Abstract:
This chapter documents the critical role of service providers in the development of today's digital media systems. It illustrates how an ethnographic approach to media infrastructures helps to connect hard infrastructural forms, such as wires, transmissions towers, and buildings, with soft infrastructural forms, including institutions, protocols, and social practices. It then focuses on circumvention practices in Turkey. The work of Turkey's cybercafé operators forms a key component of Internet infrastructure, critically shaping the social topography of media in the country. The cafés and their operators coordinate disparate technologies and communities and are sites where different protocols are negotiated. At the same time, in these locations, state infrastructural control, surveillance, and censorship can be undermined.

Abstract:
Populations, as with individuals, have unique patterns of disease. The science of epidemiology, which straddles biology, clinical medicine, social sciences, and ecology, seeks to describe, understand, and utilize these patterns to improve population health. Epidemiology’s central paradigm is that analysis of population patterns of disease, particularly by linking these to exposure variables (risk factors), provides understanding of their causes. Epidemiology is useful in other ways, including preventing and controlling disease in populations and guiding health and health-care policy and planning. Epidemiology can help clinicians to manage the health care of individuals. Epidemiology has a large toolbox. At its core lies the measurement of the prevalence and incidence of risk factors and outcomes. These measurements are generated by study types (designs) that serve the various purposes of epidemiology. Of the many kinds of studies available, the most important are case series (register studies), cross-sectional, case–control, cohort, and intervention (trials) studies.

This chapter provides case examples from the authors’ work within human service organizations that illustrate the importance of addressing OSCs: including culture, climate, and worker attitudes. These examples of the influence of OSC provide the reader with an understanding of how social contexts affect human services quality and outcomes along with implications for improving them. The chapter explains the sensitivity of human service effectiveness to OSC and describes the social processes that explain its influence. Case examples are used to illustrate the influence of shared mindsets and worker attitudes within OSCs. These examples include the influence of shared beliefs, assumptions, and attitudes of service providers on client and staff relationships that affect services quality.

Abstract:
This chapter uses a randomized community development programme in rural Pakistan to assess the impact of citizen engagement on public service delivery and maternal and child health outcomes. The programme had a strong emphasis on organizing women, who also identified health services as a development priority at baseline. At midline, we find that the mobilization effort alone had a significant impact on the performance of village-based health providers. We detect economically large improvements in pregnancy and well-baby visits by female health workers, as well as increased utilization of pre- and post-natal care by pregnant women. In contrast, the quality of supra-village health services did not improve, underscoring the importance of community enforcement and monitoring capacity for improving service delivery.

Abstract:
The role of co-operatives as providers of goods and services, as in the industrial age, more recently became overshadowed by their potential as providers of social services. In the post-industrial or service society, co-operatives are found in a growing number of countries. Co-operative enterprises have a unique capacity to mobilize social capital and provide relational goods that neither public nor private for-profit providers demonstrate. This brings co-operative enterprises full-circle in terms of their historical political role as democratic pioneers, since they can now also contribute to reducing the growing democratic deficit. This chapter explores the political and social dimensions of co-operative enterprises that pursue multiple goals. It also introduces a dynamic model of co-operative development that can be fruitfully employed for analysing the social and political dilemmas faced by co-operative enterprises.
