Abstract:
The Jewish Orphanage in Leiden was the last one of 8 such care homes to open its doors in The Netherlands before the Second World War. After spending almost 39 years in an old and utterly inadequate building in Leiden’s city centre, the inauguration in 1929 of a brand-new building, shown on the front cover, was the start of a remarkably productive and prosperous period. The building still stands there, proudly but sadly, to this day: the relatively happy period lasted less than 14 years. On Wednesday evening, 17th March 1943, the Leiden Police, under German instructions, closed down the Orphanage and delivered 50 children and 9 staff to the Leiden railway station, from where they were brought to Transit Camp Westerbork in the Northeast of the country. Two boys were released from Westerbork thanks to tireless efforts of a neighbour in Leiden; one young woman survived Auschwitz, and one young girl escaped to Palestine via Bergen-Belsen. The 55 others were deported to Sobibor, not one of them survived. Some 168 children lived in the new building at one time or another between August 1929 and March 1943. This book reconstructs life in the orphanage based on the many stories and photographs which they left us. It is dedicated to the memory of those who perished in the holocaust, but also to those who survived. Without them this book could not have been written.

This work is about what matters in survival, that is, about what relation to a future individual gives you a reason for prudential concern for that individual. For common sense there is such a relation and it is identity, but according to Parfit, common sense is wrong in this respect. Identity is not what matters in survival. In this work, it is argued that this Parfitian thesis, revolutionary though it is, does not go far enough. The result is the highly radical view, “Survival Nihilism,” according to which nothing matters in survival. Although we generally have motivating reasons to have prudential concern, and perhaps even indirect normative reasons for such concerns—such as a commitment to find a vaccine for the Covid-19 virus—there is no relation that gives you a basic, foundational normative reason for prudential concern. This view goes beyond what Parfit calls the Extreme View. It is the More Extreme View, and is, in effect, something like an error theory about prudential reason as a special kind of normative reason.

Soft tissue sarcoma (STS) is a rare tumor, occurring in approximately one to four of every 100,000 individuals worldwide. Soft tissue sarcomas can form anywhere in the body, including muscle, tendons, fat, blood vessels, lymph vessels, nerves, and tissues around joints. They are most common in the head, neck, arms, legs, trunk, and abdomen. Prognosis is generally poor, with a relative survival rate of approximately 65% at five years, with little difference by race. Approximately 11,930 cases and 4,870 deaths from STS occurred in the United States in 2015. The etiology of STS is still poorly understood, which makes prevention of this relatively rare cancer difficult. A major complication in studying STS is the histologic diversity —more than 100 subtypes. Newer investigations are evaluating molecular characteristics and prognostic factors but continue to be hampered by a lack of standardized histology.

Squamous cell cancer (SCC) of the anus is a rare cancer whose incidence appears to be increasing. Anal cancers are more common in women than men ( 1 , 2 ) and just under half the patients are over the age of 65. An indolent natural history and a low rate of distant metastases ( 3 , 2 ) determine locoregional control as the primary aim of treatment. The relative 5-year survival rate is 62 % ( 4 ) , and has changed little for patients treated in the last two decades.

What happens to insurgent social movements that emerge during a democratic transition but fail to achieve their goals? How influential are they? Are they able to survive their initial mobilizing boom? Using the development of the Zapatista movement during Mexico’s democratic transition in the 1990s, this book seeks to answer these questions. The Zapatista movement is probably the best example of an influential and salient insurgent social movement emerging during a democratic transition that successfully mobilized sympathy and support for the indigenous agenda inside and outside of the country, yet failed to achieve its goals vis-à-vis the Mexican state. Why did such an influential movement fail to have its demands fully met? The answer is illustrated using a sliding door analogy to explain how the Zapatista movement developed within almost simultaneous openings and closings of political opportunities for its mobilization, success, and survival. Framing the relative achievements and failures of the movement within Mexico’s democratization is essential to understanding how social movements develop and survive and how responsive an electoral democracy can actually become. As such, this book offers a test of the quality of Mexico’s democracy and the resilience of the Zapatista movement, identifying the extent to which emerging political forces have incorporated dissident and previously excluded political actors into the new polity.

Metastatic epidural spinal cord compression (MESCC) is a potentially disabling complication associated with advanced cancer. To address whether decompressive surgery followed by radiation therapy is superior to radiation therapy alone, this multi-institutional randomized trial compared outcomes among 101 patients with MESCCC. The study assessed functional outcomes such as ability to ambulate posttreatment, length of ambulation and maintained continence posttreatment, survival time after intervention, and additional functional, quality of life, and medication use outcomes. The practice-changing results of this study indicate that patients who received decompressive surgery and radiation had a longer length of posttreatment ambulation (122 days vs. 13 days, P = 0.03), better overall survival (126 days vs. 100 days, Relative risk 0.60, P = 0.033), lower doses of palliative medications, as well as better performance on several other secondary outcomes. From this landmark study, in appropriately selected patients with MESCC, surgery followed by radiation has become the standard of care.

This chapter considers some of the general mechanisms by which artistic figures are consecrated and weighs their relative contribution to the construction of Debussy’s reputation. Drawing on Gladys Engel Lang and Kurt Lang’s analysis of the survival of reputation in the fine arts, four areas emerge that would seem to be particularly relevant to Debussy: (1) the initiatives undertaken by the composer to establish his own legacy; (2) the posthumous reception of the corpus of works left behind; (3) the actions of heirs and family members on behalf of the deceased: and (4) the efforts of the composer’s close friends and collaborators. Yet, as Chapter 1 demonstrates, the first two were rendered less effective because of the particularities of Debussy’s case—namely, his protracted illness and his death during the First World War.

Melanoma is a cancer of melanocytes, cells that produce the brown-black skin pigment melanin. Melanocytes originate in cells of the neural crest and migrate during embryogenesis, principally to the epidermis, eyes, and some mucous membranes (mouth, nose, esophagus, anus, genitourinary organs, and conjunctiva). Cutaneous melanoma afflicts mainly fair-skinned people of European origin, among whom sun exposure is the major cause. Five-year relative survival can exceed 90%. Invasive cutaneous melanoma in US whites occurs mostly on the trunk (34%), and upper limbs and shoulders (26%). Melanoma incidence rates have been increasing predominantly in European-origin populations. Ultraviolet (UV) radiation, from the sun or artificial tanning devices, probably both initiates and promotes melanoma. Nevi are markers of increased melanoma risk and direct precursors in some cases; nevus-prone people may require only modest sun exposure to initiate melanoma. Other risk factors include family history and sun sensitivity.

A critical variable determining the future relationship between China and the United States is the change in the relative balance of power, which depends on the sustainability of China’s economic growth. While China has produced three decades of double-digit growth, this paper argues that China’s rise has peaked. The factors that have contributed to China’s rapid growth, such as efficiency gains produced by market-oriented reforms, practically unlimited access to global markets, and the demographic dividend, are either disappearing or dissipating. Simultaneously, obstacles to future growth, such as systemic corruption, environmental degradation, and demographic ageing, are becoming more salient. Economic slowdown will threaten the survival strategy of the ruling Chinese Communist Party (CCP). Since the 1989 Tiananmen crackdown, the CCP has followed a sophisticated strategy to maintain power. An economic slowdown will likely cause an unravelling of this strategy, which depends on revenues generated by growth for its sustainability.

The non-Hodgkin lymphomas (NHL) are a heterogeneous group of over forty lymphoid neoplasms that have undergone a major redefinition over the last twenty-five years, in part due to advances in immunology and genetics as well as implementation of the WHO classification system. NHLs are considered clonal tumors of B-cells, T-cells, or natural killer (NK) cells arrested at various stages of differentiation, regardless of whether they present in the blood (lymphoid leukemia) or lymphoid tissues (lymphoma). In the United States, the age-standardized NHL incidence rate (per 100,000) doubled from 1973 (10.2) to 2004 (21.4) and then stabilized, while five-year relative survival rates improved from 42% in 1973 to 70% in 2004. Established risk factors for NHL or specific NHL subtypes include infectious agents (HTLV-1, HIV, EBV, HHV8, HCV, H. pylori), immune dysregulation (primary immunodeficiency, transplantation, autoimmunity, and immunosuppressive drugs), family history of lymphoma, and common genetic variants identified by genome-wide association studies.

The lack of a precise biochemical definition of acute kidney injury (AKI) resulted in at least 35 definitions in the medical literature, which gave rise to a wide variation in reported incidence and clinical significance of AKI, impeded a meaningful comparison of studies.The first part of this chapter describes and discusses different definitions and classification systems of AKI. Patient outcome and the need for renal replacement therapy are directly related to the severity of AKI, an observation that supports the use of a categorical staging system rather than a simple binary descriptor. The severity of AKI is commonly characterized using the relative changes in serum creatinine and urine output. Recently introduced staging systems including the RIFLE classification and the Acute Kidney Injury Network (AKIN) use these relatively simple and readily available parameters allowing the assignment of individual patients to different AKI stages. More recently, a Kidney Disease: Improving Global Outcomes (KDIGO) workgroup developed a consensus-based AKI staging system drawing elements of both RIFLE and AKIN. The potential pitfalls and limitations of the proposed definitions and classifications are briefly described.The second part of the chapter describes the epidemiology of AKI in different clinical settings; the intensive care unit (ICU), the hospitalized population, and the community. The different spectrum of AKI in the emerging countries is discussed and the most important causes and aetiologies of the major clinical types of AKI, prerenal, renal, and post-renal are summarized in table form. Finally the patient survival and renal functional outcome of AKI are briefly discussed

Abstract:
Canonical and non-canonical gospels are typically studied in relative isolation from each other. The book shows why this need not be the case. Early Christian authors produced a mass of gospel literature to meet the demands of a growing Christian reading public for ever more material relating to Jesus and his earthly existence, and out of that proliferating body of work a consensus formed around a fourfold gospel whose originally anonymous components were ascribed to Matthew, Mark, Luke, and John. Yet a significant number of marginalized non-canonical texts have survived, in whole or in part, and the canonical boundary should not inhibit exploration of their relationship to their historically more successful counterparts. Thus the purpose of this book is to trace some of the many thematic similarities and differences within the field of early gospel literature, and to develop an interpretative practice that respects the integrity of that field.

Andy Grove enhanced the microprocessor manufacturer Intel in the 1980s–90s by learning to adapt to the crises that he later termed ‘strategic inflection points’. The first section describes how and what CEO Grove learnt from these ‘inflection point’ adaptive crises. The second section describes his three-stage adaptive framework for dealing with these adaptive crises. It includes rapid adaptation and two other rational methods, diverse and institutionalized deliberation, plus a pre-deliberation experimental process that ‘delivers’ new product ideas. The third section focuses on this experimentation process, so important for high-tech firms seeking product innovation and diversification. Packard’s memoir The HP Way described the Hewlett-Packard decentralized, multi-divisional, experimentation system that produced a long series of innovations and diversifications. The final section discusses Steve Jobs’s relatively top-down, hands-on approach to experimentation and diversification, which provided Apple with the iPod, iPhone, and iPad.

Abstract:
The title of this chapter derives from Aboriginal elders, whose rich cultural tradition survives, not in written texts, such as the Bible, but in their remarkable ability to “read” the stories/Dreamings, songlines, spiritual presences, sacred sites, and laws “written” on the Australian landscape. Borrowing from this hermeneutical tradition, the chapter focuses on how the narrator of a biblical narrative “reads the landscape,” constructing, and relating characters to, the environment in the context of the plot and perspectives espoused in the plot. It explores the phenomenon of “place” as crucial for an appreciation of location in reading the environment and considers examples of “emplacement,” “displacement” and “re-placement” in key narratives of the Pentateuch. “Place” is ultimately where characters belong in the ecosystem of the narrative. By reading the landscape the chapter examines how the narrator constructs the environment in relation to the plot, characters, and the focus of the narrative.
