Abstract:
There is considerable supportive evidence to suggest that increased levels of DNA damage are associated with an increased risk of developing neoplastic lesions in the human colon and rectum. Within this thesis, several different topics related to DNA damage were explored in detail, principally using the single cell gel electrophoresis assay (the comet assay) to measure DNA damage both in cell line studies and in human colorectal mucosal biopsies from patients undergoing routine endoscopic examinations of the colon and rectum.

Abstract:
Cette thèse qui porte sur le rectum et le tissu périrectal comporte 3 axes principaux : l’étude morphologique du rectum et du tissu périrectal, l’évaluation des possibilités de reconstructions sphinctériennes sans implantation de matériel d’électro-stimulation et l’évaluation de la faisabilité de la modélisation pelvienne dynamique patient spécifique. L’étude morphologique du rectum nous a permis de démontrer la variabilité importante de la longueur du rectum et de la hauteur de la ligne de réflexion péritonéale. Cette variabilité suggère que l’évaluation de la localisation des tumeurs du rectum utilisant une simple mesure centimétrique est insuffisante. L’étude morphologique nous a également permis de démontrer une corrélation entre le nombre de noeuds lymphatiques du rectum et la morphométrie du mésorectum. Cette corrélation pourrait être prise en considération pour déterminer le nombre minimal de noeuds lymphatiques à examiner après la chirurgie pour les malades ayant un cancer du rectum. Dans la deuxième partie de cette thèse nous avons démontré la faisabilité anatomique des reconstructions sphinctériennes utilisant le muscle gracile réinnervé par le nerveux pudendal avec une anastomose nerveuse termino-terminale ou termino-latérale. Cette technique de reconstruction pourrait permettre au muscle transposé de développer les caractéristiques d'un sphincter anal externe et d’intégrer des mécanismes sensitifs. La dernière partie de cette thèse a permis le développement d’un modèle dynamique complet de la cavité pelvienne spécifique à chaque patient. La validation d’un tel modèle est la première étape avant l’élaboration d’un modèle spécifique simulant les troubles de la statique pelvienne d’un malade et leurs corrections. Ce modèle permettra d’évaluer avant l’intervention le résultat fonctionnel de la correction chirurgicale envisagée. Cette thèse est une base pour le développement de travaux cliniques dont le but est d’améliorer la prise en charge et la qualité de vie des malades ayant une pathologie fonctionnelle ou tumorale du rectum
The aim if this thesis was to evaluate the rectum and perirectal tissue with 3 principal views: the morphologic study of the rectum and perirectal tissue, the evaluation of the feasibility of reconstructing the anal sphincter without electro-stimulation material and the evaluation of the feasibility of pelvic dynamic patient-specific modelling. The morphological study of the rectum allowed us to demonstrate the important variability of the rectal length and of the height of rectal peritoneal insertion line. This variability suggests that the evaluation of the location of the tumours of the rectum using a simple centimetric measure is insufficient. The morphological study also allowed us to establish a correlation between the number of the rectal lymph nodes and the rectal morphometry. This correlation should be considered when determining the minimum number of lymph nodes required for histological examination after surgery for patients with rectal cancer. In the second part of this thesis we have demonstrated the technical feasibility of reconstructing the anal sphincter using a transposed gracilis muscle and a pudendal terminoterminal or termino-lateral nerve anastomosis. This procedure could allow transposing gracilis muscle to achieve the characteristics of the external anal sphincter and to integrate intact sensory mechanisms. The last part of this thesis allowed generating a complete dynamic model of female pelvis specific to each patient. The validation of this model is the first step before the simulation of a model of pelvic floor diseases and the simulation of their corrections. This model would be a tool for the planning of surgical procedure and for a preoperative functional evaluation of the correction of pelvic floor disorders. This thesis is a basis for the development of clinical works the purpose of which is to improve the healthcare and quality of life of patients with a functional or tumoral pathology of the rectum

Abstract:
Le cancer colo-rectal est un problème de santé publique majeur dans les pays développés. Le cancer du rectum est défini comme une marge distale de la tumeur à moins de 15 cm de la marge anale. Sa prise en charge optimale impose une discussion entre une chirurgie mutilante emportant les sphincters ou une chirurgie conservatrice laissant en place l'appareil sphinctérien et évitant l'incontinence et la colostomie. L’Imagerie par résonance magnétique (IRM) est l’examen de référence pour le diagnostic de la tumeur initiale. Dans projet, nous avons utilisé des outils IRM modernes comme la volumétrie et l’imagerie de diffusion dans l’évaluation thérapeutique des cancers du rectum après chimioradiothérapie.Nous avons ainsi montré que la variation de volume tumoral au cours du traitement du cancer du rectum était associée à une survie sans récidive à 5 ans : les patients dont le volume tumoral diminuait d'au moins 70% au cours du traitement néoadjuvant avaient une survie à 5 ans sans récidive et un grade de régression histologique significativement plus élevée que chez les patients dont le volume diminuait de moins de 70%. En utilisant la même technique de volumétrie tumorale tridimensionnelle au cours d'un essai thérapeutique, nous avons également montré pour la première fois que la variation précoce de volume tumoral du rectum au cours du traitement néoadjuvant par chimiothérapie seule pouvait prédire le succès du traitement ultérieur par radio-chimiothérapie puis chirurgie. L'étude de la variation précoce de volume tumoral est actuellement utilisée dans un essai randomisé prospectif multicentrique dans lequel la variation de volume tumoral du rectum après radio-chimiothérapie néoadjuvante chez des patients avec tumeur avancée (T3c, T4) est le déterminant de la poursuite d'un traitement radio-chimiothérapique néoadjuvant ou le traitement chirurgical radical (essai GRECCAR4, ClinicalTrials.gov NCT01333709). Nous nous sommes ensuite intéressés à l’imagerie de diffusion et en particulier l’IVIM marqueur prédictif de réponse thérapeutique. Dans cette étude, la diffusion pure permettait d’obtenir des résultats prometteurs pour l’évaluation de la réponse au traitement néoadjuvant. Finalement, nous présenterons les perspectives de recherche concernant un type particulier de réponse, les réponses colloïdes
Colo-Rectal Cancer is the third most commonly diagnosed cancer in males and the second in females. Around 30% of all colorectal cancers are diagnosed in the rectum. Despite the major improvements that have been made management of rectal cancer still remains a challenge. Chemoradiotherapy (CRT) followed by surgery has been widely adopted for the management of locally advanced rectal cancers because this approach increases the probability of anal sphincter preservation, decreases the local recurrence rate and decreased the risk of colostomy. As we enter the era of personalized medicine with therapies stratified according to the risk of local or distant recurrence, imaging has become an essential tool in the preoperative decision making, to avoid both under- and overtreatment. Magnetic Resonance (MR) imaging is now an essential tool to enable the oncology team to make appropriate treatment decisions. First, we demonstrated that tumor shrinkage after preoperative chemotherapy–radiation therapy was associated with good response. Second, we demonstrated that early tumor volume decrease after induction chemotherapy before chemoradiotherapy was as well associated with good patient prognosis. Third, we demonstrated the added value of DW MR imaging for predicting tumor response using IVIM, a more sophisticated diffusion analytic approaches, which allows quantitative parameters that reflect tissue microcapillary perfusion and tissue diffusivity to be derived. In this study we demonstrated that true diffusion was associated with regression grade on pathology. Finally, we will present our perspectives especially in tumor response evaluation in patient with colloidal response
