Abstract:
Background: Neuropathic pain is an injury or disease of the central and/or peripheral somatosensory nervous system, and it has a significant impact on quality of life, especially since it is often refractory to treatment. Rehabilitative intervention is considered in various guidelines on neuropathic pain treatment, although not in an organic nor detailed way. The aim of this systematic review was to analyze the most indicated therapeutic strategies, providing rehabilitative recommendations in the management of neuropathic pain. Methods: A systematic review was performed according to PRISMA guidelines. The scientific search, carried out until July 2020, considered guidelines in English language of the last thirteen years. Results: Six guidelines were analyzed, from which emerges that a multidisciplinary approach, comprehensive of pharmacologic and nonpharmacologic interventions, should drive neuropathic pain management. A relevant role in non-pharmacological intervention is played by rehabilitation, through an adequate tailored rehabilitation program and physical therapies. Conclusion: This analysis highlights the importance of rehabilitation but also the lack of evidence on various rehabilitative practices. Arises hence the need for further studies in this field to better define a rehabilitative treatment strategy.

The aim of the presented paper is to analyze the approaches to the non-pharmaceutical rehabilitation of the drug addicts based on the example of the Social and Rehabilitative Centre «Roza Vetrov» of the municipal pedagogic institution «Podrostok». The following rehabilitative approaches were used: (1) labor and environmental rehabilitation; (2) group and family psychotherapy; (3) different trainings; (4) supported employment and workplace accommodation. Destigmatization is an obligatory psychotherapeutical procedure starting the rehabilitation. Rehabilitative environment is a special micro social group with adjusted behavior. Trainings are a complex of psycho corrective procedures with different targets. Adaptive psychotherapy is the necessary part of the rehabilitation and bringing the patients back to the micro social group they are used to be a member of. Family psychotherapy is performed by discussing the problems of co-abuse, possible options of manipulative behavior in drug addicts and their families. At the stage of workplace accommodation and professional orientation behavior modes are formed, necessary for successful entry into the professional labor.

The fields of rehabilitation robotics and virtual reality (VR) are becoming a growing area in the clinical rehabilitation of people with motion impairments. These systems have the potential to assess abilities through physiological measurements and modelling activities such as posture, gait, and balance. They can be used as rehabilitative tools by providing patients with task-specific training in a motivating and engaging way too. Although the potential advantages of such systems, until now there is a general limitation of their use in rehabilitative practice. Robotics and VR systems can be challenging, engaging and fun, particularly for children with disabilities, since they are often not very motivated to comply with conventional therapy. The aim of this work is to accurately describe the clinical use of innovative rehabilitative technologies and their use for the development of two technology-based rehabilitation pathways for the treatment of gait disorders following obesity and neurological diseases in treatment of pediatric patients.

The rehabilitation needs of individuals undergoing thoracic surgery are changing, especially as surgical management is increasingly being offered to patients who are at risk of developing functional limitations during and after hospital discharge. In the past rehabilitative management of these patients was frequently limited to specific respiratory physiotherapy interventions in the immediate postoperative setting with the aim to prevent postoperative pulmonary complications. In the past two decades, this focus has shifted toward pulmonary rehabilitation interventions that aim to improve functional status of individuals, both in the pre- and (longer-term) postoperative period. While there is increased interest in (p)rehabilitation interventions the majority of thoracic surgery patients are however currently on their own with respect to progression of their exercise and physical activity regimens after they have been discharged from hospital. There are also no formal guidelines supporting the referral of these patients to outpatient rehabilitation programs. The current evidence regarding rehabilitation interventions initiated before, during, and after the hospitalization period will be briefly reviewed with special focus on patients undergoing surgery for lung cancer treatment and patients undergoing lung transplantation. More research will be necessary in the coming years to modify or change clinical rehabilitation practice beyond the acute admission phase in patients undergoing thoracic surgery. Tele rehabilitation or web-based activity counseling programs might also be interesting emerging alternatives in the (long-term) postoperative rehabilitative treatment of these patients.

Neurological diseases severely affect the quality of life of patients. Although existing treatments including rehabilitative therapy aim to facilitate the recovery of motor function, achieving complete recovery remains a challenge. In recent years, regenerative therapy has been considered as a potential candidate that could yield complete functional recovery. However, to achieve desirable results, integration of transplanted cells into neural networks and generation of appropriate microenvironments are essential. Furthermore, considering the nascent state of research in this area, we must understand certain aspects about regenerative therapy, including specific effects, nature of interaction when administered in combination with rehabilitative therapy (regenerative rehabilitation), and optimal conditions. Herein, we review the current status of research in the field of regenerative therapy, discuss the findings that could hold the key to resolving the challenges associated with regenerative rehabilitation, and outline the challenges to be addressed with future studies. The current state of research emphasizes the importance of determining the independent effect of regenerative and rehabilitative therapies before exploring their combined effects. Furthermore, the current review highlights the progression in the treatment perspective from a state of compensation of lost function to that of a possibility of complete functional recovery.

This paper looks at rehabilitation from a legal perspective and aims to clarify the current meaning and content of the principle of rehabilitation. It does this by focusing on the questions of whether and on what grounds rehabilitation can be considered a positive obligation on the part of the State and, if so, what the consequences are of recognising rehabilitation as a positive obligation.

Background and Aim: Mental health services in Denmark offer various rehabilitative treatment interventions to people with severe mental illness. Complementary and Alternative Medicine (CAM) such as National Acupuncture Detoxification Association (NADA) ear acupuncture are used as rehabilitative interventions. We investigated the use of CAM in four psychiatric residential homes, and whether CAM supports residents’ rehabilitation and personal recovery. Methods: Residents in four selected residential homes participated in a questionnaire study, which elicited information on the use of CAM offered in a residential home setting. The study was conducted from February to November 2017. Recovery scores were assessed using the Mental Health Recovery Measure (MHRM). Results: Of 131 eligible respondents, 68 (52%) participated. CAM was used by 84% of the residents. NADA ear acupuncture (38%) and music therapy (37%) were the most commonly used types of CAM. Conclusions: CAM is a commonly used rehabilitative intervention and more than 50% receiving treatment with CAM believed that it has supported their recovery process.

Offender rehabilitation is a challenging goal that calls for ongoing creative innovations. Amongst is a non-doing rehabilitative initiation that is inspired by spiritual traditions. The aim of this paper is to present an application of non-doing offender rehabilitation that has no declared intention to rehabilitate, carried by a peacemaking Islamic Sufi route. Based on the positive criminology approach, we conducted a qualitative phenomenological study consisted of interviews with 11 ex-prisoners who were employed in the Shadhiliyya-Yashrutiyya Sufi order as construction workers and also with 35 Sufi disciple and leaders. We identified five themes of non-doing: (1) atmosphere; (2) modeling; (3) social inclusion and suspension of judgement; (4) spiritual meaning; (5) feasibility of transformative processes. The discussion presents principles of a model of non-doing rehabilitation in a spiritual community and emphasizes the research innovation in presenting non-doing as a holistic method of inclusion within a transformative faith community.

MS clinical features vary between patients. In approximately 60% of cases, cognitive deficits are associated with motor disabilities, with consequences on both walking and maintaining balance and cognitive efficiency. Multimodal programs are very infrequent for MS patients and cognitive rehabilitation is not provided by the Italian health system, which only favors access to motor rehabilitation. Dual-task studies showed how motor and cognitive skills are closely associated. Therefore, physiotherapy exercises may favor an indirect improvement in cognition. The aim of this study is to understand which rehabilitative approach may increase both cognitive and motor efficiency, avoiding the waste of time and resources. In this multi-site single-blind parallel controlled clinical trial, we will compare three rehabilitative approaches: cognitive training, motor training and combined cognitive–motor training. We also aim to evaluate: whether self-perception and objective improvement correspond; the impact of each rehabilitation program on patients’ QoL, mood and self-perception; and long-term effects. A total of 60 patients will be randomly assigned to one of the three treatments for two 45-min sessions/week for 12 weeks. All participants will undergo a complete cognitive, motor, clinical assessment together with mood, self-perception, and QoL questionnaires before, immediately after and 6 months after the training period.

Il lavoro analizza il ruolo fondamentale delle prognosi nel sistema sanzionatorio penale. Dopo una breve presentazione delle questioni problematiche che emergono nella formulazione delle prognosi, vengono esaminate le singole disposizioni che richiedono al giudice una valutazione del rischio di recidiva e degli effetti special-preventivi della risposta al reato. Sul piano normativo, le tipologie di giudizio prognostico si differenziano a seconda delle informazioni di cui il giudice dispone per prevedere il comportamento futuro dell'autore di reato (o dell'imputato). Dinanzi a questo quadro normativo, l'analisi si sposta sull'importanza del sapere empirico e delle scienze umane per attribuire maggiore affidabilità e controllobilità ai giudizi prognostici. Solo attraverso una formulazione legislativa più accurata delle disposizioni che richiedono una valutazione prognostica e un ampliamento delle basi conoscitive necessarie per effettuarla, sembra possibile dare concreta attuazione alla finalità rieducativa della pena.
The work analyzes the fundamental role of prognosis in the penal sanction system. After a brief presentation of problematic issues that emerge in the formulation of the prognosis, the study examines the provisions that require recidivism risk assessment by. On regulatory level, prognostic judgments differ according to the information available to the judge in order to predict the future behavior of the offender (or defendant). Faced with this regulatory framework, the analysis shifts to the great importance of empirical and scientific knowledge to give greater reliability to the prognostic judgments. Only through a more careful legislative formulation of provisions that require prognostic judgments and the widening of knowledge necessary to carry it out, it seems possible to implement the rehabilitative purpose of the penal sanction system.

Abstract Hearing aid (HA) rehabilitation was studied in northern Finland. The costs of HA fitting were examined at two hospitals, Kainuu Central Hospital and Oulu University Hospital. The patients were visited and interviewed at their home and use of HAs was charted. The benefit of HAs was evaluated using generic and disease-specific questionnaires. The effect of follow-up counselling of HA users on HA use and on the benefit of HAs was studied. The proportion of all HA possessors that are regular users has clearly increased during the past twenty years, and the number of non-users, in particular, has decreased significantly. Only 5.3% of first fitted HAs were not in use in 2001. The costs of HA fitting in 2000 were approximately € 900. There was not much difference in the costs between Kainuu Central Hospital and Oulu University Hospital. The price of a HA accounted for somewhat less than half of the total hospital costs, and the costs of the audiology personnel made up roughly a third of the overall costs. Emotional problems of HA users were significant before HA fitting, but six months after HA fitting the number of patients who felt handicapped by their hearing problems had decreased significantly. This could be seen in the results of both the social and the emotional items of the disease-specific health-related quality-of-life (HRQOL) measure, the short version of the Hearing Handicap Inventory for the Elderly (HHIE-S). The benefit could not, however, be shown with the generic HRQOL instrument, the EuroQol questionnaire (EQ-5D), which apparently lacks sensitivity for measuring changes brought about by audiological intervention. Follow-up counselling of HA users can significantly increase HA use and decrease the number of non-users. It can also significantly increase the users' handling skills. The cost of follow-up counselling is approximately € 83 per fitted HA, which is an 8.7% increment to the calculated cost of fitting a HA.

Abstract:
This thesis details the development of a dual robotic system for upper limb rehabilitation after stroke. The literature shows that stroke is major cause of upper limb impairment in the UK. Rehabilitation is used to affect long term recovery of upper limb function. However, health services have limited resources and struggle to provide sufficient contact time for effective rehabilitation. Robotic rehabilitation technologies offer an opportunity to augment existing rehabilitation practice and improve the treatment recieved by patients. The motivation for this research stems from the need for a robotic rehabilitation system 'that can fully support and assist the upper limb without restricting movement. A prototype single robot rehabilitation system was considered. Testing through simulation and physical models revealed a number of limitations. A dual robot system was developed from the single robot system to address its deficincies. A novel cooperative admittance control scheme was developed for the dual robot system. When compared to the single robot system the dual robot system showed a number of distinct advantages; the' ability to actively support the upper limb, coordination of natural movement patterns, the capacity to target assistance to individual upper limb joints. Preliminary trials with stroke subjects showed that the dual robot system did not impair normal movement patterns and, using a basic control mode, could improve reach movements.

The aim of this project was to design and build a tabletop robot that could move the arm of a patient with limited motor function around on a table in any given trajectory to aid the patient in regaining function. The design that resulted from bench level experiments was that of an arm brace mounted to a moving base. The base needed to be omni directional to accommodate all possible trajectories of motion, the arm brace needed to be able to move vertically as well as be flexible enough to accommodate yaw, pitch and roll of the forearm. After choosing a three-wheeled design using TransWheelsTM, the base of the robot was built and programmed by Adam Kraft. The arm brace, which I designed and built, had a rack and pinion setup with a variable voltage regulator to control its vertical motion and a foam and linear spring combination to allow for yaw, pitch and roll while still providing for support. Testing of the prototype proved extremely valuable in refining the requirements of the robot as well as the design. Issues that were discovered during testing of the robot included drift of the robot, the inability to orient the robot the same each time, the slipping of the pinion on the rack if too much downward force is applied to the arm brace and the stiffness of the arm brace during yaw, pitch and roll.
(cont.) Several suggestions were made for possible solutions to the issues, all which seem very feasible to implement. As it is the robot can only move the patient's arm, the patient cannot move the robot since the motors are not back-drivable. This is an impediment in allowing the patient to initiate movement, which is a critical part of therapy. Solutions were proposed that are worth further examination to determine their feasibility. In addition, with a few changes, the robot act as a guide to move a patient's passive arm along a preprogrammed trajectory to aid the patient in performing tasks such as reaching. Even if the robot is unable to match its initial goal, it has great potential to become a valuable asset to stroke patients with limited arm motor function.

Abstract:
The long-term outcomes of subjects fitted with a bone anchored hearing aid (BAHA) for a unilateral profound sensorineural hearing loss (UPSHL) are still evolving. Previous studies have focused on the comparison between shortterm outcomes obtained with hard-wired contralateral routing of signal (CROS) hearing aids and those obtained with BAHA devices. Published results on subjects who have worn their BAHA devices for UPSHL for more than twelve months are limited. This study explored the long-term outcomes of adults fitted with a BAHA for UPSHL. The aims were firstly to examine subjects' pre-operative and postoperative speech perception in quiet and noise, as well as administer two standardised questionnaires, the Abbreviated Profile of Hearing Benefit (APHAB) and the Glasgow Hearing Aid Benefit Profile (GHABP). The second aim was to evaluate the responses of implanted subjects following the preoperative test protocols using a supplementary questionnaire, the Single Sided Deafness Questionnaire (SSDQ). The third aim was to monitor the subjects' implant or repair issues. In addition, questionnaire results were compared to subjects who underwent pre-operative assessment but were not implanted. All subjects had a UPSHL resulting from various aetiologies including vestibular schwannoma or other skull base tumour removal, viral infections, cochlear trauma, idiopathic sudden hearing loss, and Meniere's disease. There was a significant difference between the implanted groups' pre- and post-operative outcomes measures, indicating a treatment effect from the fitting of the BAHA device. No significant changes were found with the non-implanted groups' longterm outcome measures in regards to their perceived hearing difficulties. No significant correlations were found between outcome measures and gender, age of fitting, length of deafness, or ear affected for either group.

Humanitarian aid workers live and work in harsh circumstances far from loved ones and support mechanisms. The problem is that international aid must continue to work effectively despite stress levels. The purpose of this phenomenological study was to understand how aid workers perceive their work-related stressors, examine their subsequent experiences of in-house stress management services, and describe the factors that influence aid workers' decisions to access in-house stress management services. The conservation of resources theory was used to understand aid workers experience of stress. A purposive sampling technique was used to identify 12 aid workers with a minimum of 5 years of field experience. Data were collected through semistructured interviews conducted via Skype. After the data were reviewed, unit meanings were assigned and grouped to develop themes. The themes generated were organizational culture, social support, operational environment, the aid worker, adapting and strategizing, stress management services, and services. The greatest stressors participants reported related to the lack of safety and unpredictable working environments, and their experience of accessing stress management services were varied. Assurances of confidentiality and professionalism were the key factors that influenced aid workers engagement with stress management services. Participants received a summary of the findings which included recommendations for aid organizations on improving stress management services for aid workers. The results of this study contribute to the body of knowledge regarding the well-being of aid workers, supporting social change to improve the quality of care for the affected populations they serve.

Abstract:
Impaired motor function following neurological injury may be overcome through therapies that induce neuroplastic changes in the brain. Therapeutic methods include repetitive exercises that promote use-dependent plasticity (UDP), the benefit of which may be increased by first administering peripheral nerve stimulation (PNS) to activate afferent fibers, resulting in increased cortical excitability. We speculate that PNS delivered only in response to attempted movement would induce timing-dependent plasticity (TDP), a mechanism essential to normal motor learning. Here we develop a brain-machine interface (BMI) to detect movement intent and effort in healthy volunteers (n=5) from their electroencephalogram (EEG). This could be used in the future to promote TDP by triggering PNS in response to a patient’s level of effort in a motor task. Linear classifiers were used to predict state (rest, sham, right, left) based on EEG variables in a handgrip task and to determine between three levels of force applied. Mean classification accuracy with out-of-sample data was 54% (23-73%) for tasks and 44% (21-65%) for force. There was a slight but significant correlation (p<0.001) between sample entropy and force exerted. The results indicate the feasibility of applying PNS in response to motor intent detected from the brain.

Cognitive impairments, such as memory and intelligence, attention and mental performance, usually suffer in diffuse brain damage, in particular in chronic cerebral ischemia. However, they can also occur in focal brain lesions, such as cerebral infarcts, reaching the degree of dementia. The aim of the study was to investigate the peculiarities of non-dementia cognitive disorders in patients with the consequences of cerebral infarction. We examined 30 patients with chronic cerebral ischemia with cerebral infarction at the age of 58-66 years (M=62.4) and a similar comparison group was chosen. The main results of the cognitive status assessment in patients with CHEM with suffered cerebral infarction showed a decrease in short-term visual and figurative memory, more so in men and with increasing age. There were also decreased productivity of attention, decreased level of abstract thinking, and weakened connection between logical thinking and attention, more pronounced in men and in the older age group. With a disease duration of more than 3 years, the volume of figurative memory and the abstract component of thinking decreased. The main conclusions are: cognitive disorders developing against the background of CIM worsen to a greater extent in men, in persons older than 60 years and in 3 years from the moment of cerebral infarction; short-term medication therapy has no significant effect on cognitive dysfunction, the significance of these results determines the need for long-term, at least 6 months, rehabilitative medication therapy.

Review question / Objective: What is the current status of implementation of interventional programs on early functional rehabilitation during acute, hospital-based medical care, specifically tailored for older patients with CI and what are the most appropriate programs or program components to support early rehab in this specific population? This study combines a systematic umbrella review with a scoping review. While an umbrella review synthesizes knowledge by summarizing existing review papers, a scoping review aims to provide an overview of an emerging area, extracting concepts and identify the gaps in knowledge. The study focuses on older hospitalized adults (>65 yrs.) receiving ward based early rehabilitation. The focus within this review is on study participants with cognitive impairment or dementia. The study targets at controlled trials independent of their randomization procedure reporting on an early functional rehabilitation during hospitalization. Trials that were conducted in different or mixed settings (e.g. inpatient and aftercare intervention) without a clear focus on hospital based rehabilitation were excluded. The study aim is to identify the presence of CI specific features for early rehabilitation including: CI/dementia assessment, sub-analysis of results according to cognitive status, sample description defined by cognitive impairment, program modules specific for geriatric patients CI.

Aim: This study aimed to explore how qualitative data about rehabilitation for patients with intermittent claudication do provide an enhanced understanding of the quantitative experimental results. Background: The study was a randomized clinical trial comparing a rehabilitation intervention with usual care. A statistically significant difference between rehabilitation and usual care was found in walking distance, physical activity, quality of life and diet. The findings from the quantitative and qualitative analyses were analysed separately on their own tradition. In this study, mixed methods address whether the qualitative results could help explain the quantitative results and bring forward additional information. Design: Complex mixed-method intervention design with a convergent questionnaire variant. Methods: From April 2017- May 2019, patients diagnosed with intermittent claudication were included in a randomized clinical trial (N=118). In addition, qualitative interview informants from the intervention group were sampled from the quantitative study population for a survey (N=43) and focus group interviews (N=10). Interviews were conducted from April 2018-August 2019. Results: Integrated analyses identified how improvement in walking distance, physical activity, diet and quality of life was affected by team spirit, pedometer, education and fun exercise in a local setting. Quantitative and qualitative findings primarily confirmed and expanded each other; however, two discordant results were also evident. Conclusion: Our study adds empirical evidence regarding how a mixed-methods study can be used to obtain a more nuanced understanding of complex healthcare problems. The study provides new knowledge concerning how to set up a rehabilitation programme for patients with intermittent claudication.

This paper explores three case studies of the Al-Nouri Mosque, Al-Hadba Minaret, and Lalish Temple in the aftermath of the ISIS war of 2014–17 and the occupation of Nineveh governorate. It analyses the role of these sites as part of northern Iraq’s heritage and the mechanisms in place for their reconstruction and preservation. The paper explores the role of the different actors and how these have influenced different understandings of heritage and therefore different responses and approaches to restoration. It argues for the role that international organisations such as UNESCO have in monitoring reconstruction processes, as well as the role of international aid in heritage protection, along with the importance of community consultations and of responding to the needs of affected communities in the rehabilitation process.

The purpose of our critically appraised topic is to combine the best evidence regarding the long-term outcomes in children with acute flaccid myelitis (AFM) regarding posture and movement, gross and fine motor control, and activities of daily living (ADL) performance. The final portfolio contains eight articles. The study designs of these articles include a retrospective cohort study, two retrospective non-randomized studies without a control group, a retrospective review, a nationwide follow-up questionnaire analysis study, a case report, a case series, and a multiple quantitative case study. All studies related directly to our evidence-based PICO question and were used to determine the best evidence of the long-term outcomes in children with AFM. Overall, our findings showed that functional improvements were seen in most i ndividuals, however, this varied from complete to incomplete recovery along with some persistent motor and functional deficits. Every case is different depending on when they were diagnosed, and how quickly they were able to implement a rehabilitation program into their everyday routine.

Condition being studied: Currently there is an increase in cases of dental wear, due to several factors such as: excessive consumption of carbonated drinks, a diet high in acids, gastric diseases, anorexia, bulimia, dental grinding, use of highly abrasive toothpastes, or a combination of these(9) (10) (11) (12); which affect the patient in several aspects: loss of vertical dimension, sensitivity due to the exposure of dentin, esthetics, affectation of the neuromuscular system(11) (13) (14). With the advent of minimally invasive dentistry, occlusal veneers have been found to be a valid option to rehabilitate this type of cases and thus avoid greater wear of the dental structure with full coverage restorations. Sometimes when performing a tabletop it is not necessary to perform any preparation, thus preserving the maximum amount of dental tissue(3) (6) (15). Due to the masticatory load either in patients without parafunction where the maximum masticatory force is approximately 424 N for women and 630 N for men or in those who present parafunction where the maximum bite force can vary from 780 to 1120N(7), it is necessary that the occlusal veneers support that load which makes indispensable a compilation of studies investigating both fatigue and fracture resistance and the survival rate of occlusal veneers in different materials and thicknesses.

In 2017, the State of California adopted landmark legislation to increase the funds available for transportation in the state: Senate Bill 1 (SB1), the Road Repair and Accountability Act of 2017. Through a combination of higher gas and diesel motor fuel taxes, SB1 raises revenue for four critical transportation needs in the state: road maintenance and rehabilitation, relief from congestion, improvements to trade corridors, and improving transit and rail services. To help state leaders identify the most important projects and programs to fund within those four topical areas, we conducted an online survey that asked a sample of 3,574 adult Californians their thoughts on how the state can achieve the SB1 objectives. The survey was administered from April to August 2019 with a survey platform and panel of respondents managed by Qualtrics. Quota sampling ensured that the final sample closely reflects California adults in terms of key socio-demographic characteristics and geographic distribution. Key findings included very strong support for improving all transportation modes, reducing air pollution and greenhouse gas emissions from transportation, and more convenient options to travel without driving. Respondents placed particular value on better maintenance for both local streets and roads, as well as highways. Finally, the majority of respondents assessed all types of transportation infrastructure in their communities as somewhat or very good.
