Abstract:
Health always remains a core component in the life of human beings and considered as a major instrument of overall socio-economic development. Concept of health has been defined and understood in different ways by different cultures. The two major division of medicine namely curative and preventive medicine came into light at the close of 19th century. Although origin of Medicine and Public health goes back to ancient Greek mythology, Asclepius was the Greek god of Healing. He was the son of Apollo and the Coronis. Asclepius was married to Epione, with whom he had five daughters: Hygieia, Panacea, Aceso, Iaso, and Aglaea. Hygieia was the goddess of health, cleanliness and hygiene. Follower of panacea practiced curative medicine and were called healer and followers of Hygieia were called as Hygienist or Public Health experts (1). Concept of Public Health came from this hygienist category of healthcare personals. The Constitution of the World Health Organization, which came into force on April 7, 1948, defines health “as a state of complete physical, mental and social well-being.”(2) In today’s world more emphasis is given on physical health and most of the resources are diverted for the care of physical health especially for the medical treatment. Other domains of health care remain neglected. Public health applies to the holistic approach for the care of health. Physical health is best taken care by the judicious mix of public health and medical domain of health. The evolution of concepts of public health tries to address new challenges of population health like non communicable diseases, cancers, accidents, injuries, inequities, increasing cost of care, poor quality of care, and poor patient satisfaction etc.

Abstract:
The purpose of the article is to determine the main strategic directions of public policy development of the regional health care system. The definition of «health policy» is proposed. The main types of policy in the field of health care are identified: by objects of regulation – socio-economic, medical, technological; by levels – municipal, regional, state, international, world. It is proposed to define the concept of «public regional health policy», its objects and subjects. The functions of public policy in the development of health care in the region are considered. Methods of realization of the purposes of public regional policy in the field of public health services are defined: organizational-legal, economic and social. The need for careful study and analysis of regional, domestic and international experience in health care development is noted. The importance of public-private partnership in the health care system by optimizing costs and creating conditions for technological breakthrough and stable growth in the industry is noted. The actions of the regions to implement the main directions of the health care modernization program are analyzed. The main shortcomings and positive changes inherent in the current management practice of developing and implementing public policy in the field of health care at the regional level are identified. The process of development and implementation of public regional policy, which is carried out in 4 stages: policy initiation; policy implementation and monitoring involves the choice of methods and management technologies for the implementation of strategies; policy implementation; policy evaluation – assessment of the obtained effects, detection of deviations from the standard and regulation (adjustment) of the policy. In order to achieve these goals, the main strategic directions of public policy for the development of the regional health care system have been developed: 1) optimization of financial activities of the regional health care system; 2) optimization of the use of labor resources. It is noted that the proposed measures are long-term strategic in nature.

Background: In older adults (aged 60 years and above), mental health problems are gaining public health importance because of the increasing prevalence, disease burden, disability, morbidity, and mortality. Epidemiological studies on major mental health disorders such as depression and dementia in older adults have contributed to a better understanding of the distribution and determinants of these conditions. Identifying potential risk factors has stimulated interventional research on preventing these conditions under the public health framework towards their management. The increasing burden of geriatric mental health conditions like dementia in developing countries like India can contribute to significant challenges if there is no adequate strengthening of the public health response. This includes scaling up the measures of prevention, public awareness, early diagnosis, and quality health and social care equitably available to all sections of the population. The Decade of Healthy Ageing (2021–2030) provides the opportunity for concerted and coordinated initiatives to improve intrinsic capacity (physical and mental) and offer an age-friendly environment to enhance the functional ability of all older adults. Methods: This article reviews the critical public health issues related to geriatric mental health in India.

According to Congressional Budget Office projections, the Patient Protection and Affordable Care Act (Affordable Care Act or Act) — assuming it survives the pending legal challenges and is fully implemented — will provide health insurance to 34 million additional Americans by 2021. This will increase the percentage of non-elderly Americans with health insurance from the current rate of 83 percent to 95 percent. Although enactment of the Affordable Care Act constitutes a historic step forward in the nearly century-long effort to ensure universal health insurance coverage, relatively little in the Act focused on the potentially transformative impact of public health efforts that prevent disease. Chronic diseases such as heart disease, cancer, stroke, and diabetes are now responsible for more than 75 percent of health care costs, and reducing the incidence of these diseases — which are often entirely preventable — could dramatically improve the nation’s physical and fiscal health.
