This chapter discusses the way in which Hades is presented in the Iliad. Although the Underworld remains remote and unreachable for the most part, its presence is always felt on the battlefield through the deaths of heroes. Hades also features in the poem in other ways, as, for example, in Iliad, 5 where the cap of Hades is used to provide absolute invisibility for Athena. The overall argument of the chapter is that Homer presents Hades in the Iliad as the realm of absolute invisibility which cannot be reached or transcended even by the gods. In doing so he sets a challenge for his poetic sight, which he takes on in Iliad 23 by temporarily transferring Hades to earth.

Abstract:
The core of the asylum regime is the principle of non-refoulement that prohibits governments from sending refugees back to their persecutors. Governments attempt to evade this legal obligation to which they have explicitly agreed by manipulating territoriality. A remote control strategy of “extraterritorialization” pushes border control functions hundreds or even thousands of kilometers beyond the state’s territory. Simultaneously, states restrict access to asylum and other rights enjoyed by virtue of presence on a state’s territory, by making micro-distinctions down to the meter at the borderline in a process of “hyper-territorialization.” This study analyzes remote controls since the 1930s in Palestine, North America, Europe, and Australia to identify the origins of different forms of remote control, explain how they work together as a system of control, and establish the conditions that enable or constrain them in practice. It argues that foreign policy issue linkages and transnational advocacy networks promoting a humanitarian norm that is less susceptible to the legal manipulation of territoriality constrains remote controls more than the law itself. The degree of constraint varies widely by the technique of remote control.

Telemedicine overcomes barriers to the delivery of health care services including, distance, cost, gaps in distribution and availability of providers. Anesthesia and surgery applications include remote robotic anesthesia and surgery, preoperative assessment at a distance, physiologic monitoring, remote mentoring, and comprehensive critical care services. The Patient Protection and Affordable Care Act and other legislation supports expansion of telemedicine by enabling telemedicine regulation and reimbursement. Tele-Anesthesia applications are expanding and future economically viable telehealth programs will address a variety of anesthesia specific challenges and domains including pain management, postoperative assessment, just in time training for new clinical applications and anesthesia procedures. Telemedicine techniques include synchronous and asynchronous interactions, virtual presence, mobile-health, and haptics enabled robotics, which are used alone or in combination. Focus areas for development of efficient and effective new Tele-Anesthesia programs include chronic care, remote emergency care, and disaster support. Provider acceptance in tele-anesthesia requires education and training to achieve the full advantages of telemedicine.

Non-operating room anesthesia (NORA) is increasingly prevalent and presents unique challenges to the anesthesiologist. NORA includes procedures performed by gastroenterologists, interventional cardiologists, interventional pulmonologists, radiologists, and surgeons. Members of the anesthesia team must maintain a level of vigilance in the specialized areas where these procedures are performed that is equal to that in a traditional operating room while making accommodations for specialized equipment and rapid turnover. Anesthetic techniques must be chosen so as not to interfere with the proceduralist’s ability to diagnose and treat the underlying problem. In order to ensure optimal safety standards in NORA settings, anesthesiologists should maintain open channels of communication with procedural staff and should become involved in the design and outfitting of NORA locations.

Cancer produces endocrine and metabolic complications in two ways. Firstly, the primary tumour or its metastases may interfere with the function of endocrine glands, kidneys, or liver by invasion or obstruction. Secondly, tumours may give rise to remote effects without local spread and these actions are termed paraneoplastic manifestations of malignancy. Generally, these paraneoplastic syndromes arise from secretion by tumours of hormones, cytokines, and growth factors, but also occur when normal cells secrete products in response to the presence of tumour. This chapter reviews the pathogenesis, epidemiology, and management of the commonest paraneoplastic endocrinopathies including hypercalcaemia, Cushing’s syndrome, the syndrome of inappropriate antidiuresis, non-islet cell tumour hypoglycaemia, enteropancreatic hormone syndromes, Carcinoid syndrome, phaeochromocytoma, gonadotrophin secretion syndromes, prolactin and oxytocin secretion, and paraneoplastic pyrexia. The chapter concludes with a brief discussion of the management of metabolic disease in the context of advanced malignancy including hyperglycaemia, thyroid dysfunction, metabolic bone disease, renal failure, liver failure, and lactic acidosis.

Colonization of Remote Oceania resulted in the discovery of thousands of islands spread across an enormous area of the Pacific Ocean. Beginning as early as approximately 3500 cal. B.P. in Western Micronesia, populations began an expansion westward eventually settling East Polynesia over two millennia later. Although this general pattern is well-established, the reliability of colonization chronologies for particular islands and island groups varies significantly. This chapter synthesizes and critiques current interpretations of radiocarbon and other dating estimates for colonization of the major islands across the region and provides recommendations for future research and chronology building, highlighting the potential for Bayesian analyses. Estimates for the colonization of Hawai'i are presented as a case study.

Settlement pattern archaeology has had a major impact on archaeological research in Oceania. This article reviews the history of the settlement pattern approach in Polynesia and provides case studies from the archipelagos of Samoa and Hawai’i. The primary theoretical and methodological foundations and limitations of settlement pattern archaeology are discussed. Recent technological innovations in spatial analysis, including remote sensing, computer analysis, and geographical information systems, are presented. Finally, the chapter concludes with a brief discussion of current and future avenues of development for settlement pattern studies, including the use of remote sensing technology and non-site approaches to archaeological survey and recording.

In this chapter the way Hades is portrayed in the Odyssey is explored. Following the discussion from Chapter 1, it is argued that Hades’ invisibility is literally spelled out in the Odyssey, bringing to the forefront the poetic challenge Homer’s gaze had already faced in the Iliad. Hades, by being the most remote realm within the—already remote from Olympian influence—universe of Odysseus’ ‘Apologoi’, presents the poet with the most difficult poetic goal to be conquered. Homer, by sending his protagonist into Hades, gains access to the invisible realm, which further means that he and the audience can access the stories of the past, thus initiating the poetics of Hades.

Functionalism in the theory of mind requires an account of function that has a normative component—mere conditional connection (whether indicative or sub-junctive) is not enough. For instance, a component of a computing system isn’t an adder just in case its output is always or would always be the sum of the inputs, since any computing system in a world with as much indeterminism as ours can err or malfunction. Two general reductions of normative language have been proposed that one might wish to apply to the problem of defining proper function: the evolutionary reduction (Wright, Millikan) and the agential reduction (Plantinga). We argue that whatever the merits of the reductions in other contexts, a functionalist theory of mind that defines proper function in either of these ways must fail. The argument proceeds by first showing the agential reduction is viciously circular in the context of a functionalist theory of agency. Second, if functionalism about mind is true and proper function is reducible evolutionarily, then it is possible to have a situation in which the presence or absence of mental properties depends in an implausibly spooky, acausal way on remote facts. It is plausible that the only currently avail-able way for the functionalist to meet these challenges is to accept irreducible end-directed causal powers of minds and/or their functional parts, in accordance with a broadly Aristotelian tradition.

Although uncommon, a trapped central venous catheter (CVC) can present a significant problem for the interventionalist and pose considerable risk to the patient. The use of chronic long-term CVCs is on the rise, with an average catheter dwell time of 10 months. Although all CVCs are prone to complications, chronic catheters exhibit a higher rate of complications. Chronic catheters are also at risk of becoming trapped, whereby they cannot be removed by standard technique. A simple, elegant technique to remove a trapped CVC via inserting an angioplasty balloon into the lumen of a stuck catheter has been described, as has a modified technique with inclusion of a hemostatic sheath and a stiff guidewire inserted into the cut catheter to ensure hemostasis, prevent air embolism or endoluminal thrombosis, and avoid injury to the central veins and heart. Examples of both applications are provided in this chapter.

Abstract:
This book is a philosophical exploration of the various facets of the problem of hell, the reasons that the usual responses to the problem are unsatisfying, and the way that an adequate solution to the problem can be constructed. What drives discussion of the problem of hell, most fundamentally, is the question of why a perfectly good and loving God would consign anyone to eternal suffering in hell. Four main lines of response have been developed to answer it—viz., traditionalism, annihilationism, the choice model, and universalism—but, for different reasons, each of these standard options ends up being deficient in some crucial respect. The alternative view that the author defends in this book, the divine presence model, stands within the tradition that understands hell to be a state of eternal conscious suffering, but develops this idea in a way that is able to avoid the worst problems of its counterparts. The key idea is that the suffering of hell is not the result of any divine act that aims to inflict it, but rather the way that a sinful creature necessarily experiences the unmitigated presence of a holy God. Heaven and hell are not two “places” to which the saved and damned are consigned, respectively, but instead are two radically different ways that different persons will experience the same reality of God’s omnipresence once the barrier of divine hiddenness is finally removed.
