Full textRoot canal Retreatment is an alternative in the face off endodontic failure. This procedure aims to reduce the symptoms that may occur or worsen after the endodontic treatment. The root canal retreatment procedure follows the premise that apparently the quality of the filling material in the entire canal system is deficient, therefore, all the material must be removed. Currently, with better techniques of image analysis it is possible to identify the origin of the apical lesion, therefore, it is possible to select the pulp canals with poor treatment and be able to treat them individually. In this way, the selective root canal retreatment could avoid the unnecessary wear of root canals in good condition.

Full textTechnological advances to improve the quality of endodontic treatment in all aspects are insufficient if they are not complemented by adequate prosthetic treatment. The bacterial cause caused by the maladaptive prosthetic treatments can promote bacterial migration towards the root canal and the filling material, in case of endodontic treatment. If this situation is not controlled, the infection could continue into the periapical tissues, causing a periapical cyst. To treat the periapical cyst, identify with the tooth where the infection originated, portray the root canals and perform the surgical removal of the cyst. However, it is possible to perform a non-surgical approach with favorable results.

Full textDental malposition influences the aesthetics and functionality of patients. In this situation, it is possible to make the correction by means of fixed orthodontic treatment; However, at the consultation, patients with clinical features such as reduced vestibular cortices, root resorption and positions of teeth not compatible with alveolar bone, which limit the possibility of achieving the objectives of orthodontic treatment. Despite this, it is possible to complement orthodontic treatment with periodontal procedures to optimize dental movement and reduce adverse effects on supportive tissues. This work reports the orthodontic treatment performed in six months of a 25-year-old female patient supplemented with corticotomy and bone graft to correct the crowding of the dental pieces without adverse effects of the support tissues. It is advisable to have a tomographic analysis prior to treatment in order to correctly assess the topography of the alveolar bone, so that factors that can alter the stability of the orthodontic treatment can be identified.

Full textObjetivo: Determinar el Perfil de farmacorresistencia del Mycobacterium tuberculosis en pacientes que ingresaron a retratamiento de los establecimientos de las redes de Salud Bonilla, Bepeca y Ventanilla, DIRESA Callao, 2006-2010 Materiales y mtodos: Se tom a los pacientes con diagnóstico de tuberculosis frotis (+) y pruebas de sensibilidad a fármacos anti-tuberculosos que ingresaron a Re-Tratamiento, se realizaron pruebas de sensibilidad en el Instituto Nacional de Salud (INS), se utilizaron las frecuencias de las variables establecidas y se utilizaron las medidas de tendencia central y de dispersión en las variables cualitativas. Resultados: La prevalencia farmacorresistencia a uno o más fármacos fue de 29%, Multi-resistencia fue 21%. La frecuencia en general de resistencias fue isoniacida 97,34%(n=404), rifampicina 81,20%(n=337), estreptomicina 46,41%(n=188). Conclusiones: En el presente trabajo se encontró que existe una elevada prevalencia de TBC resistente en los pacientes que ingresaron a retratamiento de los establecimientos de las redes de Salud Bonilla, Bepeca y Ventanilla, DIRESA Callao siendo la forma la de mayor prevalencia la TBC-MDR, siendo el fármaco que con mayor frecuencia genero resistencia en los pacientes con monoresistencia la isoniacida.

Full textObjective: The general objective of this study has been to evaluate the cleanliness of the dentin tubules achieved through different irrigation techniques and with a specific solvent, in cases of endodontic re-treatment. Materials and Methods: 100 recently extracted single-canal inferior premolar human teeth were utilized (n = 100), with their roots intact, coming from extractions of patients within an age range of 25 and 45. After decontaminating and cleaning the teeth were radiographed in orthoradial and proximal views to corroborate the presence of only one canal and to evaluate the angle of the curvature. Once coronal portions were sectioned and roots were standardized at 16mm, the samples were placed in a model for endodontic treatment using the Mechanized System by Níquel-Titanio MTwo. Then they were sealed with a side compaction technique of lateral compaction using gutta-percha cones and AH Plus sealant. After its conservation at 37°C temperature and 100% humidity, the gutta-percha obturation was removed with the system MTwo-Retreatment to carry out a complementary irrigation procedure with different techniques and irrigants. The aim of these procedures has been to evaluate the degree of cleanliness achieved. The samples were randomly distributed in three study groups; G1: Manual irrigation (n:40), G2: Passive Ultrasonic Irrigation (n:40) and G3: Control (n:20). Likewise, each group was divided into two subgroups: SG1: Irrigation with NaClO and EDTA and SG2: Irrigation with Endosolv R. After the complementary irrigation was completed, each sample was sectioned in vestibular-lingual direction to visualize the canalicular dentil wall and tubules. This observation was first carried out using a stereoscopic microscope (Macroscopic Study) and later using a Scanning Electron Microscope (Microscopic Study). For statistical purposes, the Analysis of Variance (ANOVA), Repeated Measures (MR) model of two factors, Inter and Intra-subjects, was applied with a significance level of p <0.05. It was complemented with a Generalized Linear Regression model, including the following factors: Radicular Third, Irrigation Technique and Irrigant Solution. Results. The subgroups that obtained the highest levels of cleaning after the supplementary irrigation procedure were those irrigated manually with a syringe and needle; according to the macroscopic study it was the case of those irrigated with the specific solvent Endosolv R (SG 1.2) ; and according to the microscopic study , it was the case of those irrigated with NaClO / EDTA (SG 1.1). The lowest levels of cleaning and highest percentages of tubular obstruction were observed in SG 2.2 irrigated with Endosolv R and Ultrasonic Activation; this pattern was even more evident in the apical third. Conclusions: The methods used to evaluate the cleaning of the root dentin in cases of Endodontic Re-treatment were very different. The macroscopic evidence of a clean and detritus-free surface did not guarantee the existence of a dentin substrate with open tubules free of micro-contaminants. In the macroscopic study, the variable that most influenced to obtain a clean surface was the root third followed by the type of irrigant solution used, while in the microscopic study this situation was reversed: the irrigating solution was followed by the root third as most important variables. The chemical nature of the Irrigant solution had more influence on the cleaning of the dentin surface (macro and microscopically) than the irrigation system used.
