangerous, as it can lead to the development of terminal renal failure that may require replacement renal therapy. Amyloidosis often leads to kidney failure in patients with rheumatoid arthritis. Renal amyloidosis more often develops in patients with acute course of rheumatoid arthritis and under maximal immunological disorders. In patients with renal amyloidosis against the background of rheumatoid arthritis, manifestations of joint affection decrease, while the renal-uremic syndrome takes a predominant role. Signs of nephrotic syndrome and chronic renal failure develop gradually. Kidney damage can be caused by medications for rheumatoid arthritis. The choice of the optimal scheme of individual-centred therapy is vitally important for patients, since every aggravation of both rheumatic disease and secondary renal damage leads to the progression of chronic renal failure.

Rheumatoid cachexia is associated with rheumatoid arthritis and it increases mortality and morbidity. Adjuvant-induced arthritis is an experimental model of rheumatoid arthritis that causes anorexia and muscle wasting. α-Melanocyte-stimulating hormone (α-MSH) has anti-inflammatory actions, and it is able to decrease inflammation in several inflammatory diseases including experimental arthritis. In this study we tested whether systemic α-MSH treatment is able to ameliorate cachexia in arthritic rats. On day 8 after adjuvant injection control and arthritic rats were treated with α-MSH (50 μg/rat ip) twice a day, until day 16 when all rats were euthanized. Arthritis decreased food intake, but it increased hypothalamic expression of neuropeptide Y (NPY) and Agouti-related peptides (AgRP) as well as interleukin-1β (IL-1β) and cyclooxygenase-2 (COX-2) mRNA. In arthritic rats, α-MSH decreased the external signs of arthritis and increased food intake ( P < 0.01). In addition, α-MSH decreased hypothalamic expression of IL-1β, COX-2, proopiomelanocortin, and prohormone-converting (PC) enzymes PC1/3 and PC2 mRNA in arthritic rats. In control rats, α-MSH did not modify food intake or hypothalamic expression of aforementioned mRNA. α-MSH prevented arthritis-induced increase in gastrocnemius COX-2, muscle-specific RING-finger protein-1 (MuRF1), and atrogin-1 expression, and it increased fast myofiber size. In conclusion our data show that in arthritic rats peripheral α-MSH treatment has an anti-cachectic action increasing food intake and decreasing muscle wasting.

Background:Chikungunya virus (CHIKV) infection is an emerging disease which is responsible for several epidemics around the world1. Systematic review and meta-analysis had shown that approximately 25% of cases of Chikungunya(CHIK) would develop CHIK-Chronic Inflammatory Rheumatism and 14% would develop persistent arthritis (or spondylitis)2.Objectives:To describe the frequency of the clinical patterns of chronic arthritis & to characterize the clinical symptoms in a Bangladeshi cohort of CHIK patients 12months post-infection.Methods:In 2017, a Chikungunya outbreak occurred in Dhaka, Bangladesh, during which a prospective cohort of CHIK patients with confirmed diagnosis was constituted. A longitudinal follow up of 60 patients from an initial cohort of 142 patients, attending the out-patient department of Rheumatology, BSMMU, was done. Patients having arthritis/ arthralgia or both lasting more than 3 months were considered as chronic cases. Their baseline and follow-up symptoms at 3m, 6m and 12months were evaluated. Functional status was assessed with the Bengali Version Health Assessment Questionnaire (HAQ).Results:Of the initial 142 patients enrolled in the study, 135(95.1%) had CHIKV-IgM and 29(20.4%) had IgG positive. Patients that followed up in-person were predominantly adult (age 43.73 ± 11.09 years) and female 34 (56.7%). The majority of the patients 35 (58.3%) had undifferentiated arthritis. After three months, 8 (16.3%) had oligoarthralgia, 26 (53.1%) had polyarthralgia and 8 (16.3%) had polyarthralgia with oligoarthritis. At the end of one year, 13 (21.7%) patients underwent complete remission. Among the 47 patients, 21 had joint involvement where 11(52.4%) had polyarthralgia, 5(23.8%) had polyarthralgia with oligoarthritis, 5 (23%) had oligoarthralgia and 4 (19%) had monoarthralgia. Among the 47 patients, mild, moderate and severe functional disability was present in 89.4%, 6.4% and 4.3% patients respectively.Conclusion:After one year of follow up, one-third of the patients remained symptomatic. Puskesmas Tanah Garam Kota Solok ". Based on the results of research conducted on 40 respondents, it can be concluded that there is a relationship between genetics and the incidence of rheumatism, there is a relationship between age and the incidence of rheumatism, there is a relationship between sex and the incidence of rheumatism, there is a relationship between obesity and the incidence of rheumatism and a relationship between lifestyle and incidence rheumatism in Kelurahan VI Tribe, Tanah Garam Public Health Center, Solok City.

Abstract:
ABSTRACTObjective: Rheumatoid arthritis is chronic inflammation disorder that can be treated with anti-inflammatory drug such as methylprednisolone.Several medicinal plants shave the potential to be used in arthritis therapy, one of which is Muntingia calabura L. The objective of this research wasto study the antirheumatic activity of M. calabura leaves ethanol extract and its n-hexane, ethyl acetate, and water fractions in a rheumatoid arthriticrat model.Methods: Rheumatoid was induced in rats by intraplantar injection of Complete Freund’s Adjuvant (CFA) into the right foot paw of Wistar rats. Thepaw volume was measured by a plethysmometer. The histology of right foot paw was carried out. For chronic CFA inflammation, induced paw edemaassay was used, whereas chronic inflammation was assessed by CFA induced arthritis. The histopathology of synovial joints (paw) was carried out.Result: The results of the study revealed that in CFA-induced arthritic rats, methylprednisolone, M. calabura ethanol extract, and its fractions wereable to reduce inflammation. Histopathology examination also showed a reduction of in cartilage destruction, influx of inflammatory cells, pannusformation, fibrin deposition, and synovitis.Conclusion: M. calabura L. n-hexane fraction showed a better anti-rheumatic activity in rats among other fractions.Keywords: Muntingia calabura L. leaves, Ethanol extract, N-hexane fraction, Ethyl acetate fraction, Antirheumatic.

<p><strong><em>Background:</em></strong><em> </em><em>One of the chronic diseases is rheumatoid arthritis (RA) is the most common auto immune disease, which is inflammation of the joints that occurs in adulthood and the elderly. According to the American Collage Of Rheumatology, 2015, rheumatoid arthritis has a significant negative impact on the ability to move, both work and household chores and affect quality of life and increase mortality</em><em>. </em><em>Indications of an increase in cases of rheumatism in the community one of them because of the lack of family attention to the prevention and care of family members who have rheumatic diseases. </em><strong><em>Aim</em></strong><em>: To find out Family Nursing Care with the Application of Complementary Therapy in Rheumatism Cases in </em><em>one of Kanagarian Sumatera Barat</em><em>.</em><em> </em><strong><em>Method</em></strong><em>: </em><em>In implementation, complementary therapy is applied to Rheumatic patients, namely through routine health checks, physical activity, rheumatic diet, gymnastics elderly, and consumption fro.</em><em> </em><strong><em>Results</em></strong><em>: </em><em>Rheumatism pain that was experienced Ny. Z has decreased from pain scale 6 (moderate) to 2 (mild) after ingestion of the frost for ± 6 days</em><em>.</em><em> </em><strong><em>Conclusion</em></strong><em>: </em><em>: </em><em>The application of complementary therapies showed improvement in patients suffering from rheumatism, therefore, the application of evidence-based nursing in providing nursing care is recommended</em><em>.</em><em> </em><em></em></p>
