Abstract:
Fibrillary or immunotactoid nephropathy is a rare deposition disease of unknown cause in which highly organized deposits containing immunoglobulin and complement are found in the glomerular basement membrane and mesangium. These deposits are not amyloid fibrils and do not stain with Congo red. They are usually polyclonal and are not associated with monoclonal paraproteins, or with cryoglobulins or systemic lupus, distinguishing them from other non-amyloid fibrillary glomerulopathies. There is debate about whether there is a useful distinction between distinct fibrillary and rarer immunotactoid variants, the rarer immunotactoid variety being associated with larger, more organized, microtubular fibrils and possibly more commonly associated with malignancy (usually lymphoproliferative). However, clinically the conditions are similar. Patients usually present with heavy proteinuria or nephrotic syndrome. Microscopic haematuria is common and many have reduced glomerular filtration rate at presentation. Fifty per cent reach end-stage renal failure in 3–5 years and there is no convincing evidence that any treatment is effective in altering this outcome. Other organs appear to be affected only rarely (lung, liver). The disease may recur post-transplant but not always and usually more slowly, so does not preclude transplantation.

Postoperative visual loss (POVL) has multiple diagnoses and contributory causes including emboli, direct globe compression, prolonged elevation of venous pressure in the head with associated large fluid shifts, prolonged hypotension, periorbital trauma, preexisting ophthalmologic anatomic risk factors, and many other associated factors. It frequently results in permanent injury and severe disability. Though any loss of vision postoperatively should prompt an emergent or urgent ophthalmologic consultation, some of the rarer causes of POVL are considered true medical or surgical emergencies. This chapter briefly discusses these rarer causes and primarily focuses on the more common POVL diagnoses, including the mechanism of injury, appropriate assessment, and initial management.

In interviews in cities and towns across the United States, from New York to Los Angeles, and from Madison to Dallas, members of 40 black and white pairs of friends reflect on how they became friends, how racial issues are addressed, and how their friendships have influenced their views and, in some cases, their actions. Utilizing a sociological framework to examine the friendships, Korgen offers readers a rare glimpse into an even rarer phenomenon and sheds light on important aspects of race relations in America. How do close friendships between blacks and whites develop? Why are cross-racial friendships so rare? How do these friendships navigate the issue of race? Crossing the Racial Divide answers these questions through a lively discussion of the problems and issues and through the voices of members of cross-racial friendships. In interviews in cities and towns across the United States, from New York to Los Angeles, and from Madison to Dallas, members of 40 black and white pairs of friends reflect on how they became friends, how racial issues are addressed, and how their friendships have influenced their views and, in some cases, their actions. Utilizing a sociological framework to examine the friendships, Korgen offers readers a rare glimpse into an even rarer phenomenon and sheds light on important aspects of race relations in America. Challenging both the traditional notion that blacks and whites are opposites and the increasingly popular notion of colorblindness, the author reveals that, while close black/white friendships follow the concept of homophily, we cannot just wish away the tensions and disparities that exist between most white and black Americans. Cross-racial friendships provide a unique perspective that makes racism and racial separation both more visible and more vulnerable. Put into sociological context, the stories revealed in this book make evident the institutional barriers existing between most black and white Americans and offer insight into the means to dismantle them.
