Abstract:
BackgroundThe persistent trigeminal artery (PTA) is an adult carotid-basilar anastomosis with debated pathologic aspects, such as its association with brain aneurysms. True trigeminal artery aneurysms are rare vascular anomalies, reported in a few case reports.ObjectiveTo report our experience with a ruptured trigeminal artery aneurysm and to provide a systematic review of the literature in order to analyse potential links between the anatomic configuration of the PTA and PTA aneurysm (PTAA) type, and implications of each PTAA type for the diagnostic and therapeutic approach.MethodsWe reviewed the medical literature on trigeminal artery aneurysms according to the PRISMA guidelines. Population characteristics, aneurysms features, and PTA type and side were assessed.Results40 previously published cases of PTAAs were included in the analysis. The mean age of subjects was 55 years, with a strong female predominance (77%). Four PTAAs were accidentally discovered, while 16 caused compressive symptoms and 20 were ruptured. Successful endovascular treatment was performed in 62% of cases.ConclusionsPTAAs are rare vascular anomalies, underdiagnosed in the presence of a trigemino-cavernous fistula. Parent vessel occlusion seems to be the best therapeutic option for ruptured or symptomatic unruptured PTAAs in Saltzman type II and III PTAs. Patency of the parent vessel is the main target in Saltzman type I PTA.

As obstacles like stairs occur often, paramedics must frequently manually carry patients and are thereby exposed to loads multitudes higher than recommended. This creates the need for patient transport aids (PTA), which can physically support paramedics in a wide variety of transport situations, without slowing down the transport. In this paper a workflow analysis for transport missions in an urban context and basic tasks for PTAs are presented. Subsequently, the high-level task modelling and human-centered risk analysis according to the HiFEM method are presented for the use case of a patient transport over stairs with a passive PTA, like a rescue chair, and an active PTA like the novel SEBARES prototype. The analysis shows that conventional PTA’s have a simple linear use process, however, impose excessive physical workloads, which cause risks like the paramedic or the PTA falling down the stairs. Contrary, active PTA’s reduce physical workloads, however, introduce additional concurrent steps, like identifying and correcting misalignments, which create further risks. In order to mitigate risks with active, stair climbing PTAs, either new kinematic designs or intelligent assistance functions, like automatic stair detection, are necessary.

<p>Penyelenggaraan PTAIS di Kopertasis X Jawa tengah terdapat 36 PTAIS yang telah melaksanakan kegiatan proses belajar mengajar, ada 11 PTAIS yang telah mengantongi Akreditasi. Dari sebelas PTAI dapat akreditasi A hanya satu yaitu FT. Unissula. 11 prodi mendapat akreditasi B dan 3 prodi mendapat akreditasi C. Sedangkan yang lain belum mengurus akreditasi Dari data tersebut dapat dilihat fenomena bahwa beberapa PTAIS tidak mempunyai ijin penyelenggaraan pendidikan dan akreditasi. Dari potret tersebut dapat dipahami bahwa kualitas PTAIS masih minim. Oleh karena itu dalam rangka meningkatkan mutu PTAIS diperlukan rumusan strategi peningkatan kinerja PTAIS untuk mencapai visi dan misi PTAI melalui meningkatkan kualitas pendidik dan tenaga. Oleh karena itu peneliti menawarkan konsep <em>balanced scorecad</em> yang bersifat teknis, taktis dan operasional dengan mengimplementasikan prinsisp-prinsip<em> good governance</em> sebagai bahan kebijakan strategi pengembangan PTAIS di masa yang akan datang.</p>Peneliti mengembangkan model <em>balanced scorcard</em> dari Kaplan (1996) yang didasarkan pada perspektif pemangku kepentingan <em>(stakeholders), </em>Adapun tujuan penelitian adalah (1) untuk mengetahui implementasi <em>balanced scorecard</em> pada PTAIS, (2) untuk strategi peningkatan mutu PTAIS berdasarkan pendekatan <em>balanced scorecard</em> kurang baik, hal ini disebabkan bebrapa keterbatasan antara lain : Mahasiswa Baru Sebagian besar PTAIS mengalami penurunan. Abstract:
Renal angioplasty and stenting (PTRAs) resolves renal artery stenosis, but inconsistently improves renal function, possibly due to persistent parenchymal damage. We developed a bioengineered fusion of a drug delivery vector (elastin-like polypeptide, ELP) with vascular endothelial growth factor (VEGF), and showed its therapeutic efficacy. We tested the hypothesis that combined ELP-VEGF therapy with PTRAs improves renal recovery more efficiently than PTRAs alone, by protecting the stenotic renal parenchyma. Unilateral renovascular disease (RVD) was induced by renal artery stenosis in 14 pigs. Six weeks later, stenotic kidney blood flow (RBF) and glomerular filtration rate (GFR) were quantified in vivo using multidetector CT. Blood and urine were collected during in vivo studies. All pigs underwent PTRAs and then were randomized into single intrarenal ELP-VEGF administration or placebo ( n = 7 each) groups. Pigs were observed for four additional weeks, in vivo CT studies were repeated, and then pigs were euthanized for ex vivo studies to quantify renal microvascular (MV) density, angiogenic factor expression, and morphometric analysis. Renal hemodynamics were similarly blunted in all RVD pigs. PTRAs resolved stenosis but modestly improved RBF and GFR. However, combined PTRAs+ ELP-VEGF improved RBF, GFR, regional perfusion, plasma creatinine, asymmetric dimethlyarginine (ADMA), and albuminuria compared with PTRAs alone, accompanied by improved angiogenic signaling, MV density, and renal fibrosis. Greater improvement of renal function via coadjuvant ELP-VEGF therapy may be driven by enhanced MV proliferation and repair, which ameliorates MV rarefaction and fibrogenic activity that PTRAs alone cannot offset. Thus, our study supports a novel strategy to boost renal recovery in RVD after PTRAs.

Abstract:
Background. The Ottawa Decision Support Framework (ODSF) has guided the development of patient decision aids (PtDAs) for 20 years and needs updating across a range of decisions and hypothesized outcomes. Purpose. To determine the effectiveness of ODSF-developed PtDAs on hypothesized outcomes and to recommend framework changes. Data Source. A subanalysis of randomized controlled trials included in the 2017 Cochrane review of PtDAs comparing PtDAs to usual care in adults considering health treatment or screening decisions (searched to 2015). Study Selection. Trials in the original review that evaluated ODSF-developed PtDAs. Data Synthesis. Meta-analyses of ODSF outcomes with similar measurements and descriptions of other reported outcomes. Results. Of 105 trials, 24 evaluated ODSF-developed PtDAs. Compared with usual care, ODSF PtDAs improved knowledge (mean difference [MD] 13.85; 95% confidence interval [CI] 10.32−17.37; 14 trials), increased accurate risk perceptions (risk ratio [RR] 2.41; 95% CI 1.66−3.48; 7 trials), and increased congruence between informed values and chosen options (RR 1.32; 95% CI 1.09−1.59; 4 trials). They reduced perceived decisional needs as measured using the Decisional Conflict Scale (MD −5.92; 95% CI −8.58 to −3.26; 15 trials) and the proportion of undecided patients (RR 0.65; 95% CI 0.50−0.83; 13 trials). Non-ODSF PtDAs, designed with or without a specific framework, also outperformed usual care. Few ODSF trials measured secondary outcomes. Limitations. The included trials had heterogeneity. Conclusion. ODSF PtDAs address decisional needs and improve decision quality; the best indicator of addressing perceived uncertainty is “proportion undecided.” Secondary ODSF outcomes should be reduced to adherence to one’s chosen option and use/costs of health services, which warrant further research.

Abstract:
Background. Although patient decision aids (PtDAs) have been shown to improve patient knowledge and satisfaction, they are infrequently used in the real world. We aimed to understand how our publicly available PtDAs developed for destination therapy left ventricular assist device (DT LVAD) were implemented in clinical practice and characterize factors influencing adoption. Methods. We contacted 39 people, 31 who had independently emailed inquiring about our DT LVAD PtDAs and 8 identified through snowball sampling. Thirty people from 23 programs participated in semistructured interviews, which were analyzed using normalization process theory. Results. Eleven programs currently use the PtDAs, 5 plan to use them but have not yet, and 7 do not currently use them nor have active plans to use them. Due to major tradeoffs and preference sensitivity of the DT LVAD decision, participants recognized a role for shared decision making and a need for significant information transfer. Due to a relative lack of resources, participants saw the PtDAs as a way to help facilitate a higher quality decision-making process. Limited time, lack of personnel, and perceived burden to implementing system-level change were cited as barriers to use. Initial implementation was accomplished by a champion of the PtDAs. Actual use of the PtDAs most commonly occurred through LVAD coordinators at the start of formal patient teaching sessions, where the PtDAs could be integrated into the existing LVAD consent and education structure. Conclusion. Interest in and implementation of PtDAs occurred independently at several LVAD programs due to a favorable decisional context, including a perceived role for shared decision making in the high-stakes decision around DT LVAD, unmet informational needs, preexisting education sessions, and invested clinical champions.

This paper studies the effect of preferential trade agreements (PTAs) on multilateral trading system using a sequential bargaining game. The study considers two formats of PTAs, i.e., when the PTA tariffs are specified before the formation of PTAs and when PTA members have to negotiate PTA tariffs after the formation of PTAs. The study finds that PTAs with specified tariffs can be building blocks to multilateral liberalization while PTAs without specified tariffs are stumbling blocks. The paper also concludes that PTAs can eliminate bargaining inefficiency called forward manipulation when PTA tariffs are specified before PTA negotiation.

Abstract:
Objective. To identify predictors of post-fetoscopic twin anemia polycythemia sequence (pTAPS) after fetoscopic laser photocoagulation (FLP) of placental anastomoses for twin–twin transfusion syndrome (TTTS). Patients and methods. The study included 195 patients who underwent FLP of anastomoses for TTTS during the period from 2010 to September 2019. A group of 13 pregnant women in whom pTAPS developed in the postoperative period was selected. Results. Out of 169 pregnancies lasting for more than 2 weeks after FLP of anastomoses, pTAPS developed in 13 (7.7%). It was revealed that FLP was performed before 20 weeks in 85% of those who developed pTAPS, while the average period in the group was 22.1 weeks. Localization of the placenta along the anterior wall of the uterus with pTAPS was observed 3 times more often than along the posterior wall, while in the rest of the operated patients this ratio was approximately 1:1. The number of anastomoses ranged from 3 to 11, most frequently – 5. Conclusion. With the development of pTAPS, there is a high probability of an unfavorable outcome. Localization of the placenta along the anterior wall of the uterus, localization of the anastomoses in the amniotic cavity of the donor twin are the factors that predispose to the development of pTAPS. The presence of 1–2 large AV anastomoses together with no more than 5 small anastomoses may serve as a probable predictor of the pTAPS development. This research study attempted to gather, present and analyse information regarding the current role of the Parent-Teacher-Student-Associations in the democratic governance of secondary schools for purposes of contributing towards the education governance policy discourse as South Africa moves away from apartheid to democracy. Central to this largely fact-finding exercise was an attempt to make a contribution to an understanding of how the Parent-Teacher-Student-Associations, in the execution of their duties, practice both democracy and accountability; operate; resolve tensions and/or differences among parents, teachers and students; impact upon the schools in general; and relate to both the Department of Education and Training and other organs of civil society. Further, this survey attempted to ascertain m which crucial areas the Parent-Teacher-Student-Associations are most lacking, and how these could be strengthened. And finally, an attempt was made in this study to contribute to the possible future role of the Parent-Teacher-Student Associations in the new democratic education dispensation, and how, in the execution of this new role the Parent-Teacher-Student-Associations should relate to the new, future democratic government. The study concluded with a number of recommendations for policy in the area of democratic school governance. The study used largely a survey method. The Parent-Teacher-Student-Associations of three secondary schools under the auspices of the Department of Education and Training in the Western Cape region were surveyed. The single most important data-gathering instrument used was the interview. Numerous conclusions were arrived at. First, the Parent-Teacher-Student-Associations studied were found to be very powerful and effective in their areas of operation in school governance despite their inability to have access to resources of power, wealth and expertise. These Parent-Teacher-Student-Associations are important education policy actors who are not only influencing policy, but are in total control of very crucial policy areas in their schools. Second, the study concluded that whereas the Parent-Teacher-Student-Associations studied represent an important step towards the full democratisation of education in general, and in their schools in particular, their role in school governance could not be described as an unqualified success. However, despite the problems associated with the PTSAs involvement in school governance, their role does have the potential to make for better schools. And finally, because of the limited nature of the study in terms of the methodology, scope and time, the conclusions arrived at here cannot and should not be generalised beyond the confines of the study as no attempt was made to embark upon regional or national research exercise.

Memoria para optar al título de Ingeniera Civil Química
El crecimiento de la población mundial y aumento del desarrollo económico de los países ha generado una mayor producción de residuos, los que se asocian directamente a la actividad humana. En Chile, la mayor parte de estos es dispuesta en rellenos sanitarios y vertederos, emitiendo GEI directamente a la atmósfera, y generando lixiviados que pueden contaminar las aguas y provocar pérdida de nutrientes en los suelos. Bajo este contexto, el presente trabajo estudia la coincineración de residuos mediante el proceso Thermylis® como una alternativa a la disposición actual de lodos provenientes de PTAS, tomando como caso base los generados en distintas regiones del país. Se evalúa este proceso a través de una comparación técnica, económica y ambiental de distintas opciones de alimentación, consistentes en una mezcla de lodos con otros residuos sólidos seleccionados con el fin de aumentar el poder calorífico. Considerando la población residente, el número de sitios de disposición final, y la generación y destino de los residuos, se definen tres casos de estudio: Gran Santiago Región Metropolitana (caso 1), Rancagua y alrededores Región de O Higgins (caso 2), y Gran Concepción y alrededores Región del Biobío (caso 3). Se consideran los lodos generados en las principales PTAS de cada región, con lo que los flujos de alimentación para cada caso son 1.158[t/día], 105[t/día] y 249[t/día], respectivamente. A partir de la evaluación técnica, en los tres casos se seleccionan alimentaciones compuestas por lodos y plásticos, debido a que son las más favorables desde el punto de vista energético. Además, se determina que en todos ellos se cumple con la normativa ambiental vigente para este tipo de procesos (D29, MMA). Por otra parte, se realiza una evaluación económica a partir de un flujo de caja puro a 15 años, con lo que se obtiene un VAN de -195 [MUSD] para el caso 1, -45 [MUSD] para el caso 2 y -73 [MUSD] para el caso 3. Con esto, se concluye que el proyecto no es rentable desde el punto de vista económico en los escenarios planteados, debido a la alta inversión requerida por Thermylis® y el bajo costo por disponer residuos en Chile.

A América do Sul, na história recente, foi palco de diversas ações de integração regional relacionadas ao comércio, influenciando os posicionamentos dos países em matéria de políticas de comércio interncional bem como formando um emaranhado de relações que se sobrepõe muitas vezes de formas antagônicas. Neste contexto, a presente dissertação aborda o histórico das principais blocos regionais que envolvem a América do Sul como base para mostrar a atual fragmentação deste subcontinente em três visões principais de política de comércio internacional, sendo uma visão intermediária caracterizada pelo Brasil e acompanhada pelos membros do MERCOSUL em uma vertente regional-multilateralista. Em um extremo estão Venezuela, Equador e Bolívia, representantes da ALBA, com seus governos de posicionamento extremo-nacionalista. E em posição antagônica a estes estão Chile, Peru e Colômbia, que conformam o eixo liberal-bilateralista. Objetivando comprovar que há uma tendência de fortalecimento da visão liberal-bilateralista na região será feito um estudo com base nas suas principais formas de atuação, ou seja, por meio da análise de Acordos Preferenciais de Comércio, mais especificamente FTAs bilaterais celebrados com países de distintas regiões com foco especial ao período imediatamente após a eclosão da crise de 2008, de 2009-2014. Finalmente, se a hipótese se confirmar, restando evidenciada a retomada do posicionamento bilateralista, após período de predominância da visão multilateralista na América do Sul, é importante apontar os desafios para o Brasil neste cenário.
South America, in recent history, has had several regional integration actions related to trade, influencing the countries positions in terms of international trade policies as well as forming a tangle of relationships that often overlaps with contrary views. In this context, this thesis addresses the history of the main regional blocs involving South America as a basis to show the current fragmentation of this subcontinent in three main views of international trade policy, with an intermediate vision characterized by Brazil and followed also by members of the MERCOSUR in a regional-multilateralist position. At one extreme, the countries Venezuela, Ecuador and Bolivia, ALBA representatives, present their extreme-nationalist position. And in an antagonistic position, Chile, Peru and Colombia constitute the liberal-bilateralist axis. In order to prove that there is a trend towards the strengthening of the liberal-bilateralist vision in the region, a research based on analysis of Preferential Trade Agreements will be held, focusing bilateral FTAs concluded with different countries regions with special attention to the period immediately after the outbreak of the 2008 crisis, from 2009 to 2014. Finally, if the hypothesis is confirmed, demonstrating the resumption of the bilateralist position after a period in which the multilateralist vision was predominant in South America, it is important to point the challenges of this scenario for Brazil.

Com uma área plantada em torno de aproximadamente 1 milhão de hectares o país mantém a posição de maior produtor mundial de citros. Desse total, 77% encontram-se localizados na região Sudeste. O sistema citrícola representa 4,47% das exportações de produtos do agronegócio nacional, sendo que o suco de laranja concentrado representa 72% do valor dessas exportações. Entretanto, todo este potencial econômico vem sendo prejudicado por várias doenças dentre elas uma denominada Cancro Cítrico, a qual é causada por um fitopatógeno bacteriano denominado Xanthomonas citri subsp. citri (Xcc). Com o objetivo de analisar os feitos da interação entre essa bactéria e seu hospedeiro natural, utilizou-se a técnica de Análise da Transcrição Reversa em Tempo Real (qRT-PCR) para quantificar a expressão dos genes que compõem o SSTT, SSTQ, algumas ORFs hipotéticas e as quatro cópias do gene pthA. Para tanto, realizou-se um estudo prévio com 10 genes candidatos visando identificar os mais adequados para serem usados como genes de referência no processo de normalização de dados em experimentos dessa natureza. O experimento contou com quatro replicatas biológicas e duas replicatas técnicas. Com base nos resultados, verificou-se que os genes rpoB e rpoC parecem ser os mais indicados para a normalização de dados em experimentos de expressão gênica por qRT-PCR que envolvam analises intergrupos. Abstract:
Piercing problems arise often in facility location, which is a well-studied area of computational geometry. The general form of the piercing problem discussed in this dissertation asks for the minimum number of facilities for a set of given rectangular demand regions such that each region has at least one facility located within it. It has been shown that even if all regions are uniform sized squares, the problem is NP-hard. Therefore we concentrate on approximation algorithms for the problem. As the known approximation ratio for arbitrarily sized rectangles is poor, we restrict our effort to designing approximation algorithms for unit-height rectangles. Our e-approximation scheme requires nO(1/ε²) time. We also consider the problem with restrictions like bounding the depth of a point and the width of the rectangles. The approximation schemes for these two cases take nO(1/ε) time. We also show how to maintain a factor 2 approximation of the piercing set in O(log n) amortized time in an insertion-only scenario.

When timing constants are uncertain (in early the design phase, or due to slight variations of the timing bounds), timed model checking techniques may not be satisfactory. In contrast, parametric timed model checking synthesizes timing values ensuring correctness. takes as input an extension of parametric timed automata (PTAs), a powerful formalism to formally verify critical real-time systems. extends PTAs with multi-rate clocks, global rational-valued variables and a set of additional useful features. We describe here the new features and algorithms offered by 3, that moved along the years from a simple prototype dedicated to robustness analysis to a standalone parametric model checker for timed systems.

Electrorheological (ER) fluids are typically composed of polarizable particles dispersed in a non-conducting fluid. Upon the application of an electric field, chain-like or fibrillar aggregates of the suspended particles are oriented along the direction of the electric field, thereby inducing viscoelasticity and a drastic increase in viscosity. In our study, Poly(3-thiophene acetic acid), PTAA, has been developed for using as ER material. The rheological properties of this PTAA suspension upon the application of electric field were investigated under various deformations; oscillatory shear flow, steady shear, and creep. We found that PTAA based ER fluid exhibited viscoelastic behavior and showed the excellent responses under an applied electric field. Moreover, the ER response of this PTAA fluid was amplified with increases in electric field strength, particle concentration, and particle conductivity. Under the oscillatory shear, the dynamic moduli, G′ and G″, increased dramatically by 10 orders of magnitude, when the field strength was increased to 2 kV/mm. The suspensions exhibited a transition from fluid-like to solid-like behavior as the field strength increased. While under steady shear flow, the yield stress increased with electric field strength, E, and particle volume fraction, φ, according to a scaling law of the form, τy α Eαφγ. Furthermore, the creep curves of this ER fluid consisted of both elastic and viscous responses and this fluid exhibits partially elastic recovery after the removal of applied stress. The creep properties strongly depended on the magnitude of an applied stress.
