Abstract:
Humulin 70/30 insulin is an intermediate acting medication used for treatment diabetes mellitus. According to systematic review and there have been forty four case reports of overdoses with different types of insulin. We present a unique case of insulin Humulin 70/30 overdose due to suicidal attempt that presented with persistent hypoglycemia and required prolonged hospital treatment. A47 years old female patient presented to the emergency department due to decreased level of consciousness after self -administration of 2000 international unit Humulin insulin 70/30 suicidal attempt. In the emergency department patient treated with 250 ml D10% then shifted to the medical department. The patient continue treatment with dextrose 10% 200 ml/hour as continuous infusion, unfractionated heparin 7500 international unit/12 hour, hydrocortisone 100mg /6 hours, octreotide 100 microgram /8 and glucagon 1 mg . On the fourth day all medication and D10% stopped and the patient still under observation then discharged on will general condition without hypoglycemia attacks. A single high overdose of Humulin insulin can present with prolonged persistent hypoglycemia that is refractory to D10% alone. All physicians should know the guidelines to treat severe form of hypoglycemia and when to admit into medical department or intensive care unit with follow up for blood glucose and serum electrolyte monitoring.

Abstract Because the strength of structural steel decreases sharply at high temperature, it must be sprayed with fire-resistant coating or fire-resistant materials for composite use. The strength of refractory steel can be reduced by no more than 1/3 of the room temperature at high temperature, and maintain good high temperature strength. This article mainly summarizes the development history of refracto-ry steel in China and other countries, analyzes the material selection basis and strengthening mecha-nism of refractory steel, and finds that by combining Mo element solid solution strengthening, phase transformation strengthening, and Nb, the strength guarantee of refractory steel at high temper-atures and V element precipitation strengthening are obtained. Designers can calculate the service temperature of steel structure through the initial stress and design strength of components to select refractory steel. Finally, the application status of refractory steel and its application prospect in the field of oil rescue are summarized and prospected.

Abstract:
The investigating results are given for the periclase-chromite refractories' composition and structure which are in contact with the pulverized coal and gas medium in the coppersulfide smelting furnaces. The high-temperature burnt copper concentrate and the sulfur dioxide gas suspensions combined action changes the surface and deep refractories layers chemical composition, with that the impurities content reach the value in weight percent: Fe 54,0, Cu 7,2, Zn 6,4, S 1,8. The refractory's surface layer saturation with the iron and non-ferrous metals oxides decreases the porosity and gives rise to low-melting compositions and eutectics. The refractory decomposition is induced by the shelling of the refractory surface layers with the filled porous taking place in course of the heating-cooling cycling because of the phase's thermal linear expansion coefficients. When the spent refractory disposal, it is feasible to separate mechanically the surface layer for the non-ferrous metals extracting, the rest part can be used for obtaining the refractory powder of various purpose.

Abstract:
AbstractMedication overuse headache (MOH), new daily persistent headache (NDPH), and persistent refractory headache attributed to severe acute respiratory syndrome coronavirus 2 (SARS-CoV2) infection represent a significant burden in terms of disability and quality of life, and a challenge in terms of definition, pathophysiology, and treatment. Regarding MOH, prevention without withdrawal is not inferior to prevention with withdrawal. Preventive medications like topiramate, onabotulinumtoxinA, and calcitonin gene-related peptide (CGRP) monoclonal antibodies improve chronic migraine with MOH regardless of withdrawal. The differential diagnosis of NDPH is broad and should be carefully examined. There are no guidelines for the treatment of NDPH, but options include a short course of steroids, nerve blocks, topiramate, nortriptyline, gabapentin, CGRP monoclonal antibodies, and onabotulinumtoxinA. The persistence of headache 3 months after SARS-CoV2 infection is a predictor of poor prognosis.
