This thesis explores how doctors and nurses use documents to share their knowledge within and across healthcare settings. In addressing this question I draw on a 15-month, multi-sited ethnographic study in several pediatric health care settings, following patients from primary care clinics, to emergency rooms, and in-patient units. The analysis focuses on the practices that go into documenting patients' histories and care, which include recordings on various on-line systems, preprinted forms, and whiteboards. By combining the previously distinct lenses of 1) knowing in practice, 2) time-space analysis of social interaction, and 3) communicative genre and genre systems, I suggest that doctors and nurses employ various types of document genres to manage, not only their distributed knowing about patients' care, but also their own movements across time-space. I outline a perspective on documents and knowing which attempts to highlight the role of human practice in how people use documents to coordinate their activities, share their capabilities, and get things done in complex distributed organizational work. The data suggest that doctors and nurses use medical documents as maps and itineraries to organize their distributed work practices. Doctors and nurses record patients' histories many times in different documents, with each document serving as a map and itinerary for a different constituency of people. Each of these documents is rarely used in isolation from other documents. Doctors and nurses constantly recombine the documents they use, which allows them to both appropriate documents from other settings into their local organization of work and build unique local combinations of documents.
(cont.) I introduce the concept of "re-localizing" to describe how doctors and nurses use documents to share their knowing within and across healthcare settings. Re-localization involves many healthcare professionals' parallel rewriting of a patient's history based on a recombination of each other's maps and itineraries and the patient's own accounts. By integrating the concrete case and the maps and itineraries based on those cases the notion of relocalization overcomes the dichotomy between the abstract and the situated, the local and global. Documents are not seen as mere vessels for abstract representations, but integral parts of distributed knowing within and across settings.

Full textThe aim of this study was to investigate records management practices in government ministries in the Kingdom of Swaziland. The study sought to determine how records are generated, used and disposed of in the government ministries; the types of records that are generated within the Swaziland government; how the records are organized and retrieved; the training needs of the Swaziland government’s registry staff; the attitude of staff towards records management practices; the level of staff awareness sound records management practices; the suitability of records storage facilities; and the challenges of e-records management in the government of Swaziland. A mixed method research paradigm consisting of a survey and observation was used to carry out the study. The target population consisted of action officers and records officers (registry staff) from government ministries in Swaziland. 92 action officers and 29 registry staff were involved in the study. 31 of the action officers were from top management, 31 from middle management, and 30 from lower level management. The results revealed that government ministries in Swaziland do not practice sound records management in line with the Swaziland National Archives Act (No. 5 of 1971). There was no uniformity with respect to filing methods and the manner in which records were destroyed or deleted. There were no control measures for ensuring the care and safe custody of records. Electronic records management policy, filing procedure manuals and a disaster recovery plan did not exist at the time of writing. The lack of records management practices directly and indirectly affects the information flow within the ministries. It is expected that the findings of this study will inform policy on records management in the government of Swaziland; facilitate effective records management in the government; create awareness about sound records management; help inform a records management training plan; and help establish integrity and transparency in the functioning of government. The study recommends that research should be extended to parastatal organisations in Swaziland to ensure that the management of records, both in the government and the public sector, is harmonized to enhance service delivery, accountability and transparency in the management of the country’s public affairs.

Full textRecords management seeks to efficiently and systematically control the life-cycle of records that are routinely generated as a result of activities and transactions. Records are a vital asset in ensuring that the institution is governed effectively and efficiently, and is accountable to its staff, students and the community that it serves. Records support decision-making, organize documents, provide evidence of policies, decisions, transactions and activities, and support the university in cases of litigation. There have been many instances at the University of Zululand whereby records were reported to be misplaced or missing thus raising a question on how are records managed and what system and policies are used in terms of records management. This Master‘s dissertation sought to investigate the status of records management at the University of Zululand. The study used the survey research method, with the questionnaire as the primary data collection instrument. The study targeted both administrative and academic (Heads of Departments) employees at the University of Zululand. Purposive sampling was used to select 26 academic Heads of Departments and 24 administrative employees, resulting in a total of 50 respondents. The findings revealed that the University of Zululand still has a long way to go with respect to day-today records management. Apart from the Integrated Tertiary Software (ITS) which is an Enterprise Management System, no formal records management system was identified at the University of Zululand that deals with either paper-based or electronic records such as emails. There also appeared to be no specific policy for records management. There were no qualified records management practitioners at the University of Zululand, even though there are employees who are hired by the university as so-called =records officers‘. Proper training, workshops and skills in records management are therefore lacking amongst academic and administrative practitioners at the University of Zululand. v The study recommends the development and adoption of an official records management policy, the appointment of a records manager, and the establishment of a records management training program for staff. Furthermore, the study recommends the acquisition and implementation of a suitable campus-wide records management model such as Windows SharePoint Server (WSPS) or other relevant system, and the establishment of a formal records department for record keeping.

A review of national health records (NEHR) systems shows that privacy and security risks have a profound impact on the success of such projects. Countries have different approaches when dealing with privacy and security considerations. The aims of this study were to explore how governments can design secure national health records systems. To do that systematically, we developed a framework to analyze NEHR systems. We then applied the framework to investigate the privacy and security risks in these systems. The studied systems demonstrate that getting privacy and security right have a considerable impact on the success of NEHR projects. Also, our study reveals that the healthcare system structure has a substantial impact on the adoption and usage rates of the system. The studied cases uncover many opportunities for improving privacy and security measures in future projects. The framework demonstrates the utility of applying it to the three cases.

Full textENGLISH ABSTRACT: Competitive pressures in every company worldwide always have, and probably always will, drive leading institutions to adopt smarter, faster and more cost effective ways of providing customers with feature-rich products and outstanding service, in order to differentiate specific offerings to local and global markets. This approach is both inevitable and laudable. At the same time, communications (physical and electronic) have made the world a smaller place and presented a larger target audience for each company's goods and services. However, customers have become better educated, more sophisticated and more demanding, whilst competitors have raised standards and the fight for customer loyalty has intensified. As these pressures increased, the appropriate level of corporate governance has not always been applied, as evidenced by the well publicised failure of Enron in the USA, and many others worldwide. The typical reaction has been to produce an increasing raft of legislation and regulation, designed to protect the consumer against 'cavalier' corporate behaviour, and an equally increasing demand for more and better information. The Banking Industry has not escaped this trend. Its failures have not been as dramatic as Enron and the consequences not as severe. Nevertheless, the potential impact of failure through poor corporate governance is huge and legislation and regulation has become highly visible through acts such as the 'Financial Advisory and Intermediary Service Act' (FAIS), the 'Financial Intelligence Centre Act' (FICA) and the Basel II Committee, - all emphasising integrity, authenticity, completeness, transparency, security, retention, protection, and eventual disposal of information. This assignment examines the relationship between the concepts information management, content management and records management and their importance as enablers of corporate governance. It highlights the current records management capability- maturity within the FirstRand Banking Group and presents a future solution and a practical approach to migration. Whilst understanding the challenges associated with implementing a records management strategy and programme, it emphasises the needs and the huge benefits for the FirstRand Group in meeting its corporate obligations, improving customer service and reducing operating costs. The future solution is set within a Records Management Capability Framework which encompasses how records must be managed by a consistent set of rules. It presents a typical records management life cycle from creation to disposal; it examines how practical systems design can balance the need for structure and stability whilst providing flexibility for operational use; it documents design principles and critical standards; it outlines implementation guidelines; and it recommends policies, ownership, organisational structures and roles, and governance mechanisms within the FirstRand Business Model. FirstRand's records management competency was audited and measured against a capability maturity model (the Carnegie Mellon University's Software Engineering Institute's approach). The results indicated significant cultural and operational challenges in building a records management capability, but it was concluded that knowledge management and sharing of information and intellectual assets (facilitating faster access to better quality and up-to-date information and building corporate memory and business agility) could be delivered only by a well conceived and carefully implemented Records Management Capability Framework.

Full textE-records readiness is key to the implementation of electronic records management programmes and ultimately the e-Government in the public sector. However, past studies about Swaziland show no evidence of research that ascertains the depth of e-Records readiness in the context of the current e-Government strategy. Studies that have been documented on records management systems in the country have largely focused on paperbased records management in government ministries. The aim of the study was to assess e-Records readiness in the Swaziland government ministries with a view to conceptualising framework for the effective management of e-Records as a facilitating tool for e- Government. The study sought to establish the national legal and policy framework governing management of electronic records in Swaziland’s government ministries in the context of e-Government; the level of compliance to policies, standards, tools, procedures and responsibilities for e- Records management in the government ministries; establish the e-Records management products and technologies existing in the government ministries; examine resource capacity and training for e-Records management staff; find out if there is internal awareness of the link between the e-Record management and the e-Government strategy; as well as explore the depth of the government wide digital preservation strategy. The IRMT (2004) E-Records Readiness Tool was adapted and used as the analytical framework for this study. This tool is designed to assist organizations to benchmark themselves and to determine where they stand in respect to the management of electronic records. The study mainly used the quantitative research approach exploiting a survey that utilized a questionnaire as a data collection instrument. The quantitative approach was however complemented by a qualitative research approach which utilized observation and interview techniques. Using both quantitative and qualitative approaches made it possible to triangulate and validate the findings. The unit of analysis was the Swaziland government ministries, with a survey design involving all the 19 Government Ministries. The target study participants comprised of the Directorate of Swaziland National Archives, the Directorate of Computer Services, the Directorate of E-Government, the records officers and the action officers.

Full textObjectives: First, to identify the core Electronic Health Records (EHR) functionalities available to physicians who work in private and public health care facilities in Trinidad and Tobago and the extent to which physicians are using each function. Second, to understand the rate of adoption of Electronic Health Records in private and public hospitals/clinics, and finally, to identify the barriers to adoption of Electronic Health Records in private and public hospitals/clinics in Trinidad and Tobago. Background: The two largest public hospitals in Trinidad and Tobago, Port of Spain General Hospital and San Fernando General Hospital, utilized paper medical records. In Trinidad and Tobago, there is little known about the EHR functions available and being used, adoption rates, and barriers to adoption of EHR in the private and public sectors. Method: Electronic Health Records (n = 130) questionnaires were sent to number of health care practices in the private and public facilities in the five regions of Trinidad and Tobago, in order to understand availability and use of EHR, adoption rates, and barriers to the use of EHR. Results: The most commonly available function for the private and public physicians was Health Information and Data with respective scores of 58% and 29%. Sixty-three percent of the private physicians who adopted EHR reported using the Result Management and Order Management functions. The public physicians who had adopted EHR reported they were not utilizing the Decision Support, Result Management, and Order Management functions. There was no statistical difference between private and public physicians for the available and used functions. A total of 53 private and 19 public physicians responded to the survey (55% response rate). Thirteen (25%) private physicians reported adopting EHR and 2(11%) public physician reported adoption of EHR. Private and public physicians cited start-up cost and technical limitations of systems as the barriers to their practices' adoption of EHR. Conclusion: Findings showed the same availability and use of core functionalities, as well as adoption rate among the private and public facilities, and slightly fewer barriers in the private practices. A larger sample is merited to understand if there is any statistically significant difference between the two groups.

Full textThis master's thesis will present a literature review and analysis ofthe implementation and use of Electronic Medical Records (EMR). The literature review will discuss reasons that support implementation of EMRs, factors that are necessary for successful implementation and barriers that impede implementation. Also, real-world examples of implementation for medical billing in healthcare organizations will be discussed, as well as the disparity in implementation rates between larger and smaller healthcare organizations.
The analysis portion of this thesis will include data from the 2009 National Ambulatory Medical Survey (NAMCS) EMR Supplement and, through the application of the Chi-Square statistical test using SPSS, will assess whether size of the medical practice in terms of number of physicians is significantly associated with EMR implementation and functionality, the latter of which includes clinical reminders and prescription ordering. It will be shown that physician size is indeed significantly associated with implementation and functionality.

MedRec is a simple, distributed system for personal control of identity and distribution of personal information. The work is done in the context of a medical information distribution system where patients retain control over who can access their data. We create a network of trusted data repositories, the access to which are determined by a set of 'smart contracts'. These contracts are stored on a distributed ledger maintained by those who generate data. The distributed nature of the system allows unified access from diverse sources in a single application with no intermediary. This increases patient control while retaining a measure of privacy of both data content and source. MedRec is amenable to extensions for decentralized messaging and distribution of information to third parties such as medical researchers, healthcare proxies, and other institutions. The system is based on a blockchain that contains smart contracts defining user identity and distribution specifics.

Full textENGLISH ABSTRACT: The past decade has seen a huge increase in the implementation and use of Electronic document and records management systems (EDRMS) in many public spheres. The South African government, in this case the Presidency, has now also adopted this system to comply with current legislation requiring government institutions to replace paper with electronic media as the preferred way of conducting their official business. In general, and as is evident from the available literature, it seems that Government organisations have been lagging behind in this regard. The failure to reap the expected benefits from this step by government institutions such as the Presidency forms the basis for this investigation, which examines the failures and successes in the implementation of an EDRMS and the factors affecting this process. The study used triangulation approach in order to arrive at a more reliable conclusion. Literature research has been used as a main data collection method for this study. It also serves as a basis for the approaches to subject matter that have been adopted. Data was also collected by conducting semi –structured interviews with government institutions, including the subject of this research, that have implemented EDRMS. The critical factors that have had an effect on the implementation of EDRMS were found to be of a strategic, social and technical nature. The study also found similarities in the factors affecting EDRMS implementation within government and identifies the need for further research focusing on how these factors differ between various types of organisations.
AFRIKAANSE OPSOMMING: Die afgelope dekade was daar ‘n ontsaglike toename in die instelling en toepassing van elektroniese dokument- en rekordbestuurstelsels (EDRMS) op talle openbare terreine. Die Suid-Afrikaanse regering, in hierdie geval die kantoor van die Staatspresident, het nou ook hierdie stelsel aangeneem om te voldoen aan huidige wetgewing wat staatsinstellings verplig om papier te vervang met elektroniese media as die aanbevole manier om hulle amptelike besigheid te bedryf. Oor die algemeen, en soos dit ook blyk uit die beskikbare bronne, lyk dit egter asof regeringsinstansies agter geraak het wat dit betref. Die feit dat die verwagte voordele van hierdie stap nie deur staatsinstansies soos die Presidensie benut is nie, vorm die grondslag vir hierdie ondersoek, wat die prestasies en weiering wat gepaard gaan met die instelling van `n EDRMS en die faktore wat hierdie proses affekteer van nader beskou. Navorsing van die literatuur is gebruik as die belangrikse datainsamelingmetode vir hierdie ondersoek. Dit dien ook as die basis vir die manier waarop die onderwerpmateriaal benader is. Data is ook ingesamel van staatsinstansies wat `n EDRMS ingestel het, soos die instansie wat hier bestudeer word. Die kritiese faktore wat ‘n invloed gehad het op die instel van `n EDRMS het geblyk van `n strategiese, sosiale en tegnies aard te wees. Die ondersoek het ook ooreenkomste gevind in die faktore wat die instel van `n EDRMS in die regering affekteer en indentifiseer ook die behoefte aan verdere navorsing wat gerig is op hoe hierdie faktore van organisasie tot organisasie verskil.

Full textABSTRACT Electronic medical record (EMR) use has improved significantly in health care organizations. However, many barriers and factors influence the success of EMR implementation and adoption. The purpose of the descriptive qualitative single-case study was to explore health care professionals? perceptions of the use of EMRs at a hospital division of a major medical center. The study findings highlighted the challenges in transitioning from paper records to EMR despite the many benefits and potential improvement in health care. A description of the 16 health care professionals? perceptions of EMR use emerged by adopting the unified theory of acceptance and use of technology (UTAUT) model and NVivo 10 computer software to aid with the analysis of semi-structured, recorded, and transcribed interviews. Themes emerging from the analysis were in five categories: (a) Experience of health care professionals with a subtheme of workflow, (b) Challenges in transition from paper to EMR, (c) Barriers to EMR acceptance, with a subtheme of privacy, confidentiality, and security, (d) Leadership support, and (d) Success of EMR. The findings of the case study may inform health care industry decision makers of additional social and behavioral factors needed for successful EMR strategic planning, implementation, and maintenance.

Full textENGLISH ABSTRACT: The research study assessed records management practices in the Limpopo Department of Agriculture (LDA), with the focus being on human resources records. Human resources records management practices were assessed in four key performance areas, namely: policy and regulatory framework, storage requirements, integrity of paper-based and electronic records, and efficiency and effectiveness of the registry system. The assessment tool, as prescribed in the Best Practice Model for Keeping and Managing Paper-Based Employee Records, was used. Research findings showed that LDA's performance in terms of records management practices do not comply with the set policies and regulatory framework and that the storage conditions are insecure. Research findings were evaluated and possible strategies for improving the management of human resources records are recommended. The adoption of awareness campaigns for staff, a coordinated training programme as well as the provision of support of top management, are some of the strategies recommended.
AFRIKAANSE OPSOMMING: In hierdie navorsingstudie is rekords van bestuurpraktyke in die Limpopo Departement van Landbou (LDL) geassesseer, met die fokus op menslikehulpbronrekords. Die menslikehulpbronrekords is geassesseer op grond van vier kernprestasie-areas, naamlik die beleids- en regulatoriese raamwerk, bergingsvereistes, integriteit van papiergebaseerde en elektroniese rekords, en doeltreffendheid en effektiwiteit van die registerstelsel. Die assesseringstelsel soos voorgeskryf deur die Best Practice Model for Keeping and Managing Paper-Based Employee Records is gebruik. Navorsingsbevindings het getoon dat die LDL se prestasie ten opsigte van rekordbestuurpraktyke nie voldoen aan beleide nie en dat ‟n regulatoriese raamwerk en veilige bergingstoestande nie bestaan nie. Navorsingsbevindings is geëvalueer en moontlike strategieë om die bestuur van menslikehulpbronrekords te verbeter, is aanbeveel. Die ingebruikneming van bewusmakingsveldtogte vir personeel, 'n gekoördineerde opleidingsprogrm en ook die verskaffing van steun deur topbestuur is van die strategieë wat aanbeveel word.

Full textInformation plays a very significant role in the economic development of the enterprises. It is increasingly used as a corporate resource to improve productivity, efficiency and effectiveness. This means that information in the form of records, is a strategic weapon to gain competitive advantage for the enterprise that creates and receives them. The study explores the problems that are facing terminology in the study of information management and records management. The problems that exist in terms of terminology are so intense that there is yet no universally acceptable definition for records and records management. The basic elements of records management are explored in attempt to position records management as an information management function. It demands records as vital information resources be managed throughout their life cycles, at a strategic level in much the same way as other economic resources such as people, money and land. In managing records effectively, the education and training of records managers is regarded as imperative. In order to test the information management function of records management, a survey is administered within Enterprise ABC to observe the records management practices amongst the records staff. The findings of the survey reveal that the records management practices within Enterprise ABC are chaotic in the sense that there are no standards and procedures that govern the flow of records within the enterprise.

Full textThe introduction of technology has affected the service standards of most service organizations. Organizations were either negatively or positively affected. (Polokwane Municipality is no exception). Records management as a major function of service organizations needs to be developed to cope with the challenges brought about by technology. The study, therefore, investigated the possible role of electronic records management in a service organization. The aim of electronic records management is to improve service quality. By making use of an electronic records management system, organizations can render quality service. The introduction of technology in records management affects several aspects such as space, time, costs, customer satisfaction and the role of the records manager. Although technology may improve the standard of service in organizations, it is important for organizations to embark on staff development programmes. Employees should be trained to use the existing system, and encouraged to further their studies. Furthermore, organizations should encourage employees to affiliate with professional organizations, as the latter enables employees to embark on a process of lifelong learning. Once a well-trained workforce has been established, responsibilities must be allocated. Employees should be made aware of their limitations. Some should be tasked only with scanning in new information, while others could be made responsible either for maintaining or disposing of records. The most senior member of staff in the records management section should be responsible for any actions with regard to the disposal of records. Other departments such as Information Technology (IT) should assist with their expertise.
