In this paper we introduce a series of articles which describe user centred practice in a south London community-based rehabilitation service for the severely mentally ill. We emphasise the medical consultation style adopted, the service management style and specific initiatives allowing users to influence our practice and shape functional and structural components of the service.

Abstract:
The purpose of this paper is to review scientific evidence for the co-occurrence of major depressive illness and disability, and to examine this phenomenon specifically for women in the United States today. Following a discussion of different ways of operationalizing the concept of disability, the analysis addresses gender biases in disability measurement as well as in recent research on depression and functional impairment. Next, the results of research regarding co-occurrence of disability and depression are reviewed, highlighting those studies focused specifically on women. Turning to research on rehabilitation services for those with psychiatric disabilities, studies suggest that service delivery models fail to address needs specific to women with mental illness in general, as well as those women experiencing severe depression. Thus, the analysis concludes with a series of suggestions and future directions for investigators seeking to better understand the linkages between depression, disability, and rehabilitation services for women.

<p>The improvement of rehabilitation service capacity is an important part of the construction of a healthy China, and intelligent technology is a powerful means of rehabilitation development. This paper reviews the background of a series of national policies for the construction of a healthy China, analyzes and summarizes the many shortcomings that currently restrict the improvement of rehabilitation service capabilities, and proposes the implementation path of intelligent rehabilitation. By expounding the service process of intelligent rehabilitation, and analyzing in detail the intelligent technical means suitable for integration from the four key links of real-time health monitoring, remote home intelligent rehabilitation intervention, health classification evaluation standard system and health intervention standard system, the general framework of implementation path of intelligent rehabilitation is built. Taking hypertension rehabilitation as an example, the article introduces the intelligent rehabilitation practice exploration and reference model in three aspects: The research and development of hypertension intelligent equipment, the clinical research of hypertension rehabilitation and the construction of hypertension rehabilitation database. Finally, combined with the concept of intelligent interconnection of all things, the definition of “rehabilitation Internet of things” is proposed, and the time is right for intelligent rehabilitation in the context of building a healthy China.</p>

AbstractSexuality and intimacy are important aspects of life that are frequently compromised after severe injury or illness, yet these aspects are often overlooked by medical and rehabilitation professionals. This case series describes the Occupational Therapy Sexuality and Intimacy program at a Military Treatment Facility (MTF). Three diverse clients with a range of physical, cognitive and emotional impairments were chosen to illustrate complexities of the Occupational Therapy Sexuality and Intimacy Program at this MTF, and unique skills employed by Occupational Therapists. Consistent themes discovered include: perceived value of the program; appreciation of safe spaces to discuss personal topics; and enhanced awareness of role identity, body image, and emotional regulation. These cases illustrate that sexuality and intimacy interventions may have profound effects on injured service members, improving social reintegration, and quality of life.

There is a paucity of research concerning service usage and needs of female long-term psychiatric patients. A series of studies comparing the provisions for chronically mentally ill men and women in a south London community-focused rehabilitation and continuing care service indicate marked differences in the services received by men and women, and raise questions concerning whether the needs of female patients are adequately served. As a group the women had been in contact with services for longer, had received less intensive input from services, and it appeared that the services had been less responsive to their changing needs. Among those in high contact with the services, the functioning of men and women did not differ, yet women were over-represented in workshops designed for those functioning at a lower level. Women over 45 years of age seemed to be particularly badly served by the organised activities offered.

ABSTRACTIn view of the growing numbers of physically and developmentally handicapped older persons in Canada, this study attempts to determine the effects of basic client characteristics on the delivery of services to these groups. Using data from the 1984 Disability Information System of Calgary, a series of models are estimated which focus on the characteristics of age, gender, geographical location, and level of physical and cognitive functioning as determinants of the receipt or non-receipt of residential, professional, support and day program services. The results indicate that there is a weak relationship between age, gender and level of cognitive functioning and the overall receipt of services. However, when each service is analyzed separately, only day programs are adequately explained. As was anticipated, it appears the needs of the elderly handicapped are not being met within the rehabilitation service system.

Commonwealth Rehabilitation Services (CRS) Australia provides vocational assessment services to a client group whose rehabilitation is funded by Family and Community Services. Currently, the assessment of client skills, abilities and needs is based on the case management model. This model was expanded in the Assessment Plus study with the addition of a four-week assessment schedule and a series of educational group sessions. The program was conducted by a multi-disciplinary team at CRS Australia's Maroubra Unit, NSW. Forty-six CRS clients meeting inclusionary criteria formed the subject group for the study. Data was collected for the first eight months following implementation of the Assessment Plus program. The outcomes for study participants were compared with a matched group of 46 clients referred for assessment during the same period in 1999 prior to the implementation of Assessment Plus. The results of the study program showed a statistically significant increase in employment outcomes in the Assessment Plus group, with 12 (26%) clients recorded in employment compared with 3 (6%) in the control group. There was also an improvement in service delivery factors. These included a shorter assessment phase, an increase in the number of vocational goals identified and required reports filed. The results suggest that providing a time-based assessment frameworkplusintensive client involvement and participation improves vocational outcomes and service delivery.

The Ministry of Justice’s Consultation Paper – Strengthening Probation, Building Confidence – launched by Justice Secretary David Gauke in July 2018, represents a revisionist view of the recent history of the probation service in which many of its assertions are incoherent, disingenuous and disconnected from the lived realities of both those who offend and local communities having to deal with the impact of austerity on local services. In addition, the consultation process itself is disingenuous in that it presents the failure of the Transforming Rehabilitation initiative as one of technical oversights and misjudgements that can be put right through a series of relatively minor adjustments. Answers to the 17 consultation questions, however insightful and helpful they may be, will do nothing to deal with the underlying difficulties of Transforming Rehabilitation.

<p>The Seal Island Bridge is a steel through-arch truss bridge in Cape Breton Island, Nova Scotia, Canada with a main span of 152 m. At over 60 years in service, the bridge is near the end of its design life and showing structural and operational difficulties such as a previously failed truss diagonal, cracked floorbeams, steel material property complexities, wind-induced vibrations, and restricted access due to narrow deck geometry. A series of bridge inspections were performed which included visual inspections, non-destructive testing, and material testing. The inspections revealed the presence of tack welds and associated cracking, steel corrosion, concrete deterioration, seized bearings, and vibrating bracing elements. Additionally, a structural health monitoring (SHM) program was implemented to determine the current bridge behaviour. To assess the structure, a finite element (FE) model was created and calibrated using the SHM data and the inspection findings. The FE modelling is the focus of this paper. During preliminary analysis, it was determined that the structure was sensitive to wind loading. Therefore, a detailed wind buffeting analysis was performed to refine the wind loading used in the analysis. Based on the results of the analysis and investigations, a rehabilitation plan is currently being developed to ensure that the bridge can remain in service for an additional 15 years. Additionally, a benefit-cost analysis is being performed to assess potential rehabilitation and replacement options for the Owner, the Province of Nova Scotia.</p>

Based on a proposal made by the Architecture and Rehabilitation Service of the Department of Vertebration of the Territory, Mobility and Housing of the Government of Aragon and in collaboration with the City Council of Sierra de Luna (Zaragoza), the San Jorge University was asked the possibility to carrying out a study that would serve as a debate and reflection through the analysis of the physical and constructive state of some buildings in the nucleus and the possibilities of reusing its buildings. All this has been carried out by a group of students of the subjects of Constructionof the second, third and fourth courses (all four-monthly). The study has been developed as a Vertical Workshop, with several visits to the nucleus. Through the analysis of factory injuries, the current regulations, and the conversations held with the owners of the three selected buildings, a series of questions are raised to be taken into account in a future intervention and some possible proposals open to new debates. A partir de una propuesta realizada desde el Servicio de Arquitectura y Rehabilitación del Departamento de Vertebración del Teritorio, Movilidad y Vivienda del Gobierno de Aragón y en colaboración con el Ayuntamiento de Sierra de Luna (Zaragoza), se planteó a la Universidad San Jorge la posibilidad de realizar un estudio que sirviera como debate y reflexión a través del análisis del estado físico y constructivo de algunas edificaciones del citado núcleo y las posibilidades de reutilización de sus edificaciones. Todo ello se ha llevado a cabo por un grupo de estudiantes de las asignaturas de Construcción de los cursos segundo, tercero y cuarto (todas ellas cuatrimestrales). Abstract A 22-inch onshore natural gas transmission pipeline experienced an in-service rupture in January of 2018. The rupture was caused by an axial crack that occurred 150 Km (93 miles) downstream of the closest compression station. While the root cause analysis of the rupture was in progress, excavations of indications reported by a Hard Spot In-Line Inspection (ILI) were undertaken starting approximately 42 km (26 miles) downstream of the rupture. These excavations discovered predominantly circumferential crack colonies, with a number of through-wall cracks being leak. The root cause analysis concluded that both the axial and circumferential external surface cracks are consistent with hydrogen environment assisted cracking generated by the impressed-current cathodic protection system, operated near the pipe-to-soil recommended potential limit of −1200 mV as measured with respect to a saturated copper/copper sulfate reference electrode (CSE). Transportadora de Gas del Norte (TGN) implemented a series of preventive and corrective measures that included pressure testing, cathodic protection improvement, coating rehabilitation programs and In-Line Inspection programs among others. This paper presents the background and approach used in the root cause analysis, summarizes the methodology, results and findings, and discusses their implications in the management of the Pipeline Integrity of the pipeline system operated by TGN.

To further measure the present purpose have been performinga usability test, furthermore scientific experiments withamputees patients. These experiments were carried together byphysiotherapists of the physic rehabilitation service. The evaluationperformed about the tool indicated promising results with ausability and mechanic appropriated, basing the use of PhysioPong.

Abstract:
The Washington Metropolitan Area Transit Authority (WMATA) contracted with Booz Allen Hamilton to conduct a non-destructive structural assessment of the 2000 and 3000 series rapid transit rail cars. The main goals of the assessment were to identify any significant changes that have occurred to key elements of the car body structure since their date of manufacture and to confirm that no significant and obvious structural deterioration or damage exists. The methodology behind selecting a small representative sample of cars is discussed as an introduction to this work. The paper then discusses the process by which several critical areas were selected for non-destructive inspection. One of the biggest challenges in successfully completing this project was not interfering with WMATA’s fleet operating requirements. The use of WMATA facilities had to be coordinated to create minimum interference with WMATA’s daily maintenance activities. Also, the inspection work had to be planned in such a way as to minimize the amount of vehicle component disassembly in order to return the vehicle to revenue service as soon as possible. The inspections produced valuable results regarding the construction and condition of these cars. The structural welds of the car appear to have performed satisfactorily through the operational life of the cars to date without any significant deterioration. Some corrosion was noted in the door opening areas of the cars, particularly at the door thresholds. The findings of this report will be used to target specific areas of the car during the upcoming mid-life rehabilitation project.

Residential rehabilitation for alcohol and other drug dependence aims to provide a structured environment for people to break addiction and reintegrate into society within a community rather than in outpatient or other settings. This Evidence Snapshot aimed to identify the community impacts of residential alcohol and drug rehabilitation services. Ten studies were reviewed, seven of which were from the US and two from Australia. A consistent theme across five studies examining community perceptions and impact was that initial concerns around the potential impacts of drug treatment facilities, including residential rehabilitation, were largely unfounded or did not materialise in the long term. Studies also reported a number of positive impacts on communities as reflected by participation in events at community rehabilitation centres, residents making contributions to communities through volunteering, and longer-term employment and associated economic impacts stemming from successful reintegration into the community following rehabilitation. Three studies examining impacts on property values reported mixed findings. One study reported negative impacts, however two studies demonstrated either no effect or higher sales in houses close to sober-living houses over time. A large study examining crime rates showed that drug treatment centres had similar crime rates to areas around liquor stores and lower rates than near corner and convenience stores. A US-based study of economic impacts reported positive impacts. The review findings raise a number of important considerations. Initial community concerns are not based on lived experience of residential rehabilitation in their area and appear to diminish once the centres are established. A number of gaps in the evidence - including a dearth of studies; lack of exploration of community knowledge; and potential under-measurement of complex outcomes such as community attitudes and sentiment – should be borne in mind when interpreting review findings.

Cracks and their formations in concrete structures have been a common and long-lived problem, mainly due to the intrinsic brittleness of the concrete. Concrete structures, such as rigid pavement and bridge decks, are prone to deformations and deteriorations caused by shrinkage, temperature fluctuation, and traffic load, which can affect their service life. Rehabilitation of concrete structures is expensive and challenging—not only from maintenance viewpoints but also because they cannot be used for services during maintenance. It is critical to significantly improve the ductility of concrete to overcome such issues and to enable better infrastructure quality. To this end, the self-healing cementitious composites (SHCC) investigated in this work could be a promising solution to the aforementioned problems. In this project, the team has designed a series of cementitious composites to investigate their mechanical performances and self-healing abilities. Firstly, various types of fibers were investigated for improving ductility of the designed SHCC. To enhance the self-healing of SHCC, we proposed and examined that the combination of the internal curing method with SHCC mixture design can further improve self-healing performance. Three types of internal curing agents were used on the SHCC mixture design, and their self-healing efficiency was evaluated by multiple destructive and non-destructive tests. Results indicated a significant improvement in the self-healing capacity with the incorporation of internal curing agents such as zeolite and lightweight aggregate. To control the fiber distribution and workability of the SHCC, the mix design was further adjusted by controlling rheology using different types of viscosity modifiers. The team also explored the feasibility of the incorporation of colloidal nano-silica into the mix design of SHCC. Results suggest that optimum amounts of nano-silica have positive influence on self-healing efficiency and mechanical properties of the SHCC. Better hydration was also achieved by adding the nano-silica. The bonding strength of the SHCC with conventional concrete was also improved. At last, a standardized mixing procedure for the large scale SHCC was drafted and proposed.

Because the construction phase accounts for the majority of project costs for pavement rehabilitation projects, most research on infrastructure project cost estimating focuses on that phase, rather than on the preconstruction phases. Nevertheless, costs incurred prior to construction, referred to in this report as "preconstruction costs" are significant and worthy of consideration (See Section 2.1 of the report for a more detailed and precise definition of preconstruction). In the 20202021 fiscal year, for instance, the California Department of Transportation (Caltrans) spent more than $169 million on preconstruction work for pavement rehabilitation projects. This report presents the results of a study of preconstruction cost estimating for pavement rehabilitation projects undertaken by Caltrans. It uses data on the 139 pavement rehabilitation projects for which Caltrans opened bids in the five-year period from April 26, 2016 to May 11, 2021. A data set was developed that combined the preconstruction hours for each project with the primary bid items for the pavement rehabilitation projects. Two models were developed to estimate preconstruction hours from the bid items, one using an Artificial Neural Network (ANN) and the other a parametric exponential model developed using multiple regression. The models had coefficients of determination of 0.85 and 0.80, respectively. Tools were then developed to assist professional users in validating their preconstruction cost estimates using each of the models. CTC staff or Caltrans can use these tools to evaluate the reasonableness of the preconstruction estimate on an individual project, or on the sum of an entire biennial SHOPP pavement rehabilitation portfolio, in order to assure the most efficient use of infrastructure funding to best serve the community's transportation needs.

Problem-solving courts (PSC) are a problem-solving approach targeting the complex needs of individuals within the criminal or family justice systems. Over the last 20 years, PSC have been introduced into the UK to address the personal, social and structural factors underlying behavioural issues that often contribute to re-offending. In June 2023, the Ministry of Justice launched three courts with problem-solving components referred to as Intensive Supervision Courts (ISC). Focusing on rehabilitative outcomes, PSC combine intervention programmes with judicial oversight through regular reviews. By placing judges and magistrates at the centre of rehabilitation, PSC target individuals or families with complex needs, who might not benefit from standard court proceedings and supervision, with an aim to improve long-term life outcomes. This POSTnote provides an overview of PSC in England and Wales. It outlines different PSC and courts with PSC elements that operate within adult criminal courts, family courts and youth courts across England and Wales, drawing data from case studies in the UK and, where relevant, internationally. It also discusses potential challenges to fully implement PSC and their approaches as well as opportunities for more effective implementation of PSC across England and Wales. Key points Key elements of PSC include intensive intervention programmes, that seek to address underlying social and health issues through regular judicial monitoring and cross-governmental collaborative efforts. Several ongoing PSC and courts with PSC elements operate within adult criminal courts, family courts and youth courts across England and Wales. Although there is a substantial international evidence base, there seems to be limited evidence about the effectiveness of PSC in the UK due to inconsistent implementation and evaluation. Challenges to PSC implementation can include costs, lack of funding, limited evidence, procedural issues and lack of widespread judicial engagement. Opportunities for effective PSC implementation include use of existing resources, multi-agency partnerships, advocating for specialist services and a change in culture within the judiciary.
